과호흡증이 동반된 수면마비환자의 일례 (A Case of Sleep Paralysis Associated with Hyperventilation)

  • The Authors report a case of sleep paralysis patient who is 25-years old man. He complained that he has been experiencing episode "being suffocated" during sleep and could not move by himself while semiconsciousness state. He was a only son of divorced parents and could not attach anybody from yong childhood. His symptoms was began intermittently after his early teens. Medical history and physical examination revealed no significant finding. A nocturnal polysomnogram showed loss of muscle tone during terminal REM period and sudden hyperventilation attack followed by wake-up. Psychosocial stress and insufficient sleep were presumed to be an etiological factors. Imipramine 50mg per day almost completely relieved symptoms.

  • Sleep is an essential physiological process, especially for proper brain function through the formation of new pathways and processing information and cognition. Therefore, when sleep is insufficient, this can result in pathophysiologic conditions. Sleep deficiency is a risk factor for various conditions, including dementia, diabetes, and obesity. Recent studies have shown that there are differences in the prevalence of sleep disorders between genders. Insomnia, the most common type of sleep disorder, has been reported to have a higher incidence in females than in males. However, sex/gender differences in other sleep disorder subtypes are not thoroughly understood. Currently, increasing evidence suggests that gender issues should be considered important when prescribing medicine. Therefore, an investigation of the gender-dependent differences in sleep disorders is required. In this review, we first describe sex/gender differences not only in the prevalence of sleep disorders by category but in the efficacy of sleep medications. In addition, we summarize sex/gender differences in the impact of sleep disorders on incident dementia. This may help understand gender-dependent pathogenesis of sleep disorders and develop therapeutic strategies in men and women.

입원환자의 수면형태 및 수면에 영향을 미치는 요인 (Sleep pattern changes and related factors in general hospital inpatients)

  • Sleep is an inevitable part of human existence and a part of the dynamic quality of life. The purpose of this study is to explore sleep pattern changes and related factors of patients hospitalized. The participants are 158 patients hospitalized in one general hospital. The questionnaire was developed by modification of VSH sleep scale (1987). Open question was used for research of related factors. $SPSS/PC^+$ are utilized for data analysis. The results are as follows ; There is significant difference before and after admission in the sleep disturbance. There is retroverse significant difference before and after admission in the sleep effectiveness. There is no significant difference between two groups in the nap supplementation. The patients reported pain by reason of sleep latency and mid-sleep awakening and noise due to early awakening. The reasons for lack of sleep were pain and noise. The method for overcoming their sleep disturbance, most of the subjects waited with closed eyes, imaged by themselved and took medicate sleeping pills. Discomfort manifested in the case of insufficient steepness was fatigue, headache and aggressiveness.

불면증의 원인과 발생기전 (The Causes and Developmental Mechanism of Insomnia)

  • With the recent development of sleep medicine, insomnia has been perceived as a disease from a simple symptom. As there are various causes in a disease, proper treatment according to each cause is necessary for a more effective treatment In general, insomnia is classified into five categrories of physical, physiological, psychological, psychiatric and pharmacological aspects. However, such categorizations are often insufficient in treating insomnia. Furthermore understanding of the developmental mechanisms of insomnia is required. The function of sleep is developed and maintained through the balance of the reciprocal forces of sleep and arousal. These forces are contantly regulated by what is called a circadian rhythm. Sleep is induced by this rhythm which is affected by factors such as awakening time in the morning, amount of intellectual function, amount and time length of physical exercise and sunlight Insomnia could develop when this rhythm is delayed and leads to a "forbidden zone" which is a very difficult period for inducing sleep about two to four hours before the routine bedtime. Whereas sleep gradually develops in line with the circardian rhythm, arousal can occur very abruptly by any cause triggered by emotional discomfort or anxiety. Such characteristic and emotional factors as perfectionism, separation anxiety, secondary gain, insecurity, and negative cognition may provoke the inner anxiety and fear for insomnia, which can lead acute insomnia to a chronic one. As chronic insomnia is developed by multiple causes and factors, integrated approaches through analysis of above mentioned factors will be more effective in the treatment of insomnia than a simple administration of hypnotics.

염증 반응과 수면 장애 (Inflammation and Insufficient or Disordered Sleep)

