Purpose: A study analysed the stress distribution of abutment screw and supporting bone of fixture by the tightening torque force of the abutment screw within clinical treatment situation for the stability of the dental implant prosthesis. Methods: The finite element analysis was targeted to the mandibular molar crown model, and the implant was internal type 4.0 mm diameter, 10.0 mm length fixture and abutment screw and supporting bone. The occlusal surface was modeled in 4 cusps and loaded 100 N to the buccal cusps. The connection between the abutment and the fixture was achieved by combining three abutment tightening torque forces of 20, 25, and 30 Ncm. Results: The results showed that the maximum stress value of the supporting bone was found in the buccal cortical bone region of the fixture in all models. The von Mises stress value of each model showed 184.5 MPa at the 20 Ncm model, 195.3 MPa in the 25 Ncm model, and 216.5 MPa in the 30 Ncm model. The contact stress between the abutment and the abutment screw showed the stress value in the 20 Ncm model was 201.2 MPa, and the 245.5 MPa in the 25 Ncm model and 314.0 MPa in the 30 Ncm model. Conclusion: The increase of tightening force within the clinical range of the abutment screw of the implant dental prosthesis was found to have no problem with the stability of the supporting bone and the abutment screw.
Oral implants must fulfill certain criteria arising from special demands of function, which include biocompatibility, adequate mechanical strength, optimum soft and hard tissue integration, and transmission of functional forces to bone within physiological limits. And one of the critical elements influencing the long-term uncompromise functioning of oral implants is load distribution at the implant- bone interface, Factors that affect the load transfer at the bone-implant interface include the type of loading, material properties of the implant and prosthesis, implant geometry, surface structure, quality and quantity of the surrounding bone, and nature of the bone-implant interface. To understand the biomechanical behavior of dental implants, validation of stress and strain measurements is required. The finite element analysis (FEA) has been applied to the dental implant field to predict stress distribution patterns in the implant-bone interface by comparison of various implant designs. This method offers the advantage of solving complex structural problems by dividing them into smaller and simpler interrelated sections by using mathematical techniques. The purpose of this study was to evaluate the stresses induced around the implants in bone using FEA, A 3D FEA computer software (SOLIDWORKS 2004, DASSO SYSTEM, France) was used for the analysis of clinical simulations. Two types (external and internal) of implants of 4.1 mm diameter, 12.0 mm length were buried in 4 types of bone modeled. Vertical and oblique forces of lOON were applied on the center of the abutment, and the values of von Mises equivalent stress at the implant-bone interface were computed. The results showed that von Mises stresses at the marginal. bone were higher under oblique load than under vertical load, and the stresses were higher at the lingual marginal bone than at the buccal marginal bone under oblique load. Under vertical and oblique load, the stress in type I, II, III bone was found to be the highest at the marginal bone and the lowest at the bone around apical portions of implant. Higher stresses occurred at the top of the crestal region and lower stresses occurred near the tip of the implant with greater thickness of the cortical shell while high stresses surrounded the fixture apex for type N. The stresses in the crestal region were higher in Model 2 than in Model 1, the stresses near the tip of the implant were higher in Model 1 than Model 2, and Model 2 showed more effective stress distribution than Model.
Purpose: Given the predictability of dental implant procedure from the studies of successful osseointegration, implant dentistry is often the treatment of choice to replace missing teeth in edentulous patient instead of the fixed prosthesis or removable denture. The $Renova^{(R)}$ dental implant has a RBM(Resorbable Blast Media) surface, internal hex prosthetic connection and a tapered design. At this study gives the analysis of the implant and the short term survival rate of the implant. Material and Methods: In this study, a multilateral analysis was performed on the subjects undergoing placement with $Renova^{(R)}$ implant between August 2006 and February 2008 in Yonsei University dental hospital. 96 implants were placed in 56 patients and they were surveyed for cumulative survival rate. Among them 78 implants in 44 patients were surveyed for the rest analyses. Result: 1. The cumulative survival rate was 96.88% of 96 implants in 56 patients. 2. The mean marginal bone loss was 0.803mm and the marginal bone loss in augmentation group has higher value than the marginal bone loss in non augmentation group. 3. The health scale for the implants were 87% in success group, 9% in satisfactory survival group, 1% in compromised survival group, and 3% in failure group. 4. Two implants placed in poor bone posterior area by 2-stage failed during prosthetic procedure. Conclusion: $Renova^{(R)}$ dental implant showed high cumulative survival rate in installation on partial edentulous ridge and could be a predictable implant system.
