근관절가동기법이 퇴행성 디스크 질환을 가진 환자의 통증과 유연성에 미치는 영향 (The Effects of Joint and Muscle Mobilization on Pain and Flexibility of the Patients with Degenerative Disc Disease)

  • Purpose : The purpose of this study is to find out the effects of Joint and Muscle Mobilization applied to the patients with Degenerative Disc Disease by measuring, assessing and analyzing the changes on pain and in flexibility before and after Joint and Muscle Mobilization. Method: Surveyed from June. 2008 to July. 2009 were 10 patients suffering from DDD. Joint and muscle Mobilization were applied for 15 minutes in total. After Joint Mobilization, the Oswestry Disability Index(ODI), Visual Analogue Scale (VAS) were used to measure the degree of the pain on the patients. After Joint and Muscle Mobilization, Spinal-$Mouse^{(R)}$ were used to measure the degree of the flexibility on the patients. Result: 1. There was significant decrease in the numerical values of the VAS & ODI after Joint and Muscle Mobilization (p<0.01). 2. There was significant decrease in the degree of the pain on the patients after Joint and Muscle Mobilization (p<0.01). 3. From the analysis into DDD in the degree the pain before and after Joint and Muscle Mobilization with Paired Sample T-test, It became evident that the longer the period of treatment was, the higher the pains decreases drastically, while significant difference was shown in the flexibility and the degree of the pain (p<0.01). Conclusion: Summed up, it can be generally concluded that Joint and Muscle Mobilization is an effective treatment to rid the patients with DDD of pains safely and promptly. It is, therefore, suggested to continue and expand the study on the cure of DDD and to motivate patients. Joint and Muscle Mobilization is considered as safest and most efficient pain remedy.

관절가동술과 근력강화운동이 앞쪽머리자세 환자의 목기능과 근긴장도에 미치는 영향 (The Effects of Joint Mobilization and Muscle Strength Exercise on Neck Function and Range of Motion in Forward Head Posture)

  • Background: The purpose of this study was to investigate the effects of joint mobilization and muscle strengthening exercises on neck function, range of motion, and muscle tone in patients with forward head posture. Methods: A total 32 subjects were divided into muscle strengthening exercise (n=16) and joint mobilization (n=16) who met the diagnostic criteria for the forward head posture, and the study method used the neck disability index (NDI) and BPM Pro to compare the range of motion. Results: As a result of the study, there was no significant difference between each group, and when looking at the differences before and after each group, there was a significant difference in neck pain in both groups. There was a significant difference in muscle tension in the joint mobilization group, but there was no significant difference in the muscle strengthening exercise group. In the range of joint mobilization, there was a significant difference in the range of left joint mobilization of the joint mobilization group, there was no significant difference in the range of right joint mobilization, and there was no significant difference in the range of joint mobilization of the muscle strengthening exercise group. Conclusion: The above results revealed that joint mobilization and muscle strengthening exercises were effective on neck function, range of motion, and muscle tone in forward head posture patients.

경추관절가동술이 정상성인의 경부 관절가동범위와 정적근지구력에 미치는 영향 (The Effects of Cervical ROM and Muscle Endurance on Cervical Joint Mobilization of Normal Adults)

  • The purpose of this study was to evaluate the influence of cervical ROM and muscle endurance on cervical joint mobilization of normal adults. Both joint mobilization group(n=20) and control group(n=20) were measured an cervical ROM and muscle endurance time by CROM and stop watch at pre-test, post-test in 1 weeks, post-test in 2 weeks and post-test in 3 weeks. The flexion ROM, extension ROM and muscle endurance of the joint mobilization group were significantly different among the experimental period (p<.05). The Correlation of muscle endurance and extension ROM is significantly Correlation (p<.05). In conclusion, we were found that cervical joint mobilization could increase cervical ROM and muscle endurance and Correlation of muscle endurance and extension ROM.

