Design and Construction of Integrated Database for Contents Development of Pulse Analysis System (맥진기 컨텐츠 개발을 위한 통합 데이터베이스 설계 및 구축)

  • The development of medical devices based on the theory of tranditional Korean medicine is increasing due to population aging and emerging demand for medical services. Various clinical trials are being conducted to ensure the clinical effectiveness of the developed medical devices, and the importance of systematic management of variously collected information in this process is increasing. In this paper, we designed and constructed the database that can systematically manage the medical information of the patient and the pulse measurement information in conducting clinical trials by using pulse analysis system which is a representative diagnostic method of traditional Korean medicine. The constructed database can be efficiently used for the verification of existing algorithms or developing new algorithms through the quality control of various information and measured data on clinical trials. It also has several advantages in the control of clinical data. The results of this study can contribute to the establishment of Korean medical device data standard for the construction of integrated database management system that can share information with different kind of pulse analysis system and other bio-signal measuring devices.

  • Recently, researches for the integration and standardization of clinical data in the Western medicine and Korean medicine is in progress. If an integration of similar clinical data as well as heterogeneous clinical data is possible based on one standardization, we can able to derive implicit medical knowledge from integrated clinical data. In this paper, we implemented Korean clinical database based on internationally known CDISC standardization to efficiently store Korean clinical data and constructed E-CRF system for convenient data input in clinical sites. Furthermore, we showed example of an integration of Korean clinical data from 4 clinical sites. The results of our study will help to establish the foundation for the extraction of implicit medical knowledge from integrated clinical data. Also, our results may support efficient management through data integration, prevention of repetitive or unnecessary clinical trials, facilitation of collaborative study and convenient research through the distribution of refined clinical data.

  • Clinical trials are an important part of developing new medical devices or developing new drugs. However, it takes many time and expense to conduct clinical trials. In order to solve this problem, it define the standard format of the data collected in clinical trials in major countries and in Korea. Clinical trials are also underway in korean medicine to develop new medical devices. If it define the standards for the data we collect in clinical trials, there are likely to be a variety of ways to use them. If it collect and manage various clinical trial data as one standardized data, you will be able to draw various results. Based on these findings, we established a system for collecting and managing integrated data on clinical trials. And let's look at how to use it.

  • The advances in electronic medical records (EMRs) and bioinformatics (BI) represent two significant trends in healthcare. The widespread adoption of EMR systems and the completion of the Human Genome Project developed the technologies for data acquisition, analysis, and visualization in two different domains. The massive amount of data from both clinical and biology domains is expected to provide personalized, preventive, and predictive healthcare services in the near future. The integrated use of EMR and BI data needs to consider four key informatics areas: data modeling, analytics, standardization, and privacy. Bioclinical data warehouses integrating heterogeneous patient-related clinical or omics data should be considered. The representative standardization effort by the Clinical Bioinformatics Ontology (CBO) aims to provide uniquely identified concepts to include molecular pathology terminologies. Since individual genome data are easily used to predict current and future health status, different safeguards to ensure confidentiality should be considered. In this paper, we focused on the informatics aspects of integrating the EMR community and BI community by identifying opportunities, challenges, and approaches to provide the best possible care service for our patients and the population.

A Review of Clinical Studies of Integrated Traditional Chinese and Western Medicine Therapy for Legg-Calve-Perthes Disease Using China National Knowledge Infrastructure Database: Focused on Clinical Studies after 2000 (대퇴골두의 연소성 골연골증의 중·서양 협진치료에 대한 임상 연구 동향: 2000년 이후 발표된 연구를 중심으로)

  • Objectives This study was conducted to investigate clinical studies about Integrated Traditional Chinese and Western Medicine therapy for Legg-Calve-Perthes disease. Methods We searched clinical studies about Integrated Traditional Chinese and Western Medicine therapy for Legg-Calve-Perthes disease through China National Knowledge Infrastructure. 17 articles published from 2000-2021 were finally chosen and analyzed by published year, study design, number of samples, diagnosis criteria, evaluation criteria, treatment period, follow up period, treatment method. Results Herbal medicine, external treatment, Chuna massage therapy were performed for traditional Chinese medical treatment. For Western medicine treatment, conservative treatment and surgical treatment were performed. Above them, herbal medicine and conservative treatment were mostly used for treating Legg-Calve-Perthes disease. Conclusions By analyzing clinical studies, We found that Integrated Traditional Chinese and Western Medicine therapy can be helpful for treating Legg-Calve-Perthes disease. In Korea, more clinical research about Legg-Calve-Perthes disease is still needed. This study will be helpful for future research on Korean medicine for Legg-Calve-Perthes disease.