연구목적: 골유착이라는 개념이 도입된 후, 임플란트의 등장은 치과계에 많은 변화를 일으키게 되었다. 임플란트를 이용한 상실치아의 수복은 안정된 시술방법으로 시행되어져 왔으며 그 적용 범위가 확대되었고 사용 빈도도 급격히 증가해왔다. 이에 따라 국산 임플란트의 사용 빈도도 높아지고 그 종류 또한 증가하였으나 장기간의 임상적, 객관적인 자료를 가진 국산 임플란트의 수는 많지 않은 상태이다. 본 연구는 연세대학교 치과대학 병원에 내원한 환자 중 SLA 표면 처리 및 미세나사선을 가진 내측 연결형의 $Implantium^{(R)}$ 임플란트를 이용하여 치료받은 38명의 환자에게 식립된 106개의 임플란트를 대상으로 6개월에서 30개월까지의 임상적, 방사선학적 결과를 후향적으로 분석하였다. 연구재료 및 방법: 진료기록부를 통해 성별, 연령, 식립 위치, 식립 부위의 골 상태, 식립된 임플란트의 직경 및 길이, 2차수술및골이식여부, 대합치의 유형, 임상적 합병증의 유형 및 빈도 등을 조사하여 그에 따른 분포 및 생존율의 차이와 함께 이들 항목이 변연골 흡수량에 미치는 영향을 조사하였다. 방사선 사진분석으로 임플란트 식립시, 보철물 장착 후 첫 6개월 내지 1년간, 그 이후 연간 변연골 흡수량을 측정하여 분석하였다. 결과: 1. 총 38명에게 식립된 106개의 임플란트 중 1개가 실패하여 누적 생존율은 99.1%로 나타났다. 2. 보철물 장착 후 최소 6개월 이상 정기검진이 가능했던 96개의 임플란트에서 상악에서의 생존율이 97.0%, 하악에서의 생존율은 100%였고, 구치부에서의 생존율은 98.9%였으며 전치부에서의 생존율은 100%였다. 3. 보철물 장착 후 첫 1년간의 변연골 흡수량은 평균 0.17 mm 이었으며, 1년 이후의 연간 변연골 흡수량은 평균 0.04 mm로 통계학적으로 유의차 있게 적은 골소실량을 나타내었다(P<.05). 4. 보철물 장착 후 첫 1년간 연령에 따른 변연골 흡수량의 차이는 보이지 않았으나(P>.05), 1년 이후의 정기 검진 기간에서 50세 이상의 환자에서 50세 미만의 환자보다 큰 연간 변연골 흡수량을 보였다(P<.05). 5. 성별 간, 악궁 간, 전치부와 구치부위 간, 2차 수술 및 골 이식 여부, 보철물의 유형, 대합치의 유형에 따른 변연골 흡수량의 차이는 보이지 않았다(P>.05). 결론: 이상의 결과를 토대로 변연골 흡수량에 영향을 주는 요소로 연령을 들 수 있었으며 성별, 악궁 간, 악궁 내 위치, 2차 수술 및 골 이식 여부, 보철물의 유형, 대합치의 유형에 따른 변연골 흡수량의 차이는 없었다. 본 연구에서 최대 30개월까지의 기간 동안 SLA 표면 처리 및 미세나사선을 가진 내측 연결형의 국산 임플란트의 임상적인 성공률은 만족스러운 결과를 보였으며 변연골 흡수량도 임플란트 성공 기준에 부합하였다.
