Folope et al(1999) reported soft tissue giant cell tumor which was similar to malignant giant cell tumor in clinical, pathologic, and immunohistological aspect but represented low malignancy. We reported a 30-year-old female suffered from pain and palpable mass on the anterolateral aspect of the right knee for one year. Excisional biopsy from the lesion revealed some giant cells and polymorphous cells containing eosinophilic cytoplasm and vacuolated nucleus. Histopathologic findings of the lesion were consistent with soft tissue giant cell tumor of low malignant potential. Hereby, we report a case of soft tissue giant cell tumor of low malignant potential with a review of the literature.
Osteoarthritis is the most common chronic joint disease in the world. With its progression, cartilage thickness tends to diminish, which causes severe pain to human being. One way to examine the stage of osteoarthritis is to measure the cartilage thickness. When it comes to inter-subject study, however, it is not easy task to compare cartilage thickness since every human being has different cartilage structure. In this paper, we propose a method to assess cartilage defect using MRI inter-subject thickness comparison. First, we used manual segmentation method to build accurate atlas images and each segmented image was labeled as articular surface and bone-cartilage interface in order to measure the thickness. Secondly, each point in the bone-cartilage interface was assigned the measured thickness so that the thickness does not change after registration. We used affine transformation and SyGN to get deformation fields which were then applied to thickness images to have cartilage thickness atlas. In this way, it is possible to investigate pixel-by-pixel thickness comparison. Lastly, the atlas images were made according to their osteoarthritis grade which indicates the degree of its progression. The result atlas images were compared using the analysis of variance in order to verify the validity of our method. The result shows that a significant difference is existed among them with p < 0.001.
Chondromyxoid fbroma (CMF) is a rare, benign tumor of the bone that represents fewer than $2\%$ of all benign tumors of bone. CMF is most often found in the long tubular bones, especially the tibia and femur near the knee joint. Less common sites included the pelvis, fibula, calcaneus and rib. A 54-year-old male patient presented to us with history of swelling and mild, intermittent local pain without any rise in overlying skin temperature in lateral portion of left 7th rib for one-month duration, which was diagnosed as benign rib tumor by plain chest X-ray and CT scan, and treated successfully by excision of rib with good result. Pathologic diagnosis of this tumor was CMF. Without any medical therapy, there was no evidence of recurrence after operation. We report this case and follow-up of the patient.

Erdheim Chester disease (ECD) is very rare non-Langerhans cell histiocytosis (LCH) which occurs in the skeletal system and multiple organs. As it is progressive, sometimes it causes fatal results. However, it is often misdiagnosed as LCH or multiple bone metastasis and, thus, is very difficult to diagnose. In Korea, only 10 cases were first reported in 1999. In particular, there have been a few orthopedic approaches or reports in English-speaking literatures, and no report has been issued in Korea. The authors performed bone biopsy in patients with knee and lower extremity pain who were referred for the integrated treatment. We attempts to report this diagnosis experience with literature review.

Objectives : The purpose of this report was to introduce 3 cases of patients diagnosed with both medial collateral ligament tear and meniscal tear that showed clinical improvement after Korean medicine treatment. Methods : These patients received Korean herbal medicine, pharmacopuncture treatment, and acupuncture treatment. We assessed these cases by using the Numeric Rating Scale (NRS), Western Ontario and McMaster Universities'Arthritis Index (WOMAC), and the European Quality of Life-5 Dimension Index (EQ-5D). Results : After Korean medicine treatment, most of the NRS, WOMAC, and EQ-5D scores showed improvement on every discharge date of these 3 cases. The NRS and WOMAC scores of all cases showed a decrease. The EQ-5D of all cases showed an increase. Conclusions : Through this research, we concluded that Korean medicine treatment can be helpful in relieving pain and improving the quality of life and function of knee joint movement in patients with medial collateral ligament tear and meniscal tears.
A 34-year-old man with ACL total rupture due to slip down injury, had received ACL reconstruction using autogenous hamstrings tendon with cross-pin femoral fixation. Postoperative course was as usual. But postoperative 3 months later, he complained posterolateral knee pain, recurrent effusion and mild instability. He was managed repeatitive aspiration and nonsteroid antiinflammatory drugs but was failed to relieve symptoms & signs. In CT scans, perforation of posteromedial femoral cortex of lateral femoral condyle was found. In second look arthroscopy, two pieces of broken femoral cross pin were found in between tibiofemoral Joint which was badly injured cartilage. We considered malposition of pins was the main cause of failure. We propose that femoral tunnel must be made more acute angle and femoral cross-pin guide must be positioned more external rotation 10-20 degree than transepicondylar axis made confirm the cross-pin tunnel position in order to avoid posterior cortex perforation and early failure.

