광 집적회로용 실리콘 기반 궤도 각운동량 부호 변환기 (Silicon Electro-optic Orbital Angular Momentum Sign Modulator for Photonic Integrated Circuit)

  • 본 논문에서는 실리콘 기반의 광 집적회로에서 외부 전압 조절을 통해 |l|=1 궤도 각운동량 모드의 궤도 각양자수를 변조할 수 있는 전기 광학 변조기를 설계하였다. 설계된 전기 광학 변조기는 위치별로 서로 다른 도핑농도를 가지는 실리콘 코어와 실리콘 산화막으로 구성되어 있으며, 도핑농도의 분포를 통해 궤도 각운동량 모드를 구성하는 두 고유 모드의 전파 손실과 유효굴절률 변화량을 조절할 수 있도록 설계되었다. 변조기는 역전압을 기준으로 -0.33V에서는 궤도 각운동량 모드의 부호가 유지되는 광도파로로, 10V에서는 궤도 각양자수 부호 변환기로서 동작한다. 고유 모드 확장법으로 계산한 신호변조 후의 전기장 분포를 통해 얻은 궤도 각양자수는 두 동작모드에서 모두 |l|>0.92 으로 매우 높은 궤도 각운동량 모드 순도를 보였다.

입원환자의 낙상 실태 및 위험요인 조사연구 (Incidence of Falls and Risk Factors of Falls in Inpatients)

  • Purpose: The objective of this study was to report the incidence of falls in hospitals and analyze the risk factors for falls. Methods: This study used data on 1,216 patients who experienced falls from 2015 to 2017 during their hospitalization. The data was collected from the falls incident reports and patient' electronic medical record of hospital. Data were analyzed with descriptive statistics using Chi-square test, Fisher's exact test and multiple Poisson regression analysis with the SAS 9.4 Results: The incidence of falls was 1.38 per 1,000 patients days (2015), 1.81 per 1,000patients days (2016) and 1.99 per 1,000patients days (2017). The incidence of injury caused by falls (level III~V) was 0.05 per 1,000patients days (2015), 0.04 per 1,000patients days (2016) and 0.06 per 1,000patients days (2017). The largest number of falls occurred during night shift (42.5%), specifically in the patients' room (70.8%), and medical unit (66.0%). Average age of fallers was 69.1 years and 61.7% of them were older than 71 years. CCI and the patient's department have statistically significant differences in injury or injury levels from falls, but the integrated nursing care services had no significant difference in injury or injury levels from falls. Conclusion: The result of this study can be used as a reference for establishing a fall prevention strategy for hospitalized patients by presenting index values such as the fall rate.

한반도 겨울철 강수 유형에 따른 전지구 수치모델(GRIMs) 예측성능 검증 (Evaluation of Predictability of Global/Regional Integrated Model System (GRIMs) for the Winter Precipitation Systems over Korea)

  • This paper evaluates precipitation forecast skill of Global/Regional Integrated Model system (GRIMs) over South Korea in a boreal winter from December 2013 to February 2014. Three types of precipitation are classified based on development mechanism: 1) convection type (C type), 2) low pressure type (L type), and 3) orographic type (O type), in which their frequencies are 44.4%, 25.0%, and 30.6%, respectively. It appears that the model significantly overestimates precipitation occurrence (0.1 mm d-1) for all types of winter precipitation. Objective measured skill scores of GRIMs are comparably high for L type and O type. Except for precipitation occurrence, the model shows high predictability for L type precipitation with the most unbiased prediction. It is noted that Equitable Threat Score (ETS) is inappropriate for measuring rare events due to its high dependency on the sample size, as in the case of Critical Success Index as well. The Symmetric Extreme Dependency Score (SEDS) demonstrates less sensitivity on the number of samples. Thus, SEDS is used for the evaluation of prediction skill to supplement the limit of ETS. The evaluation via SEDS shows that the prediction skill score for L type is the highest in the range of 5.0, 10.0 mm d-1 and the score for O type is the highest in the range of 1.0, 20.0 mm d-1. C type has the lowest scores in overall range. The difference in precipitation forecast skill by precipitation type can be explained by the spatial distribution and intensity of precipitation in each representative case.

