  • Streptococus pneumoniae is an important cause of invasive infections as well as non-invasive infections such as acute otitis media and sinusitis both in children and adults. Resistance of S. pneumoniae to multiple antimicrobials is increasing and poses therapeutic challenges, and prevention became more important. 23-valent polysaccharide vaccine has been used for the last several decades, but is not effective in children <2 years of age, the highest risk group of invasive diseases. Recently, a 7-valent pneumococcal protein conjugate vaccine(PCV) which is effective in infants and young children has been developed. The efficacy of PCVs against invasive pneumococcal disease and pneumonia is well established and is documented in several well-conducted studies. However, the effect of PCVs on otitis media is less obvious and more complex. PCVs clearly reduce diseases caused by vaccine-type(VT) pneumococci, but replacement of VT serotypes by non-VT serotypes in nasopharyngeal carriage of S. pneumoniae is responsible for the increase in acute otitis media caused by non-VT serotypes. Three years after introduction of PCV in the US, some increase of invasive infections with serotype 19A possibly due to serotype switching within certain vaccine type strains has been noted. Since most antibiotic-resistance in S. pneumoniae is confined to VT serotypes, vaccine use also reduces antibiotic resistance. With development of PCV, there was a great advance in the prevention of pneumococcal diseases, but replacement with potential virulent organisms and development of antibiotic resistance in non-VT pneumococci is a possibility that needs careful monitoring.

  • 의학의 발달과 더불어 종양에 대한 국소치료는 광범위근치술(extended radical surgery) 로부터 기능보존술(function preserving surgery)로 발달하고 이것은 다시 최소침습술(minimal-invasive surgery)로 발전하여 왔다. 그러나 최근 들어서는 기술의 혁신적인 진보로 비침습적수술법(non-invasive surgery)이 개발되어 감마나이프(gamma knife), 사이버나이프(cyber knife), 및 하이푸나이프(HIFU knife) 등이 출현하게 되었다. 본 논문에서는 이들 중 하이푸나이프를 이용한 체외강력집속초음파치료(extracorporeal high intensity focused ultrasound therapy)에 대하여 발달사, 치료기 구조 및 치료과정, 조직학적 변화와 기전, 임상적용, 장단점 및 전망 등을 살펴보고자 한다.

  • This is a case report of invasive aspergillosis of the right maxillary antrum and the left lung in a 43 year-old male patient with diabetes mellitus and liver cirrhosis. The results were as follows. 1. Invasive aspergilliosis should be considered in compromised patients who have nasal symptomatology, infraorbital swelling and pain. 2. Invasive aspergillosis is a rapidly progressive mycotic infection of the sinus which may extend to the orbit, nasal cavity and cheek. 3. Surgical intervention is particularly urgent in orbital aspergillosis in patient in whom the eyesight is still preserved. 4. Diagnosis depends upon pathological demonstration of tissue invasion by organism with the typical morphology of aspergillus species. 5. Long-term antifungal drugs should be administrated postoperatively in pathint with invasive aspergillosis.

미주신경 자극을 위한 새로운 방법 : 비침습적 TENS 적용에 대한 미주신경 자극의 치료적 효과 (New Methods of Vagus Nerve Stimulation : Therapeutic Effects of Non-Invasive Vagus Nerve Stimulation by TENS Application)

  • Purpose: The purpose of this study was to investigate the therapeutic effect of non-invasive vagus nerve stimulation by transcutaneous electrical nerve stimulation application on the autonomic nervous system of human body. Methods: Participants were seventeen healthy adults. Standard deviation of all normal N-N intervals(SDNN), root mean square of successive differences(RMSSD), low frequency(LF), high frequency(HF) were compared in pre and post Mean values after intervention. Data were analyzed in Wilcoxon's signed-ranks test. Results: The results of this study is that sistolic blood pressure and pulse rate decreased mean value after non-invasive vagus nerve stimulation by transcutaneous electrical nerve stimulation. High frequency, low frequency, SDNN, RMSSD increased mean value in heart rate variability after intervention. But that is not significant except for SDNN. Conclusion: Non-invasive vagus stimulation by transcutaneous electrical nerve stimulation effect on parasympathetic nerve stimulation, and then it might be effective method for autonomic nerve balance control.

Invasive Aspergillosis Involving the Lungs and Brain after Short Period of Steroid Injection: A Case Report

  • Invasive pulmonary aspergillosis (IPA) has emerged as a severe infection in patients with immunocompromised hosts. However, recently, several IPA cases, without an apparent predisposition to immunodeficiency, has been reported. A 72-year-old woman was admitted for evaluation of general weakness and poor oral intake. She reported no medical history, except for intraarticular injection of a corticosteroid for joint pain for the duration of two months. A chest radiography revealed multiple cavitary nodules in both lungs. Examination of specimens, obtained by percutaneous needle biopsy, led to a diagnosis of invasive aspergillosis. Brain magnetic resonance imagining revealed numerous peripheral thin enhancing cystic nodules in both cerebral hemispheres. We initiated intravenous administration of amphotercin B. However, the patient died after nine days. Here, we report an invasive aspergillosis case, which involves the lungs and brain after a short period of steroid injection.

