Purpose: This study evaluated the pretreatment expression patterns of MDM2, p53, and pRb proteins to determine if the expression patterns could predict the outcome of concurrent chemoradiotherapy (CCRT) for esophageal squamous cell carcinoma and aid in the decisions for the selection of treatment modalities. Materials and Methods: Fifty-one patients that were treated with definitive chemoradiotherapy for stage $I{\sim}IVa$ esophageal squamous cell carcinoma were selected for this study. Radiotherapy was administered with daily $1.8{\sim}2\;Gy$ fractions up to a median dose of 54 Gy for primary tumors, and with four cycles of cisplatin/5-fluorouracil chemotherapy that was administered every 4 weeks, the first two cycles of which were administered concurrently with radiotherapy. Expression of MDM2, p53, and pRb was investigated by immunohistochemical analysis using pretreatment biopsy specimens. Results: MDM2, p53, and pRb were detected with high immunoreactivity in 19.6%, 27.5%, and 66.7% of the patients, respectively. However, there was no significant correlation between expression of these factors and clinical outcome. By the use of multivariate analysis with nine covariates-age, tumor location, tumor length, stage, pathological response, clinical response, MDM2 expression, p53 expression, and pRb expression, only pathological response and stage were significant factors for cause-specific survival. Conclusion: Expression of MDM2, p53, and pRb was not found to be clinically significant for predicting outcomes after CCRT in this study. Further studies with a larger patient population and longer follow-up periods are needed to re-evaluate the expression pattern and to identify new predictors for CCRT response.
Purpose : This study was undertaken to analyze the efficacy and sphincter preservation rate of platinum based neoadjuvant chemotherapy Plus radiotherapy versus abdominoperineal resection and Postoperative radiotherapy for anal cancer. Materials and Methods : Data of forty-two patients with anal cancer were retrospectively analyzed. Among thirty-eight patients with epidermoid histology, four patients received radiotherapy, and nineteen patients received abdominoperineal resection and adjuvant radiotherapy with or without chemotherapy $(APR+RT{\pm}CT)$, and fifteen patients received neoadjuvant chemotherapy and radiotherapy (CRT). The CRT regimen was composed of three cycles of 5-fluorouracil $(1,000\;mg/m^2\;bolus\;on\;D1\~5)$ and cisplatin $(60\;mg/m^2\;bolus\;on\;D1)$ followed by 50.4 Gy to the tumor bed and regional lymphatics over 5.5 weeks. Both inguinal lymphatics were treated with an identical dose schedule. Residual disease was treated with an additional three cycles of identical adjuvant chemotherapy. An identical dose schedule was used for post-operative radiotherapy. Median follow-up period was eighty-five months. Results : Overall five-year survival rates were $80.3\%$, 88.9 and $79.4\%$ for entire patients, $APR+RT{\pm}CT$ group, and the CRT group, respectively. No significant difference was found between the two groups (p=0.49). Anus preservation rate for the CRT group was $86.7\%$. Age (0=0.0164) and performance status (p=0.0007) were found to be significant prognostic factors by univariate analysis. Age (p=0.0426), performance status (p=0.0008), and inguinal lymph node metastasis (e=0.0093) were statistically significant prognostic factors by multivariate analysis. No case of RTOG grade 3 complication or higher was reported. Conclusion : This and other recent studies have shown that combined chemotherapy plus radiotherapy for anal cancer results in a high rate of anal sphincter preservation as well as local control and survival. Furthermore, neoadjuvant use of chemotherapy with a cisplatin based regimen rather than a concurrent regimen may lead to a decrease in complications.
Purpose: To evaluate radiation sensitivity of dendritic cells in comparison with lymphocytes. Materials and methods: T lymphocytes captured from peripheral blood were irradiated by 0 Gy, 10 Gy, 30 Gy. Apoptosis was measured by flowcytometry for staining of Annexin V 4 hours after irradiation. Immature and mature dendritic cells processed from blood hematopoietic stem cell were irradiated by 0 Gy, 10 Gy, 30 Gy, 100 Gy respectively and apoptosis was measured by flowcytometry with time difference as 4h, 24h and 48h after irradiation. Morphometric analysis by percent nucleus was measured in three cell groups, also. Results: Lymphocytes showed radiation sensitivity by increasing apoptotic fraction according to radiation dose. However, both mature and immature dendritic cells showed consistent fraction of apoptosis in spite of increasing radiation dose. Percent nucleus ratio is significantly higher in lymphocytes than that of mature or immature dendritic cells. Stimulation of T-cell by dendritic cells was not changed after irradiation. Conclusion: Dendritic cells showed radioresistance which was associated with small size of nucleus in comparison with lymphocytes and this result would be used as a basal data of radio-labelling for the cellular trafficking studios in nuclear medicine fields.
The work presents a realistic human exposure assessment of indoor radon released from groundwater in a house. At first, a two-compartment model is developed to describe the generation and transfer of radon in indoor air from groundwater. The model is used to estimate radon concentrations profile of indoor air in a house us]ng by showering, washing clothes, and flushing toilets. Then, the study performs an uncertainty analysis of model input parameters to quantify the uncertainty in radon concentration profile. In order to estimate a daily internal dose of a specific tissue group in an adult through the inhalation of such indoor radon, 3 PBPK(Physiologically-Based Pharmaco-Kinetic) model is developed. Combining indoor radon profile and PBPK model is used to a realistic human assessment for such exposure. The results obtained from this study would be used to the evaluation of human risk by inhalation associated with the indoor radon released from groundwater.