A 42 year old male patient complained of increasing pain and mass around the right ankle with 4 months duration. Simple Roentgenogram and CT showed ill-defined osteolysis and cortical perforation in the neck of the right talus. The pathologic findings showed high grade osteoblastic osteosarcoma. The tumor mass of the ankle increased and pulmonary metastasis was found in spite of administrating of two cycles of preoperative chemotherapy. Below-knee amputation and open wedge pulmonary resection were performed for primary lesion and multiple pulmoanry metastasis, respectively. However, the patient died with multiple pulmonary metastasis in spite of three cycles of postoperative chemotherapy at ten months after the operation.
Objectives The purpose of this study was to find out the effects of ChondroT on arthralgia of the Collagenase-induced osteoarthritis in rats. Methods Osteoarthritis was induced into rat by injecting Collagenase in its knee joint. Rats are divided into a total of 8 groups (n=6). Normal group was not induced for osteoarthritis whereas control groups were induced for osteoarthritis by Collagenase. Positive-A (Indomethacin) was injected with Collagenase and after 8 days, 2 mg/kg of Indomethacin was medicated. Positive-B (JOINS TAB) was injected with Collagenase and after 8 days, 20 mg/kg of JOINS TAB was medicated. Experimental groups (Chondro T) at three dose levels (50, 100 and 200 mg/kg) were injected with Collagenase and after 8days they were medicated with 10 ml/kg. Indomethacin, JOINS TAB and ChondroT were medicated each substances once a day for 10 days. Thereafter, the changes in plantar withdrawal response of osteoarthritis rats by dynamic plantar aesthesiometer were observed and then RT-PCR analysis was done to investigate the expression of related proteins. Results 1. ChondroT significantly decreased withdrawal response of mechanical allodynia compared with control group in all of the experimental groups (ChondroT-A, ChondroT-B, ChondroT-C). 2. ChondroT significantly reduced Bax/Bcl-2 ratio in all of the experimental groups (ChondroT-A, ChondroT-B, ChondroT-C). 3. ChondroT significantly reduced the expression of INF-${\gamma}$ compared with control group in group ChondroT-B, ChondroT-C. Conclusions This results suggest that ChondroT may be meaningful for suppressing the pain of osteoarthritis. Further study is needed to conduct a rigorous clinical research.
In determining the cause of dysmenorrhea, it is necessary to investigate postural alignment and balance. The purpose of this study is to compare and analyze whether there is a difference in posture alignment and foot pressure balance according to the dysmenorrhea degree. The subjects were twenty female students in their 20s, who had pain caused by dysmenorrhea. According to the degree of dysmenorrhea, the subjects were divided into mild and severe groups. In the results, there was a significant difference only in shoulder height asymmetry angle between the mild and severe groups (p<0.05), and there was no significant difference in pelvic and knee joint alignment. In the correlation analysis, there was a significant positive correlation between the dysmenorrhea score (MDQ) and shoulder height asymmetry angle was found. These results indicate that dysmenorrhea symptom and asymmetric alignment of shoulder are related. To analyze these factors, further research will need to investigate the correlation between dysmenorrhea and spinal alignment.
Purpose: The aim of this study is to compare the immediate effects of weight-assisting methods on vastus medialis oblique (VMO) and vastus lateralis (VL) muscle activation, on the VMO/VL muscle activation ratio, and on muscle onset time in healthy subjects when ascending stairs. Methods: Healthy participants were randomly assigned to the belt group (n = 11), hand group (n = 11), and control group (n = 11). In the belt group, a belt was wrapped around the sacrum and pulled forward with both hands, moving the center of weight forward, while ascending stairs. The hand group grasped the hips with both hands and climbed stairs, assisting their weight from the rear and moving the center of weight backward, and the control group climbed the stairs without any intervention. Results: Muscle activation of the VMO decreased significantly after the intervention in the belt and hand groups, and activation of the VL muscle in both groups showed a greater decrease than that of the VMO muscle. Further, the VMO/VL muscle activation ratio increased significantly, with an improvement shown in the order of the belt group, hand group, and control group, while muscle onset time also improved in the order of the belt group, hand group, and control group. Conclusion: The belt group demonstrated the greatest effect across all dependent variables, confirming that in clinical practice, these two weight-assisting methods are more effective interventions during stair ascent for patients with knee joint instability, pain, and imbalance than no assistance.
