Objectives : To compare the efficacy of local acupoint with distal acupuncture at relieving pain and improving function in knee osteoarthritis. Designs : A randomized, single-blinded, crossover clinical trial. Settings : One outpatient clinic(department of acupuncture & moxibustion) located in academic teaching hospital, South Korea. Patients : 17 patients with osteoarthritis of the knee(mean age 62.76[$SD{\pm}4.37$] years). Interventions : The trial had 4 stages : baseline(2weeks), phase I and II(each 2weeks), washout period(2weeks). Patients were randomly assigned to either group A or group B. Group A received acupuncture at local acupoints during phase I, then acupuncture at distal acupoints in phase II. Group B received the treatments in reverse order. In each phase, the patients were treated with acupuncture for 6 times. Measurements : The primary outcome was subjective pain as measured by a 100mm visual analogue scale(VAS) ranging from 0(no pain) to 10(worst pain ever). Secondary outcomes were changes in the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC) total and pain scores. Measurements were obtained at baseline, 1st day of phase I and II, and 2 days after last treatment of phase I and II. Results : The 17 participants in 2 groups were well matched for age, sex, target knees, baseline VAS score, WOMAC pain score and WOMAC score. Participants in local acupoint group experienced greater improvement than distal acupoint group at 2 days after last treatment in WOMAC total score(mean difference, -10.65[95% CI, -20.56 to -0.74] ; P=0.036) but not in VAS(mean difference, -12.41[95% CI, -29.56 to 4.73] P=0.15) and WOMAC pain score(mean difference, -1.82[95% CI, -3.98 to 0.33] ; P=0.094). Conclusions : Local acupoints are more effective than distal acupoints at relieving pain and improving function in knee osteoarthritis.

Was used to measure recovery or worse of patient's condition, muscle assessment questionnaire(MAQ) was used to measure patient's muscular strength, Endurance, coordination/balance, KWOMAC was used to pain, stiffness, and physical function, and BPM was used to measure velocity, anterior-posterior. Results : This study results in following conclusions. 1. MAQ score was significantly decreased in modality group(p<.05). 2. KWOMAC score was significantly decreased in modality group(p<.05). 3. BPM were score was significantly decreased in modality group(p<.05). Conclusions : From this result the modality treatment retrogression characteristic will be effective in treatment of patient.

Results : This study results in following conclusons. 1. MAQ score was significantly decreased in PNF group(p<.05). 2. KWOMAC score was significantly decreased in PNF group(p<.05). 3. BPM were score was significantly decreased in PNF group(p>.05). Conclusion : From this result the PNF treatment retrogression characteristic will be effective in treatment of patient, with the arthritis and widely may be applied at a therapist.

Purpose: The purposes of this study were to compare clinical results after total knee arthroplasty (TKA) using the Attune and PFC Sigma knee designs and to investigate whether the use of the Attune prosthesis increased the risk of patellar injury in Asian patients. Materials and Methods: Three hundred knees that underwent TKA using Attune (group A) were compared to 300 knees that underwent TKA using PFC Sigma (group B). The Knee Society Knee Score (KS) and Function Score (FS), and range of motion (ROM) were compared. The residual patellar thickness was compared to evaluate the risk of patellar injury. Results: The postoperative KS and ROM of group A were better than those of group B (93.1 vs. 88.8, p<0.001 and $131.4^{\circ}$ vs. $129.0^{\circ}$, p=0.008, respectively). The postoperative FS did not differ significantly between the two groups (80.9 vs. 78.7, p=0.427). The residual patella was thinner in group A (14.8 mm vs. 15.7 mm, p=0.003), which made up a higher proportion of the high-risk group for patellar fractures with a residual thickness of <12 mm (7.5% vs. 2.1%, p=0.003). Conclusions: TKA using the Attune prosthesis provided more favorable clinical results than TKA using PFC Sigma. However, the risk of injury in the residual patella was increased with use of the Attune prosthesis in Asian patients.
