Meta-Analysis of Correlation Between Subjective and Objective Cognitive-Linguistic Tests : Focused on Normal Aging, MCI, and Dementia (메타분석을 통한 주·객관적 인지-언어 평가 간 상관성 연구 : 정상 노년층, MCI, 치매 환자를 중심으로)

  • Subjective cognitive-linguistic complaints in older adults contribute to the diagnostic and prognostic investigation of MCI or dementia. However, the utility of subjective test for predicting cognitive-linguistic decline is controversial. Few domestic studies have included the correlation between subjective and objective tests systematically. The current study analyzed 26 studies published since 2000, and the effect sizes of their correlation coefficients between two tests were computed. The results of qualitative analysis indicated that the number of subjects ranged from 26 to 657. Subjective tests included the self-report 75.4% and the informant-report 24.6%. In objective tests, memory comprised the largest proportion, followed by global cognition, and language, etc. As a result of meta-analysis, self-report test had the predictive value for dementia, and informant-report test contributed to discriminate among 3 groups. In the elderly group, self-report test was correlated with reasoning, and informant-report test with memory and language. In MCIs, self-report test predicted several abilities including language, and informant-report test signaled the future decline of domains like global cognition. Two types of subjective tests in dementia also represented memory, language, and global cognition accurately. This study provides evidence-based information to support relationships between subjective and objective tests for cognitive-linguistic ability in 3 groups.

The Influence of Depression and School Life on the Quality of Life of Korean Child and Adolescent Patients with Attention-Deficit/Hyperactivity Disorder: A Comparison of the Perspectives of the Patients and Their Caregivers

  • Objectives: This study aimed to compare the quality of life reported by patients with attention-deficit/hyperactivity disorder (ADHD) to the patients' quality of life as reported by their caregivers. In addition, it aimed to examine how emotional problems, including depression and anxiety, and the severity of the symptoms affect the quality of life reported by the patients and their caregivers. Methods: The patients' quality of life and their degree of depression and anxiety were measured using the Pediatric Quality of Life Inventory (PedsQL) 4.0 Child Self-Report, the Children's Depression Inventory (CDI), and the Revised Children's Manifest Anxiety Scale, respectively. The caregivers' perception of the patients' quality of life and severity of the ADHD symptoms were measured using the PedsQL 4.0 Parent Proxy Report and the Conners' Parent Rating Scale (CPRS), respectively. A total of 66 participants completed the survey. The independent-samples t-test, Pearson's correlation analysis, and multiple regression analysis were conducted. Results: The mean score of the PedsQL 4.0 Child Self-Report was significantly higher than the mean score of the PedsQL 4.0 Parent Proxy Report. However, for school function, the PedsQL 4.0 Child Self-Report score was significantly lower than that of Parent Proxy Report. The correlation between the PedsQL 4.0 Child Self-Report and PedsQL 4.0 Parent Proxy Report scores was significant only for emotional function and social function. The multiple regression analysis showed that the PedsQL 4.0 Child Self-Report and PedsQL 4.0 Parent Proxy Report scores were significantly predicted by the CDI and CPRS scores, respectively. Conclusion: Our results demonstrate that there are clear differences between the quality of life reported by the patient themselves and that reported by their caregivers. In addition, the findings suggest that it is critical to treat the patients' accompanying depressive symptoms.

  • Purpose: The objective of the present study was to develop and evaluate a legal communicable diseases (LCDs) electronic system for infection control. Method: The system was developed through the procedure of analysis, design, implementation, application and evaluation, and was applied within an OCS (order communication system). Results: As indicated by the main menu, the present system is composed of an improved perception system for enhancing perception of LCDs, LCDs identification system, and improved efficiency in the report system. Detailed items included in the main menu are introduction and log-in screen, pop-up window for checking the outbreaks of LCDs, decision making icon, electronic signature icon, electronic report form, email system, etc. The total number of reports was greater after the application of the system (n=99) than before (n=80), and the adequacy of report time was statistically significantly higher after application of the system (P<0.05) Conclusion: The present system suggests a new method for LCDs report and infection control, and is expected to be adopted by other medical institutions in the future.

