Shoulder Arthrokinematics of Collegiate Ice Hockey Athletes Based on the 3D-2D Model Registration Technique

  • Objective: There is a lack of studies using the 3D-2D image registration techniques on the mechanism of a shoulder injury for ice hockey players. This study aimed to analyze in vivo 3D glenohumeral joint arthrokinematics in collegiate ice hockey athletes and compare shoulder scaption with or without a hockey stick using the 3D-2D image registration technique. Method: We recruited 12 male elite ice hockey players (age, 19.88 ± 0.65 years). For arthrokinematic analysis of the common shoulder abduction movements of the injury pathogenesis of ice hockey players, participants abducted their dominant arm along the scapular plane and then grabbed a stick using the same motion under C-arm fluoroscopy with 16 frames per second. Computed tomography (CT) scans of the shoulder complex were obtained with a 0.6-mm slice pitch. Data from the humerus translation distances, scapula upward rotation, anterior-posterior tilt, internal to external rotation angles, and scapulohumeral rhythm (SHR) ratio on glenohumeral (GH) joint kinematics were outputted using a MATLAB customized code. Results: The humeral translation in the stick hand compared to the bare hand moved more anterior and more superior until the abduction angle reached 40°. When the GH joint in the stick hand was at the maximal abduction of the scapula, the scapula was externally rotated 2~5° relative to 0°. The SHR ratio relative to the abduction along the scapular plane at 40° indicated a statistically significant difference between the two groups (p < 0.05). Conclusion: With arm loading with the stick, the humeral and scapular kinematics showed a significant correlation in the initial section of the SHR. Although these correlations might be difficult in clinical settings, ice hockey athletes can lead to the movement difference of the scapulohumeral joints with inherent instability.

Intervention Strategy Applied ICF Checklists for Sitting Cross-Legged in Patient with Multiple Ligament Knee Injury: Single Subject Study

  • Purpose: This study applies the ICF to identify the patient's body function, structure, and participation, evaluates the patient's environmental factors and individual factors, and is a high level of movement to return to the society of patients with multiple ligament injury of the knee joint. Methods: Progressive strength training and ROM exercise were performed 30 minutes a day, 5 times a week for 6 weeks. The evaluation was performed by examining the ROM, length, MMT, instability, dynamic balance, pain and depression. Results: The ROM of the knee joint was improved from 110° to 135° after intervention, and the knee flexion length decreased from 69 cm to 45 cm. Knee flexor is Good after intervention from Poor-, and knee extensor is Good+ after intervention from Poor, and the plantar flexor of the ankle joint improved from Poor- before intervention to Good after intervention and dorsi-flexor of the ankle joint improve to Good from Poor. Pain index was moderate before and after the intervention, with a score of 3, 2 after the intervention, and when maintaining the sitting cross-legged, the before intervention score was 7 to 4 after the intervention. Conclusion: The patient's posture of sitting cross-legged was maintained from 30 seconds before intervention to 14 minutes after intervention. These results were able to set the hypothesis design, intervention method and goal that the multifaceted approach of environment and individual factors as well as body function and structure area, activity and participation area using ICF checklists, it is helped the patient to return to daily life.

변형된 엉치엉덩관절 테이핑의 요통 효과 사례보고 (The effect of Modified Sacroiliac Joint Taping on Back pain_A case report)

  • 본 연구는 엉치엉덩관절의 불안정성에 초점을 맞춘 테이핑 방법이 요통에 도움이 될 수 있는 잠재 중재 방법인지에 대해 고려해 보고자 한 사례연구이다. 해당 사례는 58세 과거 디스크탈출증 및 협착증으로 인해 허리 수술 병력이 있는 남성이 지속적인 통증 감소를 위해 참여한 테이핑 교육 세션에서 나타난 주목할 만한 결과를 사례로 정리하였다. 중재 방법으로 테잎을 엉치뼈 2~4사이에 가시결절로부터 양쪽으로 80%< 장력으로 늘려 엉치엉덩관절을 지나게 붙이고 이후 끝 쪽은 양쪽 약 45° 방향으로 올려 중간볼기근(gluteus medius m.) 쪽으로 향하게 붙였다. 그 다음 엉덩뼈능선(iliac crest)로부터 척추기립근을 따라 10번째 갈비뼈 수준까지 이르는 양쪽 테이핑 방법이 적용되었다. 해당 중재를 통해 VAS와 ODI 두 검사도구로부터 VAS는 5에서 0, 장애지수는 13에서 0으로 각각 기록되는 긍정적 사례 결과를 관찰할 수 있었다.