The Influence of Family Function on Occupational Attitude of Chinese Nursing Students in the Probation Period: The Moderation Effect of Social Support

  • Purpose: The purpose of this study was to explore the factors influencing the occupational attitudes of nursing students in the probation period. Methods: Nursing students in the probation period from five hospitals completed an anonymous survey. The instruments included the nursing occupational attitude scale, family adaptability, partnership, growth, affection, and resolve index, and perceived social support scale. The study examined the moderation model between family function, perceived social support, and occupational attitudes using PROCESS 3.2. Results: For nursing students, when social support was low, family function had a significant positive impact on occupational attitudes and intentions, and the effect was much higher than that of perceived social support. Conclusion: Family function has a significant positive explanatory effect on attitude and intention (β = .13, p < .001 and β = .12, p < .001); the interaction term between family function and perceived social support are significant (β = .01, p < .001 and β = .01, p < .001). Perceived social support has a significant moderating effect on the relationship between family function and occupational attitudes of nursing students in the probation period. Family function has a significant difference in the occupational attitudes and intentions of nursing students with low perceived social support. Nursing students perceive social support in the probation period has a significant moderation effect in the relationship between their family function and occupational attitudes. Interns with low family function should be given more social support to improve their occupational attitudes.

코로나-19 무증상 감염 환자의 한약 치료 방안에 대한 고찰 (A review on the treatment of Herbal Medicine for asymptomatically infected COVID-19 patients)

  • Objectives: Objective: The purpose of this study is to investigate the treatment of herbal medicine for patients with COVID-19 asymptomatic infections. Method: In English, search engines such as 'PubMed', 'Science Direct', and 'Cumulative Index to Nursing and Allied Health Literature (CINAHL)' were used. In Chinese, search engines such as 'China National Knowledge Infrastructure (CNKI)' and Wanfang were used. The keywords for the search engines were 'COVID-19', 'asymptomatic infection', 'Korean Medicine', 'traditional Chinese medicine', 'herbal medicine', and etc. Only clinical studies using herbal medicine for patients without fever or respiratory symptoms were selected. We excluded the cases that do not fit the research topic. Results: A total of 5 studies were finally selected. Among them, four of them used integrated herbal medicine and Western medicine, and one of the studies treated exclusively for herbal medicine. There were a total of seven prescriptions for herbal medicine used in the study. Outcome variables were used as following: lab test, nucleic acid conversion time, hospitalization period, chest CT, and etc. In the RCT study, herbal medicine and Western medicine decreased nucleic acid conversion time, average hospitalization time compared to the control group, but it was not statistically significant. No other adverse reactions were reported in all studies. Conclusion: According to the results, integrated herbal medicine and Western medicine might be an effective treatment for patients with COVID-19 asymptomatic infection reducing hospitalization period, time of nucleic acid turning negative. No severe adverse effects were reported. However, it is thought that better-designed research will be needed in the future.

살포식 패류 양식어장 건강도 평가기준 설정 (Establishment of Integrated Health Evaluation Criteria for Coastal Aquaculture System)

  • We investigated the physio-chemical and geochemical parameters in the spraying shellfish aquacultures (Yeoja and Gangjin Bay) to establish the systematic strategy for effective environmental management. Spatial variation of each parameter showed partially significant difference (P<0.05) between Yeoja and Ganjin Bay, inferring the discriminative progress (i.e., accumulation and degradation) of the autochthonous organic matter within the aquaculture environments. We additionally integrated various properties (e.g., water/sediment quality, natural hazard, and biological health) which may affect the biological growth within the aquaculture habitats based on the biogeochemical cycles related to environmental components and aquaculture species. We used a screening approach (i.e., one out-all out; OOAO) which can permit the assessment of the health levels of aquaculture species, the scoring for other parameters (seawater, sediment, and natural hazard) as three levels (excellent, moderate and poor) depending on the complex interactive properties occurring in the aquaculture environments. Actual, discriminative scores obtained via our case studies may confirm that these stepwise processes are effectively evaluated for optimal health conditions within the aquaculture habitats. Thus, this approach may provide valuable insights for effective environmental management and sustainable growth of aquaculture operation.

A computer based simulation model for the fatigue damage assessment of deep water marine riser

  • An analysis for the computation of Fatigue Damage Index (FDI) under the effects of the various combination of the ocean loads like random waves, current, platform motion and VIV (Vortex Induced Vibration) for a certain design water depth is a critically important part of the analysis and design of the marine riser platform integrated system. Herein, a 'Computer Simulation Model (CSM)' is developed to combine the advantages of the frequency domain and time domain. A case study considering a steel catenary riser operating in 1000 m water depth has been conducted with semi-submersible. The riser is subjected to extreme environmental conditions and static and dynamic response analyses are performed and the Response Amplitude Operators (RAOs) of the offshore platform are computed with the frequency domain solution. Later the frequency domain results are integrated with time domain analysis system for the dynamic analysis in time domain. After that an extensive post processing is done to compute the FDI of the marine riser. In the present paper importance is given to the nature of the current profile and the VIV. At the end we have reported the detail results of the FDI comparison with VIV and without VIV under the linear current velocity and the FDI comparison with linear and power law current velocity with and without VIV. We have also reported the design recommendations for the marine riser in the regions where the higher fatigue damage is observed and the proposed CSM is implemented in industrially used standard soft solution systems (i.e., OrcaFlex*TM and Ansys AQWA**TM), Ms-Excel***TM, and C++ programming language using its object oriented features.

