Comparison Study with Pain Assessment Tools of Pain Measurement in Children for Intravenous Catheter Placement in the Computed Tomography (CT 검사에서 소아 환자의 정맥 카데터 삽입 통증에 대한 측정 도구 비교)

  • Needle phobia of medical devices is a significant problem in children patients. We conducted study in 130 pediatric patients aged $8{\sim}13$(mean age 10.5) who had venipuncture performed in a computed tomography. This review aims to explore the research available relating to three commonly used pain rating scales of children, the visual analogue scale(VAS), numeric pain rating scale(NPRS) and Wong-Baker faces pain scale(WBFPS) with scores. Its validity is supported by a strong positive correlation with the three-pain rating scales(correlations ranging from 0.70 to 0.92) measure in children. There were no significant differences between the means on the VAS and either of the pain rating scales. It has the advantage of being suitable for use with the most widely used metric for scoring($0{\sim}10$), and conforms closely to a linear interval scale. These scales presented moderate to good correlation and moderate agreement, sufficient for valid use in children. All three pain-rating scales are valid, reliable and appropriate for use in intravenous catheter placement.

An Experimental Comparative Study of Radiography, Ultrasonography and CT Imaging in the IV Catheter Fragment (정맥내 카테터 조각의 엑스선, 초음파 및 CT 영상의 실험적 비교 연구)

  • The objective of this study was to detect the fragments generated during IV (intravenous) catheter injection of contrast medium and drug administration in a clinical setting and removal was performed by experimentally producing a phantom, and to compare the radiography, ultrasonography, and multi-detector computed tomography (MDCT) imaging and radiation dose. A 1 cm fragment of an 18 gage Teflon$^{(R)}$ IV catheter with saline was inserted into the IV control line. Radiography, CT, and ultrasonography were performed and radiography and CT dose were calculated. CT and ultrasonography showed an IV catheter fragment clinically and radiography showed no visible difference in the ability to provide a useful image of an IV catheter fragment modality (p >.05). Radiography of effective dose ($0.2139mSv{\cdot}Gy^{-1}{\cdot}cm^{-2}$) form DAP DAP ($0.93{\mu}Gy{\cdot}m^2 $), and dose length product (DLP) ($201mGy{\cdot}cm$) to effective dose was calculated as 0.483 mSv. IV catheter fragment were detected of radiography, ultrasonography and CT. These results can be obtained by menas of an excellent IV catheter fragment of detection capability CT. However, CT is followed by radiation exposure. IV catheter fragment confirming the position and information recommend an ultrasonography.

Respiratory Depression After the Second Dose of Intravenous Tramadol in a Rectal Cancer Patient with Normal Renal Function: A Clinical Case Report (정상 신기능 직장암 환자에서 Intravenous Tramadol 이회 투여 후 호흡곤란 발생 증례 보고)

  • We report an unusual case of respiratory depression and prolonged apnea after the second dose of 100-mg intravenous tramadol. Due to continuous pain at foley catheter keep site, intravenous tramadol was administered to the patient. Soon after the second dose of tramadol injection, the patient became apneic. The patient did not respond to verbal command and started exhibiting oxygen desaturation. The patient was quickly treated with 100% oxygen, and it took 4 hours for the spontaneous respiration to return to regular. This case report demonstrates that even two doses of tramadol administered intravenously may manifest as sudden and prolonged apnea. Respiratory depression with tramadol has been reported in patients with impaired renal functions and Cytochrome P(CYP) 2D6 gene duplication.

[ $N_2O-O_2$ ] INHALATION SEDATION WITH SUCTION CATHETER IN FULL MOUTH BREATHING PATIENTS (구호흡 소아환자에서 흡인도관을 이용한 $N_2O-O_2$ 진정)

  • There are some problems in inhalation sedation of non-cooperative pediatric patients. Usually the pediatric patients reject the nasal hood and there's no cooperation for administration of nitrous oxide gas. In mouth breathing patient, other technics of sedation such as intravenous or oral sedation or general anesthesia were recommended. Common causes of mouth breathing are common cold, allergic rhinitis, sinus problem, anatomical disorder, and habitual mouth-breathing. However in some patient not indicated the general anesthesia and high failure rate in oral and intravenous sedation. Administration of $N_2O-O_2$ with suction catheter was applied in full mouth breathing patient. Clinically effective sedation were occurred during procedure about 45 to 55 minutes. There's no any side effects by $N_2O-O_2$ inhalation sedation. The patients awoke at the end of the procedure and received 100% oxygen for 2-3 minutes. There's still some problems in use of the suction catheter such as air pollution of operation theater and elevate arterial carbon dioxide tension.

