대학생의 생활 스트레스가 측두하악관절 장애에 미치는 영향 (Influence of life stress on Temporomandibular joint disorders in undergraduate students)

  • Purpose: The purpose of this study was to investigate the factors contributing to temporomandibular joint disorder symptoms. Methods: The survey was conducted from May 1 to June 10 2017 against college students(120 males and 123 females) in Daejeon and Gangwon who understood the purpose of this study and agreed to participate. Results: First, The most frequent symptom of temporomandibular joint disorders is noises from the joint (male 45%, female 61.8%), followed by headache or neck pain (male 25.8%, female 52.8%). There was shown a significant difference by gender in all items except for jaw dropping, poor alignment of teeth and trauma(p<0.05). Second, Looking into the correlation between stress and subjective symptoms of Temporomandibular joint disorders, The stress of Temporomandibular joint disorders showed more significant relation with realistic issues such as academic task, economic burden (e.g., treatment cost), future career, and life values rather than aspect of social relationship(p<0.01). Conclusion: As a result of analyzing variables to understand the influence of stress on temporomandibular joint disorders, the accumulated number of bad habits, stress and temporomandibular joint disorders showed a significantly positive correlation with one another (p<0.05). In other words, the higher the stress level the severe the temporomandibular joint disorders; the more the bad habits the severe the temporomandibular joint disorders;

Facet joint disorders: from diagnosis to treatment

  • One of the most common sources of spinal pain syndromes is the facet joints. Cervical, thoracic, and lumbar facet joint pain syndromes comprise 55%, 42%, and 31% of chronic spinal pain syndromes, respectively. Common facet joint disorders are degenerative disorders, such as osteoarthritis, hypertrophied superior articular process, and facet joint cysts; septic arthritis; systemic and metabolic disorders, such as ankylosing spondylitis or gout; and traumatic dislocations. The facet pain syndrome from osteoarthritis is suspected from a patient's history (referred pain pattern) and physical examination (tenderness). Other facet joint disorders may cause radicular pain if mass effect from a facet joint cyst, hypertrophied superior articular process, or tumors compress the dorsal root ganglion. However, a high degree of morphological change does not always provoke pain. The superiority of innervating nerve block or direct joint injection for diagnosis and treatment is still a controversy. Treatment includes facet joint injection in facet joint osteoarthritis or whiplash injury provoking referred pain or decompression in mass effect in cases of hypertrophied superior articular process or facet joint cyst eliciting radicular pain. In addition, septic arthritis is treated using a proper antibiotic, based on infected tissue or blood culture. This review describes the diagnosis and treatment of common facet joint disorders.

골스캔을 이용한 측두하악관절장애의 임상진단에 관한 연구 (A Study on Clinical Diagnosis of Temporomandibular Joint Disorders Using Bone Scan)

  • The author examined the clinical signs and symptoms , routine radiographs, laboratory test and bone scan in 73 subjects with TMJ disorders and studied the responses of clinical test on patients, the distribution of signs and symptoms of joints, the simple uptake rate(SUR) of affected joints , the SUR of subclassified groups of TMJ disorders, active joints of subclassified groups of TMJ disorders and the SUR of joints with noises. The obtained results were as follows : 1. The percentage of joint pain on palpation, joint noises joint pain on function, mandibular dysfunction and active finding of bone scan in aptients with TMJ disorders were higher 2. The SUR was higher in joints affected by joint pain on function, joint pain on palpation, mandibular dysfunction and creptius. 3. The SUR of osteoarthrosis was the lowest. 4. The percentage of active joints were the highest in joints with discdisplacement without reduction, and followed by osteoarthritis, and disc displacement with reduction. 5. The SUR of TMJ showing joint noises only was lower.

