치매(痴呆)의 한열허실(寒熱虛實) 변증(辨證)을 위한 지표 문항 개발에 관한 기초 연구 (Preliminary Research for Development of Instrument for Cold-Heat & Deficiency-Excess Pattern Identification of Dementia)

  • This study was performed to develop cold-heat and deficiency-excess pattern identification for dementia, as well as for standard Korean medicine diagnosis and treatment. Five experts comprised of 4 neuropsychiatrists of Korean medicine and 1 statistician to develop cold-heat and deficiency-excess pattern identification for dementia. We searched studies about pattern identification and selected 507 articles using Oasis search terms provided by the KIOM. As a result, 10 pattern identification research study were recruited. Moreover, we analyzed neuropsychological assessments for dementia that evaluate Behavioral and Psychological Symptoms of Dementia (BPSD) and cognitive function using experts conferences and we selected neuropsychological instruments using pattern identification. Six cold patterns, six heat patterns, ten deficiency patterns, and four excess patterns were identified according to the cold-heat and deficiency-excess pattern identification of dementia. We selected the Caregiver-Administered Neuropsychiatric Inventory and the Korean Mini-Mental State Examination as neuropsychological assessments of dementia, which examine behavioral symptoms and cognitive function, suspectively. We formed positive and negative correlation between Korean medicine pattern identification and neuropsychological assessments for dementia. We developed and suggested a forecast module of pattern identification for dementia. But, it is necessary to perform additional clinical trials to verify its validity and accuracy.

알코올성 간염의 변증설문 개발에 관한 연구 (Study to Develop the Pattern Identification Questionnaire for Alcoholic Hepatitis)

  • I Alcoholic hepatitis is a serious liver disease that may lead to cirrhosis and carcinoma, and the short-term mortality rate is fairly high in severe patients. This study was conducted to develop the instrument of pattern identification for alcoholic hepatitis. We made the pattern identification questionnaire and symptoms indicator through reviewing traditional oriental medical literatures and got advices from the advisor committee with Delphi technique. The advisor committee on this study was organized by 10 professors of internal medicine of oriental medical colleges nationwide. The questionnaire was composed of questions about 6 pattern identification - dampness, heat, liver, spleen, cold and dryness. We gave importance to each symptoms of 6 pattern identification which had been scored on a 5-point scale. We surveyed two groups: 36 male alcoholic hepatitis patients whose Alcohol Use Disorder Identification Test(AUDIT) scores were over 12 and who drank alcohol over 40 g per day were allocated to the hepatitis group. Forty three men who did not drink alcohol were allocated to the normal group. Alcoholic hepatitis had relativities to dampness, heat among cause of disease and liver, spleen among viscera. There were statistical significances between the hepatitis group and the normal group in dampness, heat, liver questionnaire. As a result of this study we suggest that the questionnaire would be effective instruments of pattern identification for alcoholic hepatitis.

평소 증상 기반 한열변증 설문지의 신뢰도 및 타당도 연구 (Development on the Questionnaire of Cold-Heat Pattern Identification Based on Usual Symptoms: Reliability and validation Study)

  • The aims of this study were to evaluate the reliability and validity of the cold and heat pattern identification questionnaire (CHPIQ). From July 2015 to December 2015, 120 participants, university faculties, filled out CHPIQ by the way of self-reporting. Then two Korean medical doctors independently diagnosed them whether they belonged to cold pattern (CP) or not, and heat pattern (HP) or not. We evaluated the internal consistency using Cronbach's alpha coefficient, and the validity using the sensitivity and specificity through receiver operating characteristic-curve. The internal consistency (Cronbach's alpha coefficient) showed 0.754 (CP) and 0.753 (HP). The area under the curve was recorded with 0.884 (CP) and 0.786 (HP). The agreements between CHPIQ and experts were 82.8% (CP) and 72.9% (HP). The sensitivities showed 0.707 (CP) and 0.719 (HP), and the specificities were 0.935 (CP) and 0.736 (HP). This study suggests that CHPIQ is a reliable and valid instrument for estimating cold-heat pattern identification.