내측 전하방 관절낭 이동술을 이용한 다방향 견관절 불안정성의 치료 (Medial Anterior-inferior Capsular Shift in Multidirectional Shoulder Instability)

  • 목적: 이 논문의 목적은 다방향견관절 불안정증의 수술적 치료에 있어서 내측 전하방 관절낭 이동술의 유용성을 알아보고자 함이다. 대상 및 방법: 1998년 3월부터 2003년 12월까지 내측 전하방 관절낭 이동술을 시행한 19명의 환자를 대상으로 하였다. 이들 중 15례에서는 반복적인 탈구를 경험하였으며, 양측성 견관절에서만 이완을 보인 경우가 8례(42.0%), 견관절을 포함한 전신 인대 이완은 11례(58%)였으며 수의성 아탈구가 2례(10.5%)가 있었다. 평균 추시 기간은 24개월이었다(범위: 9-32 개월) 결과: 19례중 18례에서 통증이 호전되었으며, 평균 10도 정도의 외회전 감소가 있었으나 일상생활에 제한은 없었다. 술 후 모든 예에서 재탈구나 아탈구는 없었으나 2례에서는 스포츠 활동에서 불안감을 호소하였다. Rowe점수에서 모든 환자에서 최우수 또는 우수로 나타났으며 혈종 1례 및 가벼운 국소 피부 감염이 1례에서 있었으나 추후 모두 치료되었다. 결론: 다방향 견관절 불안정증에서 내측 전하방 관절낭 이동술은 통증감소, 환자의 만족도 및 견관절 안정성에 있어 만족스러운 결과를 나타냈다. Hill-Sachs병변 및 Bankart병변이 일부 있었으나 만족스러운 결과를 보였다.

반원상 연골판 손상에서의 관절선 압통의 해석 (The Interpretation of Joint Line Tenderness in Meniscal Injury)

  • 목적: 슬관절 손상의 많은 빈도를 차지하고, 가장 흔하게 수술의 원인이 되는 반월상 연골판 손상을 외래에서 쉽게 일차적으로 검사하는 관절선 압통의 유용성을 알아보고자 하였다. 대상 및 방법: 2005년 5월에서 2006년 6월까지 군병원에서 동일한 시술자에게서 반월상 연골판 손상이 의심되어 수술을 시행한 76명의 환자를 대상으로 관절선 압통(joint line tenderness)에 대한 민감도, 특이도, 정확도, 양성 예측율, 음성 예측율 등을 분석하였다. 결과: 관절선 압통의 민감도, 특이도, 정확도, 양성 예측을, 음성 예측율은 순서대로 78.8%, 32.6%, 52.6%, 47.3%, 66.7%로 나타났으며 외측과 내측 및 불안정성을 동반한 환자를 대상으로 동일하게 분석하여도 상기 결과와 비슷하게 높지 않은 결과를 나타내었다. 결론: 관절선 압통 단일 검사는 외래에서 손쉽게 행해지는 간편한 검사이나, 그 단독 검사로서는 유용성이 떨어지는 것으로 보인다. 그러므로, 여러가지 이학적 검사를 병행 시행하는 것이 좀 더 높은 진단율을 나타낼 것으로 보이고, 필요시에는 자기공명영상(MRI)이나 진단적 관절경 검사도 필요할 것으로 보인다.

무지 중수 수지 관절 척측 측부 인대 급성 완전 파열의 진단 및 치료에 대한 관절경의 유용성 (Efficacy of Arthroscopic Diagnosis and Treatment for Acute Complete Metacarpophalangeal Ulnar Collateral Ligament Tears of the Thumb)

  • 목적: 무지 중수 수지 관절 척측 측부 인대 손상은 완전 파열 시 파열된 인대의 연속성을 방해하는 Stener 병변 때문에 수술적 치료인 탐색술 및 봉합술을 시행하고 있다. 저자들은 무지 중수 수지 관절 척측 측부 인대 손상에서 관절경을 이용, 진단과 치료를 시행하고 관절경적 수기에 대한 효용성을 알아보고자 하였다. 대상 및 방법: 무지 중수 수지 관절 척측 측부 인대 완전 파열로 관절경적 진단 및 치료를 받고 1년 이상 추시 가능하였던 13예를 대상으로 하였다. 평균 연령은 35.6세었다. 관절경으로 척측 측부 인대 손상, Stener 병변 등을 진단하였으며 관절경을 이용하여 치료하고 그 결과를 관절 불안정성 여부, 무지 의 집게력 파악력, 관절 운동 범위 등으로 판정하였다. 결과: 13예중 5예에서 Stener 병변이 관찰되었다. 전예에서 추시상 무지 중수 수지 관절의 불안정성이 없었고 무지의 집게력 및 파악력 은 건측의 92%, 94%로 회복되었으며 중수 수지 관절 운동 범위는 평균 52 도로 건측과 비슷하였다. 결론: 무지 중수 수지 관절 척측 측부 인대 손상에서 관절경 수기는 Stener 병변의 확진 및 치료를 가능하게 하고 연부조직 손상을 최소화시켜 조기 기능 회복을 기대할 수 있게 하는 유용한 치료 방법으로 사료되었다.

