Case Report on the Five-year Survival of a Patient with Rectal Cancer Treated with Integrative Medicine After Resection and Chemoradiation Therapy (수술 및 항암 방사선 표준 치료 이후의 한양방 통합치료를 통해 5년 무병생존에 도달한 3기 직장암 환자 1례에 대한 보고)

  • Objectives: This is a five-year survival and complete response (CR) report on rectal cancer treated with western medicine and Korean traditional medicine. Method: A 25-year-old woman diagnosed with rectal cancer visited ⃝⃝ Korean traditional medicine hospital after neoadjuvant concurrent chemoradiation therapy, chemotherapy, and low anterior resection with regional lymph node dissection. She was treated with Korean traditional medicine, including acupuncture, abdominal moxibustion, wild ginseng pharmacopuncture, and herbal medicine, which was based on integrated medicine therapy (IMT), from January 2018 to February 2022. The tumor size was measured by scanning with computed tomography (CT), magnetic resonance imaging, and positron-emission tomography/CT. Adverse events were evaluated using laboratory conclusion and National Cancer Institute Common Terminology Criteria for Adverse Events version 5.0. Result: During four years of treatment, IMT maintained safety. The patient finally reached five-year survival without any recurrence or complication (CR) on March 11, 2022. Conclusion: We suggest that an integrative approach including Korean traditional medicine can be a meaningful treatment option for rectal cancer. Further studies should be performed to establish the proper treatment protocol of integrative medicine for rectal cancer.

Recent Clinical Research on Acupuncture Therapy for Cluster Headache (군발성 두통에 대한 최근 침치료 연구 동향)

  • Objectives: This study presents evidence by analyzing the research trends in acupuncture treatment for cluster headache in the last 10 years. Methods: Randomized controlled trials (RCTs) about acupuncture for cluster headache were searched from the China National Knowledge Infrastructure, PubMed, Cochrane Library, Oriental Medicine Advanced Searching Integrated System, ScienceON, Korean Studies Information Service System, and Research Information Sharing Service. The search terms were the combinations of "cluster headache", "acupuncture", and "needle therapy", and the articles were restricted to those published between 2013 and 2023. Only RCTs were selected. The risk of bias (RoB) was assessed according to the revised Cochrane RoB2 criteria. Results: Six RCTs were selected and analyzed in this review. All selected studies were conducted in China. All RCTs comprised 628 participants. Manual acupuncture was used in all studies. Acupuncture targeting the sphenopalatine ganglion was performed in two papers published after 2020. ST8, Ex-HIN3, and GB14 were the most frequently used acupoints in acupuncture treatment. The most commonly used indicators for evaluation were headache attack frequency, clinical efficacy, and the visual analog scale. In each study, adding acupuncture treatment to conventional therapy had significant effects in relieving the symptoms of cluster headaches. Conclusion: The results suggest that acupuncture is an effective treatment for cluster headache. To ensure objective evidence for the effectiveness of acupuncture treatment in cluster headache, it is important to continue large-scale case reports and RCTs.

Clinical Analysis of 89 Patients with Severe Acute Ischemic Stroke from 3rd Intensive Care Unit(East-West Integrated Intensive Care Uint) of Kyung Hee Medical Center (경희의료원 제3 중환자실(동서협진중환자실)에 입원한 89명의 중증 급성기 뇌경색 환자에 대한 임상적 고찰)

  • Objects : To gain better insights of East-west integrated treatment of ischemic stroke. We analyzed 89 patients with severe acute ischemic stroke who were admitted to the East-west integrated intensive care unit. Methods : Subjects enrollment was from the East-west integrated intensive care unit of Kyung Hee Medical Center from March 2006 to February 2007. Patients were admitted within 14 days after the onset of ischemic stroke. We assessed the subjects' general characteristics, risk factors, admitting routes and periods, diagnostic imaging, process of western treatment and Korean traditional treatment, complication and consequence. Results : The proportion of males was 50.6%, of females 49.4%, average age was male 66.0$\pm$10.3 and female 71.1$\pm$10.5. Average length of hospital stays was 19.5 days. Monthly admissions were highest in November and December. The admission route was through emergency room (61.8%) or ward (34.8%). Mean Glasgow coma scale score was 10.0$\pm$2.5, average time from symptom of onset to hospital admission was 2.3$\pm$2.2 days. Dominant ischemic vascular territory was middle cerebral artery (66.3%). Initial western treatment was argatroban (22.5%), urokinase (28.1%), and heparinization (38.2%). Distribution of Sasang constitution of So-yang to Tae-eum to So-eum was equal to 5.4 to 2.9 to 1.5. Major complications were observed in 40 (42.7%) patients. In hospital mortality was 12.4% (11 deaths), all of them caused by aggravation of neurological deficit and only 3 of them with major complications. There appears to be a significant positive relationship between length of hospital stay and occurrence of complications (P<0.05). After discharge from the ICU, 64 (71.9%) patients were improved, 11 (12.4%) patients had expired, and 14 (15.8%) patients were transferred. Conclusions : From this study, we suggest that patients with severe acute ischemic stroke should be treated with East-west integrated therapy for more favorable consequences and decreased mortality.

