토모테라피 치료 시 MVCT Image의 Slice Thickness 차이에 따른 선량 비교 (Extra Dose Measurement of Differential Slice Thickness of MVCT Image with Helical Tomotherapy)

  • 의료용 선형가속기 (linear accelerator)와 나선형 컴퓨터 단층 촬영 장치 (helical computed tomography scanner)의 결합 장치인 토모테라피는 세기 변조 방사선 치료 (intensity modulated radiation therapy(IMRT))의 큰 혁신을 이끌었다. 토모테라피 치료 과정에서, Megavoltage computed tomography (MVCT) 영상획득은 치료 환자의 정확한 자세 정렬을 위해 치료 전 또는 후에 이용된다. 그러나 이는 환자의 총 선량을 증가시키는 결과를 만들며, 본 연구는 이처럼 MVCT 영상 획득 시 증가되는 선량을 Cylindrical "Cheese" Phantom을 이용하여 측정, 비교하였다. 각각의 pitch 별로 (1, 2, 3 mm) 동일한 개수의 slice (10 slice), 그리고 동일한 length (약 9 cm)를 scanning하여, 이때의 선량 (MVCT Scanning Dose)을 A1SL ion chamber를 이용하여 측정하였다. 측정 결과 동일한 Slice 개수 (각각의 pitch 당 10개)일 때, MVCT scanning dose의 평균값은 각각 (pitch 1, 2, 3mm) 2.24 cGy, 1.02 cGy, 0.81 cGy가 측정되었다. 동일한 length에서 MVCT scanning dose의 평균값은 각각 (pitch 1, 2, 3mm) 2.47 cGy, 1.28 cGy, 0.88 cGy가 측정되었다. 이는 할당된 pitch와 scanning length가 MVCT scanning dose에 큰 영향을 미치는 가장 중요한 매개 변수임을 말하며, pitch는 MVCT scanning dose와 역비례 관계를 나타냈다. 때문에 적절한 pitch와 scanning length의 선택으로 치료 선량 외의 추가 선량을 최소로 줄여야 하겠다.

거대분자에 부분적으로 결합한 상자성 자기공명 조영제의 회전속도가 이완증강에 미치는 영향 (The Effects of Rotational Correlation Time of Paramagnetic Contrast Agents on Relaxation Enhancement: Partial Binding to Macromolecules)

  • Purpose : To evaluate the effect of rotational correlation time (${\tau}_R$) and the possible related changes of other parameters, ${\tau}_M,{\;}{\tau}_S,{\;}and{\;}(\tau}_V$ of gadolinium (Gd) chelate on T1 relaxation enhancement in two pool model. Materials and Methods : The NMRD (Nuclear Magnetic Relaxation Dispersion) profiles were simulated from 0.02 MHz to 800 MHz proton Larmor frequency for different values of rotational correlation times based on Solomon-Bloembergen equation for inner-sphere relaxation enhancement. To include both unbound pool (pool A) and bound pool (pool B), the relaxivity was divided by contribution from unbound pool and bound pool. The rotational correlation time for pool A was fixed at the value of 0.1 ns, which is a typical value for low molecular weight complexes such as Gd-DTPA in solution and ${\tau}_R$ for pool B was changed from 0.1 ns to 20 ns to allow the slower rotation by binding to macromolecule. The fractional factor of was also adjusted from 0 to 1.0 to simulate different binding ratios to macromolecule. Since the binding of Gd-chelate to macromolecule cab alter the electronic environment of Gd ion and also the degree of bulk water access to hydration site of Gd-chelate, the effects of these parameters were also included. Results : The result shows that low field profiles, ranged from 0.02 to 40 MHz, and dominated by contribution from bound pool, which is bound to macromolecule regardless of binding ratios. In addition, as more Gd-chelate bound to macromolecule, sharp increase of relaxivity at higher field occurs. The NMRD profiles for different values of ${\tau}_S$ show the enormous increase of low field profile whereas relaxivity at high field is not affected by ${\tau}_S$. On the other hand, the change in ${\tau}$V does not affect low field profile but strongly in fluences on both inflection fie이 and the maximum relaxivity value. The results shows a fluences on both inflection field and the maximum relaxivity value. The results shows a parabolic dependence of relaxivity on ${\tau}_M$. Conclusion : Binding of Gd-chelate to a macromolecule causes slower rotational tumbling of Gd-chelate and would result in relaxation enhancement, especially in clinical imaging field. However, binding to macromolecule can change water enchange rate (${\tau}_M$) and electronic relaxation ($T_le$) vis structural deformation of electron environment and the access of bulk water to hydration site of metal-chelate. The clinical utilities of Gd-chelate bound to macromolecule are the less dose requirement, the tissue specificity, and the better perfusion and intravascular agents.

