A Case Report of a Patient with Ptosis and Ataxia Diagnosed as Claude's Syndrome Who Was Treated with Korean Medicine (Claude's syndrome으로 인한 안검하수 및 운동실조에 대한 한방 복합 치료 치험 1례)

  • Objective: We describe the case of an 84-year-old man with a midbrain infarction causing pupil sparing oculomotor nerve palsy, with ipsilateral cerebellar ataxia diagnosed as Claude's syndrome. Methods: The patient was treated with Korean medicine, including herbal medicine, acupuncture, and electro-acupuncture, during 45 days of hospitalization. Improvements in the patient's symptoms and changes in ptosis were evaluated using the Korean version of the Scale for the Assessment and Rating of Ataxia (SARA). Results: After 45 days of treatment with the Korean medicine, the patient's symptoms, including ataxia and ptosis, improved. Conclusions: This study suggests that the treatment with Korean medicine was effective in aiding the patient's recovery.

  • The aim of this study is to find out the impact of collaborative instruction between the teacher librarian and the classroom teacher on learning interest and academic achievement by running Library and Information Life, which is the textbook of information literacy for teacher librarians and the Korean language curriculum for the first grade of middle school through library-based instruction. The result of this study has a positive influence on academic achievement by facilitating learning interest on Korean language classes under coplanning, co-operating and coevaluating in the school library. Therefore, it is important to establish the institutional system for collaborative instruction in order to increase information literacy instruction in the school community. A local school should expand the physical environment and educational equipment of its school library and form a flexible schedule to have enough time for teaching information literacy skills and for using of materials.

Effects on Aerodynamic Drag Reduction of a Passenger Car by Rear Body Shape Modifications (승용차의 후면 형상 변형이 공기저항 감소에 미치는 영향)

  • This paper suggests possible rear body shape modifications of a passenger car for the improvement of aerodynamic performance, based on the CFD analysis results. YF SONATA, a passenger car of Hyundai Motors Company, plays a major role as the baseline car in this research. Representatively, three parts(trunk rear edge, side rear edge and rear undercover) are modified in a small range in order for the total outer shapes not to be changed enough so that the modified car is not considered different, compared with the baseline. Specifically, using computational fluid dynamics, aerodynamic drag reduction is accomplished maximally about 11% in this research. Finally, it is proved that although the range of changes of the rear body shapes of a passenger car is very strictly confined, by changing a small range of rear body shapes alone the enhancement of aerodynamic performance of a passenger car can be significantly accomplished.

  • Introduction: This case report addressed the effect of Korean medicine treatments on a patient with hemorrhagic anemia due to lower gastrointestinal bleeding. Case report: An 83-year-old male with lower gastrointestinal bleeding was treated with Korean medicine, including the herbal medication Subi-jeon. We evaluated the improvements of symptoms by hemoglobin levels and numbers of transfusions. From the 6th day of taking the herbal medicine to discharge, a hemoglobin level of 7 g/dL or higher was maintained and the patient did not require blood transfusion. Conclusions: This clinical case study suggests that Subi-jeon might be effective in cases of hematosis of lower gastrointestinal bleeding.

  • This review highlights the ontogenesis and the differentiation of motor neuron in spinal cord, and introduce the survival motor neuron(SMN) which is associated with growth and survival of motor neurons. The differentiation of floor plate cells and motor neurons in the vertebrate neural tube appears to be induced by signals from the notochord. This signal is Sonic hedgehog(Shh). The early development of motor neurons involves the inductive action of Shh. The SMN gene is essential for embryonic viability. SMN mRNA is also expressed in virtually all cell types in spinal cord, including large motor neurons. The SMN protein is involved in RNA processing and during early embryonic development is necessary fer cell survival. Two SMN genes are present in 5q 13 in humans: the telomeric gene(SMNt), which is the SMA-determining gene, and the centromeric analog gene(SMNc). The majority of transcripts from the SMNt gene are full length but, major transcripts of the SMNc gene have a high degrees of alternative splicing and tend to have little or no exon 7. The SMN is involved in the RNA processing(the biogenesis of snRNPs and pre-mRNA splicing), the anti-apoptotic effects, and regulating gene expression.

Does Type I Truly Dominate Hepatic Glycogen Storage Diseases in Korea?: A Single Center Study

  • Purpose: There are no studies of hepatic glycogen storage diseases (GSDs) other than type I and III in Korea. We aimed on investigating the characteristics of hepatic GSDs in Korea diagnosed and followed at a single center. Methods: We retrospectively analyzed patients who were diagnosed as GSD and followed at Samsung Medical Center from January, 1997 to December, 2013. Clinical manifestations, laboratory results, treatment, and prognosis were investigated. Results: Twenty-one patients were included in the study. The types of 17 patients were confirmed by enzyme activity tests and/or gene analysis. GSD Ia was diagnosed in 7 patients (33.3%), Ib in 1 patient (4.8%), III in 2 patients (9.5%), IV in 1 patient (4.8%), and IX in 6 patients (28.6%). Types other than GSD I constituted 52.9% (9/17) of the patients diagnosed with a specific type of hepatic GSD. The median age at presentation was 2 years. Hepatomegaly was observed in 95.2%, elevated liver transaminases in 90.5%, and hyperlactacidemia in 81.0% of the patients. The duration for follow-up was $77{\pm}62.0$ months. Uncooked corn starch was initiated in all the patients. No mortality was observed during the follow-up period, and liver transplantation was performed in 14.3%. Conclusion: Types other than GSD I comprised more than half of the patients diagnosed with a specific type of hepatic GSD. Clinical suspicion and thorough evaluation of hepatic GSDs in Korea should be focused not only on GSD I, but also on other types.

