Single-tube nested reverse transcription-polymerase chain reaction for simultaneous detection of genotyping of porcine reproductive and respiratory syndrome virus without DNA carryover contamination (DNA 교차오염 방지기능이 있는 single-tube nested reverse transcription-polymerase chain reaction을 이용한 돼지생식기호흡기증후군바이러스 유전형 감별진단)

  • In the study, we developed and evaluated a uracil N-glycosylase (UNG)-supplemented single-tube nested reverse transcription-polymerase chain reaction (UsnRT-PCR) assay that can carried out first-round RT-PCR and second-round nested PCR in a reaction tube without reaction tube opening and can simultaneously detect EU- and NA-PRRSV. The UsnRT-PCR confirmed to have a preventing ability of mis-amplification by contamination of pre-amplified PRRSV DNA from previous UsnRT-PCR. Primer specificities were evaluated with RNAs extracted from 8 viral strains and our results revealed that the primers had a high specificity for both genotypes of PRRSV. The sensitivity of the UsnRT-PCR was 0.1 $TCID_{50}$/0.1 mL for EU- or NA-PRRSV, respectively, which is comparable to that of previously reported real time RT-PCR (RRT-PCR). Clinical evaluation on 110 field samples (60 sera and 50 lung tissues) by the UsnRT-PCR and the RRT-PCR showed that detection rates of the UsnRT-PCR was 70% (77/110), and was relatively higher than that of the RRT-PCR (69.1%, 76/110). The percent positive or negative agreement of the UsnRT-PCR compared to RRT-PCR was 96.1% (73/76) or 90.9% (30/33), showing that the test results of both assays may be different for some clinical samples. Therefore, it is recommend that diagnostic laboratory workers use the two diagnostic assays for the correct diagnosis for the relevant samples in the swine disease diagnostic laboratories. In conclusion, the UsnRT-PCR assay can be applied for the rapid, and reliable diagnosis of PRRSV without concerns about preamplified DNA carryover contamination that can occurred in PCR process in the swine disease diagnostic laboratories.

Job Analysis of Pediatric Occupational Therapists in Korea: Focused on Sensory Integration Intervention (아동작업치료사의 직무분석: 감각통합치료 중심으로)

  • Objective : The aims of the this study is to identify job characteristics, to generate job description, and additionally to investigate duty task of the job for Korean pediatric occupational therapist focused on sensory integration intervention. Methods : In the first stage, job analysis of pediatric occupational therapist focused on sensory integration intervention was performed by an advisory committee consist of the DACUM panel. Through the job analysis, job description with definition was established. In the second stage, a survey was performed for 141 pediatric occupational therapist using a questionnaire based on the job description in order to investigate importance, frequency and difficulty of duty task and task elements of the job. This process was done from May to September 2017. Results : The job definition drew from this study was 'Pediatric occupational therapist provides occupational therapy services to children and adolescents to support occupational performance and social participation including daily activities and play". Through the job description established in this study, 9 duties (consulting, evaluation, intervention plan, intervention, document, education, self-development, management, and administration), 28 tasks and 169 task elements were drew. In the survey, among the duty tasks and task elements, 'ensure physical safety' and 'perform a session' were selected as the most important task, and 'ensure safety of children, sensory integrative equipment and environment' and 'prepare a safe environment' were selected as the most important element. Conclusion : This study defined job demands and characteristics of pediatric occupational therapist who performs sensory integration intervention. It is expected that this study can be used a resource to develop and/or improve educational program related.

A Systematic review of effects and methods of treadmill training applied to stroke patients (뇌졸중환자에게 적용된 트레드밀훈련의 효과 및 방법에 대한 체계적 고찰)

  • Background: A physical therapy is required to restore motor function of stroke patients, especially gait function improvement is very important to them. Design: Systemic review. Methods: The purpose of this study is to provide objective evidence for the effectiveness of the treadmill training method applied to stroke patients. The literature search for a systematic review was performed on domestic data published from 2010 to 2020 at KISS, RISS, and DBpia. Results: 40 papers meeting the inclusion/exclusion criteria were finally selected. Data analysis was performed according to the research type, intervention method and process, measurement variables, and the risk of bias assessment was conducted as quality evaluation. Conclusion: Various training methods have been suggested and proved that treadmill intervention is an effective method for improving gait and balance. As the goal of rehabilitation for stroke patients is to improve walking and balance, we expect continuous clinical research to continue to establish a more systematic training program and present a new paradigm.