EPID의 적정관리 도구로서의 유용성에 관한 연구 (The useage of the EPID as a QA tools)

  • Purpose : The aim of this study is to conform the possibility of the liquid type EPID as a QC tools to clinical indication and of replacement of the film dosimetry. Aditional aim is to describe a procedure for the use of a EPID as a physics calibration tool in the measurements of radiation beam parameters which are typically carried out with film. Method & Materials : In this study we used the Clinac 2100c/d with EPID. This system contains 65536 liquid-filled ion chambers arranged in a $256{\times}256$ matrix and the imaging area is $32.5{\times}32.5cm$ with liquid layer thickness of 1mm. The EPID was tested for different field sizes under typical clinical conditions and pixel values were calibrated against dose by producing images using various thickness of lead attenuators(lead step wedge) using 6 & 10MV x-ray. We placed various thickness of lead on the table of linear accelerator and set the portal vision an SDD of 100cm. To acquire portal image we change the field size and energy, and we recorded the average pixel value in a $3{\times}3$ pixel region of interest(ROI) at field center was recorded. The pixel values were also measured for different field sizes in order to evaluate the dependence of pixel value on x-ray energy spectrum and various scatter components. Result : The EPID, as a whole, was useful as a QA tool and dosimetry device. In mechanical check, cross-hair centering was well matched and the error was less than ?2mm and light/radiation field coincidence was less than 1mm also. In portal dosimetry the wider the field size the the higher the pixel value and as the lead thickness increase, the pixel value was exponentially decreased. Conclusions : The EPID was very suitable for QA tools and it can be used to measure exit dose during patients treatment with reasonable accuracy. But when indicate the EPID to clincal study deep consideration required

세로토닌 5-HT1A수용체 방사성 추적자 18F-Mefway의 합성과 소동물 뇌 PET 연구 (Synthesis and Small Animal Brain PET Study of a Serotonin Receptor Radiotracer, 18F-Mefway)

  • 세로토닌 수용체 5-$HT_{1A}$에 대한 방사성추적자로 알려진 $^{18}F$-mefway를 개선된 방법으로 합성하고, 소 동물 뇌 microPET 연구를 통해서 이 수용체에 선택적으로 결합하는 지를 확인하려고 한다. 기존 합성 방법을 수정하여 전구체를 합성하고, $[^{18}F]^-$$130^{\circ}C$에서 30분간 교반하여 $^{18}F$-mefway를 합성하여, Solid phase extraction(SPE)과 HPLC를 이용해 정제한 다음, 소동물 뇌의 microPET 다이나믹 영상을 얻었다. Oxalyl chloride와 LAH/diethyl ether을 사용해서 기존 합성수율 9%에서 34%로 $^{18}F$-mefway 전구체를 향상된 수율로 합성할 수 있는 개선된 방법을 확립하였다. 이렇게 합성된 $^{18}F$-mefway는 세로토닌 5-$HT_{1A}$수용체 영상용 방사성 의약품으로서 신경정신질환 연구에 이용될 수 있을 것으로 사료된다.

