Overview of the Sonography of the Knee Joint (슬관절 초음파 개론)

  • Traditionally the diagnosis and treatment of the diseases of the knee is based on the findings of the x-rays and the MRI. The x-rays provide good information of the changes of the internal structure of the bone. However, there is a limitation in providing information of the soft tissue and the cartilage. The MRI is one of the most expensive diagnostic modalities and it can not give us a dynamic and real time information. The sonography has a role in diagnosis and treatment of the soft tissue disease and surface of the bone. It gives us a real time dynamic information and it is really cheap. In this article the sonographic findings of the normal and pathologic conditions of the knee joint are introduced in relation to the findings of the x-rays and the MRI.

Bilateral Checkrein Deformities of the Hallux Following Intramuscular Sarcoidosis: A Case Report (근육 내 사르코이드증 후 발생한 양측 무지의 체크레인 변형: 증례 보고)

  • This report presents a unique case of checkrein deformities in both halluces following isolated intramuscular sarcoidosis, a rare occurrence given the infrequent musculoskeletal involvement in sarcoidosis. Typically resulting from flexor hallucis longus tendon entrapment by scar tissue post-trauma, the checkrein deformity reported in this paper presented with unusual metatarsophalangeal joint flexion and interphalangeal joint extension during ankle dorsiflexion. A 49-year-old woman with a history of intramuscular sarcoidosis presented with a great toe deformity and discomfort while wearing shoes, leading to a diagnosis of dynamic deformity, possibly attributed to tendon tethering by sarcoidosis. Surgical treatments, including abductor hallucis muscle intratendinous tenotomy, flexor hallucis longus Z-plasty lengthening, Weil osteotomy, and Kirschner wire fixation, significantly improved the functional scores and patient discomfort. This report underscores the importance of recognizing dynamic deformities and the potential for rare diseases, such as sarcoidosis, to cause such conditions, highlighting the need for careful diagnosis and tailored surgical intervention for atypical checkrein deformities.

A Case of Disseminated Mycobacterium intracellulare Infection in an Immunocompromised Host (면역 저하 환자에서 발생한 파종성 Mycobacterium intracellulare 감염 1예)

  • We report a case of disseminated Mycobacterium intracellulare infection in a 31-year-old man who had been diagnosed as having dermatomyositis and systemic lupus erythematosus 3-years prior. The patient developed a left pleural effusion M. intracellulare was repeatedly isolated from the pleural fluid. After antimycobacterial treatment, the patient's pleural effusion resolved, but a left knee joint effusion developed newly and M. intracellulare was cultured from the joint fluid. At present, the patient has been taking antimycobacterial medication for 15 months but his left knee joint fluid remains positive for M. intracellulare. To our knowledge, this is the second reported case of disseminated NTM infection in a non-HIV infected patient in Korea.

A Web-based Survey for Assessment of Korean Medical Treatment Clinical Practice Patterns for Neck Pain and Cervical Intervertebral Disc Displacement (경항통 및 경추 추간판 탈출증의 한의진료 임상현황조사를 위한 웹기반 설문조사)

  • Objectives : While neck pain is highly prevalent in Korea, studies examining its Korean medical treatment are currently lacking. The aim of this study was to assess current Korean medical treatment practice patterns for neck pain. Methods : A preliminary questionnaire was developed to investigate current practice patterns of neck pain and cervical intervertebral disc displacement (IDD) treatment, and underwent further revision through external review. The final questionnaire was distributed as a web-based survey to 18,289 potential respondents by email. Results : The response rate was 3.34 %. Most participants replied that they received multiple Korean medicine interventions for neck pain treatment consisting of such methods as acupuncture, wet cupping, interferential current therapy and electroacupuncture. A total of 378 respondents acknowledged that diagnostic testing was needed for neck pain treatment. The most commonly used Korean Standard Classification of Diseases (KCD) principal diagnosis code for neck pain was M542, and for cervical IDD was M501. Conclusion : This survey study helps determine current practice patterns of neck pain, and recognizes the need for use of diagnostic devices in neck pain treatment. These results are further anticipated to provide basic data for clinical practice guidelines (CPGs) and future studies using Korean National Health Insurance and Health Insurance Review & Assessment Service data.

  • We can test for small structures located at the surface layer as high resolution ultrasonic techniques and small probes were developed. Accordingly ultrasound became a useful tool in the diagnosis and treatment of wrist diseases. It is important to understand the anatomical structures and apply clinical features as well as these technical aspects when using ultrasound. The purpose of this article is to evaluate the usefulness and approach of ultrasound about typical diseases according to the area of the wrist pain.