  • According to the research, the Estern and Western medical literatural records about the causes and symptoms of the heart failure, the results as follows are concluded. 1. The asthma, suceptibility to fright and severe palpitation are generally revealed from the left heart failure. The causes of it are belong to the Yang and Yeum deficiency which are mainly caused by primordial energy deficiency. 2. The symtoms of asthma which are derived from left heart failure are related to the gasping that contain dyspnea, asthma due to the accumulation of phlegm and severe palpitation and related to shortness of breath due to fluid retention (水喘) that make asthma. 3. In the right heart failure, the edema which is derived from the congestion of vein, is revealed as stoppage of main channel that is mainly caused by the Yang or the Yeum deficiency. 4. The edema which is caused by the right heart failure, is mainly related to the symptoms of Yeum type edema, also it is related to the five viscera-fluid, moreover to the heart-fluid (心水), lung-fluid (肺水) and liver-fluid (肝水). 5. In heart failure, the pathologic symptoms which are derived from the stoppage of blood circulation, are phlegm-retention disease (痰飮), diffuse fluid-retention syndrom (溢飮), fluid-retention syndrome characterized by dyspnea and edema (支飮).

A Study on the Structural Relationships between Flow Experience and Satisfaction about School Library Use (학교도서관 이용에 대한 플로우 경험과 만족도의 인과관계에 관한 연구)

  • The domestic and foreign standards of school libraries suggest that school library should be run to give useful and pleasant experiences to students. This study applies Csikszentmihalyi's 'flow theory' to analyze the relationships between students' flow experience and their satisfaction about school libraries use. The variables selected in this study are students' satisfaction, flow experience, library skill, challenge of library use, types of library use and students' learning styles. The research model is designed by using these 6 variables in this study. The data are collected from 293 students and analyzed by structural equation modeling. The results of this study are as follows: The entire casual relationships show that library skills influence in types of library use, types of library use affect students' learning styles and students' learning styles influence in satisfaction through flow. To improve students' satisfaction about school library use, this study proposes effective ways related to teacher librarians' role performances to expand students' flow experience and increase their library skills.

Differential Proteomic Analysis of Secreted Proteins from Cutinase-producing Bacillus sp. SB-007

  • Bacillus sp. SB-007 was isolated from pea leaves harvested from the southwestern parts of South Korea through screening on a minimal medium containing 0.2% purified cutin for its ability to induce the cutinase production. However, no cutinase was produced when it was grown in a minimal medium containing 0.2% glucose. A proteomic approach was applied to separate and characterize these differentially secreted proteins. The expression level of 83 extracellular proteins of the cutinase-producing Bacillus sp. strain SB-007 incubated in a cutinase-induced medium increased significantly as compared with that cultured in a non cutinase-induced medium containing glucose. The extracellular proteome of Bacillus sp. SB-007 includes proteins from different functional classes, such as enzymes for the degradation of various macromolecules, proteins involved in energy metabolism, sporulation, transport/binding proteins and lipoproteins, stress inducible proteins, several cellular molecule biosynthetic pathways and catabolism, and some proteins with an as yet unknown function. In addition, the two protein spots showed little similarities with the known lipolytic enzymes in the database. These secreted proteome analysis results are expected to be useful in improving the Bacillus strains for the production of industrial cutinases.

  • Purpose: To compare the clinical results of arthroscopic transglenoid suture with thermal capsulorrhaphy and thermal capsulorrhaphy on multidirectional shoulder instability. Materials and Methods: From January 1993 to September 2001, 23 patients who received the artrhoscopic treatment were the subjects and follow up period were at least 2 years. Each were subdivided as Group A(7 cases), which took transglenoid suture with thermal capulorrhaphy and Group B(16 cases), who took only thermal capsulorrhaphy. Clinical results were evaluated by Rowe score before and after surgery. Results: After operation according to Rowe score 4(57.1%) were excellent, 1(14.3%) were good and 2(28.6%) were fair in the Group A. for Group B 6(37.5%) were excellent, 2(12.5%) were good, 5(31.3%) were fair and 3(18.8%) were poor. Shoulder instability was recurred in 1(14.2%) case of Group A and 8(50%) cases of Group B during follow up period. Conclusion: Thermal capsulorrhaphy is thought to be a good adjuvent method, if it done with anterior capsular shift by transglenoid suture in multidirectional shoulder instability.