말디토프 질량분석을 이용한 고분자의 특성분석 (Analysis of Polymer Characteristics Using Matrix-assisted Laser Desorption/Ionization Time-of-flight Mass Spectrometry)

  • 최근에, 질량분석기술의 폴리머 분석에의 응용은 MALDI-TOF MS 개발 이후 급속도로 발전하였다. 이 리뷰 논문은 현재까지 연구된 MALDI-TOF MS의 폴리머 특성분석에의 응용에 관한 최신 논문을 정리하였다. MALDI-TOF MS는 바이오 폴리머와, 합성 폴리머의 평균분자량 분석, 폴리머의 시퀀스 분석을 통한 구조의 해석, 모노머의 조성분석에까지 이용되고 있다. 엔드그룹의 특성과 농도를 분석하는 연구도 많이 진행되었고, 복잡한 폴리머의 분자량의 분석에는 SEC와 MALDI-TOF MS를 연결한 분석법을 추천한다. MALDI에 tandem MS를 결합한 분석기술이나, 이온 모빌리티를 응용한 질량분석기, TOF-SIMS, MALDI-TOF-Imaging 기술도 급격히 발전하고 있으며, 이의 폴리머 특성분석에의 응용은 별도의 분리기술이 필요 없어 앞으로 더 많이 이용될 것으로 생각된다. 분자량, 시퀀스, 그리고 모노머의 조성을 정확하게 계산해주는 소프트웨어와 고분자량(> 100 kDa)의 분석을 가능하게 해주는 기술이 개발된다면, 폴리머를 연구하는 과학자들에게 MALDI-TOF MS의 이용은 문제점을 해결하고, 목적하는 폴리머를 합성하는 데 중요한 수단이 될 것이다.

쥐를 이용한 새로운 가돌리늄 조영제 Gd-DTPA-bis(amide)(L3)의 약동학 및 생체내 분포 특성에 대한 연구 (Pharmacokinetics and Bio-distribution of New Gd-complexes of DTPA-bis (amide) (L3) in a Rat Model)

  • 목적: DTPA-bis-amide (L3) Gd(III) 복합체의 약동학 및 생체내 분포특성을 조사하고자 하였다. 대상과 방법: Sprague-Dawley 쥐의 꼬리정맥을 통하여 0.1 mmol Gd/kg의 DTPA-bis-amide (L3) Gd(III) 복합체를 주사한 후 약동학 및 생체내 분포 특성을 조사하였다. 조직 및 장기 그리고 혈중 Gd 농도를 ICP-AES를 사용하여 정량 측정하였으며 혈중 약동학적 파라미터는 two-compartment 모델을 사용하여 계산하였다. 결과: DTPA-bis-amide (L3) Gd(III) 복합체의 혈중 반감기는 ${\alpha}$-phase의 경우 $2.286{\pm}0.11$ min 그리고 ${\beta}$-phase의 경우 $146.1{\pm}7.5$ min 이었다. 생체내 분포특성은 주요 배설 경로는 신장을 통한 배설이 주요 경로였으며 일부 담도계를 통한 배설이 확인되었다. 또한 Gd(III)의 농도비는 다른 장기에 비해 간과 비장에서 매우 높은 농도를 보였으며 일부 Gd(III)이 정맥주사 후 7일후에 혈액 및 일부 장기에서 매우 소량 검출되었다. 결론: 새로운 조영제인 DTPA-bis-amide (L3) Gd(III) 복합체는 혈중 잔류시간이 상대적으로 길면서도 체내 축적없이 체외로 배출되는 특성을 나타내었다. 따라서 친지질성과 친수성의 균형이 잘 이루어진 가돌리늄 조영제의 합성은 향후 blood pool MRI 조영제로써의 가능성이 매우 높은 것으로 판단된다.

