Teratoma Presenting as An Unilateral Mediastinal Mass with Contralateral Pleural Effusion (편측성 종격동 종괴와 반대측 흉수로 발현된 기형종 1예)

  • A teratoma is the most common benign germ cell tumor that develops in the mediastinum. Patients with a mediastinal teratoma are usually asymptomatic. However, a spontaneous rupture of a mediastinal teratoma into the pleural cavity or adjacent organs can cause severe chest pain, hemoptysis, acute dyspnea, etc. Complications such as recurrent pneumonia, pericardial effusion, pleural effusion and great vessel invasion can sometimes occur. We encountered a case of a patient with an abrupt onset of dyspnea after persistent shoulder pain for one month. The X-ray examinations revealed a unilateral mediastinal mass with contralateral pleural effusion. Subsequent evaluations confirmed a spontaneous rupture of the teratoma into the contralateral pleural cavity.

Internal Quality Evaluation and Age Identification of Fresh Korean Ginseng using Magnetic Resonance Imaging (자기공명영상을 이용한 수삼의 내부 품질평가 및 연근판정)

  • The purpose of this study is to characterize the internal physical properties of fresh Korean ginsengs (Panax ginseng C.A. Meyer) through a magnetic resonance imaging (MRI) technique. Current external visual inspection cannot determine internal quality of ginsengs successfully. Relaxation time constants, T$_1$ and T$_2$*, were obtained from a series of MR images. Calculated Ti values were varied with different physiological states of ginseng tissues. Internal imaging information was obtained nondestructively from fresh ginsengs. One- and two-dimensional image analyses were performed. One-dimensional image analysis showed a potential of age identification of ginsengs rapidly. Internal quality of normal and abnormal ginsengs was evaluated using two-dimensional MR images. Various types of internal defects such as internal cavity and rotten spot were visualized clearly. The MRI technique had a feasibility to detect internal defects of fresh ginsengs effectively.

Laparoscopic Retrieval of Ectopic Adult Heartworms from the Abdominal Cavity of a Dog with Heartworm Infestation (심장사상충 복강내 이소기생 개에서 복강경적 치료 1예)

  • A 5-year-old, intact male Beagle was presented with chronic abdominal pain. The dog was diagnosed with dirofilariasis by positive heartworm antigen detection via ELISA and concurrent microfilaria. Thoracic radiographs revealed cardiomegaly with dilation of the main pulmonary artery. Echocardiography revealed the adult worms in the main pulmonary arteries, but other abnormalities other than heartworm infection were not present. To find the cause of the abdominal pain, exploratory laparoscopy was performed. Ectopic migrating adult heart worms were visualized through exploratory laparoscopy and the clinical sign resolved after removing the heart worm. This report describes removing the ectopic migrating adult heartworms using exploratory laparoscopy in the abdominal cavity.

  • Patients with mediastinal teratoma are usually asymptomatic, but may develop symptoms by rupture into adjacent structures which result in pneumonia, hemoptysis, pleural effusion, pericardial effusion, or pneumothorax. Rarely, life-threatening acute respiratory distress require a emergency surgery. Rupture into pleural cavity may result in pleuritis and pleural effusion with severe anterior chest or back pain. The symptom must be differentiated from other common intrathoracic distress diseases. Clinical, cytologic and radiologic examinations of pleural effusion, and moreover, measurement of enzymes such as amylase or insulin, which is secreted from pancreatic tissues, in pleural effusion and cystic fluid enabled us to make the diagnosis of rupture of mediastinal teratoma preoperatively.

Effects of Bistortae Rhizoma on Hemostasia, Anti-inflammatory Action and Central Nervous System (권삼(拳蔘)이 지혈(止血).소염작용(消炎作用) 및 중추신경계(中樞神經系)에 미치는 영향(影響))

  • Objective : The purpose of these research was to investigate effects of water extract of Bistortae Rhizoma(BRE) on the hemostasia, anti-inflammatory action and central nervous system. Methods : we used mice and rats administered with the extract of the above herbs. Results : BRE decreased the permeability of evans blue into peritoneal cavity and cotton pellet granuloma formation. BRE did not decrease the acetic acid induced writhing syndrome and the histamine induced mouse paw edema. BRE inhibited the pentylenetetrazole and the strychnine induced convulsion. BRE shortened the bleeding time and plasma prtrombin time. BRE did not affect on the proliferation of Balb/c 3T3 cells. Conclusions : these results suggest that the effects of BRE are the hemostasia, anti-inflammatory action, and mild depressant activity of central nervous system.