목적: 슬관절 주위에 발생한 골종양 환자에서 종양 대치물을 이용한 재건술의 결과와 보행 분석의 유용성을 알아보고자 하였다. 대상 및 방법: 2001년부터 2010년까지 슬관절 주위에 발생한 골종양에 대하여 광범위 절제술 후 종양 대치물을 이용한 재건술을 시행 받은 30명 중 7명을 대상으로 하였으며 기능적 결과 및 보행 분석 검사를 평가하였다. 결과: SF-36 점수는 신체적, 정신적 역할제한 항목에서 각각 100% (100점)으로 높은 점수를 보였으며 일반 건강상태, 신체적 기능 정도, 활력, 사회적 기능에서 낮은 점수를 나타냈다. 또한 MSTS 평가의 종합 평균 점수는 88.1% (23.8점[17-27])였다. 보행 분석 검사상 평균 보행속도 97.2 m/s, 평균 분속수 105.6 step/min, 평균 활보장 111.3 m, 평균 보장 61.5 cm, 유각기 39.8%cycle, 입각기 60.1%cycle, 평균 단하지 지지기 37.1%cycle, 평균 양하지 지지기 13.0%cycle, 평균 발 들림시기 60.7%cycle였다. 결론: 슬관절 주위 종양에 대한 광범위 절제술 후 종양 대치물을 이용한 재건술은 좋은 기능적결과를 기대할 수 있으며, 보행 분석 검사는 술후 환자의 상태를 정량적으로 평가함으로써 보행 모습 및 기능을 객관화 할 수 있는 한 방법으로 사료되며 기능적 평가 방법과 함께 치료 및 술 후 재활 계획을 세우는데 도움이 되리라 생각된다.
Objective : Acupuncture has been widely used throughout the world for the treatment of knee osteoarthritis (knee OA). This study investigated whether acupuncture, particularly Sa-am acupuncture, could be effective in relieving pain and improving the symptoms of knee OA. Method : This study was conducted as a prospective, randomized, controlled, and patient- and investigator-blinded clinical trial. Forty volunteers with knee OA participated in the study. All participants were screened through an inclusion and exclusion criteria. Thirty four participants completed the clinical trial. In total, forty subjects were randomly selected to receive Sa-am acupuncture. Eight sessions of acupuncture were given at the contralateral side of the problematic knee for 4 weeks. Korean translation of Western Ontario and McMaster Universities Osteoarthritis Index (KWOMAC) scores were measured twice: at the beginning and end of the clinical trial period. Both the Patient Global Assessment and physical health scores based on the 36-Item Short-Form Health Survey were also used to measure the results. Results : Compared to the pre-trial scores, the Sa-am acupuncture group (n=34) showed a significant decrease in KWOMAC total scores according to a paired t-test. The Sa-am acupuncture group also showed significant improvement in the Patient Global Assessment when compared to the pre-trial. Conclusions : Sa-am acupuncture for knee OA resulted in an improved KWOMAC total score. This was mostly driven by the function component score that was greatly affected by acupuncture. However, further studies with expanded designs are needed to solidify this finding with scientific rigor.
Purpose: This study was to examine the effect of a taping therapy on pain relief and the improvement of daily living for elderly having degenerative knee arthritis. Method: This study was conducted with a non-equivalent control-group pre-test and post-test design. Data were collected conveniently with 63 elderly who had having(a) pain due to degenerative knee arthritis, and (b) inconvenience in daily life(30 for an experimental group and 33 for a control group). The subjects were recruited from the elderly, participating in welfare programs held in a welfare organization and day-care facilities. The experimental group received an intervention of taping therapy offered twice a week, for 4 weeks. The data collection from the experimental group was done from the beginning of the therapy throughout two weeks later after the end of the therapy. Results: For the experimental group, pain scores came to more decreased significantly, as the periods in which taping therapy was conducted were getting longer. Physical function scores became also more decreased at significantly level, as taping therapy was more conducted. However, compared to the control group, the score change for the experimental was not significantly showed in physical function after the therapy ended. That is, there was no longer-lasting effect on physical function improvement. Conclusion: This study found that this therapy could be a useful self-management method that the elderly with degenerative knee arthritis can use easily at home. Because of insignificant result in longer-lasting effect, this taping therapy would be applied properly with the interval of 2~3days.
