말단결장루의 합병증 발생 영향 요인 (Affecting Factors of End Colostomy-Related Complications)

  • Purpose: This study was performed to investigate the type and frequency of end colostomy-related complications and to identify the risk factors for those complications. Methods: Retrospective analysis of medical records was made in 708 patients who underwent end colostomy in Samsung Medical Center between October 1994 and February 2005. The type was divided into stomal and peristomal complications: stomal complications included bleeding, necrosis, mucocutaneous separation, prolapsed stoma, retraced stoma, stenosis, and hyperplasia; peristomal complications did peristomal varix, peristomal hernia, irritant contact dermatitis, allergic contact dermatitis, maceration, folliculitis, hyperplasia, bacterial infection, candidal infection, malignancy in the peristomal area, mechanical damage and pyoderma gangrenosum. Results: For stomal complications, hyperplasia was most common(9.0%). For peristomal complications, irritant contact dermatitis was developed in 17.4%. Sex and BMI were risk factors for irritant contact dermatitis, hyperplasia, peristomal hernia, flat stoma, and retracted stoma. Conclusion: Teaching for preventing irritant contact dermatitis such as proper pouching and peristomal skin protection, and for comprehensive weight control should be emphasized on self care program for ostomates, while ostomy care nurse should take a careful consideration of preoperative ostomy site marking in female obese patients.

  • 목초액 외용 후 발생한 자극 피부염에 의한 홍반, 소양감, 통증 및 수장족저 농포증의 악화를 보인 환자에게 한방치료를 통해 유의성 있는 치료 효과를 확인할 수 있었으며 활발한 사례보고를 통해 부적절한 민간요법의 시행에 의한 폐해로부터 예방이 필요할 것으로 사료된다.

개에서 발생한 자극성 접촉성 피부염 1례 (Irritant Contact Dermatitis in a Dog)

  • 6년령의 중성화한 암컷 코커 스파니엘이 1개월 전부터 시작된 서혜부의 홍반과 가피, 소양감을 주증으로 본원에 내원하였다. 환축은 건성 피부에 대하여 보습을 목적으로 오트밀 샴푸와 보습용 스프레이를 수 개월간 사용하였다. 전혈구 검사와 혈청 생화학 검사결과 특이 소견은 관찰되지 않았다. 조직병리학적 검사 결과, 표피의 이상각화증과 표피증식증이 확인되었으며, 진피 상층부의 혈관주위 염증이 관찰되었다. 병력, 임상증상, 신체검사 및 혈액학적 검사, 피부기본 검사와 조직병리학적 검사를 통하여 자극성 접촉성 피부염으로 진단하였다. 자극성 접촉성 피부염의 원인으로 생각되는 보습용 스프레이의 사용을 중지하였으며 2주 후 서혜부의 홍반, 가피와 소양감이 개선되었다.

Nitric Oxide Signal Transduction and Its Role in Skin Sensitization

  • Nitric oxide (NO) is a signaling molecule that plays a crucial role in numerous cellular physiological processes. In the skin, NO is produced by keratinocytes, fibroblasts, endothelial cells, and immune cells and is involved in skin functions such as vasodilation, pigmentation, hair growth, wound healing, and immune responses. NO modulates both innate and adaptive immune responses. As a signaling molecule and cytotoxic effector, NO influences the function of immune cells and production of cytokines. NO is a key mediator that protects against or contributes to skin inflammation. Moreover, NO has been implicated in skin sensitization, a process underlying contact dermatitis. It modulates the function of dendritic cells and T cells, thereby affecting the immune response to allergens. NO also plays a role in contact dermatitis by inducing inflammation and tissue damage. NO-related chemicals, such as nitrofatty acids and nitric oxide synthase (NOS) inhibitors, have potential therapeutic applications in skin conditions, including allergic contact dermatitis (ACD) and irritant contact dermatitis (ICD). Further research is required to fully elucidate the therapeutic potential of NO-related chemicals and develop personalized treatment strategies for skin conditions.

Wet-work Exposure: A Main Risk Factor for Occupational Hand Dermatitis

  • Wet-work can be defined as activities where workers have to immerse their hands in liquids for >2 hours per shift, or wear waterproof (occlusive) gloves for a corresponding amount of time, or wash their hands >20 times per shift. This review considers the recent literature on wet-work exposure, and examines wet-work as a main risk factor for developing irritant contact dermatitis of the hands. The aim of this paper is to provide a detailed description of wet-work exposure among specific occupational groups who extensively deal with water and other liquids in their occupations. Furthermore, it highlights the extent and importance of the subsequent adverse health effects caused by exposure to wet-work.

