The Effects of Exercise Program on Pain and Balance ability in Patients with Total Knee Replacement : Meta-analysis

  • This study attempted to systematically and comprehensively analyze individual studies in which exercise program on total knee replacement patients. The electronic database for literature search used the Korean educational Academic Information (RISS), Korean Academic Information (KISS) and Nuri Media (DBPIA). We investigated a prior study of exercise program applied to total knee replacement patients from 2015 to February 2021. For meta-analysis, effect size of each individual study was extracted using R project for Statistical computing version 4.0.3. Rob 2.0 tool, developed by the Cochrane group, was used to evaluate the quality of each individual study. The overall effect size exercise program with pain and balance was 0.99 (95% CI=0.53-1.44), which was significantly different than large effect size (p<0.05). The sub-group for pain was analyzed, for effect size of 0.62 and the effect size of balance scale(timed up and go test) was 1.00. Through these results, it was found that exercise program is an effective exercise for total knee replacement patients, and further research is needs.

Effectiveness of Korean Medicine Treatment for Knee Joint Pain with Plica Syndrome after Traffic Accident: Case Report (교통사고로 인한 추벽증후군에 대한 한의학적 치료의 효과: 증례보고)

  • This study aims to evaluate the effect of various Korean medicine treatment techniques on knee pain with medial plica syndrome. We treated three knee pain patients with medial plica syndrome using acupuncture, herbal medicine, pharmacopuncture and cupping during their hospitalization. EuroQol-five dimensions (EQ-5D), Western Ontario and Mcmaster Universities Osteoarthritis Index (WOMAC), numerical rating scale (NRS) and the changes of symptoms were used for patient assessment. There were notable decreases in WOMAC and NRS and a notable increase in EQ-5D of case 1 and 3. There was also an improvement in the symptoms experienced by the patients. Our study is the first case report to demonstrate the applicability and effectiveness of Korean medicine treatment compared to those of the pre-existing surgical and conservative treatments for knee pain patients with medial plica syndrome.

The Effect of 8 Constitution Acupuncture on degenerative arthritis of knee joint (퇴행성(退行性) 슬관절염(膝關節炎)에 체질침요법(體質鍼療法)이 미치는 영향)

  • Objective: This study was designed to compared the effect of general acupuncture on degenerative arthritis of knee joint with 8 constitution acupuncture by Visual Analogue Scale(VAS). Methods: VAS was used for this research, with the pain indicator of 10 and 0 which represents a pain of first visit and a status of no pain respectively. Two groups, consisting of 40 patients, was randomly allocated for this research. SPSS(Statistic Program for Social Science) 10.0 for window was used for a statistical analysis. Independent T-test and Mann Whitney U test was performed to evaluate the improvement of each group by the treatment number and to compare that of two groups. Results: As a result of the comparison analysis between two groups after 20th treatment, the improvement of subjective symptoms in 8 constitution acupuncture group was shown more effective than that in general acupuncture group, while, After 10th treatment, there was no significant difference between two groups in VAS. Conclusion: It is suggested that the effect of 8 constitution acupuncture should be significantly different from that of general acupuncture on degenerative arthritis of knee joint. Further study is needed to confirm the effectiveness of 8 constitution acupuncture.

Effects of the Moxibustion Therapy on the Pain Decrease and Joint Recovery with Degenerative Knee Arthritis (뜸 치료가 퇴행성 슬관절 환자의 동통완화와 관절기능 회복에 미치는 영향)

  • Objective : This study was designed to the effect of moxibustion therapy on the pain decrease and joint recovery with degenerative knee arthritis Methods : The research was administered from January to March 2008. The study subjects are menopause female 20 person and over 50 years old with degenerative knee arthritis patients, The Research were conducted with 10 moxibustion group and to 10 hot pack group during 4 Weeks. Results : 1. There is no significant difference of LSS of pre-treatment between hot pack group and moxibustion group(p<0.05). 2. There is no significant difference of LSS of post-treatment between hot pack group and moxibustion group(p<0.05). 3. There is no significant difference of LSS between pretreatment and post-treatment in hot pack group and moxibustion group(p<0.05). 4. There is no significant difference of VAS of pre-treatment between hot pack group and moxibustion group(p<0.05). Conclusion : This study suggests that hot pack used usually in physical therapy can decrease pain but the there is no significantly different between these two methods.

The Clinical Study of Oriental Medicine on Spontaneous Osteonecrosis of the Knee (자발성 슬관절 골괴사 환자에 대한 한방치료 증례연구)

  • Objectives The purpose of this study was to report the effect of Oriental medicine on spontaneous osteonecrosis of the knee. Methods Patients received Oriental treatment such as Bee-Venom therapy and acupuncture & moxibustion. The improvement of the pain intensity and recovery of gait was observed. Results During five weeks of treatment, patient's knee pain and gait disturbance were improved. Conclusions In this case, Oriental treatment on spontaneous osteonecrosis was effective to reduce pain and recover gait.