  • 수면현상은 인간의 삶의 1/3을 차지하는 중요한 생리적 현상으로 인간의 수면은 신체의 기능을 유지하기 위한 중요한 생리적 기능뿐 아니라 수면을 통한 휴식을 통하여 신체 건강을 유지하고 성장을 유지하는 중요한 역할을 한다. 최근의 급격한 산업화와 정보화로 인하여 급격한 사회 생활구조의 변화를 유발하여 성인뿐 아니라 소아에서도 평균수면 시간의 감소와 다양한 스트레스 환경으로 인한 수면의 질적 저하로 인한 다양한 수면 문제가 사회적 문제로 대두되고 있다. 단순히 수면 부족 또는 수면의 질적 저하뿐 아니라 수면 장애, 특히 폐쇄성 수면 무호흡증은 심혈관 질환뿐 아니라 비만, 대사증후군, 심혈관 질환, 뇌혈관 질환 등 의 다양한 만성 질병의 발생과 깊은 연관성이 있는 것으로 보고하고 있다. 특히 이러한 만성질환을 유발하는 핵심기전으로 염증 반응이 관여하고 있는 것으로 알려져 있으며, 따라서 수면 현상과 다양한 수면 장애에 대한 관심이 급격히 증가하고 있는 실정이다. 비록 수면 무호흡증으로 인한 이러한 질병의 이환에 대한 기전은 다양한 요인이 복합적으로 작용하는 것으로 알려져 있지만 핵심 기전으로 알려진 염증반응의 활성화가 병태 생리학적으로 중요한 역할을 하는 것으로 추측하고 있다. 따라서 최근 수면 무호흡증을 낮은 수준의 만성 염증 질환으로 인식되고 있으며, 더불어 비만과 수면무호흡증으로 인한 연관된 질병의 이환을 더욱 증가시키는 것으로 보고되고 있다. 따라서 본 종설에는 수면의 질적 또는 양적 저하뿐 아니라 폐쇄성 수면 무호흡증으로 인한 염증성 반응의 활성화가 인간의 신체 면역 과정 미치는 영향을 중심으로 최근의 연구 결과 및 수면 무호흡증과 비만의 잠재적 상호작용에 대하여 서술하고자 한다

호흡지지요법을 적용 중인 중환자실 입원환자의 액티그래피 측정 수면특성 (Actigraphy-Based Assessment of Sleep Parameters in Intensive Care Unit Patients Receiving Respiratory Support Therapy)

  • Purpose : This study aimed to investigate sleep parameters and to identify differences according to respiratory support therapy, sedation, and pain medication in intensive care unit (ICU) patients. Methods : In this observational study, sleep parameters were measured using actigraphy. We observed 45 sleep events in 30 ICU patients receiving respiratory support therapy. We measured the sleep parameters, time, efficiency, and wakefulness after sleep onset (WASO). The differences in sleep parameters according to the respiratory support therapy were analyzed using the Mann-Whitney U test. Results : The average daily sleep time of the participants was 776.66±276.71 minutes, of which more than 60% accounted for daytime sleep. During night sleep, the duration of WASO was 156.93±107.91 minutes, and the frequency of WASO was 26.02±25.82 times. The high flow nasal cannula (HFNC) group had a significantly shorter night sleep time (𝑥2=7.86, p =.049), a greater number of WASO (𝑥2=5.69, p =.128), and a longer WASO duration (𝑥2=8.75, p =.033) than groups of other respiratory therapies. Conclusion : ICU patients on respiratory support therapy experienced sleep disturbances such as disrupted circadian rhythm and sleep fragmentation. Among respiratory support regimens, HFNC was associated with poor sleep parameters, which appears to be associated with the insufficient use of analgesics. The results of this study warrant the development of interventions that can improve sleep in ICU patients receiving respiratory support, including HFNC.

청소년의 수면시간과 우울 및 자살생각과의 관계 (Association between Self-Reported Sleep Duration and Depression or Suicidal ideation in Korean Adolescents)

  • Objective : The purpose of this study was to examine the relationship between self-reported sleep duration and depression or suicidal ideation in Korean adolescents. Methods : The data obtained from the 11th Korea Youth Risk Behavior Web-Based Survey (KYRBWS) were analysed. 68,043 students from 797 schools participated in this nationwide survey. Results : 53.7% of male students and 68.4% of female students had reported that a weekday sleeping duration of less than 6 hours. When the depression and suicidal ideation were analyzed by age, the odds ratio was higher in middle-school students than in high-school students. Both male and female students showed a higher risk of depression and suicidal ideation when sleeping duration was less than 5 hours and the risk was slightly higher in boys than in girls. Conclusion : Insufficient sleep duration in Korean adolescents increases depression and suicidal ideation, and these results differ according to age and gender.

소아수면과다증과 수면무호흡 (Childhood Hypersomnia and Sleep Apnea Syndrome)

