  • 본 논문에서는 WBAN(Wireless Body Area Network) On-body 시스템에서 표면 지향 안테나를 사용하여 인체 내부 기관에 의한 영향을 분석하였다. 인체 내부 기관의 영향을 확인하기 위하여 인체 상반신 모델과 실제 인체에 안테나를 부착하여 수신 신호의 세기를 측정하였다. 실험은 인체에 대한 고유 영향을 보기 위하여 무반향실에서 움직임 없이 수행하였고, VNA(Vector Network Analyzer)를 이용하여 수신 신호 세기를 측정하였다. 측정된 데이터를 이용하여 인체 모델과 상반신 모델의 수신 신호 세기를 비교하였고, 인체 내부 기관이 안테나 수신 신호 세기에 미치는 효과를 분석하였다.

간문 주위 림프절병증으로 발현된 아밀로이드증 1예 (A Case of Amyloidosis Presenting as Lymphadenopathy at the Porta Hepatis)

  • We report a rare case of systemic amyloidosis with gastrointestinal and lymph node involvement. A 64-year-old woman was admitted to our hospital with dyspepsia and weight loss. Initial esophagogastroduodenoscopy (EGD) revealed nonspecific findings, and abdominal computed tomography showed necrotizing lymphadenopathy at the porta hepatis. Laparoscopic lymph node biopsy was performed under suspicion of tuberculous lymphadenopathy, but a definite diagnosis was not established. Follow-up EGD performed 6 months later revealed multiple telangiectasia-like lesions at the gastric body, and endoscopic biopsy revealed amyloid deposition. Through additional blood and urine protein electrophoresis, the patient was finally diagnosed with systemic amyloidosis associated with multiple myeloma. She was treated with dexamethasone, thalidomide, and bortezomib; however, she died 3 months after diagnosis because of pneumonia and multiple organ failure.

호흡주기에 따른 방사선입체조형치료법의 개발 (Development of Conformal Radiotherapy with Respiratory Gate Device)

  • 목적 : 호흡주기에 따른 위치변동 감지센서를 이용하여 종양의 위치가 일정워치에 있을 때만 방사선을 치료하는 호흡 동기치료기구를 제작하고 일정한 호흡주기 상태에서 수행된 CT simulation과 3차원 입체조형치료계획에 따라 방사선을 치료하는 시스템을 개발하고자 하였다. 호흡유무에 따른 종양의 치료 마진(margin)을 측정하고 계획용표적체적(planning target volume:PTV)의 크기에 따른 선량체적표(dose volume histogram:DVH)와 종양억제확률(tumor control probability:NTCP), 건강조직손상확률(normal tissue complication probability:NTCP) 및 선량 통계자료를 통하여 치료성과를 평가하고 선량증강 범위를 예측하고자 하였다. 대상 및 방법 : 종양이 비교적 작고 전이가 없는(T1N0M0) 5명의 폐암환자를 선택하여 X-선 조준장치를 이용하여 횡격막의 이동거리를 측정하는 방법으로 내부장기의 운동을 평가하였다. 호흡동기치료기구는 끌어당김 센서가 부착된 허리띠 모양으로 구성되었으며 이를 흉곽 또는 복부에 부착하여 호흡주기에 의한 흉곽의 크기변동에 따라 센서의 회로가 개폐되고 이것을 선형가속기의 조종간에 연결하는 간단한 기구로서 감도와 재현성이 높았다. 호흡을 배기한 후 일시적 호흡이 정지된 상태에서 Spiral-CT (PQ-5000)로 3차원 영상을 획득하고 Virtual CT-simulator (AcQ-SIM)에 의하여 종양의 위치와 주위 장기들을 확인 도시하였으며 3차원 치료계획장치(Pinnacle, ADAC Co.)를 이용하여 3차원 입체조형치료를 계획하였다. 치료계획의 평가는 호흡동기치료기구의 사용유무에 따른 PTV의 크기에 따라 최적 선량분포를 구사하였으며 각각의 DVH, TCP, NTCP 및 선량통계자료를 도출 비교 검토하였다. 결과 : X-선 simulation에서 폐암환자의 횡격막 이동은 약 1 cm에서 2.5 cm로서 평균 1.5 cm로 측정되었고 자유호흡시 PTV는 CTV (clinical target volume)에 약 2 cm 마진을 주었으며 호흡동기치료기구를 사용하였을 때는 0.5 cm 마진이 적당한 것으로 측정되었다. 종양의 PTV는 연장 마진의 거의 자승비로 증가하였으며 TCP의 값은 마진 범위 $(0.5\~2.0\;cm)$에 관계없이 거의 일정하였고 NTCP의 값은 마진 크기에 따라 평균 $65\%$로 급속히 증가하였다. 결론 : 호흡주기에 따른 위치변동 감지센서를 이용한 호흡동기치료기구는 종양의 위치가 일정할 때만 방사선이 조사되는 간단하고 정확한 장치로서 3차원 입체조형치료 및 강도변조방사선치료에서 매우 유용한 장치임을 확인할 수 있었다. 또한 호흡조절 방사선입체조형치료방법의 기술과 시술절차를 확립시키고 정량적인 선량평가를 위하여 DVH, TCP, NTCP 등의 정량분석과 종양의 투여 선량 증가량(dose escalation)을 예측하는 기초자료를 제공할 수 있었다.

