Trends in Domestic and Foreign Clinical Research on Ultrasound-Guided Acupuncture

  • Background: The purpose of this study was to analyze domestic and foreign clinical research into ultrasound-guided Oriental medicine acupuncture. Methods: Ultrasound-guided Oriental medicine acupuncture studies were retrieved from PubMed, CNKI, KISS, NDSL, and OASIS. Results: Of 6,260 articles, 17 articles were selected. There was 1 article in 2004, 1 in 2008, 2 in 2011, 1 in 2012, 4 in 2013, 3 in 2016, 2 in 2017, and 3 in 2018. Of the 17 selected articles 6 articles were studies of omalgia, 4 of knee pain and 2 of peroneal nerve palsy. In addition, there was 1 article of occipital headache, 1 of neck pain, 1 of tarsal tunnel syndrome, 1 of angioma and 1 of hiccup. In these 17 studies the ultrasound frequency range where mentioned, was 5-14 MHz. Conclusion: This study provided basic data as reference for the design of more diversified and systematic clinical research in the domestic Oriental medicine community in the future.

The Effect of Opae-san for Peptic Ulcer Disease: A Systematic Review and Meta-Analysis (소화성 궤양에 대한 오패산의 치료효과 : 체계적 문헌고찰과 메타분석)

  • Objectives: The aim of this study was to perform a meta analysis of randomized controlled trials (RCTs) that applied Opae-san to peptic ulcer. Methods: The databases NDSL, RISS, KISS, KISTI, Oasis, DBpia, Cochrane, EMBASE, Pubmed, and CNKI were searched to identify RCTs that evaluated the therapeutic response to Opae-san on peptic ulcer. The selected studies were assessed using Cochrane Group's risk of bias tool. Results: 12 RCTs were selected from a total of 312 identified. Combined therapies of Opae-san plus triple therapy were superior to only triple therapy in achieving the effective rate (risk ratio=1.26, 95% CI: 1.17 to 1.35, p<0.001, I2=0%), the helicobacter pylori eradication rate (risk ratio=1.23, 95% CI: 1.12 to 1.34, p<0.001, I2=7%) and the recurrence rate (risk ratio=0.31, 95% CI: 0.12 to 0.82, p=0.02, I2=0%). But only Opae-san was not superior in achieving the effective rate compared to anti gastric secretion drugs. Conclusions: The current evidence suggests that combined therapies of Opae-san plus triple therapy yield a higher effective rate, and helicobacter pylori eradication rate and a lower recurrence rate. However, in most of these studies, it is difficult to evaluate the bias and therefore better designed studies are needed.

A study on research trends for gestational diabetes mellitus and breastfeeding: Focusing on text network analysis and topic modeling (임신성 당뇨와 모유수유에 대한 연구 동향 분석: 텍스트네트워크 분석과 토픽모델링 중심)

  • Purpose: The aim of this study was to identify core keywords and topic groups in the 'Gestational diabetes mellitus (GDM) and Breastfeeding' field of research for better understanding research trends in the past 20 years. Methods: This was a text-mining and topic modeling study composed of four steps: 1) collecting abstracts, 2) extracting and cleaning semantic morphemes, 3) building a co-occurrence matrix, and 4) analyzing network features and clustering topic groups. Results: A total of 635 papers published between 2001 and 2020 were found in databases (Web of Science, CINAHL, RISS, DBPIA, RISS, KISS). Among them, 3,639 words extracted from 366 articles selected according to the conditions were analyzed by text network analysis and topic modeling. The most important keywords were 'exposure', 'fetus', 'hypoglycemia', 'prevention' and 'program'. Six topic groups were identified through topic modeling. The main topics of the study were 'cardiovascular disease' and 'obesity'. Through the topic modeling analysis, six themes were derived: 'cardiovascular disease', 'obesity', 'complication prevention strategy', 'support of breastfeeding', 'educational program' and 'management of GDM'. Conclusion: This study showed that over the past 20 years many studies have been conducted on complications such as cardiovascular diseases and obesity related to gestational diabetes and breastfeeding. In order to prevent complications of gestational diabetes and promote breastfeeding, various nursing interventions, including gestational diabetes management and educational programs for GDM pregnancies, should be developed in nursing fields.

