Screening Tool for Anxiety Disorders: Development and Validation of the Korean Anxiety Screening Assessment

  • Objective This study evaluated the psychometric properties of the Korean Anxiety Screening Assessment (K-ANX) developed for screening anxiety disorders. Methods Data from 613 participants were analyzed. The K-ANX was evaluated for reliability using Cronbach's alpha, item-total correlation, and test information curve, and for validity using focus group interviews, factor analysis, correlational analysis, and item characteristics based on item response theory (IRT). The diagnostic sensitivity and specificity of the K-ANX were compared with those of the Beck Anxiety Inventory (BAI) and Generalized Anxiety Disorder 7-item scale (GAD-7). Results The K-ANX showed excellent internal consistency (${\alpha}=0.97$) and item-total coefficients (0.92-0.97), and a one-factor structure was suggested. All items were highly correlated with the total scores of the BAI, GAD-7, and Penn State Worry Questionnaire. IRT analysis indicated the K-ANX was most informative as a screening tool for anxiety disorders at the range between 0.8 and 1.6 (i.e., top 21.2 to 5.5 percentiles). Higher sensitivity (0.795) and specificity (0.937) for identifying anxiety disorders were observed in the K-ANX compared to the BAI and GAD-7. Conclusion The K-ANX is a reliable and valid measure to screen anxiety disorders in a Korean sample, with greater sensitivity and specificity than current measures of anxiety symptoms.

The Sleep Characteristics of Employees and Its Influence on Depression and Anxiety (직장인들의 수면 양상이 우울 및 불안에 미치는 영향)

  • Objective : This study aimed to quantify the degrees of insomnia, depression and anxiety felt by employees, and to understand which components of insomnia affect depression and anxiety. Methods : One thousand employees who had medical checkups from January to December 2014 in the Workplace Mental Health Institute of Kangbuk Samsung Medical Center were selected randomly. Sleep quality, depression, and anxiety were rated using the Pittsburgh Sleep Quality Index (PSQI), the Center for Epidemiologic Studies Depression scale (CES-D), and the Beck Anxiety Inventory (BAI), respectively. Good and poor sleepers were divided into two groups according to their total PSQI score : ${\leq}5$ for good sleepers or >5 for poor sleepers. Results : Of 998 employees, 742 (74.35%) had PSQI total scores below 5, and 256 (25.65%) had scores over 5. Multivariate linear regression analysis using the 7 components of PSQI versus BAI, and CES-D scores revealed that subjective sleep quality, daytime dysfunction, sleep disturbance, sleep latency and sleep duration components were significantly associated with both BAI and CES-D. Conclusion : Among the components of insomnia, subjective sleep quality and daytime dysfunction were most strongly associated with depression and anxiety.

Analysis of Factors Associated with the Therapeutic Duration of Transforaminal Epidural Steroid Injection in Lumbar Spinal Stenosis Patients (요척주관 협착증 환자에서 경추간공 경막외 스테로이드 주입 후 통증 완화 기간과 관련된 요인의 분석)

  • Background: A transforaminal epidural steroid injection (TFESI) is one of the methods for the conservative treatment of the lumbar spinal stenosis. As efforts to prolong the therapeutic duration and to predict the outcome of TFESI are very important, we analyzed factors considered to be associated with the therapeutic duration of a TFESI. Methods: Between August 2006 and March 2007, 69 patients (Group A: patients with no pain relief, Group B: patients with pain relief of less than 6 months, Group C: patients with pain relief of more than 6 months) who failed to the medical treatment were included to undertake a fluoroscopic-guided TFESI. Prior to treatment, the VAS (visual analogue scale), ODI (Oswestry disability index), BDI (Beck depression inventory), and BAI (Beck anxiety inventory) scores were determined to evaluate the degree of pain, disability, and psychological status. The VAS and ODI scores were used to assess the degree of pain relief. To identify the total duration of pain relief, regular outpatient visits for six months were conducted, and for the patients who were not able to visit the outpatient clinic regularly, outcome was assessed by telephone interviews after six months. Results: The dural sac cross-sectional area (DSCSA), ODI, pain duration, BDI, BAI, and age showed similar distribution for patients in the A, B, and C groups. Conclusions: The DSCSA, ODI, pain duration, BDI, BAI, and age were not associated with the therapeutic duration of TFESI in lumbar spinal stenosis patients.