  • Thixoforming process has been accepted as a new method for fabricating near net shaped products with lighweight aluminum alloys. The thixoforming process consists of reheating process of billet, billet handing filling into the die cavity and solidification of thixoformed part,. in this paper the thixoforming experiments are performed with two different die temperature ({{{{ TAU _d}}}}=20$0^{\circ}C$ 30$0^{\circ}C$) and orifice gate type. The microstructures of SSM(357, A490 and ALTHIX 86S) fabricated in thixoforming process are evaluated in therms of globularization and grain size. effect of alloying elements onthe surface and internal defects is investigated. Finally the methods to obtain the thixoformed products with good mechanical propertis are proposed by solution for avoiding the surface and internal defects.

  • The nondestructive internal quality evaluation of agricultural products has been strongly required from the needs for individual inspection. In recent, ultrasonic wave has been considered as a solution for this problem. It transmit well through most materials and can handle safely and easily. However, specially in a watermelon, it is known that general frequency band (higher than 20kHz) ultrasonic waves do not transmitted well due to severe attenuation. The objectives of this study were to find out the proper waveform and frequency of the ultrasonic waves that transmit well inside the watermelon, and to analyze the transmitted waveform in order to make clear the structure of wave propagation in watermelon. The result of several experiments showed that 2kHz shear wave was the most suitable for the detection of internal cavity in the watermelon. Also, it was found that the surface wave did not influence the directly transmitted bulk wave. These results could be a basis of application of ultrasonic wave on the evaluation of internal quality of the watermelon.

  • Ascites is the retention of body fluids in abdominal cavity, which is induced by salt increase, and disorders in such things as water retention and albumin synthesis. Ascites are especially common in cases of progressed liver cirrhosis and metastatic cancer. One patient was diagnosed with liver cirrhosis and Metastatic cancer at Rt. liver from Lt. ovarian tumor by abdominal CT. To measure the response to treatment of ascites, these clinical symptoms were observed: weight, abdominal circumference, and lab findings. Clinical symptoms and ascites were improved after oriental medical treatment. Abdominal circumference decreased. Results suggest oriental medical treatment was effective for this patient for this disease, but more extensive research is needed.

  • We report here a case of oral myiasis in the Republic of Korea. The patient was a 37-year-old man with a 30-year history of Becker's muscular dystrophy. He was intubated due to dyspnea 8 days prior to admission to an intensive care unit (ICU). A few hours after the ICU admission, 43 fly larvae were found during suction of the oral cavity. All maggots were identified as the third instars of Lucilia sericata (Diptera: Calliphoridae) by morphology. We discussed on the characteristics of myiasis acquired in Korea, including the infection risk and predisposing factors.

A Study on the Noise Reduction through the Control of Internal Flow for a Slim Type External ODD (슬림타입 외장형 ODD 내부의 유동 안정을 통한 소음저감에 관한 연구)

  • The demand for the laptop computer has been increased day by day and most of users ask quiet computer and devices to work in comfortable environment. One of the devices which generate acoustic noise is an external ODD. Unlike the internal ODD, the external ODD is easy to emit noise because it runs outside of the computer and also it is packed with a thin plastic covers. As the disk rotates, vortex flow is generated inside of the cavity due to various and complicated mold parts of the cover. In addition, there is a gap between the disk tray and the upper/lower cases, through which the air flows as well as the noise leaks. In this study, we have proposed how to reduce the acoustic noise of an external ODD using numerical and experimental analysis. The pressure fluctuations and turbulent kinetic energy distributions are calculated for the developed model. The results show that the sound pressure level is reduced by 2.3dB through simple modifications of ribs of the top cover, which remove or suppress flow instabilities inside of the cavity.