Analytical method for the out-of-plane buckling of the jib system with middle strut

  • The jib system with middle strut is widely used to achieve the large arm length in the large scale tower crane and the deployability in the mobile construction crane. In this paper, an analytical solution for the out-of-plane buckling of the jib system with middle strut is proposed. To obtain the analytical expression of the buckling characteristic equation, the method of differential equation was adopted by establishing the bending and torsional differential equation of the jib system under the instability critical state. Compared with the numerical solutions of the finite element software ANSYS, the analytical results in this work agree well with them. Therefore, the correctness of the results in this work can be confirmed. Then the influences of the lateral stiffness of the cable fixed joint, the dip angle of the strut, the inertia moment of the strut, and the horizontal position of the cable fixed joint on the out-of-plane buckling behavior of the jib system were investigated.

Assessment of Fatigue and Fracture on a Tee-Junction of LMFBR Piping Under Thermal Striping Phenomenon

  • This paper deals with the industrial problem of thermal striping damage on the French prototype fast breeder reactor, Phenix and it was studied in coordination with the research program of IAEA. The thermomechanical and fracture mechanics evaluation procedure of thermal striping damage on the tee-junction of the secondary piping using Green's function method and standard FEM is presented. The thermohydraulic(T/H) loading condition used in the present analysis is the random type thermal loads computed by T/H analysis on the turbulent mixing of the two flows with different temperatures. The thermomechanical fatigue damage was evaluated according to ASME code section 111 subsection NH. The results of the fatigue analysis showed that fatigue failure would occur at the welded joint within 90,000 hours of operation. The assessment for the fracture behavior of the welded joint showed that the crack would be initiated at an early stage in the operation. It took 42,698.9 hours for the crack to propagate up to 5 mm along the thickness direction. After then, however, the instability analysis, using tearing modulus, showed that the crack would be arrested, which was in agreement with the actual observation of the crack. An efficient analysis procedure using Green's function approach for the crack propagation problem under random type load was proposed in this study. The analysis results showed good agreement with those of the practical observations.

산업용 로봇의 작업공간 제한이 없는 교시 방법 (Teaching Method Without Work Space Limit for Industrial Robot)

  • Teaching an industrial robot is still a dangerous and time-consuming process. It is expected that a robot can track a trajectory that is repeatedly taught by a human operator. Teaching a robot in joint space is easier than that in Cartesian space or a work space because the robot will never lose its stability when it is taught and operated in a joint space. However, it is very easy for a robot to lose its stability when it is taught in a work space. This is because of the singular points problem in kinematics for manipulators. Thus, experts should teach a given task to a robot in a careful manner. A new algorithm that avoids the problem of singular points is proposed. Using this proposed method, a user can freely teach a robot without the chance of instability in an entire work space.

  • This paper deals with determination of motions of a humanoid robot using genetic algorithm. A humanoid robot has some problems of the structural instability basically. So, we have to consider the stable walking gait in gait planning. Besides, it is important to make the smoothly optimal gait for saving the electric power. A mobile robot has battery to move autonomously. But a humanoid robot needs more electric power in order to drive many joints. So, if movements of walking joint don't maintain optimally, it is hard to sustain the battery power during the working period. Also, if a gait trajectory doesn't have optimal state, the expected lift span of joints tends to be decreased. Also, if a gait trajectory doesn't have optimal state, the expected lift span of joints tends to be decreased. To solve these problems, the genetic algorithm is employed to guarantee the optimal gait trajectory. The fitness functions in a genetic algorithm are introduced to find out optimal trajectory, which enables the robot to have the less reduced jerk of joints and get smooth movement. With these all process accomplished by PC-based program, the optimal solution could be obtained from the simulation. In addition, we discuss the design consideration fur the joint motion and distributed computation of tile humanoid, ISHURO, and suggest its result such as structure of the network and a disturbance observer.