Development of An Evaluation Tool for the Quality of Patient Care Chonic Renal Failure (만성 신부전 환자간호의 질평가 도구개발)

  • Provision of better nursing care to patients is a difficult but important task. The first problem for nursing quality improvement is development of evaluation tool for the quality of nursing care. This study tries to develop a patient care tool for patients with chronic renal failure. This study as a procedural evaluation of patient care, tries to show what, how, at what order to provide care to patients with chronic renal failure. This study is divided into process of development of tool, its reliability and validity. Among process of development of tool is focal group, small expert group and expert evaluation group. To develop approprieteness of tool, nurses working is four major hospitals is Seoul were selected. To evaluate the credibility of subjects, 19 patient who were hospitalized and discharged within 3 months were selected. The period for collecting data for reliability and valiability evaluation was between Sept. 20 to Oct. 18, 1995. The development process of this study is as follows ; 1. Make preliminary list of the tool by focal group consisting of 8 clinical nurses. 2. Modify and add preliminary list by 4 expert nursing panel. 3. Calculate content validity of the tool by 23 nursing expert panel of judge. 4. Verity relability and validity of the tool. 5. Finalize an evaluation tool for the quality of nursing care in chronic renal failure patient. The result of this study were as follows ; 1. Development of evaluation tool for the quality of nursing care in chronic renal failure. 1) The evaluation of this study was developed 5 standards, 28 criteria. and 130 indicators 2) Nursing care evaluation scores for chronic renal failure patients were average 68.8. 2. Verity reliability and validity of the tool. 1) 5 standards were divided into 4 point scale and according to 28 creteria, indicators of standard were 3.72 and of criteria were 3.77 2) Inter - rater reliability (consentaneity score) of the tool by pearson correlation coefficient betwwen rates were r= .72, r= .75 and interreliabilities by single - facet crossed design were r= .96. 3) The alpha coeffecient relating to internal consistency was .7259 over 27 items of 28 criterias of developed tool. Through this study, I'm sure that the developed tool for the quality of patient care in chronic-renal failure patient will show the way of more improvement of the quality of nursing care and effective nursing intervention.

The Development and Evaluation of Web-based Flash Content for Fighting Spirit Promotion in Patients with Cancer (암 환자 투병의지 증진을 위한 플래시 콘텐츠 개발 및 평가)

  • Purpose: The purpose of this study was to develop a Web-based flash content for fighting spirit promotion and to test its effect on cancer patients' fighting spirit. Methods: The 15-minute long Web-based flash content was developed using the following 5 process: analysis, planning, development, program operation and evaluation stages and utilized the multiple edition and revision processes from December 2005 to August 2006. The evaluation was done by one group pretest-posttest design. Study subjects were 17 cancer patients undergoing chemotherapy. The group were received a 2-week intervention, a day weekly, and measured the variables at baseline, 4 weeks later. The study was performed from August 2006 to February 2007 at a cancer center in Korea. Fighting spirit was measured by Mini-MAC(Mental Adjustment to Cancer). Data was analysed by descriptive statistics and paired t-test using SAS 9.13 program. Results: On the Web-based flash content, there are 4 menu bars that consisted of cancer diagnosis, symptom management, stress management, and cancer survivorship. The study group revealed significantly more fighting spirit than pre-test(t=-3.04, p=.008). Conclusion: This Web-based flash content can be utilized in psychosocial interventions for promoting fighting spirit in patients with cancer.

A Case study of Elementary Mathematics Class in a Constructive View (초등수학에서 구성주의적 관점에서의 수업 사례연구)

  • The purpose of this paper is to compare and analyze the two different teaching methods of elementary mathematics in the traditional method and in the constructive view. To do so, the actual class in the constructive view has been made for about four months using a class of 45 students in the second grade of an elementary school. After the class was finished, we collected diverse data from the class, such as the responses from the children(self-evaluation, mathematics diary, observation by the investigator, daily report), class evaluation report by other teacher and so on. The results of this research are as follows: First, the traditional class reaches at the goal of learning in a unit time because the class is guided by the teacher but the class in the constructive view is a little flexible because it is contextual. Second, in the constructive process of mathematical knowledge we knew that small group activities or discussion without intervention of teacher was often ended in exhaustive argument without arriving at valid social consensus. Third, the attitude in mathematics was changed from the passive one to the self-regulated ones. Fourth, the class in the constructive view could extend not only the ability of mathematical communication but also the ability of self-directed learning of children. Fifth, it was a considerable change the role of teacher, that is, guide of instruction instead of unique specialist in the classroom. Sixth, finally, the evaluation was made after finishing a unit class in the traditional instruction but it was integrated in a class in a constructive view.

The Analysis of Nursing Research in Korea in relation to the Evaluation of Quality of Nursing (간호의 질 평가에 관한 국내 간호연구 분석)

