Objectives: The purpose of this study is to investigate the influencing factor of the quality of life of the elderly with mild dementia in rural community. Methods: A total of 123 elderly who were managed by public health center participated. They were diagnosed as mild dementia by MMSE-DS and CDR. Data were collected during the period from May to September in 2017 by a structured questionnaire that included general characteristics, depression, dependence, comorbidity, sleep scale. Data were analyzed by t-test, ANOVA, Pearson's correlation analysis, and multiple regression analysis. Results: The quality of life was correlated significantly with depression. Depression was the factor affecting the quality of life of the elderly with mild dementia in rural community. Depression accounted for 54.0% as a variable of quality of life. Conclusion: This study was meaningful in that it directly measured the quality of life of the elderly with mild dementia is known to be difficult to measure and found meaningful results. it is necessary to identify the cause of depression and establish a linkage system between public health centers and health related organizations in the community. We suggest the need for individualized preventive intervention program and integrated psychological health service considering depression.
The purpose of this study was to investigate and compare health, medical and welfare service providers' and local residents' perception on community care. The participants were 68 service providers and 95 local residents. The data were analyzed using independent t-test and chi-square test. The results showed that the degree of agreement of service providers on the need for policy, and visiting services were higher. Service providers' ability to link local residents with a need for care to community resources was higher, but in the activation of self-help group in community, local residents agreed more. In add budget of policy to health insurance, local residents agreed less. In perception on priority for community care policy, high priorities were preparation of financial resources and care culture. To effectively operate community care policy, it is necessary to establish a care culture through active exchanges for reducing differences in policy and opinions of recipients.

Objective: Lao PDR's recent accession to the World Trade Organization necessitates a greater understanding of the patterns and risk of livestock production in order to better align national policy with the Agreement on the Application of Sanitary and Phytosanitary Measures. This eco-health study was conducted to improve understanding of the interrelations between market chains and zoonotic infection risks at two strategic cross border points between Lao PDR, Thailand and Viet Nam. Methods: Information gained from smallholder farmer/trader interviews was integrated with serological surveys for pig-associated zoonoses-including hepatitis E virus (HEV), Taenia solium (T. solium) and trichinella-to identify potential linkages between disease risk and pig production and slaughter in low input systems common across the country. Results: Trichinella and HEV exposure was high in both humans and pigs in both study areas, significantly associated with pig slaughter and the subsequent consumption and handling of raw pork products. T. solium demonstrated a strong geographical and ethnic association with the northern study area bordering Vietnam. With the right knowledge and accessible, affordable inputs, the majority of smallholder farmers indicated a willingness to invest more in pig production, which could simultaneously improve livelihoods and decrease exposure to HEV, Trichinella, and T. solium through increased access to formal markets and an improved slaughter processes. Conclusion: The linkages identified when assessing disease risk in the context of potential economic and cultural drivers of transmission highlight the importance of a systems-based approach for the detection and control of zoonotic disease, and contributes to an improved understanding of the Lao PDR livestock sector.
While the local health insurance and the employment-based insurance were integrated in July 2000, the insured is divided into employment-based insured and the local insured and the relevant premium has been applied to both groups. The health insurance premium having the feature of social solidarity has to be determined depending on income, that is, the ability to pay in accordance with the principles of social insurance. While employment-based insurance premium has been determined depending on the earned income, the local insurance premium for the local insured has been determined by scoring gross income(evaluated income), property and possession of automobiles. A variety of improvement approaches has been implemented including introduction of the employment-based insurance premium ceiling system (2002) and the change of property scoring system for the local insured (2006). However, the health insurance system which was merged in 2000 has been implemented up to now without significant change even though there were lots of socio-demographic change including increase of income level and the population structure such as low birth and aging. In other words, it is required to implement the premium rating system securing the income-based equity. Nevertheless, it was inevitable to apply the diverse rating standards in the early stage because it was very difficult to verify the income of the self-employed. Although the income verification rate was significantly increased from 23% in 1989 to 44% in 2010, the irrational standards including property, automobiles, living standard and activity rate have been still applied to the local insured because it is difficult to secure the validity of insurance premium rating system and it severely lacks of security. This paper investigated whether the current insurance premium rating system for the local insured imposing the premium on the basis of 'gender' and 'age' complies with the basic human rights secured by the current Constitution of the Republic of Korea with respect to the practical and theoretic irrationality of insurance premium rating system and standards for he local insured. In accordance with the analysis results, this paper proposed the approach to improve the system.
