The Increased Expression of Gelatinolytic Proteases Due to Cigarette Smoking Exposure in the Lung of Guinea Pig (기니픽에서 흡연 노출에 의한 젤라틴 분해 단백 효소의 발현 양상에 관한 연구)

  • Background : Chronic obstructive pulmonary disease(COPD) is one of the major contributors to morbidity and mortality among the adult population. Cigarette smoking(CS) is undoubtedly the single most important factor in the pathogenesis of COPD. However, its mechanism is unclear. The current hypothesis regarding the pathogenesis of COPD postulates that an imbalance between proteases and antiproteases leads to the destructive changes in the lung parenchyma. This study had two aims. First, to evaluate the effect of CS exposure on histologic changes of the lung parenchyme, and second, to evaluate the effect of CS exposure on the expression of the gelatinolytic enzymes in BAL fluid cells in guinea pigs. Methods : Two groups of five guinea pigs were exposed to the whole smoke of 20 commercial cigarettes per day, 5 hours/day, 5 days/week, for 6weeks, and 12 weeks, respectively, using a smoking apparatus. Five age-matched guinea pigs exposed to room air were used as controls. Five or more sections were microscopically extamined(${\times}400$) and the number of cellular infiltration of the alveolar wall was measured in order to evaluate the effect of CS exposure on the histologic changes of lung parenchyme. The statistical significance was analyzed by a linear regression method. To evaluate the expression of the gelatinolytic enzymes in intraalveolar cells, BAL fluid was obtained and the intraalveolar cells were separated by centrifugation (500 g for 10 min at $4^{\circ}C$). Two sets of culture plates were loaded with $1{\times}10^6$ intraalveolar cells. One plate, contained O.1mM EDTA, a inhibitor of matrix metalloproteases(MMPs), and the other plate had no EDTA. Both plates were incubated for 48 hours at $37^{\circ}C$. After incubation, gelatinolytic protease expression in the supernatants was analyzed by gelatin zymography. Results : At the end of CS exposure, the level of blood carboxy Hb had increased significantly(4.1g/dl in control group, 24g/dl immediately after CS exposure, 18g/dl 30 min after CS exposure, 15g/dl 1 hour after CS exposure). Alveolar inflammatory cells were identified in the CS exposed guinea pigs. The number of alveolar cellular cells observed in a microscopic field ($400{\times}$) was $121.4{\pm}7.2$, $158.0{\pm}20.2$, $196.8{\pm}32.8$, in the control, the 6 weeks, and the 12 weeks group, respectively. The increased extent of inflammatory cellular infiltration of the lung parenchema showed a statistically significant linear relationship with the duration of CS exposure(p=0.001, $r^2=0.675$). Several types of gelatinolytic enzymes in the intraalveolar cells of CS exposed guinea pigs were expressed, of which some were inhibited by EDT A. However, the gelatinolytic enzymes were not expressed in the control groups. Conclusion : CS exposure increases inflammatory cellular infiltration of the alveolar wall and the expression of gelatinolytic proteases in guinea pigs. EDTA inhibits some of the gelatinolytic proteases. These findings suggest a possibility that CS exposure may increase MMP expression in the lungs of guinea pigs.

Clinical Relevance of Bronchial Anthracofibrosis in Patients with Chronic Obstructive Pulmonary Disease Exacerbation

  • Background: Bronchial anthracofibrosis (BAF), which is associated with exposure to biomass smoke in inefficiently ventilated indoor areas, can take the form of obstructive lung disease. Patients with BAF can mimic or present with an exacerbation of chronic obstructive pulmonary disease (COPD). The purpose of the current study was to investigate the prevalence of BAF in Korean patients with COPD exacerbation as well as to examine the clinical features of these patients in order to determine its clinical relevance. Methods: A total of 206 patients with COPD exacerbation were divided into BAF and non-BAF groups, according to computed tomography findings. We compared both clinical and radiologic variables between the two groups. Results: Patients with BAF (51 [25%]) were older, with a preponderance of nonsmoking women; moreover, they showed a more frequent association with exposure to wood smoke compared to those without BAF. However, no differences in the severity of illness and clinical course between the two groups were observed. Patients in the BAF group had less severe airflow obstruction, but more common and severe pulmonary hypertension signs than those in the non-BAF group. Conclusion: Compared with non-BAF COPD, BAF may be associated with milder airflow limitation and more frequent signs of pulmonary hypertension with a more severe grade in patients presenting with COPD exacerbation.