A Retrospective Study on the Effect of Complex Korean Medicine Treatment on Social Anxiety Disorder (사회 불안 장애에 대한 한의복합치료의 효과: 후향적 관찰 연구)

  • Objectives: This study was conducted to observe the progression of symptoms according to the treatment period of patients with social anxiety disorder who received complex Korean medicine treatment. Methods: The medical records of 25 patients who were diagnosed with social anxiety disorder according to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, and received complex Korean medicine treatment (herbal medicine, acupuncture, and Korean psychotherapy) for 12 weeks were analyzed. The State-Trait Anxiety Inventory (STAI), the Beck Depression Inventory-II (BDI-II), the Beck Anxiety Inventory (BAI), the Korean-Social Avoidance and Distress scale (K-SAD), and the Korean-Fear of Negative Evaluation (K-FNE) were measured at the initial hospital visit and during the 4, 8, and 12 weeks of treatment to evaluate the effectiveness of treatment. Missing values were replaced with the average evaluation index value at that time. Results: 1) Statistically significant changes in STAI-X1, STAI-X2, BDI-II, BAI, K-SAD, and K-FNE scores were seen according to the time of treatment. 2) When scores were compared between each treatment time point, STAI-X2 and BDI-II showed statistically significant changes between the initial visit and four weeks of treatment, the initial visit and eight weeks of treatment, and the initial visit and 12 weeks of treatment. STAI-X1 and K-FNE showed statistically significant changes between the initial visit and eight weeks of treatment and the initial visit and 12 weeks of treatment. There was a statistically significant change in BAI scores between the initial visit and the 12th week of treatment. Conclusions: Complex Korean medicine treatment alleviated anxiety, depression, fear, and avoidance of social situation symptoms in patients with social anxiety disorder. The longer the treatment period, the more symptoms tended to be alleviated. However, the evidence should be supplemented with prospective, controlled research.

Genetic analysis of protoplast fusants of candida pseudotropicalis (Candida pseudotropicalis 융합세포의 유전적 분석)

  • The genetic analysis and characterization of protoplast fusion hybrids between complementary auxotrophic mutants of Candida pseudotropicalis were carried out. Nuclear fusion appeared to occur in fusion hybrids (e.g., F15 and F33), as strongly suggested by isolation of recombinants after mitotic segregation of parental genetic markers. This was confirmed by KNA content, nuclear staining and comparison of survival rate to UV light. After keeping fusion hybrids for approximately one year, the frequency of spontaneous mitotic segregation was $3.0\times 10^{-4}$ - $8.1\times 10^{-4}$ while that of induced mitotic segregation was $1.4\times 10^{-3}$- $1.7\times 10^{-3}$. These results suggested that they maintained stable karyogamy state. It was also found that the production of $\beta$-D-galactosidase from F15, F33 and F158 was somewhat increased when compared with that from either auxotrophic parents or wild type.

A Case Report of Panic disorder treated by Yonggyechulgam-tang based on Shanghanlun provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 령계출감탕을 투여한 대양병(大陽病) 공황장애 증례 1례)

  • Objective : This case report show the effect of Yonggyechulgam-tang on Panic disorder. Methods : The patient was diagnosed Panic disorder based on DSM-5. According to Shanghanlun six meridian patterns diagnostic system, the patient treated with Yonggyechulgam-tang for 4 months. The level of panic disorder was evaluated by VAS and BAI. Results : After the treatment, VAS and BAI decreased $10{\rightarrow}2$, $42{\rightarrow}4$ each. Conclusions : Yonggyechulgam-tang improved the symptoms of Panic disorder on this study. Although Yonggyechulgam-tang is not an usual treatment for Panic disorder, we could prescribe the medicine due to Shanghanlun six meridian patterns diagnostic system.