Clinical Observations on the Association Between Diagnosis of Lung Cancer and Serum Tumor Markers in Combination

  • Objective: To evaluate the association of a diagnosis of lung cancer and combined detection of serum carcinoembryonic antigen (CEA), carbohydrateantigen 19-9 (CA19-9), neuron specific enolase (NSE) as well as the cytokeratin 19 fragment (CYFRA21-1). Methods: Serum CEA, CA19-9, NSE and CYFRA21-1 were assessed in 150 patients with lung cancer, 100 patients with benign lung disease and 100 normal control subjects, and differences of expression were compared in each group, and joint effects of these tumor markers in the diagnosis of lung cancer were analyzed. Results: Serum CEA, CA19-9, NSE and CYFRA21-1 in patients with lung cancer were significantly higher than those with benign lung disease and normal controls (p<0.01). It is suggested that these four tumor markers combined together could produce a positive detection rate of 90.2%, significantly higher than that of any single test. Conclusion: Combination detection of CEA, CA19-9, NSE and CYFRA21-1 could significantly improve the sensitivity and specificity in diagnosis of lung cancer, and could be important in early detection.

Invasive Aspergillosis Involving the Lungs and Brain after Short Period of Steroid Injection: A Case Report

  • Invasive pulmonary aspergillosis (IPA) has emerged as a severe infection in patients with immunocompromised hosts. However, recently, several IPA cases, without an apparent predisposition to immunodeficiency, has been reported. A 72-year-old woman was admitted for evaluation of general weakness and poor oral intake. She reported no medical history, except for intraarticular injection of a corticosteroid for joint pain for the duration of two months. A chest radiography revealed multiple cavitary nodules in both lungs. Examination of specimens, obtained by percutaneous needle biopsy, led to a diagnosis of invasive aspergillosis. Brain magnetic resonance imagining revealed numerous peripheral thin enhancing cystic nodules in both cerebral hemispheres. We initiated intravenous administration of amphotercin B. However, the patient died after nine days. Here, we report an invasive aspergillosis case, which involves the lungs and brain after a short period of steroid injection.

A Clinical Survey of Patients of Neuro-Pain Clinic 1 Year Period (신경통증클리닉 환자의 1년간 통계 고찰)

  • 1,666 patients treated by nerve block from September 1994 to August 1995 we statistically analyzed according to sex, age, diseases, and kinds of nerve blocks. Most patients were in the range from 30 to 60 year old, with a distribution of 43.9% male and 56.1% female. Diseases and ailments were as follows: low back pain 30.6%, frozen shoulder 14.0%, facial spasm 10.0%, cervical syndrome 9.7%, headache 7.3%, and hyperhidrosis 7.2%. Most common nerve blocks were stellate ganglion block 30.9%, epidural block 25.6%, trigger point injection 16.1%, and suprascapular nerve block 6.7%. Nerve blocks under fluoroscopic guide were as follows: facet joint block 28.6%, spinal root block 22.9%, thoracic sympathetic ganglion block 21.7%, and lumbar sympathetic ganglion block 15.4%.

A Study for Indication and Adopted Techniques of Chuna Treatment-Focusing on Domestic Clinical Studies (추나치료의 적응증별 적용 기법에 관한 연구 - 국내 임상 논문을 중심으로-)

  • Objectives : The present study examines indications and treatment techniques of Chuna manual therapy. Methods : We searched the clinical studies on Chuna manual therapy through 5 korean web databases, 2 journals.(key word 'chuna') After selecting appropriate clinical studies(Bongchuna, massage, side effect, contraindication are excepted), we analyzed them according to diseases, Chuna techniques, type of clinical trials. Results : 101 studies are selected and analyzed. They studied about cervical(26%), thoracic(12%), lumbar (29%)spinal disorders, temporomandibular joint disorders(4%), pelvic region disorders(4%), upper(6%), lower extremity(5%) disorders and other diseases(15%). Spinal disorders are studied frequently. Mild techniques like supine position both hand cervical spine flexion distraction, JS supine position cervical spine distraction, flexion distraction technique are used more frequently than adjustment by thrust. Conclusions : We have to enhance education and training about techniques that are frequently used and studied. Research and improvement of infrequently used techniques are needed. In-depth research about side effects and contraindication is needed. Highquality clinical research and systematic review should be needed to prove the therapeutic effect of Chuna treatment.

Future of Scientific Research on Preventive Medicine in Korea (예방의학의 발전을 위한 학술활동)

  • The Korean Society of Preventive Medicine has undergone continuous development, after overcoming the difficult early years, in the 59 years since its establishment in 1947. It has repeatedly upgraded its quality and quantity of research with the first journal edition in 1968 and the continuous increase in publication numbers, scientific articles, joint research projects, intra-field exchanges and participation in various international scientific activities. In the future, we should gather a more extensive collection of opinions regarding the introduction of clinical preventive medicine specialists and prepare for the establishment of a training program for clinical specialists into a preventive medicine residency course. Moreover, we should raise interest in the importance of protecting individual information and maintaining medicine ethics. It's impossible to develop academic activities without cooperation. We need such cooperation with basic medical approaches across a wide range of fields. Furthermore, we should strengthen our cooperation with aspects of clinical and drug epidemiology in many fields including public health, statistics, and dietetics. Finally, we should raise the level of international cooperation with many countries, including North Korea, to prevent diseases and promote health. Preventive medicine is a science in which practice is as important as theory. We must aim to nurture preventive medicine specialists who practice in many areas of society with the goal of preventing diseases, promoting health, improving fertility, and securing healthy elderly life for individuals and the entire population. To this end, we will endeavor to promote both theoretical and practical components of academic development.