측두하악관절 장애 환자의 임상증상과 자기공명영상에 관한 비교 연구 (A comparison of clinical symptoms and magnetic resonance images in temporomandibular joint disorders)

  • Purpose : To determine the relationship between clinical symptoms and magnetic resoncance (MR) images in patients presenting with temoporomandibular joint (TMJ) disorders. Materials and Methods: This study was based on 172 joints in 86 patients presenting with TMJ disorders. Joint pain and sound during jaw opening and closing movements were recorded, and the possible relationship between disc positions and bony changes of the condylar head and the articular fossa in MR images in the oblique sagittal planes were examined. Data were analyzed by Chi-square test. Results : There was no statistically significant relationship between clinical symptoms and MR images in the patients with TMJ disorders. Conclusion: In the patient with TMJ disorders, joint pain and sound could not be specific clinical symptoms that are related with MR image findings, and asymptomatic joints did not necessarily imply that the joints are normal according to MR image findings.

측두하악장애 환자의 임상적 양태에 대한 연구 (A prevalence of clinical sign and symptom in temporomandibular disorders patients)

  • A prevalence of temporomandibular disorders(TMD) based on the clinical sign and symptom in 155 patients were investigated. History taking with interview chart and clinical examination were performed. Age and gender of the patients, duration of TMD, location of pain, joint sound, limitation of mouth opening and more detailed diagnostic name were identified. The results of this study were as follows: 1. TMD was more prevalent in female than in male. TMD was the most prevalent at the age of 20s and decreased with age. 2. Acute TMD was more prevalent than chronic one. 3. About ninety percent of TMD patients had pain. Pain had mainly a unilateral origin. Muscle pain was mainly related with the masseter muscle. 4. Joint sound was identified in about fifty-six percent of the TMD patients and unilateral joint sound was more prevalent than bilateral one. 5. Limitation of mouth opening was observed in about forty percent of the TMD patients. 6. In TMD patients, muscle dysfunction was the most prevalent one. In muscle disorders local muscle soreness was the most prevalent one. In joint disorders, capsulitis was the most prevalent one. In muscle-joint disorders, trigger point pain with retrodiscitis was the most prevalent one.

Mischievous mandibular third molars camouflaging temporomandibular joint disorders

  • Objectives: The aim of this study was to observe the relationship between impacted mandibular third molars and development of temporomandibular joint (TMJ) disorders. Knowledge of the factors that have an adverse effect on the TMJ is necessary for proper diagnosis, treatment, and prognosis of TMJ disorders. Materials and Methods: The study was performed on 80 patients aged between 20 and 60 years with impacted mandibular third molars, over a period of two months. The patients were examined clinically and radiologically to determine the type of impaction and detect the associated TMJ symptoms or disorders. Results: In the 80 patients, 63.8% (51/80) of TMJ disorders were found in the horizontal group, 46.3% (37/80) in the mesioangular group, 42.5% (34/80) in the distoangular group, and 30.0% (24/80) in the vertical group of impacted mandibular third molars. Conclusion: The study concluded that type of impacted mandibular third molar is factor in the development of temporomandibular disorders.

성인의 턱관절장애 자각증상에 영향을 미치는 요인 (Factors affecting subjective symptoms of temporomandibular joint disorders in adults)

  • Objectives: The purpose of this study is to identify the perceived symptoms of oral and temporomandibular joint disorders in adults and to analyze the factors affecting subjective symptoms of temporomandibular joint disorders. Methods: 249 adults over 20 years old who had subjective symptoms of temporomandibular joint disorders were surveyed and analyzed. Independent t-test and ANOVA test were used to examine the relationship between oral habits and temporomandibular joint disorder according to general characteristics. $Scheff{\acute{e}}$ test was used for post-hoc analysis. Multiple regression analysis was conducted to examine the factors affecting oral habits and temporomandibular joint disorder. Results: First, the factors affecting oral and habitual behaviors were married (p<0.05) and monthly income between 1 million~1.9 million won (p<0.001), higher temporomandibular joint disorder (p<0.01) And the degree of habit was increased. Second, the factors affecting temporomandibular joint disorder were increased in occupation (p<0.05) and the degree of oral habit (p<0.01). Conclusions: In conclusion, it was confirmed that oral habit influences temporomandibular joint disorder. Especially, it is suggested that prevention and promotion of temporomandibular joint disorder are needed to recognize the removal of oral habits.