정천화담항기탕(定喘化痰降氣湯)이 ovalbumin으로 유발된 천식(喘息)에 미치는 영향(影響) (The Histopathologic Effects of Jungcheonwhadamgangki-tang on the Experimental Asthma induced by Ovalbumin)

  • Objectives: In order to evaluate the effect of jungcheonwhadamgangki-tang on Balb/c mouse of allergy-sensitive to bronchial asthma induced by ovalbumin. Methods: The changes of diameter lumen of trachea which was upper respiratory organ, weight and gross appearance of lung, histology of lung and trachea, numbers of inflammatory cells in the bronchoalveolar lavage fluld(BALF) were observed. Results : The results are obtained as follows. 1. The diameters of trachea lumen were significantly increased in Jungcheonwhadam gangki-tang treated group as compared with control group. 2. Inflammatory cells including neutrophil and eosinophil in BALF were significantly. in Jungcheonwhadamgangki-tang treated group as compared with control group. 3. Weight of lung and black spots, which resulted from infiltration of inflammatory were significantly decreased in Jungcheonwhadamgangki-tang treated group as compared with control group.4. Hypertrophy of mucous membrane of trachea and bronchus and bronchioles in the lung, peritracheal, peribronchus and peribronchiolar inflammatory cell infiltration, and mucoid exudate deposit in the lumen were significantly decreased in Jungcheonwhadamgangki-tang treated group as compared with control group. Conclusions : It is considered that Jungcheonwhadamgangki-tang has somewhat favorable effect on the bronchial asthma because the bronchial asthma specific series of abnormalities in respiratory system were decreased after oral administration of Jungcheonwhadamgangki-tang in this study. In future, it is needed that the toxicological and dosage specific study of Jungcheonwhadamgangki-tang to use against bronchial asthma with safe.

  • Immunoglobulin (Ig) G4-related disease is a recently recognized systemic fibroinflammatory condition characterized by a lymphoplasmacytic infiltrate rich in IgG4-positive plasma cells with elevated circulating levels of IgG4. The disease can either be localized to one or two organs, or present as diffuse multi-organ disease. Furthermore, lesions in different organs can present simultaneously or metachronously. In the pulmonary manefestations, lesions associated with IgG4-related disease have been described in the lung parenchyma, airways and pleura, as well as the mediastinum. We report a case of IgG4-related disease presenting as massive pleural effusion and thrombophlebitis.

  • The ulcerative colitis is a chronic inflammatory bowel disease with an unknown etiology. The major symptoms of ulcerative colitis are diarrhea, abdominal pain and hematochezia. However, arthritis, skin disorders, hepatobiliary inflammation and uveitis are occasionally recognized as systemic complications. Although there are few reports of coexistent pulmonary and inflammatory bowel disease, the lung is not generally considered to be a target organ in ulcerative colitis. We report a patient with ulcerative colitis-related bronchilolitis obliterans organizing pneumonia confirmed by video-assisted thoracoscopic surgery, who responded to corticosteroid therapy.

귀비탕(歸脾湯)이 비소세포 폐암세포(NCI-H520) 이식 마우스에서 항암 및 면역 활성에 미치는 영향 (Anticancer and Related Immunomodulatory Effects of Kwibi-tang on Non-small Cell Lung Carcinoma, NCI-H520, Xenograft Mice)