형광화학센서를 이용한 살아있는 세포 내에서의 중금속이온검출 (Heavy Metal Ion Detection in Living Cell Using Fluorescent Chemosensor)

  • 세포내의 중금속이온의 형광검출은 유기분자화학과 세포생물학분야에서 높은 관심을 갖는다. 이 연구는 형광화학센서(FS)를 이용한 Hg$^{2+}$ 과 Zn$^{2+}$의 세포 내 검출을 목적으로 하였다. FS는 약한 형광을 나타내지만 Zn$^{2+}$와 결합 시에는 강한 방출 형광은 낸다. 2FS/Zn$^{2+}$의 형광증가는 FS-Hg$^{2+}$결합의 형성할 때 Hg$^{2+}$ 1당량만 추가에도 완전한 형광감소를 나타낼 수 있었다. 네가지의 세포주(LLC-MK2, Hela, HT29 and AMC-HN3)는 공촛점 현미경에 대한 형광이미지를 위하여 사용하였다. 세포생존능은 LLC-MK2 세포주에 FS, Zn$^{2+}$, FS-Zn$^{2+}$, Hg$^{2+}$의 처리 후에 평가하였다. FS의 세포독성능은 80%이상의 생존능을 보였다. 본 연구에서는 FS가 살아있는 세포내의 Hg$^{2+}$과 Zn$^{2+}$의 선택적 이미지를 검출할 수 있음을 보여주었다.

Dosimetric Study Using Patient-Specific Three-Dimensional-Printed Head Phantom with Polymer Gel in Radiation Therapy

  • Purpose: In this study, we aimed to manufacture a patient-specific gel phantom combining three-dimensional (3D) printing and polymer gel and evaluate the radiation dose and dose profile using gel dosimetry. Methods: The patient-specific head phantom was manufactured based on the patient's computed tomography (CT) scan data to create an anatomically replicated phantom; this was then produced using a ColorJet 3D printer. A 3D polymer gel dosimeter called RTgel-100 is contained inside the 3D printing head phantom, and irradiation was performed using a 6 MV LINAC (Varian Clinac) X-ray beam, a linear accelerator for treatment. The irradiated phantom was scanned using magnetic resonance imaging (Siemens) with a magnetic field of 3 Tesla (3T) of the Korea Institute of Nuclear Medicine, and then compared the irradiated head phantom with the dose calculated by the patient's treatment planning system (TPS). Results: The comparison between the Hounsfield unit (HU) values of the CT image of the patient and those of the phantom revealed that they were almost similar. The electron density value of the patient's bone and brain was 996±167 HU and 58±15 HU, respectively, and that of the head phantom bone and brain material was 986±25 HU and 45±17 HU, respectively. The comparison of the data of TPS and 3D gel revealed that the difference in gamma index was 2%/2 mm and the passing rate was within 95%. Conclusions: 3D printing allows us to manufacture variable density phantoms for patient-specific dosimetric quality assurance (DQA), develop a customized body phantom of the patient in the future, and perform a patient-specific dosimetry with film, ion chamber, gel, and so on.

Planning and Dosimetric Study of Volumetric Modulated Arc Based Hypofractionated Stereotactic Radiotherapy for Acoustic Schwannoma - 6MV Flattening Filter Free Photon Beam