본 연구는 퇴행성 관절염으로 인해 무릎관절 전치환술을 실시한 환자를 대상으로 체간 안정화 운동이 무릎기능, 균형, 보행능력 향상에 어떠한 영향을 미치는지 알아보고자 한다. 본 연구 대상자는 퇴행성 관절염으로 진단 받아 무릎관절 전치환술을 실시한 환자 24명을 선정하여, 무작위 배분으로 일반 물리치료를 실시한 대조군 12명, 체간 안정화 운동을 실시한 실험군 12명으로 구성하였다. 체간 안정화 운동은 선행 연구를 참고하여 구성한 3가지 운동을 총 4주간 실시하였다. 측정도구로는 무릎기능 측정은 무릎관절 주관적 기능 점수 척도를, 균형능력은 일어나 걷기 검사, 한발 서기 검사를 보행능력은 10미터 걷기 검사를 이용하였다. 통계학적 분석은 각 그룹 내 비교는 대응표본 t-검정, 각 그룹 간 비교는 독립표본 t-검정으로 분석하였다. 본 연구 결과 중재 후 실험군이 대조군에 비해 모든 측정 결과가 유의하게 향상되어 나타났다(p<.05). 이와 같은 결과를 바탕으로 무릎관절 전치환술 환자를 대상으로 무릎관절 치료와 함께 체간 안정화 운동을 실시한다면, 환자의 무릎기능, 균형, 보행에 좀 더 효과적이라고 판단된다.
Background: The purpose of this study was to investigate the effects of physical therapy intervention with local vibration on the physical function of patients with traumatic patella fractures. Methods: This study recruited 6 subjects who had suffered traumatic patella fractures. The study was conducted for an average of 12.8 weeks. Before the treatment (2 weeks post-surgery), they were evaluated using the numeric pain rating scale (NPRS), the Korean-version of the impact of event scale-revised (IES-R-K), pressure pain threshold (PPT), range of motion (ROM) of the knee joint, and the Korean knee injury and osteoarthritis outcome score (K-KOOS) and were reevaluated after 7 and 12 weeks, post-surgery. This study was conducted according to ORIF Patella Fracture Post-Operative Rehabilitation Protocol after applying local vibration. The protocol consists of Phases 1~5 and this study has been applied from phase 2. Results: A comparison of the performance of the participants before and after the intervention showed a decrease in NPRS (9.83±.41→4.83±.98), IES-R-K (68.67±2.73→23.83±2.40), and K-KOOS (Function, Daily living: 70.5±5.96→34.0±3.35, Function, Sports and Recreational activities: 22.83±2.32→10.77±1.37, Quality of Life: 19.33±7.33→7.33±.52) scores. And the ROM (Knee flexion: 30.0±4.47°→128.73±3.6°, Knee extension: -6.83±2.48°→-1.33±1.03°) and PPT (9.67±.52kg/cm2→22.44±2.33kg/cm2) scores increased. Conclusion: These results show that physical intervention with local vibration using a Blackroll® booster and head can help to improve pain, physical function, and psychological status. Also, it was possible to select interventions depending on the patient's condition and the desired goal, using physical intervention with the Blackroll® booster technique.
The purpose of this study is to illustrate the potential for clinical improvement in treating a patient with a case of Posttraumatic Knee Stiffness with a combination of Korean medicine therapy. We treated the patient with acupuncture, electroacupuncture, CHUNA manual therapy and Interferential Current Therapy from $5^{th}$ December 2016 to $13^{th}$ January 2017 (total 29 times) by evaluating knee function with VAS score. After six weeks of treatment, this patient achieved effective outcome following the technique, showing that clinical symptom as able to walked and pain was relieved, VAS changed from 10 to 2 and the knee flexion ROM (active/passive) changed from 30/60 degrees to 120/140 degrees. This result shows that Korean Medicine therapy may be an effective option for Posttraumatic Knee Stiffness. Further clinical studies are needed to clarify the effect of Korean Medicine therapy on Posttraumatic Knee Stiffness.