A Case Report of Congestive Heart Failure Treated with Acupotomy Therapy (심부전으로 인한 호흡곤란 환자에게 시행한 침도침 시술 1례 보고)

  • Objectives : The purpose of this report is to report the effect of acupotomy for patients with congestive heart failure. Methods: We treated 1 patient who has dyspnea due to congestive heart failure with acupotomy. To check the effect and satisfaction of acupotomy we used New York Heart Association class(NYHA class), Modified Borg Scalw Dyspnea Index(Borg Index), Baseline Dyspnea Index(BDI), Five-point Likert scale. Results : After 1 month of treatment, the patient felt much better in breathing, and had better score in NYHA class, Borg Index, BDI. Conclusions : This report demonstrates that acupotomy therapy has useful effect on congestive heart failure, but the more cases and researches are needed.

Predictors of Intention to Report Elder Abuse among Elderly Women (여성노인에서 학대 시 신고 의향에 영향을 미치는 요인)

  • Purpose: This study was designed to identify the predictors of elderly women's intention to report elder abuse. Methods: A cross-sectional survey design was used. The participants were 204 elderly women aged over 60 living in Seoul. Data were collected using self-reported questionnaires by convenience sampling. Data were analyzed with frequencies, $x^2$ test, t-test and binary logistic regression. Results: Logistic regression analyses showed perception of elderly welfare law, perception of seriousness of elder abuse, subjective economic status, and exposure to elder abuse information were significant predictors of elderly women's intention to report elder abuse. Conclusions: The results of study suggest that the provision of information related to elder abuse including elderly welfare law is crucial toward elderly women in preventing elder abuse.

  • Among medical claims review, decision of malignancy is very important. According to the pathologic report may be ordinary pathway. Some tumors are not completely studied especially malignancy. Wheather malignancy or benign is the important thing in medical claims review. We here disscuss on the debatable tumors such as carcinoid tumor, gastrointestinal stromal tumor (GIST), desmoid tumor, MALToma, and pseudomyxoma peritonei. Another controversial subject in the medical claims review is selection of pathologic report. If the result of the pathologic report is not same in one patient, We prefer the selsection of the report from more professional hospital. We have called this professional hospital l as "third hospital" or 'refferal hospital".

Introduction to Korea Institute of Oriental Medicine's Korean Medicine Case Report Program

  • Several modalities that have been used in Korean medicine have not undergone evaluation. The Korea Institute of Oriental Medicine (KIOM), a South Korean national research agency, launched the Korean medicine case Report (KORE) program in May 2019 to promote case reports, especially among clinicians. The aim was to (1) introduce the KIOM-initiated KORE program, (2) report the results from the KORE program, and (3) present the challenges and limitation from the KORE program. We conducted a narrative review of the cases to which KORE was applied. A total of 16 proposals were reviewed since May 2019. Five proposals were rejected, and 7 case reports were included. Case reports for 3 therapies have been created under guidance. The KORE program makes it easier for Korean medicine doctors to initiate and complete case reports.

  • Renal transplant recipients who received immunosuporessive agent are in high risk of development Kaposi's sarcoma. In Korea a few report of Kaposi's sarcoma has been pubilshed but any report of intrathoracic Kaposi's sarcoma provedn by open lung biopsy has not been pulbilshed until now. We report a case of intrathoracic Kaposi's sarcoma developed in a 25 year old Korean man, who had been operated renal transplantation due to end stage renal disease and received cyclosporine and prednisolone as immunosuppessive agent, without any other organ involvment and was proven by open lung biopsy. Although discontinuation of immunosuppressive agent, temporary symptomatic and radilolgic improvement were observed, he died 11 days later after open lung biopsy because of intractable resiratory failure.

  • According to the Fourth Assessment Report of Intergovernmental Panel on Climate Change(IPCC) Working Group III, climate change is already in progress around the world, and it is necessary to execute mitigation in order to minimize adverse impacts. This paper suggests future climate change needs, employing IPCC Special Report on Emissions Scenarios(SRES) to predict temperature rises over the next 100 years. This information can be used to develop sustainable architecture applications for energy efficient buildings and renewable energy. Such climate changes could also affected the resent supplies of renewable energy sources. This paper discusses one recent Fourth Assessment Report of IPPC (Mitigation of Climate Change) and the Hadley Centre climate simulation of relevant data series for South Korea.