Herbal Medicine for the Treatment of Non-Erosive Reflux Disease: A Systematic Review and Meta-Analysis Protocol

  • Introduction: Non-erosive reflux disease (NERD) is the most common subtype of gastroesophageal reflux disease (GERD). This study aims to synthesize evidence on the efficacy and safety of various herbal medicines for the treatment of NERD. Methods and analysis: Ten electronic databases will be examined: MEDLINE (via PubMed), Cochrane Central Register of Controlled Trials, Embase, Allied and Complementary Medicine Database, China National Knowledge Infrastructure Database, Citation Information by Nii, Korean Medical Database, Korean Studies Information Service System, National Digital Science Library, and Oriental Medicine Advanced Searching Integrated System. All randomized controlled trials published from inception to May 2023 that meet the eligibility criteria will be selected. Two independent researchers will extract data, such as publication year, study design, intervention details, outcome measures, main results, and adverse events. The risk of bias and quality of evidence will be assessed, and subgroup analyses will be performed according to the type of control intervention and herbal medicine. The analysis process will be conducted using Review Manager 5.4 software. Discussion: This review will present a summary and rationale for herbal medicine's effectiveness in treating NERD. The findings of this review can help those who want to apply herbal medicine to the treatment of NERD.

  • The treatment of Atopic dermatitis and its untapped herbal elements are recognized as possible solutions to incurable diseases stemming from overdosage of chemical drugs. Synthesizing clinical results from the Oriental medicine with scientific results from the field of Molecular biology is in the mainstream. The main goal of this research is, by objectifying the methodology of abstracting information into a compendium, to establish knowledge-based database on natural materials mingling every single related information from both Oriental and Western medicine. The constructed system, ATODERM, is the first database system about the medical herb for the treatment of Atopic Dermatitis which bearing the integrated information on Oriental medicine and Western medicine. The construction of this database renders new ways of treatment and prescriptions for researchers in the field of the Oriental medicine. Using the database that has been intensively accumulated, the possibility of creating the plausible cure for the disease can be anticipated.

  • 과거와 다르게 최근의 병원들은 정보화로 인해서 상당한 양의 의료 데이터가 저장되어 있어서 이의 효과적인 이용에 관심을 가지고 있다. 그러나 기존 통합병원정보시스템(Integrated Hospital Information System)은 아직까지 일반관리와 원무관리 중심에서 벗어나지 못하고 있다. 품질 좋은 의료 서비스를 제공하기 위해서 환자 중심의 진료 및 진료지원, 임상연구 등을 종합적으로 지원하기 위한 데이터 웨어하우스의 필요성이 대두되기 시작했다 .이 글에서는 최근 임상 데이터 웨어하우스(Clinical Data Warehouse)에 초점을 둔 J병원 사례와 연구를 중심으로 기술한다.

Identification of Hub Genes in the Pathogenesis of Ischemic Stroke Based on Bioinformatics Analysis

  • Objective : The present study aimed to identify the function of ischemic stroke (IS) patients' peripheral blood and its role in IS, explore the pathogenesis, and provide direction for clinical research progress by comprehensive bioinformatics analysis. Methods : Two datasets, including GSE58294 and GSE22255, were downloaded from Gene Expression Omnibus database. GEO2R was utilized to obtain differentially expressed genes (DEGs). Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis of DEGs were performed using the database annotation, visualization and integrated discovery database. The protein-protein interaction (PPI) network of DEGs was constructed by search tool of searching interactive gene and visualized by Cytoscape software, and then the Hub gene was identified by degree analysis. The microRNA (miRNA) and miRNA target genes closely related to the onset of stroke were obtained through the miRNA gene regulatory network. Results : In total, 36 DEGs, containing 27 up-regulated and nine down-regulated DEGs, were identified. GO functional analysis showed that these DEGs were involved in regulation of apoptotic process, cytoplasm, protein binding and other biological processes. KEGG enrichment analysis showed that these DEGs mediated signaling pathways, including human T-cell lymphotropic virus (HTLV)-I infection and microRNAs in cancer. The results of PPI network and cytohubba showed that there was a relationship between DEGs, and five hub genes related to stroke were obtained : SOCS3, KRAS, PTGS2, EGR1, and DUSP1. Combined with the visualization of DEG-miRNAs, hsa-mir-16-5p, hsa-mir-181a-5p and hsa-mir-124-3p were predicted to be the key miRNAs in stroke, and three miRNAs were related to hub gene. Conclusion : Thirty-six DEGs, five Hub genes, and three miRNA were obtained from bioinformatics analysis of IS microarray data, which might provide potential targets for diagnosis and treatment of IS.