The protective effect of hypoxic therapy on paraquat-induced toxicity rat model (백서를 이용한 고독성 제초제 파라쿼트 중독 치료를 위한 저농도산소요법의 효과 연구)

  • Paraquat (PQ) is a very effective and widely used herbicide that was commercially introduced in 1962. In this study, instead of using antioxidants like in the past, to inhibit the formation of PQ-induced ROS, we attempted to reduce the oxygen concentration by using non-lethal hypoxia therapy. Therefore, we studied the toxicity of PQ in vivo, analyzed the major effects of ROS on the targeted lung tissue and compared the results with the gross histological changes after the cell protective effect of non-lethal hypoxia therapy. In vivo studies demonstrated that low-concentration oxygen therapy (i.e., 10-12% oxygen) in rats administered with PQ was associated with a higher survival rate than in rats that received only PQ. In vivo non-lethal hypoxia treatment showed better survival and less lung tissue damage. Using a hypoxic/anaerobic incubator with integrated multifaceted molecular analysis, including MDA assay, glutathione assay, and SOD assay, we established an optimal, significantly reduced in vivo non-lethal hypoxia treatment by exploiting the PQ-induced cytotoxicity responses.

Moxibustion for Benign Prostatic Hyperplasia: A Systematic Review and Meta-analysis (전립선 비대증에 대한 뜸치료의 효과 : 체계적 문헌고찰과 메타분석)

  • Objective: This study evaluates the effectiveness and safety of moxibustion for benign prostatic hyperplasia (BPH). Methods: Using the keywords "benign prostatic hyperplasia", "benign prostatic hypertrophy", "benign prostatic enlargement", "prostatic hyperplasia", and "moxibustion", we searched papers in numerous databases, including National Discovery for Science Leaders (NDSL), Korean Traditional Knowledge Portal (KTKP), Oriental Medicine Advanced Searching Integrated System (OASIS), Research Information Sharing Service (RISS), PubMed, Embase, and CENTRAL. The search range included randomized controlled trials (RCTs). Papers not matched with inclusion criteria were excluded. The methodological quality of each RCT was assessed using the Cochrane risk-of-bias tool. Where appropriate, meta-analyses were performed. Results: Initially, 77 studies were found. Of these, 11 duplicate studies were removed and 27 were excluded following title and abstract screening. After the remaining 39 papers were scanned, 13 RCTs were selected and analyzed. Among these 13 RCTs, five compared moxibustion therapy and oral medication, seven compared moxibustion plus acupuncture therapy and oral medication, and one compared moxibustion plus acupuncture therapy and sham-moxibustion. The meta-analysis showed positive results for the use of moxibustion therapy in terms of International Prostate Symptom Score (IPSS), Quality Of Life (QOL), Maximum Flow Rate (Qmax), Prostate Volume (PV), and the efficacy rate. The meta-analysis showed positive results for the use of moxibustion plus acupuncture therapy in terms of IPSS, QOL, and the efficacy rate. Conclusions: This meta-analysis of clinical trials suggests that moxibustion is effective intreating BPH patients. The results of this study could be applied to clinical treatment of BPH. However, additional large-scale clinical researches should be conducted.

Effects of High-frequency Diathermy Integrated into Suboccipital Release on Tenderness and Neck Mobility and Disability in People with Chronic Tension-type Headache (뒤통수밑근 이완기법과 병행된 고주파 심부투열이 만성 긴장성 두통 환자들의 압통과 목 가동성 및 기능장애에 미치는 영향)

  • Background: Active trigger points (TrPs) of the suboccipital muscles greatly contribute to the occurrence of chronic tension-type headache, with increased sensitivity of TrPs and facilitated referred pain. Objects: This study aimed to investigate whether the integration of high-frequency diathermy into suboccipital release is more beneficial than the use of suboccipital release alone. Methods: Thirty subjects were assigned to either experimental group-1 (EG-1) to undergo suboccipital release combined with high-frequency diathermy (frequency: 0.3 MHz, and electrode type: resistive electronic transfer), or EG-2 to undergo suboccipital release alone, or the control group (CG) with no intervention, with 10 subjects in each group. The assessment tools included the headache impact test 6 (HIT-6), perceived level of tenderness, neck disability index, and neck mobility. Intervention was performed for 10 minutes, twice per week, for 4 weeks, and measurements were performed before and after the interventions. Results: The between-group comparison of the post-test values and changes between pretest and post-test showed significant differences for all parameters at p<.05, except for the left-to-right lateral bending range. In the post hoc test, EG-1 showed significant differences for the parameters in comparison with the CG, while no significant differences in the perceived tenderness level, on both temporal regions, were found between EG-2 and CG. Furthermore, the HIT-6 score and perceived tenderness level, in the right temporal region, showed significant differences between EG-1 and EG-2. In the within-group comparison, EG-1 and EG-2 appeared to be significantly different between pretest and post-test (p<.05), except for the perceived tenderness level in the right temporal region, with significance for the EG-1 group only (p<.05). Conclusion: These findings suggest that the suboccipital release technique may be advantageous to improve headache, tenderness, and neck function and mobility, with more favorable effects with the incorporation of high-frequency diathermy.