  • Objectives : This study was to observe anticancer and related immunomodulatory effects of Kwibi-tang extracts (KBTe) on non-small cell lung carcinoma (squamous epithelial carcinoma), NCI-H520, xenograft Balb/c nu-nu nude mice. Methods : Three different dosages of KBTe, 50, 100 and 200 mg/kg were orally administered once a day for 42 days from 11 days after tumor cell inoculation. Six groups, each of 8 mice per group were used in the present study. Changes in body weight, tumor volume and weight, lymphatic organs (spleen and popliteal lymph node), serum interferon (IFN)-${\gamma}$ levels, splenocytes NK cell activity and peritoneal macrophage activities, splenic tumor necrosis factor (TNF)-${\alpha}$, interleukin (IL)-$1{\beta}$ and IL-10 contents were observed with tumor mass and lymphatic organ histopathology to detect anticancer and immunomodulatory effects. The results were compared with a potent cytotoxic anticancer agent, 5-FU (5-Fluorouracil) 30 mg/kg, intraperitoneal treatment (3-day intervals for 42 days, the optimal effective treatment regimes already confirmed). Results & Conclusions : This study suggest that over 50 mg/kg of KBTe showed favorable anticancer effects on the NCI-H520 cell xenograft with immunomodulatory effects. Although relatively lower anticancer effects were observed in KBTe 200 mg/kg treated mice as compared with 5-FU 30 mg/kg treated mice, no meaningful favorable immunomodulatory effects were observed after 5-FU treatment in the present study.

아차(兒茶)가 Cationic Bovine Serum Albumin 투여로 유발된 Membranous Nephropathy Mouse Model에 미치는 영향 (Effects of the Acasia Catechu Extract on the Membranous Nephropathy Induced by Cationic Bovine Serum Albumin in Mice)

  • Objective : Membranous nephropathy(MN) is an organ-specific autoimmune disease and a relatively common cause of nephrotic syndrome in adults worldwide. But treatment of MN is not defined. This study was to evaluate the effects of Acasia Catechu extract(ACE) on the MN induced by cBSA in mice. Methods : Mice were divided into 4 groups. The normal group was injected with a saline solution. The control group was treated with cBSA(10 mg/kg i.p.) only. The third group was treated with cBSA (10 mg/kg i.p.) and ACE (250 mg/kg, p.o.). The fourth group was treated with cBSA (10mg/kg i.p.) and ACE (500mg/kg, p.o.). After cBSA and ACE treatment for 6 weeks, we measured change of body weight, 24hrs proteinuria, serum albumin, total cholesterol, triglyceride, BUN, creatinine, TNF-$\alpha$, IL-6, IL-$1{\beta}$, IFN-$\gamma$, IgA, IgM and IgG levels. The morphologic changes of renal glomeruli were also observed with a light microscope. Results : The levels of 24 hrs proteinuria, total cholesterol, triglyceride, IgG, IgM, IgA, TNF-$\alpha$, IL-6, IL-$1{\beta}$, IFN-$\gamma$ significantly decreased in both ACE groups. The level of albumin significantly increased in both ACE groups. The mRNA expression of IL-$1{\beta}$ in splenocytes considerably decreased in the ACE-500 group. In histological findings of kidney tissue, thickening of GBM decreased in both ACE groups. Conclusions : This study shows that ACE might be effective for treatment of MN. More clinical data and studies are to be done for efficient application.

야관청혈탕이 고지방식이로 유도된 흰쥐의 비만 억제에 미치는 영향 (Effect of Yagwan-cheunghyeoltang on Obesity in Rats Induced by High Fat Diet)

  • Background : Obesity, the syndrome caused by a high fat diet, is a disease. At the same time, obesity causes diabetes mellitus, hyperlipidemia, and cardiovascular disease. Recently, its prevalence rate is increasing. Yagwan-cheunghyeoltang (YCT) used in this experiment is the prescription of Yagwanmoon added to Cheunghyeol-tang which is reported to be very effective in weight loss controlling and serum cholesterol. It is also reported that Yagwanmoon has significant antioxidant effects and YCT has a significant effect on blood glucose control. Objectives : This study was conducted to experimentally evaluate the effects of YCT on obesity in rats induced by high fat diet. Methods : The experiment was conducted with 4-week-old male rat s divided into 5 groups. They were a normal diet group, a high fat diet group, a positive drug control group, a 1% YCT group, and a YCT 3% group, and were tested for eight weeks. After four weeks of inducing obesity by a high fat diet, rats were allowed to lose weight by following the normal diet group, approximately 30% compared with 10 rats in each group were determined as still obese. Changes in body weight and organ weight and serum cholesterol, triglyceride, glucose-density, low density lipoprotein cholesterol, antioxidant activity were checked. Results : In the experimental groups, we observed weight loss and visceral fat reduction, improvement of liver function, reduction of serum glucose, activation of HMG-CoA reductase inhibitor, reduction of concentrations of leptin and it showed a significant effect on antioxidants and lipid peroxidation. Conclusions : YCT has significant effects on the regulation of hyperlipidemia and lipid peroxidation associated with obesity and has significant effects on, antioxidants and lipid peroxidation, too. Additional clinical studies are needed.