안착형 외부 연결 임플란트를 적절히 사용하기 위해서는 식립 시에 임플란트 간의 간격에 주의해야 하며 2차 수술 시에 임플란트 상부로 올라간 골을 다듬어야 한다. 육각형태를 가진 지대주가 원형 지대주에 비해 유용하다. 외부 연결형 임플란트는 악간간격이 제한된 경우에 적용할 수 있다. 안착형 외부연결(조직수준) 임플란트는 상부 직경의 두께로 인한 생역학적 장점과 내재된 점막관통부로 인한 생물학적 장점을 가지고 있다. 조직수준 임플란트를 수복할 때 지대주 수준과 임플란트 수준의 변연 중 악간공간에 따라 선택한다. 어떤 한 가지 임플란트 종류로는 모든 증례를 만족시키기 어려우므로 교합력 및 골조건을 고려하여 적절한 임플란트를 선택하는 것이 필요하다.
목적: 본 연구의 목적은 내부연결 원추형 임플란트의 육각구조 유무가 임플란트 지대주 장축변위와 임플란트 지대주 나사의 풀림토크에 미치는 영향을 평가하고자 하였다. 연구 재료 및 방법: 내부연결 원추형 임플란트를 육각구조를 가진 지대주 그룹(HEX)과 육각구조를 갖지 않는 지대주 그룹(CON)으로 나누고 각 그룹 당 10쌍의 시편을 체결하였다. 지대주 나사를 30 Ncm 조임회전력을 가하여 체결한 뒤 장축변위 및 풀림토크값을 측정하고 250 N 수직적 반복하중을 100,000회 가한 뒤 장축변위 및 풀림토크값을 측정하였다. 각 단계마다 디지털 마이크로미터를 이용하여 임플란트 직경과 수직적 높이를 측정하였고 전자 토크게이지를 이용하여 풀림토크값을 측정하였다. 각 군간의 값의 유의차를 확인하기 위하여 독립 표본 T 검정을 이용하여 통계분석하였다(α = 0.05). 결과: HEX 군은 조임회전력을 가한 뒤 유의하게 높은 장축변위를 나타내였다(P < 0.05). HEX 군과 CON 군은 수직적 반복하중 후 장축변위에 유의한 차이를 보이지 않았다(P = 0.052). HEX 군과 CON 군은 수직적 반복하중 전, 후 풀림토크값 모두 유의한 차이를 보이지 않았다(P = 0.057 and P = 0.138). 모든 그룹에서 반복하중 후 풀림토크값 상실율이 증가하였다(P < 0.05). 결론: 내부연결 원추형 임플란트에서 육각구조를 가진 경우 장축 변위가 더 컸으며, 그 외의 연결부 안정성은 유사하였다. 모든 그룹에서 반복하중 후 풀림토크값 상실율이 증가하였다.
골유착성 임플란트 보철물에서 피로나 과하중에 의해 보철유지 나사나 지대주 나사의 파절, 보철물의 파절, 또는 고정체의 파절 등과 같은 기계적 강도와 연관된 문제점이 발생할 가능성이 높다. 임플란트 시스템의 기계적인 강도에 영향을 주는 요소에는 고정체와 나사의 직경, 재료적 특성, 연결부 디자인, 지대주 디자인 등이 있으며, 이 중 임플란트 고정체와 지대주간의 연결부 디자인은 임플란트 시스템의 기계적인 연결상태와 연결부 안정성을 결정하는 주요소이다. 대부분의 기계적 강도에 관한 연구에서처럼 단일하중에 의한 압축굽힘강도나, 단기적인 반복하중 후의 결과만을 평가하여 임플란트 시스템의 장기적인 안정성을 예측하기에는 한계가 있다. 연구 목적: 이에 본 연구에서는 external butt joint와 internal conical joint를 갖는 임플란트 시스템(오스템사)의 연결부 디자인에서 각각 다른 두 가지 지대주를 사용하여, 연결부 및 지대주 디자인이 기계적 강도에 미치는 영향을 압축굽힘강도는 물론 내구성한계를 측정하여 알아보고자 하였다. 연구 재료 및 방법: External butt joint인 US II에서는 통상적인 UCLA 지대주 형태의 Cemented abutment(BJT)와 굽힘 저항성을 증가시키기 위해 나사 두부가 지대주 상단에 위치하도록 설계된 Safe abutment(BJS)를, internal conical joint인 SS II에서는 one-piece형의 Solid abutment(CJO)와 two-piece형의 ComOcta abutment(CJT)를 지대주로 사용하였다. ISO 규정을 참고하여 단일 임플란트, 변연골 흡수, 그리고 $30^{\circ}$ 경사하중 조건에서 압축굽힘강도와 내구성한계를 측정하였고 실패 양상을 관찰한 결과 다음과 같은 결론을 얻었다. 결과 및 결론: 1. 압축굽힘강도는 BJS군(1392.0N), CJO군(1261.8N), BJT군(1153.2N), 그리고 CJT군(1110.2N) 순으로 낮아졌으며(P<.05), CJT군과 BJT군 사이에는 차이가 없었다(P>.05) 2. 내구성한계는 CJO군(600N), CJT군(453N), BJS군(360N) 그리고 BJT군(300N) 순으로 낮아졌다. 3. 압축굽힘강도는 연결부 디자인 또는 지대주 디자인에 따라 차이를 보였으며, 내구성한계에 있어서는 연결부 디자인이 더 주된 요소로 작용하였다.
PURPOSE. The purpose of this study was to evaluate the effects of abutment screw length and cyclic loading on the removal torque (RTV) in external hex (EH) and internal hex (IH) implants. MATERIALS AND METHODS. Forty screw-retained single crowns were connected to external and internal hex implants. The prepared titanium abutment screws were classified into 8 groups based on the number of threads (n = 5 per group): EH 12.5, 6.5, 