가정건강관리를 위한 커뮤니티케어 작업치료프로그램 적용 효과 (Effect of Implementing a Community-Care Occupational Therapy Program for Home Health Management)

  • Purpose : Community care occupational therapy is an integrated mediation method such as evaluating and mediating daily life activities, evaluating and modifying the environment, and providing information by visiting the home where the client resides. In this study, we examine the effect of implementing a community-care occupational therapy program on the levels of depression, quality of sleep, and quality of life of elderly individuals living alone. Methods : Among the users of organizations C and D located in City B, province A, South Korea, a total of 32 elderly citizens aged above 65 were chosen as the study subjects and randomly divided into an experimental group and a control group. For the experimental group, a total of 24 sessions, each lasting 60 minutes, comprising a community-care occupational therapy program and each organization's own program, were conducted, with each session conducted twice a week for a total of 12 weeks. On the other hand, those in the control group participated in the existing programs of their organizations. Changes in the levels of depression, quality of sleep, and quality of life before and after the intervention were compared using the GDSSF-K, GQOL, and GQOL. Results : Overall, the levels of depression, quality of sleep, and quality of life were found to improve among the experimental group after the intervention. In a between-group comparison of the changes before and after the intervention, the experimental group was found to exhibit a more statistically significant difference than that of the control group. Conclusion : The results obtained in this study confirm that community-care occupational therapy programs have a positive effect on the levels of depression, quality of sleep, and quality of life of elderly individuals living alone. This implies that such programs may represent an effective solution for the problems of these individuals.

치매안심센터 종사자의 직무만족, 조직몰입 및 이직의도 분석 (Analysis of Job Satisfaction, Organizational Commitment, and Turnover Intention of Dementia Care Center Employees)

  • Purpose : This study aimed to identify the job satisfaction, organizational commitment, and turnover intention of workers at dementia care centers, and the correlations between these factors. Methods : A survey of 131 dementia care center employees was conducted. This comprised 48 questions covering respondent characteristics, organizational satisfaction, organizational commitment, and turnover intention. Results : Employees with no intention to change jobs had significantly higher job satisfaction and organizational commitment than those with job change intentions. Turnover intention was significantly correlated with both job satisfaction and organizational commitment. The higher the organizational commitment and the lower the turnover intention, the higher the job satisfaction. The higher the job satisfaction and the lower the turnover intention, the higher the organizational commitment. Analysis of the sub-components of the main variables found The lower the work environment, compensation, and sense of belonging to the organization, the higher the turnover intention. Conclusion : Regular educational programs are needed to improve the expertise of workers at the dementia care center, and opportunities for employees to communicate their needs should be maximized. In addition, higher salaries will attract more able and willing employees, reduce turnover, and provide and a sense of of belonging to the organization. Attention to the proper distribution of work and recruitment of appropriate human resources will further improve the working environment. If the job satisfaction and organizational commitment of workers at the dementia care center are increased and the number of employees intending to change jobs is reduced, the length of employee service will increase and contribute to improved care and service quality for dementia patients.

체외막산소공급(ECMO) 치료 환자의 간호 중재에 대한 체계적 고찰 (A Systematic Review of Nursing Interventions in Patients with Extracorporeal Membrane Oxygenation (ECMO))

  • Purpose : This study aimed to systematically review the effectiveness of nursing interventions for patients receiving extracorporeal membrane oxygenation (ECMO). As the use of ECMO increases in critical care settings, it is important to understand how nursing interventions affect patient outcomes, survival, and complication rates. Methods : This systematic review followed the preferred reporting items for systematic reviews and meta-analysis guidelines. A literature search was performed using terms related to ECMO and nursing interventions in several international electronic databases including CINAHL, Embase, MEDLINE, and Web of Science. Studies were screened and selected according to predefined eligibility criteria, focusing on those that evaluated the impact of nursing interventions on adult. Data extraction and risk-of-bias assessment were independently performed by two researchers. Results : A total of 647 studies were identified, and seven met the inclusion criteria for qualitative analysis. The included studies demonstrated that high-quality nursing care significantly improves clinical outcomes and reduces complications in patients receiving ECMO. Effective nursing interventions included prone positioning combined with ECMO for patients with acute respiratory distress syndrome, meticulous infection control, comprehensive and continuous nursing protocols, skilled nursing, and multidisciplinary management. These interventions have been shown to improve oxygenation, reduce complications, such as bleeding, manage blood pressure, and enhance overall clinical outcomes. Conclusion : High-quality nursing interventions are critical to improve survival and reduce complications in patients receiving ECMO. Implementing a multidisciplinary approach and comprehensive nursing protocols, including infection control and psychological support, is essential for the effective management of these patients. The findings of this study provide a foundation for the development of practical guidelines and educational programs to improve the quality of care for patients undergoing ECMO, ultimately enhancing the effectiveness of ECMO treatment and patient outcomes.