As the results achieved by the evaluation of toxicities on an aphid parasitoid, Aphidius colemani by 79 pesticides registered as horticultural pesticide and 4 adjuvants far pest control, 6 insecticides including a-cypermethrin, 13 fungicides including metalaxyl-M+mancozeb and 4 acaricides including bifenazate showed low toxicity against A. colemani adult. Low toxicity was showed in all the 4 adjuvants as well. In residual toxicity test from 40 pesticides which showed toxicity more than 50%, A. colemani was safe from 11 pesticides from the 3th day after treatment, 7 pesticides from the 5th day after treatment and 14 pesticides from the 7th day after treatment, respectively. But, chlorpyrifos-methyl, diflubenzuron+chlorpyrifos, etofenprox+diazinone and imidachloprid+chlorpyrifos showed high toxicities reaching 100%, 97.7%, 100% and 100% respectively, even from the 7th day after treatment. To evaluate the control effect by A. colemani against Myzus persicae in a greenhouse, A. colemani was released at parasitoid versus aphids rates of 1:50 and 1:100 when the population of M persicae was 50 per plant. After release, aphids population remained steady for 20 days after release at the level of around 60 aphids per plant. During the investigation, insecticides fur thrips control and fungicides for powdery mildew control were treated, but didn't affect the mummy forming of A. colemani. It may be suggested from these results that the selected insecticides, fungicides, acaricides and adjuvent could be incorporated into the integrated M. persicae management system with A. colemani on greenhouse cultivation.
The current study was conducted to produce basic data for the policy establishment to improve the life satisfaction of residents in regional communities. The study subjected total 1,400 residents living in youngnam province in our nation for the survey by using convenience sampling. The survey took about 4 months started from March 1, 2007 to June 30, 2007. The abstract and conclusion of the result could be summarized as in below. 1. In subjective health status, 45.4% responded as normal, and the health status scored $2.68{\pm}0.67$ in 5 point scale(53.6 in 100 point scale). 2. The level of life satisfaction was found to show the score of $19.57{\pm}3.39$ in 30 point scale(65.2 in 100 point scale). Characteristically, the highest score was found in male in both age groups younger than 19 and in 20s, but lowest score was found in the age group in 40s(p<0.001). In the level of life satisfaction, 29.6% responded as satisfactory and 11.9% responded as not satisfactory. 3. In reviewing the life satisfaction by the health related characteristics, 65.7% of whom responded as very healthy answered satisfactory in life, and the life satisfaction was significantly high in peoples who do not smoke. However, the level was significantly higher in alcohol drinker than no alcohol drinker. People who have enough sleep tended to satisfy in their life, and the life satisfaction level of whom have leisure turned out to show superiorly high satisfaction level. 4. As the variables affecting on the life satisfaction, sex, age, education level, marital status, health status, stress level, life satisfaction level, healthy life activities and depression level were found, and the influence of these variables was 19.6%. As the above results demonstrate, the life satisfaction of residents in regional community was proportional to health status and highly affected by daily health life. So, the more attentions have to be made on health management and the self effort should not be neglected. Especially, since stress, depression level and leisure life did have effects on life satisfaction, more attentions have to be made on stress solving and on the development of leisure life programs. For the goal, the governments, related private organizations and specialized workforces have to make integrated efforts along with the health improvement project in large perspectives.
This paper describes an EEG(electroencephalogram) software for two-channel cerebral function monitoring system to detect the cerebral ischemia. In the software, two-channel bipolar analog EEG signals are digitized and from the signals various EEG parameters are extracted and displayed on a monitor in real-time. Digitized EEG signal is transformed by FFT(Fast Fourier transform) and represented as CSA(compressed spectral array) and DSA(density spectral array). Additional 5 parameters, such as alpha ratio, percent delta, spectral edge frequency, total power, and difference in total power, are estimated using the FFT spectra. All of these are effectively merged in a monitor and displayed in real-time. Through animal experiments and clinical trials on men, the software is modified and enhanced. Since the software provides raw EEG, CSA, DSA, simultaneously with additional 5 parameters in a monitor, it is possible to observe patients multilaterally. For easy comparison of patient's status, reference patterns of CSA, DSA can be captured and displayed on top of the monitor. And user can mark events of surgical operation and patient's conditions on the software, this allow him jump to the points of events directly, when reviewing the recorded EEG file afterwards. Other functions, such as forward/backward jump, gain control, file management are equipped and these are operated by simple mouse click. Clinical tests in a university hospital show that the software responds accurately according to the conditions of patients and medical doctors can use the software easily.