  • Background: The purpose of this study was to assess the dosimetric and clinical feasibility of volumetric modulated arc based hypofractionated stereotactic radiotherapy (RapidArc) treatment for large acoustic schwannoma (AS >10cc). Materials and Methods: Ten AS patients were immobilized using BrainLab mask. They were subject to multimodality imaging (magnetic resonance and computed tomography) to contour target and organs at risk (brainstem and cochlea). Volumetric modulated arc therapy (VMAT) based stereotactic plans were optimized in Eclipse (V11) treatment planning system (TPS) using progressive resolution optimizer-III and final dose calculations were performed using analytical anisotropic algorithm with 1.5 mm grid resolution. All AS presented in this study were treated with VMAT based HSRT to a total dose of 25Gy in 5 fractions (5fractions/week). VMAT plan contains 2-4 non-coplanar arcs. Treatment planning was performed to achieve at least 99% of PTV volume (D99) receives 100% of prescription dose (25Gy), while dose to OAR's were kept below the tolerance limits. Dose-volume histograms (DVH) were analyzed to assess plan quality. Treatments were delivered using upgraded 6 MV un-flattened photon beam (FFF) from Clinac-iX machine. Extensive pretreatment quality assurance measurements were carried out to report on quality of delivery. Point dosimetry was performed using three different detectors, which includes CC13 ion-chamber, Exradin A14 ion-chamber and Exradin W1 plastic scintillator detector (PSD) which have measuring volume of $0.13cm^3$, $0.009cm^3$ and $0.002cm^3$ respectively. Results: Average PTV volume of AS was 11.3cc (${\pm}4.8$), and located in eloquent areas. VMAT plans provided complete PTV coverage with average conformity index of 1.06 (${\pm}0.05$). OAR's dose were kept below tolerance limit recommend by American Association of Physicist in Medicine task group-101(brainstem $V_{0.5cc}$ < 23Gy, cochlea maximum < 25Gy and Optic pathway <25Gy). PSD resulted in superior dosimetric accuracy compared with other two detectors (p=0.021 for PSD.

Purinergic Receptors Play Roles in Secretion of Rat von Ebner Salivary Gland

  • The effects of adenosine triphosphate(ATP) on salivary glands have been recognized since 1982. The presence of purinergic recepetors(P2Rs) that mediate the effects of ATP in various tissues, including parotid and submandibular salivary gland, has been supported by the cloning of receptor cDNAs and the expression of the receptor proteins. P2Rs have many subtypes, and the activation of these receptor subtypes increase intracellular $Ca^{2+}$, a key ion in the regulation of the secretion in the salivary gland. The apical pores of taste buds in circumvallate and foliate papillae are surrounded by the saliva from von Ebner salivary gland(vEG). Thus, it is important how the secretion of vEG is controlled. This study was designed to elucidate the roles of P2Rs on salivary secretion of vEG. Male Sprague-Dawley rats (about 200 g) were used for this experiment. vEG-rich tissues were obtained from dissecting $500-1,000\;{\mu}m$ thick posterior tongue slices under stereomicroscope view. P2Rs mRNA in vEG acinar cells were identified with RT-PCR. To observe the change in intracellular $Ca^{2+}$ activity, we employed $Ca^{2+}-ion$ specific fluorescence analysis with fura-2. Single acinar cells and cell clusters were isolated by a sequential trypsin/collagenase treatment and were loaded with $10\;{\mu}M$ fura -2 AM for 60 minutes at room temperature. Several agonists and antagonists were used to test a receptor specificity. RT-PCR revealed that the mRNAs of $P2X_4$, $P2Y_1$, $P2Y_2$ and $P2Y_3$ are expressed in vEG acinar cells. The intracellular calcium activity was increased in response to $10\;{\mu}M$ ATP, a P2Rs agonist, and 2-MeSATP, a $P2Y_1$ and $P2Y_2R$ agonist. However, $300\;{\mu}M\;{\alpha}{\beta}-MeATP$, a $P2X_1$ and $P2X_3R$ agonist, did not elicit the response. The responses elicited by $10\;{\mu}M$ ATP and UTP, a $P2Y_2R$ agonists, were maintained when extracellular calcium was removed. $10\;{\mu}M$ suramin, a P2XR antagonist, and reactive blue 2, a P2YR antagonist, partially blocked ATP-induced response. However, when extracellular calciums were removed, suramin did not abolish the responses elicited by ATP. These results suggest that P2Rs play an important role in salivary secretion of vEG acinar cells and the effects of ATP on vEG salivary secretion may be mediated by $P2X_4$, $P2Y_1$, $P2Y_2$, and/or $P2Y_3$.
