  • Purpose : The purpose of this article was to review the literature on change of knee position sense after total knee replacement intervention in knee rehabilitation. Methods : This review outlines scientific findings regarding different literature data in knee rehabilitation, and discusses proprioceptive change of knee joints after total knee replacement intervention. Result : Severe pain and diminished joint sensation may precipitate degenerative changes of knees joint, and a strong association between decreased proprioception and function has been identified in elderly patients with knee osteoarthritis. Total knee replacement(TKR) of the knee joint are effective surgical procedures in osteoarthritis patients, resulting in decreased pain, and improved physical function and quality of life in patients. After total knee replacement, joint position sensation is partially recovered, which may improve functional stability of the knee, but aspects of physical functioning are not fully restored to preoperative status. Conclusions : Thus, recovery rate in proprioception after TKR also is considered to be important because it may be a significant risk factor in failure of total knee replacement and knee rehabilitation intervention.

  • Although there are many research studies on the effectiveness of heat and cold therapy for patients with arthritis at home or health care center, little attention has been paid to determining which therapy associates with season is effective for patients with chronic arthritis. The purpose of this study was to explore the effectiveness of heat and cold therapy associated with season for patients with arthritis. An experimental design using replications with intervention was employed. A total of 27 female arthritic patients were selected. Data were collected in summer and winter. Hot bag and ice bag were applied on each patient's knee for each 20 minutes alternatively. Joint pain, discomfort and range of motion were measured. Data were analyzed using paired t-test, and two-way ANOVA. The results of this study were ; 1. Joint pain Heat therapy was effective for pain relief, as compared with cold therapy. Heat therapy was more effective for pain relief in winter than in summer. Cold therapy was effective for pain relief, but there was no statistically significant difference of pain relief between summer and winter. 2. Discomfort Discomfort was decreased using heat therapy, whereas it was increased using cold therapy. Although discomfort was decreased using heat therapy in both summer and winter, there was no statistically significant difference of discomfort between summer and winter. Using cold therapy, discomfort was decreased in summer, but increased in winter. and season had effect on discomfort. 3. Range of motion Although there was no statistically significant difference between the range of motion for both heat and cold therapy, range of motion was Increased using both heat and cold therapy. In winter, range of motion was increased rather than in summer by using heat therapy. Using cold therapy, The range of motion was decreased in both summer and winter. There was no stastistically significant difference of range of motion between heat therapy and cold therapy. Furthermore, there was no statistically significant difference of range of motion between summer and winter. In conclusion, both heat and cold therapy were effective for pain relief, discomfort, and range of motion, especially heat therapy. Heat therapy was effective for pain relief, discomfort, and range of motion in winter, as compared with summer. Cold therapy, however, was effective for only pain relief in winter, The findings suggest the use of heat therapy for patients with arthritis especially in winter.

  • Background: We evaluated the role and effects of prolotherapy in patients presenting with lower back pain and detected sacral asymlocation, by retrospectively analyzing the results of prolotherapy performed at our institute. Methods: Twenty-three patients with referred pain in the lower back rather than distinct radiculopathy, were detected to have sacral asymlocation by simple X-ray from May 2004 through July 2005. The patients were treated with prolotherapy and manipulation by the Ongley's method around the lumbosacral junction, iliolumbar ligament, and sacroiliac joint. They were treated for approximately one to two week intervals, and during this period were rechecked by X-ray and evaluated using the visual analogue scale (VAS). Results: A total of 23 patients were included in the study (10 male and 13 female), and the average age was 41 years. The average VAS at the time of visit was B.5, the average treatment time was 4,7 days, and the average VAS after treatment was 2.1. Conclusions: Back pain, and associated leg and buttock pain, originate from several causes. In these case analyses, instability around the lumbosacral area and sacral asymlocation might have been important causes of patient back pain and associated buttock and leg pain. We therefore applied prolotherapy as well as manipulation techniques devised by Ongley to these patients, and obtained good results.