The Effectiveness of Moxibustion Treatment in Infertility with IVF-ET: A Systematic Review and Meta-Analysis (보조생식술을 시행한 난임환자에서의 뜸 치료에 대한 체계적 문헌고찰 및 메타분석)

  • Objectives: The purpose of this study is to investigate the effectiveness of moxibustion in infertility with In Vitro Fertilization and Embryo Transfer (IVF-ET). Methods: We searched 8 databases (Embase, PubMed, CiNii, CNKI, OASIS, ScienceOn, KMBASE, KISS)to identify eligible studies published before 2021 Oct. We included randomized controlled clinical trials (RCTs) using moxibustion in infertility with IVF-ET. The methodological quality of each RCT was assessed by the Cochrane risk of bias tool. Results: Two RCT studies were eligible in our review. The overall risk of bias was evaluated as unclear. The meta-analysis of 2 trials indicated that favorable results for the use of moxibustion with IVF-ET. Conclusions: This systematic review and meta-analysis of clinical trials suggests that moxibustion with IVF-ET can effect on Infertility patients. However, because of studies included analysis was biased due to unclear risk of bias and unreliable study design, future RCT studies and additional Meta-Analysis are needed to judge the supplementary treatment role of moxibustion in infertility with IVF-ET.

A Systematic Review of Acupuncture Treatment Effect for Patellofemoral Pain Syndrome (슬개대퇴통증증후군의 침 치료 효과에 대한 체계적 문헌고찰)

  • Objectives The purpose of this study is to systematically review the degree of pain and function improvement in acupuncture treatment for patellofemoral pain syndrome (PFPS). Methods Randomized controlled trials (RCTs) were searched from Medline/PubMed, Cochrane Library, Embase, WanFang Data, China National Knowledge Infrastructure (CNKI), Citation Information by NII (CiNII), Koreanstudies Information Service System (KISS), ScienceON, KoreaMed, and Korean Medical Database (KMbase). The methodological quality of selected RCTs' were evaluated according to Cochrane RoB2 tool. Results Total of 158 studies were identified and 7 RCTs were selected for systematic review. Frequently used evaluation method were visual analogue scale and Kujala scores. All studies showed significant effect on pain reduction and knee function between before and after treatment, but showed partial significance between the intervention and control group. Conclusions Research showed that acupuncture treatment is an effective treatment for improving the pain and function of PFPS, but it is difficult to conclude that it has a significant effect compared to other treatments. Selected studies included subjective evaluation indicators and the total of selected studies were small, so the interpretation of the results is limited. Also most of the included studies are evaluated to have some concerns of bias. Therefore, subsequent research should continously be done in more objective and systematic method.

Evidence Based Approach of Wheel Balance Cancer Therapy: A Review (수레바퀴 암 치료법에 대한 근거중심적 연구)

  • Background : Integrative cancer treatment is a holistic approach embracing body, mind, and spirit incorporating conventional treatments of surgery, chemotherapy, radiation and personalized complementary treatments. Wheel Balance Therapy (WBT) of East-West Cancer Center(EWCC), Dunsan Oriental Hospital of Daejeon University was developed to balance out all factors involved in cancer care based on the traditional theories of oriental medicine. Objective : This work aims to analytically review literatures on WBT and its related components. Methods : Literatures published from January 1st, 1990 to April 30th, 2011 were reviewed focusing on 4 main components of WBT; herbal medicine, immune activation, anti-cancer diet, and breathing/meditation. Data were retrieved from medical search engines and electronic data bases including Pubmed, Research Information sharing Service (RISS), Korean-studies Information Service System (KISS), China National Knowledge Infrastructure (CNKI), and Korea's National Digital Library (KNDL). Results : In this review, EWCC's most commonly prescribed formulas are explored. The composition of the formulas, their use in clinical settings as well as the background studies and other therapeutic efficacies are explained. Information on incorporating anti-cancer dietary support and breathing and meditation techniques, other therapies practiced as part of the center's integrative cancer care are also covered. Conclusion : WBT based on holistic theories of oriental medicine embracing body, mind, and spirit is expected to further contribute in promotion of cancer patients' quality of life and prolonged survival time.

Systematic Review on Complementary and Alternative Medicine for Female Sexual Dysfunction (여성 성기능장애의 보완대체의학적 치료에 관한 논문 연구)

  • Objectives: The aim of this study was to review systemically clinical trials on the trends of studies for Complementary and Alternative Medicine in the treatment of female sexual dysfunction and provide basic resource for future treatment and suggestions for improving research methods. Methods: Through medical website 'Pubmed', foreign clinical literatures about female sexual dysfunction were searched and domestic clinical literatures about female sexual dysfunction were searched using internet websites 'National assembly library', 'KISS', 'RISS', 'Korean traditional knowledge portal'. Results: Total 19 literatures were selected from January 2000 to June 2012. 15 foreign literatures were selected through medical website and 4 domestic literatures were chosen using internet websites or hand-searching. 5 literatures were published in 2008 and 13 literatures mentioned about age group of patients. FSFI is most common tool for female sexual dysfunction. 9 of 19 literatures were using Herbal medicine for treatment. 5 literatures were about CBT, 6 literatures were about Biofeedback and other one literatures are about Perineal massage. There were 14 Randomized controlled trials and 5 Non-randomized controlled trials. And randomized controlled trials were relatively low in risk of bias than non-randomized controlled trials. Conclusions: Upon these results, in similar future more clinical trials should be done and accumulate Evidence-based literatures.