Numerical Methods for Wave Response in Harbor

  • A natural and an artificial harbor can exhibit frequency (or period) dependent water surface oscillations when excited by incident waves. Such oscillations in harbors can cause significant damages to moored ships and adjacent structures. This can also induce undesirable current in harbor. Many previous investigators have studied various aspects of harbor resonance problem. In the present paper, both a localized finite element method(LFEM) which is based on the functional constructed by Chen & Mei(1974) and Bai & Yeung(1974) and an integral equation method which was used by Lee(1969) are applied to harbor resonance problem. The LFEM shows computationally more efficient than the integral equation method. Our test results show a good agreement compared with other results. In the present computations, specifically two harbor geometris are treated here. The present method by LFEM can be extended to a fully three dimensional harbor problem.

One Case Report of Panic Disorder Patient Improved by Korean Medicine Treatment Alone (공황장애 한의 단독 치료 치험 1례)

  • Objective : The purpose of this study is to report that patients with panic disorder were able to discontinue the psychotropic medication through Korean Medicine treatment alone. Methods : The patient diagnosed with panic disorder were hospitalized on the 9th and were treated steadily through outpatient treatment afterwards with herbal medicine, acupuncture, cupping therapy, and Whidam's Sugi therapy while taking the chemical. Treatment effects were evaluated through BAI, BDI, STAI-X-1, CSEI-s, GIS, NDI-K, and EQ-5D measurements. Results & Conclusions : The patient's anxiety was relieved and the psychotropic medication was discontinued, but it did not recur. And after inpatient treatment, he has been steadily receiving outpatient treatment on a regular basis, maintaining an improved state and leading a normal daily life.

  • Objectives The Purpose of this study was to conduct a characteristic study on cancer patients using the Core Seven Emotions Inventory (CSEI) followed by a correlation analysis with STAI, STAXI and BDI.Methods This study was conducted by analyzing the medical records of 21 patients who had visited ○○ University Oriental hospital and completed the Core Seven Emotions Inventory (CSEI), BAI, and BDI. A total of 21 patients diagnosed with Cancer were analyzed using SPSS (Statistical Package for the Social Science, IBM, United States of America, Version 22.0). Frequency Analysis, independent t-test, one-sample t-test, and correlation analysis were conducted.Results 1. The Seven Emotion Characteristics of the Cancer patients showed a fairly stable emotional Distribution. The Gong (恐) and Kyeong (驚) emotions were relatively higher than U (憂), Bi (悲), Sa (思), Hui (喜), No (怒). 2. According to Gender, Female patients exhibited higher Sa (思) and Kyeong (驚) emotions while male patients exhibited higher Bi (悲) Emotion. 3. No (怒), U (憂), Bi (悲), Sa (思), Gong (恐), and Kyeong (驚) emotions showed a high correlation with the BDI and BAI scores of cancer patients.

A Case Study of Chronic Anal Fissure Treated With Chijasi-tang and Counseling (8년 동안 진행된 만성치열(Chronic anal fissure) 환자에게 치자시탕과 상담요법을 사용하여 호전된 치험 1례)

  • Objectives : This study is aimed to report the outcomes of the treatment of a patient with chronic anal fissure during a 8-year period. Methods : Basic patient’s data was analyzed as a CARE guideline. And related psychological state was measured with BAI, STAI-X-II test. Results : The treatment is based on Shanghanlun and the patient reported better outcomes. The main symptoms of the patient were chronic constipation, blood in the stool. The main clinical findings were that the patient’s symptoms got worse during the examination period because of the anxiety of the exam. And loss of appetite and digestive disorders are due to Jue-Yin-Bing(厥陰病) and the clue of bleeding can be found in article 375 in Shanghanlun. The main diagnoses was chronic anal fissure and main interventions were Chijasi-tang according to article 375 in Shanghanlun and counseling on anxiety. As a results bleeding has stopped after taking herbal medicine and constipation has been treated at the same time. After 6 months, the patient performed BAI, STAI-X-II test and scores got lowered indicating decreasing anxiety level. Conclusion : Chijasi-tang with counseling achieved valid positive results to the patient with chronic anal fissure.