Effectiveness and Safety of Pharmacopuncture Therapy for Chronic Low Back Pain: A Study Protocol for a Pragmatic Randomized Controlled Trial

  • Background: Low back pain (LBP) is a common musculoskeletal disorder worldwide, with a lifetime prevalence of up to 80%. Among nonsurgical treatments for chronic LBP, Korean medicine treatments are highly preferred, and pharmacopuncture therapy combining acupuncture and herbal medicine is widely used. However, no evidence-based study has focused on the use of various types of pharmacopuncture. Methods: The pragmatic randomized controlled clinical trial will include 44 participants; recruitment will start in July 2023. All participants will receive integrated Korean medicine treatment including acupuncture, cupping, and infrared therapy, and the intervention group will also receive pharmacopuncture. After 16 treatment sessions, twice a week for 8 weeks, follow-up assessments will be performed at week 9. As a pragmatic randomized controlled clinical protocol, the type, dose, and acupoints of acupuncture and pharmacopuncture are not determined in advance but are selected and recorded according to the clinical judgment of the Korean medicine doctor. Results: The primary outcome will be measured using a visual analog scale score, and the secondary outcomes include the Oswestry disability index, patient global impression of change, no worse than mild pain, and range of motion. Safety will be assessed by examining participants' self-reported adverse events and vital signs and conducting blood tests before and after the test. Conclusion: This study aims to provide clinical evidence of the effectiveness and safety of pharmacopuncture for chronic LBP.

외상성 경추부 척수손상의 회복기 단계에 대한 한양방 통합재활치료 프로토콜 적용의 임상적 효과 증례보고 (The Clinical Effects of Applying an Integrated Rehabilitation Protocol during the Recovery Phase for Traumatic Cervical Spinal Cord Injury: A Report of Four Patients)

  • The objective of this study is to report the clinical effects of applying an integrated rehabilitation protocol to four patients with traumatic cervical spinal cord injuries. The treatments applied included acupuncture, cupping and moxibustion, chuna manual therapy, functional electrical stimulation, and other physical therapies. The evaluation methods included American Spinal Injury Association Impairment Scale, functional independence measure (FIM), modified Barthel index (MBI), numeric rating scale (NRS) and other scales. All patients presented marked improvements in FIM, MBI, NRS and other scales, along with increased muscle strength in the manual muscle test. The application of our protocol resulted in clear clinical benefits and enhanced the recovery and quality of life for the patients in this study.

Synergistic Effect of Essential Oils and Enterocin Produced by Enterococcus faecalis MSW5 against Foodborne Pathogens

  • This study determines the combinatorial effect of enterocin MSW5 and five essential oils (EOs- Thymus vulgaris, Cymbopogon martini, Origanum vulgare, Cinnamomum zeylanicum, and Cymbopogon citrus) against Staphylococcus aureus, Listeria monocytogenes, and Salmonella Typhimurium. The Minimum Inhibitory Concentration of each antimicrobial agent was determined. The MIC of enterocin MSW5 against test pathogens was in the following order: S. aureus (0.362 ± 0.01), S. Typhimurium (0.362 ± 0.05 mg/ml), L. monocytogenes (0.725 ± 0.08 mg/ml). Among all EOs, maximum activity was observed in the case of C. zeylanicum against S. aureus (78.12 ± 0.04 ppm), S. Typhimurium (78.12 ± 0.08 ppm), and L. monocytogenes (39.00 ± 0.05 ppm). Further, the checkerboard assay was used to determine the synergistic effect between antimicrobial agents and enterocin MSW5 in combination with C. zeylanicum has shown significant synergism with the Fraction Inhibitory Concentration index (0.372) against test pathogens. Additionally, individual EOs and enterocin MSW5 have shown anti-biofilm activity, whereas their combined use has shown more significant antibiofilm activity. The maximum anti-biofilm activity was observed with the combination of enterocin MSW5 and O. vulgares against S. aureus (92.86 ± 0.06%) and S. Typhimurium (73.63 ± 0.23%) and a combination of enterocin MSW5 and C. citrus against L. monocytogenes (87.84 ± 0.15%). Therefore, combinations of antimicrobial compounds can control the growth of foodborne pathogens better than the individual agent.