침습적 처치에 대한 교육 프로그램이 입원환아 어머니의 불안 및 지지행위에 미치는 영향 (The Effects of Invasive Procedure Education Program on the Anxiety and Supportive Behavior of Mothers with Hospitalized Children)

  • Purpose: This study was to examine how an invasive procedures education program affected the anxiety and supportive behavior of mothers with hospitalized children, and to help provide an efficient to improve their maternal role performance by boosting their supportive behavior. Method: To evaluate their anxiety, Spilberger(1972)'s Anxiety Inventory that was translated by Kim Jeong-taek, et al.(1978) was employed, and Weon Dae-yeoung(1999)'s modified version of Melnyx(1994)'s Index of Parent Support During Instructive Procedure was utilized to assess their supportive behavior. The collected data were analyzed with SPSS program. Statistical data on real number and percentage were acquired, and $x^2$-test and t-test were conducted. Result: There was a more favorable change in the anxiety(trait anxiety and state anxiety) of the experimental group that participated in an invasive procedures education program(a mean of -0.14) than in that of the control group that didn't(a mean of 0.16). The experimental group(an average of 4..65) outstripped the control group(a mean of 3.42) in that regard. Conclusions: The invasive procedures education program that catered to the mothers with hospitalized children turned out to be effective in easing their anxiety and enhancing their supportive behavior.

서울시청 옥상정원 '초록뜰' 모니터링을 통한 식재식물과 이입식물의 관리방안에 관한 연구 (A Study of Management Method of Planted Plants and Invasive Plants through Monitoring on Rooftop Garden "Choroktteul" in Seoul City Hall)

  • The management methods of rooftop greening must be get ready without delay, Because rooftop greening has been generalized owing to the supporting project of The Seoul Metropolitan Government. Because most of the rooftop greening system is a recently-constructed low-management type, management methods should be made to suit it. This study is intended to set up a program for low-management rooftop greening based on "Choroktteul", the low management type of rooftop peening on the roof of one of the buildings in Seoul City Hall. Through monitoring, we found that most of the planted plants grew well but a few species needed physical management. We also found that invasive species(62 sp.) were 1.5 times as many as the planted plants. Most of the invasive plants were exotic species that need to be managed. The physical management methods were "pulling up" and "cutting". These methods should be carried out according to the growth stage. It was judged that a biological-ecological management program should be prepared through the long-period of monitoring and experiment. System methods such as guidelines of management program through monitoring was also required due to the emphasis on public ownership among the managers of the invasive species.

Fluconazole prophylaxis against invasive candidiasis in very low and extremely low birth weight preterm neonates: a systematic review and meta-analysis

  • Background: Evidence shows that fluconazole prophylaxis is an effective treatment against invasive fungal infections in preterm neonates, however, the most efficient schedule of fluconazole prophylaxis for the colonization and mortality of invasive candidiasis (IC) is unknown. Purpose: This systematic review and meta-analysis aimed to assess the efficiency of different prophylactic fluconazole schedules in controlling IC colonization, infection, and mortality in very low birth weight (VLBW) and extremely low birth weight (ELBW) infants in neonatal intensive care units. Methods: We searched the PubMed, Scopus, Embase, and Cochrane databases using the keywords "candida," "invasive candidiasis," "IC," "fluconazole prophylaxis," "preterm infants," "very low birth weight infants," "VLBW," "extremely low birth weight," and "ELBW." Results: Mortality was significantly decreased in a meta-analysis of studies using different fluconazole prophylaxis regimens. The meta-analysis also indicated a significant decrease in the incidence of IC-associated mortality in ELBW infants using the same fluconazole prophylaxis schedules. Conclusion: Future studies should explore the effectiveness of other different fluconazole prophylaxis schedules on IC colonization, infection, and mortality.

요추부 최소침습적 시술 후 길랑-바레 증후군: 증례보고 (Guillain-Barre Syndrome after Minimally Invasive Lumbar Procedure: A Case Report)

  • Guillain-Barre syndrome (GBS) after trauma and general orthopedic surgery is rare. A 74-year-old woman showed ascending paralysis symmetrically, dysarthria, dysphagia and areflexia on 14 days after minimally invasive endoscopic thermoannuloplasty on L4-5 level. Brain and lumbar magnetic resonance imaging demonstrate no abnormal findings. The electrodiagnostic study showed prolonged distal motor, sensory latencies and F-wave latencies and reduced amplitude of compound muscle action potential in nerves of upper and lower extremities. In the cerebrospinal fluid (CSF) examination, total protein and IgG were increased. We diagnosed Guillain-Barre Syndrome based on clinical features, electrodiagnostic study and CSF examination and the patient improved symptoms after immunoglobulin injection and rehabilitation. Because the occurrence of GBS after minimally invasive procedure has not been reported, we report a case of GBS after minimally invasive procedure with literature review.

An overlooked invasive alien plant of Jejudo Island: Commelina caroliniana (Commelinaceae)

  • Invasive alien species management is pivotal for biodiversity conservation. Commelina caroliniana Walter, from the family Commelinaceae, is an alien plant native to the Himalayas and India, but it has been widely introduced around the world, including in the United States, Brazil, Philippines, and Japan. In Korea, the first population was found growing adjacent to agricultural land and farm roads on Jejudo Island, and field observations confirmed the presence of at least nine populations there. It is similar morphologically to C. diffusa Burm. f. but can be distinguished by involucral bracts that are ciliate at the base, hairs on the peduncle and obsolete upper cincinnus, brown spots on its 4-lobed antherode, and seed surfaces that are smooth to slightly alveolate. It was determined to have an invasiveness low score of 8 according to the Korean 'Invasive Alien Plant Risk Assessment', suggesting that it may spread to natural habitats. Although the current distribution of C. caroliniana is restricted to Jeju-si, it has spread dramatically in many other areas of the world. At present, it has had a limited impact on the local environment, but local and regulatory authorities should pay close attention to this plant and take measures to prevent its expansion in the future.