천장관절 가동술이 하지 근력에 미치는 영향 (The Influence of Sacroiliac Joint Mobilization on Lower Extremity Muscle Strength)

  • Purpose : The purpose of this study was to evaluate influence of sacroiliac joint mobilization on lower extremity muscle strength. Methods : The subjects were consisted of thirty patients who had Leg length inequality(LLI) of more than 10mm(16 females. 14 males) from 21 to 41 years of age(mean aged 24.87). All subjects randomly assigned to sacroiliac joint mobilization group(n=15), control group(n=15). sacroiliac joint mobilization group received sacroiliac joint mobilization about 10 minutes for 3 times per week during 4 weeks period. Control group not received intervention during 4 weeks period. The tape measure method(TMM) was used to measure functional Leg length inequality. Biodex System 3 Pro was used to measure strength of Knee extension & flexion. All measurements of each subjects were measured at pre-test, 2weeks post-test and 4weeks post-test. Results : 1. The LLI of sacroiliac joint mobilization group was significantly reduced according to within treatment period(p<.05), most significantly reduced between pre-test and post-test(p<.05). sacroiliac joint mobilization group significantly more reduced than control group(p<.05). 2. The knee extension strength of sacroiliac joint mobilization group was significantly increased according to within treatment period(p<.05), most significantly increased between pre-test and post-test(p<.05). sacroiliac joint mobilization group significantly more increased than control group(p<.05). 3. The knee flexion strength of sacroiliac joint mobilization group was significantly increased according to within treatment period(p<.05), most significantly increased between pre-test and post-test(p<.05). sacroiliac joint mobilization group significantly more increased than control group(p<.05). Conclusion : sacroiliac joint mobilization can reduce LLI and increased lower extremity muscle strength.

Combined Effect of Joint Mobilization and Active Stretching on Gait Speed and Ability after Stroke

  • Background: Patients with stroke have limited ankle range of motion (ROM) due to soft tissue abnormalities around the ankle and thus experience functional impairment. Increased muscle tension and reduced ankle ROM impair gait and hinder the activities of daily living. Joint mobilization and stretching are effective interventions that improve gait performance by enhancing the ankle ROM. Objectives: To investigate the effects of ankle joint mobilization and calf muscle stretching on gait speed and gait performance in patients with stroke. Design: This was a randomized controlled trial. Methods: Twenty patients with stroke patients were randomized into two groups. The joint mobilization group (JMG) underwent anteroposterior mobilization of the talocrural joint and the joint mobilization stretching group (JMSG) underwent calf muscle stretching in addition to joint mobilization. Gait speed and gait parameters were measured using the 10-meter walk test and the GAITRite. Results: Both the JMG and JMSG groups showed significant improvements in gait speed, affected-side step length, and cadence after the intervention (P<.05). Conclusion: Joint mobilization and stretching were effective interventions for improving gait performance by enhancing ankle function in patients with stroke.

경추안정화운동과 경추관절가동술이 경부의 최대근력과 정적근지구력에 미치는 영향 (The effect of cervical stabilized exercise and joint mobilization on maximum muscle strength and static muscle endurance of cervical region)

  • 경추안정화 운동과 경추관절가동술이 경부의 최대근력과 정적근지구력에 미치는 영향을 알아보기 위해 60명을 경추안정화운동군, 경추관절가동술군, 대조군의 3개의 군으로 나누어 각 그룹당 20명씩 무작위 배치하였다. 경추관절가동술과 경추안정화운동은 일주일에서 3회씩 총 3주 동안 시행되었고, 경추안정화운동군은 선자세, 바로누운자세, 엎드려누운자세에서 경추심부근육의 등척성운동을 적용하였다. 경추관절가동술군은 칼텐본 기법을 이용한 관절내 운동을 적용하였다. 경추안정화운동과 경추 관절가동술 모두 최대근력과 정적근지구력이 증가하였고 그 중 경추안정화운동이 경추관절가동술보다 최대근력과 정적근지구력에 더 많은 영향을 미쳤다.

  • Background: This study was conducted to compare the effects of an incline board, proprioceptive neuromuscular facilitation (PNF) stretching, and joint mobilization on the dorsiflexion of the ankle joint and muscle tone of the plantar flexor and ankle in stroke patients. Methods: Forty-five stroke subjects were randomly divided into three groups: each JMG (Joint Mobilization Group), IBG (Inclined Board Group) and PSG (PNF Stretching Group). The experiment was performed for eight weeks between August 1 and October 31, 2018. Results: The dorsiflexion of the ankle joint was significant between the groups and by period (p<.05). JMG had a significant change by period (p<.05). Differences between groups by period increased after 8 weeks in the PSG and the JMG (p>.05). The gastrocnemius lateral muscle tone was significant between the groups (p<.05), and medial muscle tone was interacted between groups and time (p<.05). Conclusion: An effective intervention method to increase the dorsiflexion of the ankle joint and decrease the muscle tone when performing PNF stretching, and joint mobilization in stroke patients was confirmed. Future studies are recommended to variety research of PNF stretching and joint mobilization.