The Predictive Strength of Students' Self-Efficacy, Problem Solving Skills to Perform Catheter Care

  • Purpose: The aim of this study was to determine the predictive strength of students' self-efficacy, problem-solving skills, and other characteristics in performing intravenous practices and monitor phlebitis and infiltration. Methods: This cross-sectional study was carried out with 736 third and fourth-year students studying at the Health Sciences Faculties of Balikesir and Sakarya universities. The data were collected using the Self-Efficacy Scale, Problem-Solving Inventory and Catheter Care and Infiltration Monitoring Questionnaire. Results: The participants' mean Catheter Care and Infiltration Monitoring Questionnaire score significantly and positively correlated with their mean Self-Efficacy Scale score on a moderate level (r=.25; p<.001) but significantly and negatively correlated with their mean Problem-Solving Inventory score on a moderate level (r=-.21; p<.001). In other words, because a low Problem-Solving Inventory score indicates that the person's problem-solving skill is high, the Care and Infiltration Monitoring Questionnaire score increased as the problem-solving skill increased. While the Self-Efficacy Scale predicted the year of study and catheter care and infiltration monitoring variables positively, the Problem-Solving Inventory predicted the satisfaction with the profession variable negatively. Conclusion: Self-efficacy, problem-solving, liking the profession, and year of study predict success in catheter care and infiltration monitoring. For this reason, guidance may be provided in the development of a comprehensive education system toward increasing students' problem-solving skills, self-efficacy, and professional knowledge and skills.

Continuous Irrigation of Brain Abscess Using a Double Lumen Catheter - Technical Note - (이중 도관을 이용한 뇌농양의 지속적 세척술 - 수술 수기 -)

  • We present a modified method for the treatment of brain abscess. The double lumen extraventricular drainage (EVD) catheter which was developed for the intracerebral hematoma management, was applied for the treatment of brain abscess drainage. We placed the double lumen EVD catheter into the abscess cavity by free-hand technique and irrigated the abscess cavity continuously with antibiotics solution for 7 days. Simultaneous intravenous antibiotics was administered for 4 weeks. The continuous irrigation with double lumen catheter was found to be safe and effective treatment modality in the brain abscess.

Massive hemothorax after central venous catheter insertion in a patient with multiple trauma

  • Central venous catheter (CVC) insertion is commonly used in the operating room and intensive care unit to monitor central venous pressure and secure an intravenous route to deliver medications and nutritional support that cannot be safely infused into peripheral veins. However, CVC insertion may be associated with serious complications such as arterial puncture, hematoma, pneumothorax, hemothorax, catheter infections, and thrombosis. Several methods have been recommended to prevent these complications. Here we report a case of massive hemothorax caused by attempts of CVC insertion into the internal jugular vein and subclavian vein in a patient with multiple trauma. CVC placement should be performed or supervised by an experienced physician to decrease the incidence of CVC-related complications. CVC insertion under ultrasound guidance is recommended.

Effects of a Video Education Program for Caregivers on the Maintenance of Peripheral Intravenous Catheter among Hospitalized Children (입원 아동의 말초정맥주사 유지관리를 위한 보호자 대상 동영상 교육 프로그램 개발)

  • Purpose: The purpose of this study was to develop a video education program (VEP) for the caregivers and to verify its effectiveness on the maintenance of a peripheral intravenous catheter (PIVC) among hospitalized children. Methods: The VEP was developed through a literature review, educational need assessment of caregivers, and interviews with pediatric nurses, and validation of an expert group. The effectiveness of the VEP was tested on 102 caregivers and their children in a children's hospital at D city. A nonequivalent control group pretest-posttest design was used in which different types of intervention were given to caregivers in intervention group (n=51) and control group (n=51). All caregivers received brief verbal information about the PIVC maintenance. The intervention group was additionally provided with VEP using a smartphone. Data were analyzed using SPSS/Win 21.0 program. Results: The caregivers' knowledge score on PIVC maintenance in the intervention group was significantly higher than that of the control group. The numbers of flushing in case of blockage of PIVC and gauze dressing change of the intervention group was significantly lower than those of the control group. Conclusion: These results suggest that the VEP developed in this study can be useful for the maintenance of PIVC among hospitalized children.