Improvement of skin barrier function using lipid mixture

  • Dry skin is caused mainly by the perturbation of stratum corneum lipids which affected by ageing, change of season, excess use of surfactant and the effect of disease like atopic dermatitis and psoriasis. Intercellular lipid structures in stratum corneum are responsible for the barrier function of mammalian skin. The major lipd classes that can be extracted from stratum corneum are ceramides, cholesterol and fatty acid, which make up approximately 50, 25, 10 percent of the stratum corneum lipid mass, respectively. Small amount of cholesterol sulfate, phospholipids, glycosylceramide and cholesterol esters are also present. Recent studies have shown that application of one or two these lipids to the perturbed skin delays barrier recovery; only equimolar mixtures allow normal recovery. We observed that barrier recovery rate was improved in hairless mouse by topical application of single neutral lipids (ceramide, free fatty acid, cholesterol) and lipid mixtures. Whereas the application of single lipid didn’t allows a significant enhancement comparing with normal barrier repair, the equimolar mixtures of 3 components(including synthetic pseudoceramide PC104) improved barrier repair, as assessed by the transepidermal water loss. At clinical study to the volunteers aged over sixty, skin dryness recuperated by the increase of moisture(capacitance) and the reduction of scaling. Utilization of physiologic lipid mixture containing natural ceramides or synthetic pseudoceramide could lead to new forms of topical therapy for the dryness and dermatoses(e.g., psoriasis, atopic dermatitis and irritant dermatitis).

수부(手部) 건성습진(乾性濕疹)에 대한 한방치료(韓方治療) 증례(證例) 보고(報告) (A Case Study on Xerotic Eczema Treated by Oriental Medicine)

  • Eczema is a general term for skin diseases showing itching, vessiculopapular eruption, and erythema when acute, or lichenification and scale if chronic. It is classified as irritant dermatitis, atopic dermatitis, seborrheic dermatitis, neurodermatitis, nummular eczema, statis dermatitis, infective dermatitis, xerotic eczema, and pompholyx by afflicted regions and symptoms. Therapy for eczema is allopathic, and includes application of ice- pack to erythema, and application of emollients and steroids to the affected parts during subacute or chronic stages. Eczema is understood as ulceration(瘡), tinea(癬), wind syndrome(風證), and dryness syndrome(燥證) in terms of oriental medicine. Xerotic eczema is understood in dryness syndrome. Nourishing blood(養血) therapy is used. This is a case study on which xerotic eczema occurred on a patient's afflicted left palm and fingers after stroke. The patient's xerotic eczema was attributed to dryness syndrome. Therefore, Samul-tang(四物湯) and Saenghyeoryunbu-eum(生血潤膚飮) were used for nourishing blood. This therapy was effective on eczema and accompanying symptoms such as dizziness and numbness of the left hand.

은행엽(銀杏葉)의 가토(家兎) 피부(皮膚) 접촉성염증(接觸性炎症) 유발(誘發) 독성물질(毒成物質)에 관(關)한 연구(硏究) (Studies on toxic substances of Gingko leaves inducing contact dermatitis in rabbit)

  • Gingko biloba is one of the oldest trees on earth. It can be traced 300 million years or more into past. In recent years, this tree has become increasingly popular because of its longevity. Amount of comsumption of Gingko leaves in Korea is increased to produce drugs for blood circulating system's disorder. The workers who work at the company of product drug with Gingko leaves complain pruritus at belt position of west, however toxic substance from extracts of Gingko biloba was carried out to determine what kind of the toxic substance from extract could induce a contact dermatitis by means of histopathological study. Dried leaves of Korean G. biloba were exhaustively extracted with chloroform. The aqueous chloroform residue was extracted with hexane. After evaporation, the hexane extract was passed through a silica gel column chromatography, it was separated into 9 fractions by means of gradient solvent (benzene: acetone/ 98~93 : 2~7%). The 4th and 7th fraction were idendified as bilibol and gingkolic acid by silica gel thin layer chromatography and NMR spectrophotometer. The 4th and 7th fractions of Korean Gingko leaves have an irritant substances which can be induced ear swelling and increased thickness of stratum cornium of rabbit ear exposed with the fractions (3 time/3 day, $100{\mu}l$ of 50mg/ml ethyl alcohol). In the 4th fraction, the substance have induced the surface of ear to swell and mass of unknown cell like inflammatory cell were infiltrated within the stratum corneum, and the 7th fraction have the surface of ear to loss the hairs, and sebaceous gland and follicle of hair were infiltrated with inflammatory cells. and other fractions at all did not induced a certain contact dermatitis and also Deutschen drug of Gingko (named as Gerigeron) have induced the contact dermatitis such as dematitis of Korean Gingko extract.

실험동물에 대한 para-phenylenediamino을 함유하는 염모제의 피부 및 눈에 대한 시험 (A Study on the Skin and Eye Testing of para-phenylenediamine and commercial hairdyes containing paraphenylenediamine in animals)

  • Now para-phenylenediamine(PPDA) is generally used as component of most hairdyes because it can be used more conveniently and effectively than the others. But PPDA become known to cause side effects such as skin sensitization, contact dermatitis and eye irritation. So this study was done to restudy the safety of hairdyes containing PPDA. The results of experiment were as follows. 1. As a result of primary skin irritation test and eye irritation test in white rabbits, the solution containing 5% PPDA(in saline) were classified as weak irritant but four kinds of commercial hairdyes containing PPDA were proved not to have any irritation. 2. As a result of skin sensitization test in guinea pig, four kinds of commercial hairdyes were classified as Calss I(week irritant) but three kinds of commercial hairdyes induced skin sensitization for guinea pig to 5% in test groups. These results indicate that the actual toxicity of commercial hairdyes(four kinds) is negligible. But based on the results of experiment, it is thought that there is possibility of some commercial hairdyes to induce skin sensititization.