Hospice and Palliative Care for Patients in the Intensive Care Unit: Current Status in Countries Other than Korea

  • Although most patients prefer dying at home, patients whose condition rapidly becomes critical need care in the intensive care unit (ICU), and it is rare for them to die at home with their families. Therefore, interest in hospice and palliative care for patients in the ICU is increasing. Hospice and palliative care (PC) is necessary for all patients with life-threatening diseases. The following patients need palliative care in the ICU: patients with chronic critical illnesses who need tracheostomy, percutaneous gastrostomy tube, and extracorporeal life support; patients aged 80 years or older; stage 4 cancer patients; patients with specific acute diseases with a poor prognosis (e.g., anoxic brain injury and intracerebral hemorrhage requiring mechanical ventilation); and patients for whom the attending physician expects a poor prognosis. There are two PC models-a consultative model and an integrative model-in the ICU setting. Since these two models have advantages and disadvantages, it is necessary to apply the model that best fits each hospital's circumstances. Furthermore, interdisciplinary decision-making between the ICU care team and PC specialists should be strengthened to increase the provision of hospice and palliative care services for patients expected to have poor outcomes and their families.

입원 중인 자살시도 환자에 대한 간호사의 태도; 통합적 문헌고찰 (Nurses' Attitude toward Hospitalized Patients after Attempted Suicide; Integrative Review)

  • Purpose : This study aims to identify nurses' attitudes toward patients hospitalized after attempting suicide and inform directions for research and interventions to establish a therapeutic environment. Methods : For integrative review, the five stages suggested by Whittemore and Knafl were followed. Articles and theses published before February 2021 were searched using eight databases and a manual search. The search terms were suicide, self-harm, emergency department (ED), intensive care unit (ICU), nurses, and attitudes. Results : Five studies met the inclusion criteria; three of them were descriptive studies, one was a qualitative study, and one a mixed-method study. In most studies (80%), the participants were ED nurses. ED/ICU nurses showed positive or neutral attitudes toward patients. All studies suggested that suicide-related education programs are needed to increase nurses' positive attitudes such patients. Conclusion : More research is required on ICU nurses' attitudes toward patients who have attempted suicide, and efforts to identify various aspects of such attitudes are necessary. In addition, suicide education programs and support from trained psychiatric personnel should be provided to establish a therapeutic environment.

한.양방 통합의료의 갈등과 방향에 대한 연구 - 한.양방 의료 및 관련 종사자 대상 심층면접을 중심으로 - (Research on the Conflicts and Future Direction of Integrative Medicine in Korea)

  • This study examined the knowledge and understanding of integrative medicine in Korea, specifically conflicts between western and oriental medicine within Parallel (Dual) health care systems. Qualitative methodology using grounded theory guided semi-structured, in-depth interviews with Western Medical Doctors (W.M.D., n = 6), Oriental Medical Doctors (O.M.D., n = 5) and Traditional Chinese Medicine Practitioners (T.C.M.P., n = 4). Thematic analysis was used to determine broad themes from the interviews. 15 professionals (W.M.D. (40%), O.M.D. (33%), T.C.M.P. (27%), 10 males (67%) and 5 females (33%), mean age 45) were interviewed, recorded, and transcribed. Thematic analysis revealed three key themes: systematic conflicts, integration and future directions. Subthemes of systematic conflicts included: credibility of Oriental Medicine, commercial imperatives, maintaining social standing of O.M.D., professional qualifications and lack structures supporting collaborative practice. Integration subthemes included lack of academic linkage and clarity for appropriate triage, opposing medical paradigms and limited social imperative. Future directions should include: social justification, guarantee of oriental medicine legitimacy, role of government and understanding of scientific evidence. To successfully integrate dual medical systems there is a need to address differences in social-environmental factors and perceptions of scientific understanding, as well as developing strong academic links in clinical practice.

집중치료 후 증후군에 관한 개념분석 (Concept Analysis of Post Intensive Care Syndrome)

  • Purpose: The purpose of this study was to identify the attributes, antecedents, and consequences of post-intensive care syndrome. Methods: The study was conducted in accordance with Walker and Avant's conceptual analysis process. We searched the Medline, ProQuest, Google scholar, NANET (Korean National Assembly Library), and RISS (Korean Education and Research Information Service) databases resulting in 29 studies for in-depth review. Results: The attributes of post-intensive care syndrome include (1) new or worsening impairment persisting after discharge, (2) physical impairment, (3) psychological impairment, (4) cognitive impairment, and (5) symptom experience. The antecedents of the concept include a critical condition that requires intensive care, stressful events, immobility, sedation, delirium, insomnia, frailty, and invasive procedures. The consequences of post-intensive care syndrome are increased dependency in activities of daily life, decreased social interaction, delayed return to previous occupation, decreased quality of life, familial post-intensive care syndrome, and readmission/death. Conclusion: The concept of post-intensive care syndrome has multifaceted and integrative features. Further research needs to be conducted to develop a measurement tool that reflects the attributes of the concept and to develop multidisciplinary interventions to prevent post-intensive care syndrome.

  • Ginseng has been used as a traditional herb in Asian countries for thousands of years. It contains a large number of active ingredients including steroidal saponins, protopanaxadiols, and protopanaxatriols, collectively known as ginsenosides. In the last few decades, the antioxidative and anticancer effects of ginseng, in addition to its effects on improving immunity, energy and sexuality, and combating cardiovascular diseases, diabetes mellitus, and neurological diseases, have been studied in both basic and clinical research. Ginseng could be a valuable resource for future drug development; however, further higher quality evidence is required. Moreover, ginseng may have drug interactions although the available evidence suggests it is a relatively safe product. This article reviews the bioactive compounds, global distribution, and therapeutic potential of plants in the genus Panax.