  • Background: The role of the sympathetic nervous system appears to be central in causing pain in complex regional pain syndrome (CRPS). The stellate ganglion block (SGB) using additives with local anesthetics is an established treatment modality. However, literature is sparse in support of selective benefits of different additives for SGB. Hence, the authors aimed to compare the efficacy and safety of clonidine with methylprednisolone as additives to ropivacaine in the SGB for treatment of CRPS. Methods: A prospective randomized single blinded study (the investigator blinded to the study groups) was conducted among patients with CRPS-I of the upper limb, aged 18-70 years with American Society of Anaesthesiologists physical status I-III. Clonidine (15 ㎍) and methylprednisolone (40 mg) were compared as additives to 0.25% ropivacaine (5 mL) for SGB. After medical treatment for two weeks, patients in each of the two groups were given seven ultrasound guided SGBs on alternate days. Results: There was no significant difference between the two groups with respect to visual analogue scale score, edema, or overall patient satisfaction. After 1.5 months follow-up, however, the group that received methylprednisolone had better improvement in range of motion. No significant side effects were seen with either drug. Conclusions: The use of additives, both methylprednisolone and clonidine, is safe and effective for the SGB in CRPS. The significantly better improvement in joint mobility with methylprednisolone suggests that it should be considered promising as an additive to local anaesthetics when joint mobility is the concern.

  • Objective: The purpose of this study was to determine the effect of stretching exercise on depression, pain, and fatigue in patients with systemic lupus erythematosus(SLE). Methods: The study was designed as a non-equivalent control group pre-posttest quasi-experimental design. The patients with SLE who signed the consent form were conveniently assigned into two groups(11 experimental subjects and 10 control subjects). The subjects in the experimental group were participated in 6 weeks stretching exercise program for one and a half hours per session twice a week. The data were analyzed by using a SAS-pc+ 8.01 program. Results: 'The experimental group would have less depression scores than the control group' was supported (Z=2.2, p=.025). 'The experimental group would have less pain scores than the control group' was supported (Z=3.4, p=.001). 'The experimental group would have less fatigue scores than the control group' was supported (Z=-2.0, p=.041). With this study, we identified the stretching exercise program was effective on depression, pain, and fatigue of SLE patients. Conclusions: From above results, 6 weeks stretching exercise program could be an effective nursing intervention to reduce depression, pain, and fatigue in patients with SLE.

  • Rheumatoid Arthritis(RA), a chronic and systemic inflammatory disorder, is characterized by joint pain, swelling and stiffness. Patients with RA suffer from joint pain and overall pain. The painful and disabling consequences of RA are accompanied by a variety of affective, cognitive, and behavioral changes. The purpose of this paper was to explore and describe the life experiences of RA patients by eliciting verbal description of their experiences. Participants were nine persons who were diagnosed with RA, and had had it for more than six months. They were asked open ended and descriptive questions in order for them to talk about their experiences in their on terms. Interviews were tape recorded and transcribed verbatim. The transcripts were analyzed by Colaizzi method. From the protocols, 213 significant statements about life experience were organized into 83 formulating meanings which were then grouped into five theme clusters. The major themes that emgerged from the analysis were' Emotional Disturbances', 'Trying to Adapt to the Progress of Disease', 'Progress of Treatment'. 'Change of Role Performances', 'Experiences related to Family'. The result of this study showed that RA patients and families need nursing care based on a deep understanding of their lived experiences in everyday life. Nurses and other health workers must develop rehabilitation programs that focus on the pain control, functional independence and psychosocial factors.

  • Purpose: The purpose of this study was to compare the effects of stretching exercise and aquatic exercise program on flexibility, pain and quality of life (QoL) in patients with osteoarthritis. Methods: Fifty elderly women were assigned either to the stretching exercise (SEG, n=25) or to the aquatic exercise groups (AEG, n=25) after 6-week self-help education program. SEG carried out supervised stretching exercise and water exercise training program was conducted by AEG (60 min/day, 2 days/wk) for 8 weeks. Flexibility, pain, and quality of life were measured by Senior Fitness Test Manual (Rikli & Jones, 2gr5), pain rating scale (0-10) and the questionnaire by World Health Organization Quality of Life BREF (Min et al., 2gr0), respectively. Results: Both SEG (all, p&H;.05) and AEG (all, p&H;.05) increased left upper and lower-body flexibility, measured by a measuring tape. Pain significantly decreased both in SEG (p=.000) and AEG (p=.004). Quality of life was significantly improved only for the SEG. Conclusion: The 8-week stretching exercise program would significantly improve flexibility, pain control and QoL in patients with osteoarthritis.