  • Purpose: The purpose of this study is to analyse research trends related to the evaluation of quality of nursing, thereby obtaining basic data relating to the identification of current situation of instrument development of quality of nursing, and of future research orientation and to the development of performance index of nursing organization. Method: The data were collected from June to December, 2002 through the review of total of 32 research papers which had evaluated quality of nursing among published papers in Korea from 1976 to August 2002. The analysis was done in terms of research period, the periodicals in which research papers were published, domains and the approaches of evaluation of nursing quality. The content analysis of lowest-level items amounting was performed using NIC developed by McCloskey & Bulecheck(1998) and consequential indices of quality of nursing developed by Chi(1995). Results: Twenty-three of 32 papers(71.8%) turned out to be published after 1995, indicating surging interest in the evaluation of quality of nursing from the mid 1990s. Also, ten of 12 research papers dealing with subjects' diseases were published after 1995. In content analysis of lowest-level items of the process-oriented evaluation of quality of nursing, the highest nursing intervention was patient education about procedure and treatment; followed in descending order by patient education about disease process, strengthening of communication, managing environment, infection control, admission care, defecation and urination care. In content analysis of lowest-level items of the outcome-oriented evaluation of quality of nursing, items of physical and psychological state, of patient and family satisfaction, of knowledge and home care, of change of patients' state, of addressing nursing issues, and of patient recovery were the outcome indicators in more than 60 percent research papers. Conclusion: The findings provided the foundation for their effective use in nursing practice with comparing and presenting various core evaluation items representing process and outcome domains.

Effect of Visual Feedback Training for Gradual Weight Shift in the Sit-to-stand Training on the Balance and Walking Abilities of Chronic Hemiplegia Patients (앉고 일어서기 동작에서 점진적 체중 이동을 이용한 시각적 되먹임 훈련이 만성 편마비 환자의 균형 및 보행능력에 미치는 영향)

  • Purpose: This study aimed to investigate the effect of visual feedback training-for gradual weight shift in sit-to-stand training-on the balance and walking abilities of chronic hemiplegia patients. Methods: Twenty patients with chronic hemiplegia volunteered to participate in this study. The experimental group received visual feedback for gradual weight shift in the sit-to-stand training, while the contrast group followed the standard process for the sit-to-stand training. The evaluation of the balance and walking ability was conducted with the functional reach test (FRT), Berg balance scale (BBS), five time sit-to-stand (FTSTS) test, timed up and go (TUG) test, 10 m walk test (10MWT), balancia, activities-specific balance confidence (ABC) scale, and falls efficacy scale (FES). Results: In the results before and after intervention, there was a significant difference in TUG, 10MWT, ABC, and FES in the visual feedback training group (p < 0.05). In the control group, there was a significant difference in the 10MWT and ABC (p < 0.05). Also, in the evaluation of the postural fluctuations, the control group data showed a significant increase in Covar. The visual feedback group showed a significant difference in the W average. Conclusion: The visual feedback training group showed some improvement in terms balance and walking ability and on the ABC scale and FES. Therefore, if the diagonal progressive weight bearing exercise is combined with the various patterns and basic principles of PNF, it may be a more efficient intervention method.

  • In this study, we developed a sensitive and specific reverse transcription loop-mediated isothermal amplification (RT-LAMP) assay for rapid visual detection of foot-and-mouth disease virus (FMDV) circulated in Korea. The RT-LAMP was completed in 40 min at $62^{\circ}C$ and the results of the assay were directly detected by naked eye without any detection process. The assay specifically amplified all 7 serotypes of FMDV RNAs but not amplified other viral and cellular nucleic acids. The sensitivity of the RT-LAMP was $10^2$, $10^3$ and $10^3TCID_{50}/mL$ for serotype O, A and Asia 1 FMDV, respectively, which was comparable to conventional reverse transcription polymerase chain reaction (RT-PCR) and relatively lower than that of real time quantitative RT-PCR (qRT-PCR). Clinical evaluation of the RT-LAMP using different serotypes of Korean and foreign FMDV strains showed a 100% (35/35) agreement with the results of the RT-PCR and qRT-PCR. These results indicated that RT-LAMP assay developed in this study could be a valuable diagnostic method for FMDV monitoring and surveillance.

Motivational Interviewing Training Using Role Play in Dental Hygiene Students

  • The purpose of this study was to develop a motivational interviewing (MI) training program to improve competency in communication and to effect the evaluated changes that would occur in dental students' counseling techniques as result of the training in the program. The study proceeded through the program via role-play practice, which was explained effectively during the MI program training process. A convenience sample of 43 fourth-year dental hygiene students was recruited. Twenty-two students were randomly assigned to the intervention group and 21 to the control group in the order of recruitment. The theoretical lecture was delivered over 3 hours and the practice was done in parallel by applying cases occurring in clinic settings. The practice was technical training, partner practice, and small group practice. The intervention group received three hour MI lecture while the control group received no lecture. Evaluation through role-play practice was separated according to "researcher's viewpoint," "counselor's role viewpoint," and "patient's role viewpoint." The results of the analysis showed that the intervention group had higher MI skills and ability than the control group. Furthermore, program participants showed positive impressions to the MI training program. The use of coaching sessions improved the MI techniques and counseling skills of dental hygiene students learning MI. The effect of the application of the MI training program was that the MI training improved counseling skills and interviewing abilities. Moreover, continuous training and feedback enhanced MI techniques and core skills. Training using lectures in parallel with practice rather than education through theory alone improved students' techniques. Application of role play through the combined method of the MI program was confirmed by an effective training method.