  • Nitric acid is an oxidizing agent used in metal refining and cleaning, electroplating, and other industrial applications. Its accidental spillage generates oxides of nitrogen, including nitric oxide (NO) and nitrogen dioxide ($NO_2$), which cause chemical pneumonitis when inhaled. The clinical presentation of a nitric acid inhalation injury depends on the duration and intensity of exposure. In mild cases, there may be no symptoms during the first few hours after exposure, or the typical symptoms of pulmonary edema can appear within 3-24 hours. However, in cases of prolonged exposure, progressive pulmonary edema develops instantaneously and patients may not survive for more than 24 hours. We report a case of a 44-year-old male who was presented with acute respiratory distress syndrome after nitric acid inhalation. He complained of cough and dyspnea of a sudden onset after inhaling nitric acid fumes at his workplace over a four-hour period. He required endotracheal intubation and mechanical ventilation due to fulminant respiratory failure. He was managed successfully with mechanical ventilation using positive end expiratory pressure and systemic corticosteroids, and recovered fully without any deterioration in his pulmonary function.

  • Handling of radioisotopes may cause external and internal contamination to occupational workers while using radiation for medical purposes. This research aims to monitor the internal hazard of occupational workers who handle 131I. Two methods are used: in vivo or direct method and in vitro or indirect method. The in vivo or direct method was performed by assessing thyroid intake with a thyroid uptake monitoring machine. The in vitro or indirect method was performed by assessing urine samples with the help of a gamma-ray spectroscopy practice using a High-Purity Germanium (HPGe) Detector. In this study, fifty-nine thyroid counts and fifty-nine urine samples were collected from seven occupational workers who were in charge of 131I at the National Institute of Nuclear Medicine and Allied Sciences (NINMAS), Dhaka. The result showed that the average annual effective dose of seven workforces from thyroid counts were 0.0208 mSv/y, 0.0180 mSv/y, 0.0135 mSv/y, 0.0169 m Sv/y, 0.0072 mSv/y, 0.0181 mSv/y, 0.0164 mSv/y and in urine samples 0.0832 mSv/y, 0.0770 mSv/y, 0.0732 mSv/y, 0.0693 mSv/y, 0.0715 mSv/y, 0.0662 mSv/y, 0.0708 mSv/y.The total annual effective dose (in vivo and in vitro method) was found among seven workers in average 0.1039 mSv/y, 0.0950 mSv/y, 0.0868 mSv/y, 0.0862 mSv/y, 0.0787 mSv/y, 0.0843 mSv/y, 0.0872 mSv/y. Following the rules of the International Commission on Radiological Protection (ICRP), the annual limit of effective dose for occupational exposure is 20 mSv per year and the finding values from this research work are lesser than this safety boundary.

Occupational Lifting Tasks and Retinal Detachment in Non-Myopics and Myopics: Extended Analysis of a Case-Control Study

  • Objectives: Lifting heavy weights involves the Valsalva manoeuvre, which leads to intraocular pressure spikes. We used data from a case-control study to further investigate the hypothesis that occupational lifting is a risk factor for retinal detachment. Methods: The study population included 48 cases (patients operated for retinal detachment) and 84 controls (outpatients attending an eye clinic). The odds ratios (OR) of idiopathic retinal detachment were estimated with a logistic regression model (adjusted for age, sex and body mass index). Three indexes were used to examine exposure to lifting; 1) maximum load lifted, 2) average weekly lifting, 3) lifelong cumulative lifting. Results: For all indexes, the most exposed subjects showed an increased risk of retinal detachment compared with the unexposed (index 1: OR 3.57, 95% confidence interval [CI] 1.21-10.48; index 2: OR 3.24, 95% CI 1.32-7.97; index 3: OR 2.23, 95% CI 1.27-8.74) and dose-response relationships were apparent. Conclusion: These results reinforce the hypothesis that heavy occupational lifting may be a relevant risk factor for retinal detachment.