TSAHC [4'-(p-toluenesulfonylamido)-4-hydroxychalcone] is a promising antitumorigenic chalcone compound, especially against TM4SF5 (four-transmembrane L6 family member 5)-mediated hepatocarcinoma. We evaluated the potential of TSAHC to inhibit the catalytic activities of nine cytochrome P450 isoforms and of P-glycoprotein (P-gp). The abilities of TSAHC to inhibit phenacetin O-deethylation (CYP1A2), coumarin 6-hydroxylation (CYP2A6), bupropion hydroxylation (CYP2B6), amodiaquine N-deethylation (CYP2C8), diclofenac 4-hydroxylation (CYP2C9), omeprazole 5-hydroxylation (CYP2C19), dextromethorphan O-demethylation (CYP2D6), chlorzoxazone 6-hydroxylation (CYP2E1), and midazolam 1'-hydroxylation (CYP3A) were tested using human liver microsomes. The P-gp inhibitory effect of TSAHC was assessed by [$^3H$]digoxin accumulation in the LLCPK1-MDR1 cell system. TSAHC strongly inhibited CYP2C8, CYP2C9, and CYP2C19 isoform activities with $K_i$ values of 0.81, 0.076, and $3.45{\mu}M$, respectively. It also enhanced digoxin accumulation in a dose-dependent manner in the LLCPK1-MDR1 cells. These findings indicate that TSAHC has the potential to inhibit CYP2C isoforms and P-gp activities in vitro. TSAHC might be used as a nonspecific inhibitor of CYP2C isoforms based on its negligible inhibitory effect on other P450 isoforms such as CYP1A2, CYP2A6, CYP2B6, CYP2D6, CYP2E1, and CYP3A.
Epidemiological studies have shown the association between transportation of live animals and the potential transmission of infectious disease between premises. This finding was also observed in the 2014-2015 foot-and-mouth disease (FMD) outbreak in Korea. Furthermore, slaughterhouses played a key role in the global spread of the FMD virus during the epidemic. In this context, in-depth knowledge of the structure of direct and indirect contact between slaughterhouses is paramount for understanding the dynamics of FMD transmission. But the social network structure of vehicle movements to slaughterhouses in Korea remains unclear. Hence, the aim of this study was to configure a social network topology of vehicle movements between slaughterhouses for a better understanding of how they are potentially connected, and to explore whether FMD outbreaks can be explained by the network properties constructed in the study. We created five monthly directed networks based on the frequency and chronology of on- and off-slaughterhouse vehicle movements. For the monthly network, a node represented a slaughterhouse, and an edge (or link) denoted vehicle movement between two slaughterhouses. Movement data were retrieved from the national Korean Animal Health Integrated System (KAHIS) database, which tracks the routes of individual vehicle movements using a global positioning system (GPS). Electronic registration of livestock movements has been a mandatory requirement since 2013 to ensure traceability of such movements. For each of the five studied networks, the network structures were characterized by small-world properties, with a short mean distance, a high clustering coefficient, and a short diameter. In addition, a strongly connected component was observed in each of the created networks, and this giant component included 94.4% to 100% of all network nodes. The characteristic hub-and-spoke type of structure was not identified. Such a structural vulnerability in the network suggests that once an infectious disease (such as FMD) is introduced in a random slaughterhouse within the cohesive component, it can spread to every other slaughterhouse in the component. From an epidemiological perspective, for disease management, empirically derived small-world networks could inform decision-makers on the higher potential for a large FMD epidemic within the livestock industry, and could provide insights into the rapid-transmission dynamics of the disease across long distances, despite a standstill of animal movements during the epidemic, given a single incursion of infection in any slaughterhouse in the country.

This study examined the effects of geriatric depression and geriatric stress on cognitive impairment. In particular, the dementia groups were divided into Alzheimer's disease and vascular dementia. A normal group was used as the control group. For each group, the geriatric depression using the geriatric depression scale (GDS) and geriatric stress using the geriatric stress scale (GSS) was compared with the regression and correlation results of cognitive impairment. Statistical tests, such as descriptive statistics, Kruskall-Wallis, correlation analysis, and multiple regression analysis, were performed. For the Alzheimer's group, the GDS and GSS correlated with the cognitive impairment, but only the GDS showed a cause-and-effect relationship with cognitive impairment. In particular, the male group with Alzheimer's disease showed clear confirmation. In addition, geriatric stress was found to be associated with geriatric depression. In conclusion, geriatric depression affects the cognitive impairment directly and geriatric stress affects the cognitive ability indirectly through geriatric depression. In this study, the Alzheimer's disease, vascular dementia, and control groups had a small sample size. Therefore, the external validity in future studies can be increased using a larger sample size for each group.