  • Natural sleep pattern and its physiology in childhood are much different from those in adulthood. Several aspects of clinical evaluation for sleepiness in childhood are more difficult than in adulthood. These difficulties are due to several factors. First, excessive sleepiness in childhood do not always develop functional impairments. Second, objective test such as MSLT may not be reliable since it is hard to be certain that the child understand instructions. Third, sleepiness in children is often obscured by irritability. paradoxical hyperactivity, or behavioral disturbances. Anseguently, careful clinical evaluation is needed for the sleepy children. Usual causes of sleepiness in children are the disorders that induce insufficient sleep such as sleep apnea syndrome, schedule disorder, underlying medical and psychiatric disorder, and so forth. After excluding such factors, we can diagnose the hypersomnic disorders such as narcolepsy, Kleine-Levin syndrome, and idiopathic central nervous system hypersomnia. Among the variety of those causes of sleepiness, I reviewed the clinical difference of narcolepsy and obstructive sleep apnea syndrome in childhood compared with in adulthood. Recognition of the childhood narcolepsy is difficult because even severely sleepy children often do not develop pathognomic cataplexy and associated REM phenomena until much later. Since childhood narcolepsy give srise to many psychological, academical problem. Practicers should be concerned about these aspects. Childhood obstructive sleep apnea syndrome is different from adult obstructive sleep apnea syndrome too. Several aspects such as pathophysiology. clinical feature, diagnostic criteria, complication, management, and prognosis differ from those in the adult syndrome. An important feature of childhood obstructive sleep apnea syndrome is the variety of severe complications such as behavioral disorders, cognitive impairment, cardiovascular symptoms, developmental delay, and ever death. Fortunately, surgical interventions like adenotosillectomy or UPPP are more effective for Childhood OSA than adult form. CPAP is a "safe, effective, and well-tolerated" treatment modality too. So if early detection and proper management of childhood OSA were done, the severe complication would be prevented or ever cured.

경부 및 체간 상부 근막이완기법 적용이 만성 경부통 환자의 통증, 불면증 및 수면에 미치는 영향 (Effects of Application of Myofascial Release of Neck and Upper Trunk on the Pain, Insomnia and Sleep Disturbances in Patients with Chronic Neck Pain)

  • Purpose : Chronic neck pain negatively impacts the quality of life and causes various problems in daily life due to pain, insomnia, and sleep disturbances in patients with this condition. Therapeutic interventions to solve these problems in rehabilitation and physical therapy are being introduced; however, the evidence of the efficacy of myofascial release (MFR) is still insufficient. This study aimed to investigate the effects of applying MFR on pain, insomnia, and sleep disturbances in patients with chronic neck pain. Methods : Ten patients with chronic neck pain were randomly selected and grouped into the experimental group (n1 = 10) and control group (n2 = 10) by cross-over design. Pain was measured before and after MFR intervention. Moreover, insomnia was measured only after MFR intervention. Polysomnography was performed after MFR intervention. Wilcoxon signed rank test and Mann-Whitney U test were used for the visual analog scale (VAS). Independent sample t-test was separately performed to measure insomnia and sleep. Results : After MFR intervention, the VAS score of the experimental group (p = 0.005) significantly decreased than that of the control group (p = 0.002). The insomnia score of the experimental group significantly decreased than that of the control group (p = 0.001). The total sleep time (p = 0.001), sleep efficiency (p = 0.001), and sleep latency (p = 0.001) of the experimental group significantly increased than those of the control group in the polysomnographic measurement. Conclusion : The application of MFR of the neck and upper trunk may have a positive effect on pain, insomnia, and sleep disturbances in patients with chronic neck pain. It was also suggested that an objective and quantitative polysomnography can be used more often in the field of rehabilitation and physical therapy.

제14기(2018년) 청소년건강행태조사 자료를 이용한 청소년 총 수면시간과 자살 생각 사이의 연관성 (Association Between Total Sleep Duration and Suicidal Ideation: Based on 14th Korean Youth Health Behavior Survey)

  • 본 연구의 목적은 청소년기의 총수면의 양과 그들의 정신적 건강 사이의 중요성을 알아보기 위해 2018년 청소년 건강행태온라인조사자료를 이용하여 총소년 총 수면시간과 자살생각 사이의 연관성을 규명하기 위해 수행하였다. 연구방법은 청소년건강온라인조사자료를 활용하여 카이제곱검정과 다중로지스틱회귀분석을 실시하여 결과를 도출하였다. 분석결과 수면의 질을 보정한 상태로 청소년들의 총 수면시간과 자살 생각 사이의 연관성을 분석한 결과 청소년들이 본인의 나이에 충분하다고 권장되는 시간을 충족하지 못하는 수면 양상을 보일 때 자살 생각을 하는 경향이 증가하는 것으로 나타났다. (OR: 1.17, 95% CI:1.06-1.29, p-vaule: 0.00) 이를 성별에 따라 층화분석한 결과 남학생의 경우에는 충분한 수면이 보장되면 자살 생각을 하는 빈도가 낮아지는 경향을 보이는 반면(OR:0.83, 95% CI:0.71-0.96, p-value:0.02), 여학생의 경우에는 남학생과는 반대로 충분한 수면이 보장되지 못할 때 자살 생각의 빈도가 높아지는 것으로 나타났다(OR:1.22, 95% CI:1.06-1.41, p-value:0.01). 이처럼 청소년들의 성별과 수면의 양상에 따라 총 수면시간이 자살 생각에 대해 미치는 영향이 다르기 때문에 10대의 자살 위험성을 낮추기 위해 이들의 생활습관에 맞는 새로운 정책적 고려가 필요할 것으로 생각된다.