Mechanical Behavior and Characteristics of Internal Temperature and Relative Humidity of Concrete at Early Age (초기재령 콘크리트의 역학적 특성 및 온·습도 거동 특성 분석)

  • This study is to analyze the internal temperature and relative humidity of concrete at early age, as well as the mechanical behavior. Three different levels of cement unit content were cosidered as an experimental variable. In order to measure internal temperature and relative humidity immediately after concrete placement, this study developed a unique measuring device, which provided reliable results. Different cement content did not significantly affected the strengths including compressive, tensile and flexural strength and after 7 days of curing, strengths did not increase noticeably. Internal temperature reached the maximum about 11 hours later the placement and decreased after removal of forms. The internal temperature varied depending on the location and the exposure condition. In addition, the internal relative humidity was more affected by the exposure condition rather than the cement content.

Nursing Students' Exposure to Blood and Body Fluids in Clinical Practicum (일 대학 간호학생의 임상실습 중 혈액 및 체액 노출 정도)

  • Purpose: Nursing students face constant threats of blood-borne infections such as HIV, Hepatitis B, and Hepatitis C during their clinical rotation period. This study was done to determine the frequency and risk of the exposure to blood and body fluids. Methods: The data was collected using a questionnaire. Ninety eight junior & senior nursing students were participated in this study. Results: 75.6% of the participants reported more than once during practicum. The exposure occurred most frequently while checking the blood glucose (65.3%). The general ward (45.3%) was the most frequent site for the occurrence of the exposure. However, only eight cases (10.7%) were reported to the medical or nursing personnel in charge. Conclusions: This study shows nursing students are in a great risk of the exposure to blood & body fluid. The risk is highest in the general ward during checking the blood glucose. Thorough education on preventive measures, should be provided to nursing students prior to clinical practium.

Effect of High Temperature Degradation on Microstructure and High Temperature Mechanical Properties of Inconel 617 (Inconel 617의 고온열화에 따른 미세구조 및 고온 기계적 특성)

  • Inconel 617 is a candidate tube material for high temperature gas-cooled reactors(HTGR). The microstructure and mechanical properties of Inconel 617 were studied after exposure at high temperature($1050^{\circ}C$). The dominant oxide layer was Cr-oxide. The internal oxide and Cr-depleted region were observed below the Cr-oxide layer. The depth of Cr-depleted zone and internal oxide increased with exposure time. The major phases of carbides are $M_{23}C_6\;and\;M_6C$. The composition of $M_{23}C_6\;and\;M_6C$ were determined to be Cr-rich and Mo-rich, respectively. $M_6C$ carbide is more stable than $M_{23}C_6$ at high temperature. From the results of high temperature compression test, there were no significant changes in hardness and yield strength upon increasing exposure time.

Radiological safety assessment of lead shielded spent resin treatment facility with the treatment capacity of 1 ton/day

  • The radiological safety of the spent resin treatment facility with a14C treatment capacity of 1 ton/day was evaluated in terms of the external and internal exposure of worker according to operation scenario. In terms of external dose, the annual dose for close work for 1 h/day at a distance of more than 1 m (19.8 mSv) satisfied the annual dose limit. For 8 h of close work per day, the annual dose exceeded the dose limit. For remote work of 2000 h/year, the annual dose was 14.4 mSv. Lead shielding was considered to reduce exposure dose, and the highest annual dose during close work for 1 h/day corresponded to 6.75 mSv. For close work of 2000 h/year and lead thickness exceeding 1.5 cm, the highest value of annual dose was derived as 13.2 mSv. In terms of internal exposure, the initial year dose was estimated to be 1.14E+03 mSv when conservatively 100% of the nuclides were assumed to leak. The allowable outflow rate was derived as 7.77E-02% and 2.00E-01% for the average limit of 20 mSv and the maximum limit of 50 mSv, respectively, where the annual replacement of the worker was required for 50 mSv.