운동치료와 관절가동술이 하지길이 차이에 미치는 영향 (The Effects of Joint Mobilization and Therapeutic Exercise on Difference for Length of Lower Limbs)

  • Object: to evaluate the effects of two different treatments-joint mobilization and therapeutic exercise on difference for length of lower limbs. Method: The subjects were participated twenty six who has difference for length of lower limbs more 10mm. All subjects randomly assigned to Joint mobilization group(n=13) and therapeutic exercise group. Joint mobilization group received joint mobilization for 2 minutes, Therapeutic exercise group received for 15 minutes per day and 3 times a week during 4 week period. Tape measure method was used to measure the difference for length of lower limbs. Biodex was used to measure the muscle power of lower limbs(Knee flexion, extension). Finger to floor test was used to measure the mobility of spinal column. All measurement of each subjects were measured at pre-experiment, after 2weeks and post-experiment. Result: The result of this study were summarized as follows : 1. Both treatment decreased difference for length of lower limbs while joint mobilization more decreased difference for length of lower limbs than therapeutic exercise. 2. Both treatment increased mobility of spinal column while joint mobilization more increased mobility of spinal column than therapeutic exercise. 3. Joint mobilization increased muscle power while therapeutic exercise decreased muscle power. Conclusion: in a group-wise comparison joint mobilization is more effective than therapeutic exercise.

Effects of Lumbar Mobilization on the Paravertebral Muscle Activity and Muscle Tone in Patients with Lumbar Spinal Stenosis

  • Background: Patients with lumbar spinal stenosis show abnormal changes in muscle activity due to pain and limited range of motion of the lumbar spine. Excessive increased muscle tone and decreased muscle activity patterns threaten the patients' quality of life. However, there have been a few studies showing how to improve muscle performance in patients with lumbar spinal stenosis. Among these, joint mobilization is one way of improving muscle performance through pain relief and increasing the range of motion. Objectives: To investigate the effect of lumbar mobilization by orthopedic manual physical therapy on paravertebral muscle activity and tone in patients with lumbar spinal stenosis. Design: A randomized controlled trial. Methods: In this study, 24 patients with lumbar spinal stenosis were randomized (1:1 ratio) into two groups. The experimental group underwent lumbar posteroanterior mobilization, and the control group underwent conventional physical therapy (conventional transcutaneous electrical nerve stimulation) for 15 minutes each. For outcome measures, Myoton®PRO was used to evaluate muscle tone when resting of the paravertebral muscle in the pain area. For muscle activity evaluation, the reference voluntary contraction of the paravertebral muscle was evaluated using surface electromyography. Results: Muscle tone and activity were significantly improved after intervention in both the experimental and control groups. In addition, the experimental group showed more significant decrease in muscle tone and activity than the control group. Conclusion: These results suggest that lumbar mobilization improving muscle performance in patients with lumbar spinal stenosis.

칼텐본 경추 관절가동술 후 적용된 능동적 중재가 두부 전방자세 환자의 경추 정렬과 근활성도에 미치는 영향 (Effect of Active Intervention after Kaltenborn's Cervical Joint Mobilization on The Cervical Spine Alignment and Muscle Activity in Patients with Forward Head Posture)

  • PURPOSE: The purpose of this study was to compare the effects of three interventions (intervention by passive range of motion exercise plus manual cervical traction, Mulligan's joint mobilization, and strengthening exercises) after Kaltenborn's joint mobilization on the cervical spine alignment, and muscle activity in patients with a forward head posture. METHODS: The subjects were 39 students from H University in Chungnam and C University in Jeonbuk. The subjects in each group attended training sessions three times a week for four weeks. We used one-way ANOVA and Scheffe's post hoc test to compare values between groups, and used paired t-test to compare the values of the dependent variables within groups. RESULTS: The results showed that the active intervention group experienced a significant increase compared to the passive intervention group in terms of the craniovertebral angle, cervical lordosis angle, and had significant decreases compared to the passive intervention group in terms of the upper trapezius muscle activity. The active intervention group also had significant increases in craniovertebral angle and decreased anterior scalene muscle activity than the active-assistive intervention group. The active-assistive intervention group had significant decreases compared to the active intervention group in terms of the serratus anterior, levator scapulae, and splenius capitis muscle activity. CONCLUSION: It appears that the subjects with a forward head posture had significant improvements in the cervical lordosis angle, cranial rotation angle, craniovertebral angle, and muscle activity after intervention by Mulligan's joint mobilization (active-assistive intervention component) and strengthening exercises (active intervention component) after applying Kaltenborn's joint mobilization.