성인의 악관절 증상과 요추질환의 관련성 (The Association between Temporomandibular Joint Disorders and Lumbar Diseases in Adults)

  • 본 연구는 골관절염의 유병률이 증가하는 시기인 성인을 대상으로 악관절 증상과 요추질환의 관련성 분석을 통해 구강 건강 및 일반적인 건강 향상을 위한 보건정책개발에 도움이 되고자 수행하였다. 대표성이 있는 국민건강영양조사 자료 중 제5기 3차년도(2012년) 자료를 활용하였으며 연구목적에 적합한 만 50세 이상을 대상으로 건강설문 및 요추관절 방사선 검사를 완료한 3,017명을 연구대상으로 분석을 수행하였다. 악관절 증상의 여부는 16.1%가 최근 1년간 악관절 부위에서 이상 증상을 한 가지 이상 경험했다고 답하였다. 요통의 경우 20.6%가 최근 3개월 동안 요통이 있다고 답하였으며, 요추관절 방사선 영상 진단결과에 따른 요추 골관절염이 있는 사람은 30.6%로 나타났다. 악관절 증상을 경험한 사람의 경우 요통 유병위험률이 정상군에 비해 1.70배(95% CI, 1.30~2.22) 증가하는 것으로 나타났으며, 요추 골관절염 유병 위험률은 정상군에 비해 1.20배(95% CI, 0.86~1.68) 증가하는 것으로 나타났다. 본 연구결과 성인의 악관절 증상이 요추 골관절염 유병위험률과의 관련성에서는 유의하지는 않았지만 요추질환의 전형적인 증상인 요통에는 영향을 미치는 것을 알 수 있었다. 따라서 성인들의 악관절 증상에 대한 적절한 치료 및 예방을 한다면 요추질환 유병률을 낮추고, 삶의 질을 높이는 데 중요한 작용이 될 것으로 판단된다. 또한 향후 악관절 장애와 요추질환에 대한 적절한 관리프로그램 개발 및 체계적인 지원이 필요하다고 생각된다.

전기치료, 테이핑과 관절가동기법이 급성기 편타증 손상환자에게 미치는 영향-사례연구 (The Effects of Modality, Taping and Joint Mobilization for Patients with Acute Whiplash-Associated Disorder : Case Study)

  • Background: The purpose of this study was to investigate the effects of modality, taping and joint mobilization for patients with acute whiplash-associated disorders. Methods: Joint mobilization technique and kinesio taping was done to the patients who diagnosed with acute whiplash-associated disorders due to traffic accidents. Results: We got the results that it is quite effective to increase cervical range of motion and decrease the pain, according to the results, present study was performed to assess the impact on the neck disability index and fatigability. Conclusion: Joint mobilization technique applied to a person and taping applied to the other person then both people increasing cervical range of motion and reducing pain for the treatment of patients with acute whiplash-associated disorders was effective.

측두하악장애 환자에서 과두크기와 관절융기의 후방경사에 관한 연구 (A STUDY ON THE SIZE OF CONDYLE AND THE POSTERIOR SLOPE OF THE ARTICULAR EMINENCE IN PATIENTS WITH TEMPOROMANDIBULAR JOINT DISORDERS.)

  • The purpose of this study was to investigate the interrelationship between temporomandibular joint disorders, and the condyle size and angle of the posterior slope of the articular eminence. The subjects used in this study were 100 patients with temporomandibular joint disorders and 100 volunteers with normal temporomandibular joints. All the patients and the volunteers were subjected to take panoramic and temporomandibular radiographs for the morphologic evaluation. The films were traced, measured, and analyzed. The data were processed with SPSS/PC+ package for statistical analysis. The obtained results were as follows; 1. The posterior slope of the articular eminence in the group of temporomandibular joint disorders was larger than that in the normal group (p<0.05). The mean articular eminence angle was $31.6^{\circ}\;{\pm}\;6.3$ in the group of temporomandibular joint disorder, and $29.9^{\circ}\;{\pm}\;8.4$ in the normal group. 2. There were no statistically significant differences in the discrepancies of the left and right articular eminence angular measurements between the group (p>0.05). 3. There were no statistically significant differences in the condylar ratio between the groups (p>0.05). 4. There were no statistically significant differences in the discrepancies of the left and right ramus length measurements between the groups (p>0.05). 5. The relative size of condyle to fossa in the group of temporomandibular disorders was smaller than that in the normal group (p<0.05).