The organically bound tritium(OBT) concentrations of the marine organisms(seaweed and shellfish) around Wolsung nuclear power plants were measured. The OBT concentrations of seaweeds were in the range of $0.372{\sim}2.72$ Bq/kg-fresh. The OBT concentrations of shellfish were in the range of $0.239{\sim}2.02$ Bq/kg-fresh. The internal dose due to the OBT concentration of seaweed was estimated as $3.4{\times}10^{-7}$ mSv/year.
This study describes a practical method for interpretation of bioassay results of inhaled uranium to assess the committed effective doses both for chronic and acute intake situations. Organs in the body were represented by a series of mathematical compartments for analysis of the behavior of uranium in the body according to the gastrointestinal track model, respiratory track model and biokinetic model recommended by the ICRP. An analytical solutions of the system of balance equations among the compartments were obtained using the Birchall's algorithm, and the urinary excretion function and the lung retention function of uranium were obtained. An initial or total intakes by intake modes were calculated by applying excretion and retention functions to the urinary uranium concentration and the lung burden measured with a lung counter. The dose coefficients given in ICRP 78 are used to estimate the committed effective doses from the calculated intakes.

The propagation of light radiation in a turbid medium is an important problem that confronts dosimetry of therapeutic laser delivery and the development of diagnostic spectroscopy. Scattered light is measured as a function of the position(distance r, depth z) between the axis of the incident beam and the detection spot. Turbid sample yields a very forward-directed scattering pattern at short range of position from source to detector, whereas the thicker samples greatly attenuated the on-axis intensity at long range of position. The portions of scattered light reflected from or transmitted throughphantom depend upon internal reflectance and absorption properties of the phantom. Monte Carlo simulation method for modelling light transport in tissue is applied. It uses the photon is moved a distance where it may be scattered, absorbed, propagated, internally reflected, or transmitted out of tissue. The photon is repeatedly moved until it either escape from or is absorbed by the phantom. In order to obtain an optimum therapeutic ratio in phantom material, optimum control the light energy fluence rate is essential. This study is to discuss the physical mechanisms determining the actual light dose in phantom. Permitting a qualitative understanding of the measurements. It may also aid in designing the best model for laser medicine and application of medical engineering.
Cryptosporidium parvum is known as one of the most highly resistant parasites to gamma irradiation. To morphologically have an insight on the radioresistance of this parasite, ultrastructural changes in C. parvum sporozoites were observed after gamma irradiation using various doses (1, 5, 10, and 25 kGy) following a range of post-irradiation incubation times (10 kGy for 6, 12, 24, 48, 72, and 96 hr). The ultrastructures of C. parvum oocysts changed remarkably after a 10-kGy irradiation. Nuclear membrane changes and degranulation of dense granules were observed with high doses over 10 kGy, and morphological changes in micronemes and rhoptries were observed with very high doses over 25 kGy. Oocyst walls were not affected by irradiation, whereas the internal structures of sporozoites degenerated completely 96 hr post-irradiation using a dose of 10 kGy. From this study, morphological evidence of radioresistance of C. parvum has been supplemented.
Radioactive aerosols are produced during the cutting of contaminated and activated metals. They must be collected and removed by a high-performing filtration system before releasing to the environment from the decommissioning workplace. The filtration system requires regular replacement to ensure the sufficient removal of radioactive aerosols because its filtration efficiency gradually decreases. This study evaluates the efficiency and lifetime of filters while cutting metals by using a plasma arc cutter. Particularly, this study considers the aerodynamic diameter distribution of number and mass concentrations for aerosols from 6 nm to 10 ㎛ when evaluating the performance of filters. After 20 time reuses for cutting operation performed in a cutting chamber, the removal efficiency is reduced from over 99 to below 93% at 2 ㎛. The results are used to analyze the lifetime of filters, the frequencies of their replacements, and impact on internal radiation dose.
Salivary duct carcinoma(SDC) is rare malignancy, accounting for approximately 1-3% of all malignant salivary gland tumors. Systemic chemotherapy has been used for stage IV SDC, but there is no consensus on the standard treatment. SDC is histologically similar to ductal carcinoma of breast and often overexpress HER2/neu, hence HER2/neu targeted therapy could be one of treatment options. A 75-year-old Arabian man was diagnosed as SDC of right parotid gland with extensive metastases. He received oral 5-FU as palliative chemotherapy, but he was intolerable to oral chemotherapy due to severe oral mucositis. Considering immunohistochemical stain of tumor tissue showing strong positive for HER2/neu, we decided to administer an anti-HER2/neu antibody, trastuzumab. Follow-up CT scans before the third dose of trastuzumab demonstrated remarkable regression of multiple metastases as well as primary tumor. This case suggests that HER2/neu targeted therapy may be a potential therapeutic option for the SDC patient with overexpression of HER2/neu.