3.5, 2.5 and IH 6.5, 5, 3.5, 2.5 threads. The abutment screws were tightened with 20 Ncm torque twice with 10-minute intervals. After 5 minutes, the initial RTVs of the abutment screws were measured with a digital torque gauge (MGT12). A customized jig was constructed to apply a load along the implant long axis at the central fossa of the maxillary first molar. The post-loading RTVs were measured after 16,000 cycles of mechanical loading with 50 N at a 1-Hz frequency. Statistical analysis included one-way analysis of variance and paired t-tests. RESULTS. The post-loading RTVs were significantly lower than the initial RTVs in the EH 2.5 thread and IH 2.5 thread groups (P<.05). The initial RTVs exhibited no significant differences among the 8 groups, whereas the post-loading RTVs of the EH 6.5 and EH 3.5 thread groups were higher than those of the IH 3.5 thread group (P<.05). CONCLUSION. Within the limitations of this study, the external hex implants with short screw lengths were more advantageous than internal hex implants with short screw lengths in torque maintenance after cyclic loading.
PURPOSE: The aim of this study was to evaluate the influence of the oxide layer removal process in the Co-Cr-Mo (CCM) abutment after casting procedure on the prosthesis settlement and screw stability. MATERIALS AND METHODS: CCM abutments of four different interface conditions (CCM-M; machined, CCM-O; oxide layer formed, CCM-B; blasted, CCM-P; polished after blasted) and gold abutment (Gold-C; Cast with type III Gold alloy) were used. The initial settling values of abutments were evaluated according to the difference of implant-abutment length when the tightening torques were applied at 5 Ncm and 30 Ncm, and the settling values of abutments caused by loading were evaluated according to the difference of implant-abutment length before and after loading with 250 N, 100000 cycle. The loss ratios of removal torque for abutment screws were evaluated according to the difference in value of removal torques under 30 Ncm tightening torque applied before and after cyclic loading. RESULTS: The CCM-P and CCM-B group showed a higher initial settling value compared with the Gold-C group (P<.05), while the Gold-C group showed the highest settling values caused by loading (P<.05) and no significant differences were observed for between CCM groups (P>.05). The loss ratio of removal torque values for the CCM-B, CCM-P groups did not differ significantly from that of the Gold-C group (P>.05). CONCLUSION: Even though the oxide layer was removed by different methods, CCM abutment with internal conical connection structure showed lower abutment settling and similar screw loosening after cyclic loading compared with gold abutment.