Study on Effect of KWOMAC of a Contact Lateral-Wedged Insole with Strapping in Patients with Varus Deformity Osteoarthritis of the Knee (내반슬 골관절염 환자에 대한 밀착형 외측 쐐기 스트랩 발받침이 한국형 WOMAC에 미치는 영향에 관한 연구)

  • The purpose are to demonstrate the pain, stiffness, and physical function by the Korean Western Ontario MacMaster University(KWOMAC) score of the patients with varus deformity osteoarthritis of the knee after treatment period. Twenty outpatients with knee osteoarthritis(OA) were randomized and divided into the control(n=10) and wedge(n=10) groups. The remission scores of the KWOMAC index of severity for knee OA were compared between the groups. The result of repeated two-way ANOVA's revealed that a contact lateral-wedged insole with strapping produced significantly differences in the pain and physical function score between groups after treatment period(P<0.05), but it was not significant differences in the stiffness score between groups(P>0.05). These results suggest that the pain relief and improvement in function reported by patients with osteoarthritis while using lateral-wedged insole with strapping may be achieved by a reduction in external varus moment and medial compartment load.

Clinical Study of Acupucture Effect on Microtraumatic Injuries of the Knee Joint (슬관절 미세손상환자의 침치료에 관한 임상적 고찰)

  • Objectives : This study was performed to evaluate the clinical effect of acupuncture on microtraumatic injuries of the knee joint. Methods : Among the outpatients with knee joint pain who visited to Department of Acupuncture & Moxibustion, National Medical Center from February 2000 to September 2000, we selected 15 cases that showed normal finding in both X-ray and neurological examination, and that were categorized into microtraumatic injuries by the physical examination based on the Cyriax's orthopedic medicine. During acupuncture therapy, the patients were ordered to move involved side actively. The effect was assessed through questionnaires of CNRS(Cincinnati Knee Rating System). Results : We investigated 11 female and 4 male patients. The most common distribution of age was 30's(40%). The most commom duration of symtoms was from 3 to 5 months(40%). The most common microtraumatic injury was pes anserinus bursitis(33.3%), and followed by iliotibial band friction syndrome(20%), and patellar tendinitis(20%). In the CNRS, the mean number of before treatment was 60.6 and after was 66.5. 10 cases among the 15 patients were improved(66.6%). Conclusions : These results suggest that the acupuncture therapy combined with active movements of involved side was effective treatment modality on microtraumatic injuries of the knee joint.

  • Objectives: The purpose of this study is to investigate the clinical application of Gamisopunghwalhyeal-tang to three patients traumatic knee joint injury. Methods: Patients are hospitalized at Dept. of Oriental Rehabilitation, Bu-Chun Jaseng Oriental Medicine Hospital, diagnosed as traumatic knee joint injury and treated mainly with herbal medicine ; Gamisopunghwalhyeal-tang. This study was measured by VAS(Visual Analogue Scale) score and walking time and Western Ontario and McMaster Universities(WOMAC) index score. Results: After taking Gamisopunghwalhyeal-tang, the patient's pain was controlled and increased time of walking on floor after treatment. VAS & WOMAC score were decreased. Conclusions: As seen in this three cases of traumatic knee joint injury, Gamisopunghwalhyeal-tang has a positive effect to control pain with traumatic knee joint injury.

A Study on the Experiences of Total Knee Replacement in Elderly Women (여성노인의 슬관절 전치환술 경험)

  • Purpose: The aim of this study was to explore the elderly women patients' experience of treatment for total knee replacement. Methods: The participants in this study are 10 elderly women age 65 and over who were discharged after total knee replacement. The data were collected from August 5, to September 20, 2012, and it has been done by in-depth interview. Data were analyzed the phenomenological method of Colaizzi. Results: In this study, four categories were extracted: 'Suffering due to intolerable pain', 'A stable mind and body', 'Retrospection about old wounds', 'A desire for a new life'. Conclusion: This study provides the useful basic data to understand the elderly women patients who have experienced total knee replacement. Nursing intervention is required for pain control after surgery in order to improve the quality of life. In addition, nursing care through individual counseling and emotional support during the recovery period is very important. Therefore, various programs should be developed for elderly women patients to help their body recovery as well as psychological support after total knee replacement surgery.

The Effects of Stabilization Exercises on the Pain of the Thoracic, Shoulder and Knee Joint in High School Volleyball Players (슬링의 안정화운동이 고등학교 남자 배구선수들의 흉추부, 견부, 슬관절부 통증 수준에 미치는 영향)

  • Background: This study aims to examine the effects of sling exercise and common exercise on the pain level of various body parts. Methods: This study divided the subjects into a sling-exercise group and a common exercise group to for each group across the measurement times after practicing exercises twice per week for 8 weeks (total 16times). To compare the means of the experimental group and the control group on each variable, the ANOVA for repeated measure was used and if there was any significant difference across the measurement times, post-hoc comparisons were conducted for the difference. The T-tests for group difference according to exercise type on each of the variables showed the results as follows. Results: The results of this study suggest that there were significant difference in pain in shoulder pain, but no significant difference could be found in the Thoracic pain & Knee joint pain. There was no significant difference to distinguish between the sling exercise and the common exercise. Conclusions: Currently the sling exercise is not spread out widely in Korea, so that the recognition and studies of the sling exercise is still lacking. Hence, this study can hopefully facilitate popularity of the sling exercise and systematic prospect studies on it in the near future.