  • 좋은 발성을 위하여 음성이 만들어지려면, 뇌의 언어중추의 명령에 의하여 신경망을 통하여 ‘호흡기관(폐와 흉곽. 호흡관련 근육들)’과 ‘발성기관(주로 후두 즉 성대)’ 그리고 ‘조음, 공명기관(인두, 구강 및 비강)’의 근육들이 유기적으로 작용하여야 한다. 이런 협력 체제에 문제가 생기면, 다양한 종류의 질환이 유발될 수 있다[1,2]. 현재 임상에서 음성 분석을 위해 사용되는 시스템은 대부분 성대의 진동을 측정하기 위해 stroboscopy, 전기성문파형검법 (EGG, electroglottography), 후두근전도 중의 한 방법과 음성 분석을 위한 분석 프로그램으로 구성되어 있다. 그러나, 발성은 호흡과 매우 밀접한 관계가 있어 음성, 성대의 진동, 호흡 관계를 종합적으로 관찰할 필요가 있다. 본 연구에서는 발성 시 성대 점막의 접촉 양상은 ECG 로 측정하며, 발성의 주 에너지원이 되는 호흡의 변화를 2 channel 인덕턴스 호흡감시 장치(RIP: repiratory inductive plethymography)를 이용하여 흉곽의 움직임과 상 복강의 움직임에 대하여 측정하며, 발성되는 음성은 마이크로폰을 통하여 측정하는 ‘EGG, 음성, 호흡 통합검사 장치’를 개발하였다.

Deducing the conventional biomedical therapy to Ayurvedic fundamentals: Illustrations from a case report

  • Ayurveda is often criticized for having empirical and non-evidence based approach to treat the patients. At the same time, modern medicine is also being criticized for having a non-holistic, reductionist and mechanistic approach of treating the patients which do not help in many real clinical situations. An open minded deduction of treatment approaches in both of these systems for a common patient however makes us to rethink that ideally both systems are similar with a common objective of offering a cure although in a manner which is better understood through their own methods of learning. The differences therefore, are more superficial rather than being deeply rooted in the understanding. A more tolerant viewpoint towards the competitive medical systems may therefore be a better approach to offer optimal health care to our people through a genuine amalgamation of these two health care sciences through an integrated approach. Once this tolerance is developed, it will give us an opportunity to think for a focused selection of type of health care depending upon the type of the disease and strength of the particular system in that area.

An Ergonomics Approach for Developing Korean Style Chiropractic Table (한국형 척추교정기의 개발을 위한 인간공학적 접근)

  • Spinal injuries are common these days and are increasing due to the increasing sports activities, auto-related accidents, and occupational incidences. Most of spinal injuries of strain and sprain are associated with trauma and resulted in loss of optimal musculoskeletal system in spinal column area. Some of these injuries requires surgical treatments. Fortunately, some people with spinal disorder are achieving outstanding results with the chiropractic care which utilizes muscle stimulation and strengthening, traction therapy, and spinal adjustments. These treatments using chiropractic table can ease pain by restoring alignment, improving mobility, and relieving pain and stiffness in neck and spinal areas. The purpose of this study was thus to gather the comprehensive information about spinal injuries, to define the specific dimensions, and to recommend functions of chiropractic table specially suited for Korean. This study was an integrated approach in applying the concepts of biomechanical correction of the musculoskeletal system. Also, this study was to utilize a knowledge of physics, ergonomics, and rehabilitation to the development of spine cure medicine.

  • Object: By process of treatment for a case which diagnosed as Churg-Strauss Syndrome and admitted from the 11th, pril, 2002 to the eight, May, 2002, the results are as follows. Methods: Clinical observation and analysis about a case of Churg-strauss Syndrome was done, who visited College of Oriental Medicine, Kyungsan University. This case was managed by integrated therapy of oriental Medicine. This Patient was diagnosed as deficiency of Yin and coagulation of phlegm(陰虛痰疽) and medicated chungsangboha-tang(淸上補下湯) and gamiyunpe-tang (加味潤肺湯). As acupuncture and moxibustion and oriental medication, we could get the effective results. Conclusion: We could get the effective results that case of Churg-Strauss Syndrome with dyspnea, sputum, cough, fatigue, sweating sign, was treated with oriental medicine, acupuncture, moxibustion.