Effect of Exercise Programs for Chronic Low Back Pain Patients : A Systematic Review (만성 허리통증환자의 운동프로그램에 대한 효과)

  • Background: The purpose of this study was to systematic review the effect of exercise programs for chronic low back pain patients. We needs systematic development of low back pain exercise program to reduce economic cost further doing great service to public health promotion. Methods: We searched to the effects of exercise programs for chronic low back pain patients by Dankook University electronic library databases of DBPIA, KSI KISS, CINAHL, MEDLINE and PEDro combined with a hand search of papers published in relevant journals. Any type of study relevant to the topic published during time period from 1970 to 2007 was included. Results: The literature search identified 30 studies. 1.Performing the flexion exercise increased abdominal muscle activity but acute herniated intervertebral disc should be avoided. 2.The general lumbar extension exercise used lumbar extension machine and the muscle power increases, significant probability the change. 3.The spinal segments exercise for the patients offered significant efficacy and appeared to be a reasonable therapeutic option. 4.Spinal stabilization exercises appear to improve trunk endurance and balance to patients with chronic low back pain. This exercise programs had effective decrease pain and disability. Conclusion: The review suggests that although the exercise programs for chronic low back pain patients, and we expected the efficacy of the exercise programs for chronic low back pain patients used in this study should be further investigated in a long period study and objective outcomes.

A Review of Clinical Researches on Socheongryong-Tang (소청룡탕의 임상연구 분석)

  • Objective : This study's purpose was to review the clinical studies of Socheongryong-Tang. Method : We searched papers about Socheongryong-Tang using KISS, KTKP, PUBMED, Embase, Science Direct, and the key words "Socheongryong-Tang", "Shoseiryuto", "Xiao Qing Long Tang", and "Minor Green-Blue Dragon Decoction" were used. Papers not matched with inclusion criteria were excluded. Until today, there have been 143 studies on the effects of asthma, allergic rhinitis, common cold, eczema and other various effects of Socheongryong-Tang. Of these, 15 were classified as clinical research papers. Results : There were 4 cases of asthma, 2 cases of allergic rhinitis, 2 cases of common cold, 1 case of pneumonia in patients with dementia, 1 case of eczema, 1 case of drug interaction study, 1 case of ephedrine drug removal, 3 cases of side effects causing pneumonia. Conclusion : It can be seen that Socheongryong-Tang has established the basis for application for the purpose of asthma, allergic rhinitis, wind-cold type cold, preventing pneumonia in patients with dementia, dyshidrotic eczema. On the other hand, considering three cases that cause side effects of pneumonia, it is necessary to confirm whether or not the symptoms of pneumonia are certainly progressing when observing the progress of the patient.

Storage of laboratory animal blood samples causes hemorheological alterations : Inter-species differences and the effects of duration and temperature

  • Hemorheological results may be influenced by the time between blood sampling and measurement, and storage conditions (e.g., temperature, time) during sample delivery between laboratories may further affect the resulting data. This study examined possible hemorheological alterations subsequent to storage of rat and dog blood at room temperature ($22^{\circ}C$) or with cooling ($4{\sim}10^{\circ}C$) for 2, 4, 6, 24, 48 and 72 hours. Measured hemorheological parameters included hematological indices, RBC aggregation and RBC deformability. Our results indicate that marked changes of RBC deformability and of RBC aggregation in whole blood can occur during storage, especially for samples stored at room temperature. The patterns of deformability and aggregation changes at room temperature are complex and species specific, whereas those for storage at the lower temperature range are much less complicated. For room temperature storage, it thus seems logical to suggest measuring rat and dog cell deformability within 6 hours; aggregation should be measured immediately for rat blood or within 6 hours for dog blood. Storage at lower temperatures allows measuring EI up to 72 hours after sampling, while aggregation must be measured immediately, or if willing to accept a constant decrease, over 24~72 hours.