우울증 변증도구 개발을 위한 기초연구 (Preliminary Study to Develop the Instrument on Pattern Identifications for Depression)

  • Objectives: The purpose of this study is to develop a standard tool for pattern identifications in Korean Medicine for depression. Methods: The advisor committee for this study was organized by 15 Korean Medicine neuropsychiatry professors from 11 Colleges of Korean Medicine. The items and structure of the tools were based on reviews of published literature. In order to develop the tool, we took the consultation from discussions with the advisor committee twice and also incorporated additional advices from e-mail correspondences. Results: 1) We divided the symptoms and signs of depression into 11 pattern identifications. 2) We obtained the mean weights which reflected the standard deviations from each symptoms of the 11 pattern identifications which were scored on a 100-point scale by 15 experts. 3) We designed the Korean medicine pattern identification tool for depression. It was composed of 66 questions in the question- and-answer format. Conclusions: There are some points which should be considered in this study. First, we couldn't reach a complete agreement on the concept of 11 patterns. Second, each pattern identification has a different number of symptoms and signs. In addition, the items of symptoms and signs of each pattern identification are unequal. Third, as we did not set any clinical trials when using this tool, it was not possible to test its validity and reliability. Although there are some limits in this study, the development of pattern identification tools for depression through discussions with the advisor committee is meaningful. If the validity and reliability of the Korean Medicine pattern identification tools for depression are confirmed through clinical trial,s this tool is expected to be applied to the subsequent researches in the future.

중의학 변증과 양방 검사의 상관관계 연구 현황 -CNKI를 이용하여- (A Review Study in the Correlation between Pattern Identification of Traditional Chinese Medicine and Western Medicine Examination -Research on CNKI-)

  • Objectives : The purpose of this study is to investigate the correlation between pattern identification of traditional Chinese medicine (TCM) and western medicine, examined by a systematic research of Chinese medicine papers. Methods : We searched for the papers regarding pattern identification of TCM published from 1994 to 2012 in CNKI (China National Knowledge Infrastructure http://www.cnki.net) at April, 2012. Results : A total of 30 studies were finally included; 18 studies of them were related to stroke (cerebral infarction) and there were 12 studies regarding other diseases, such as hypertension, chronic colonitis, vascular dementia, mild cognitive impairment and etc. All 30 studies were analyzed and classified by diseases, differentiation of syndromes, numbers of subjects, the instrument of pattern identification, items of western medicine examination and statistical results. Conclusions : According to our study, there are some correlations between pattern identification of TCM and various items of western medicine examination. The result suggests a possibility of using the western medicine examination data for pattern identification of TCM.

  • Objectives: This study was carried out to develop a standard tool of pattern identifications for insomnia in Korean Medicine. Methods: The advisory committee for this study was organized by 17 Korean Medicine neuropsychiatry professors who were faculty members of different Korean Medicine colleges. The pattern identifications and symptoms for this tool were extracted from published Korean and Chinese literature. The Instrument on Pattern Identifications for Insomnia was developed following discussions among internal experts and after consultations with members of the advisory committee. Results: 1) Five pattern identifications were set for the tool. 2) The mean weights which represent the importance of each symptom and scored on a hundred-point scale were obtained. 3) The Instrument on Pattern Identifications for insomnia was designed in the self-reporting format composed of 47 questions. Conclusions: An Instrument on Pattern Identifications for Insomnia was created in this study. However, owing to any clinical trials has not been set yet. Therefore its validity and reliability were not confirmed. To make up for this limitation, the further clinical study would be performed in the near future.

  • In this paper, we present the different characteristics of NDVI fluctuation pattern between irrigated and non-irrigated area in Suphanburi province, in Central Thailand. For non-irrigated rice cultivation area, there is a strong correlation between NDVI fluctuation and peak rainfall, while there is a lower correlation with irrigated area. In this study, the 'peak detector' classifier was developed to identify the area of non-irrigated and irrigated cropping and its cropping intensity (number of crops per year). This classifier was created based on cropping characteristics such as number of crops, time or planting period of each crop and its relationship with the peak of rainfall. The classified result showed good accuracy in identification irrigated and nonirrigated rice cultivation areas.

간울형 불임 변증 설문지 개발 연구 (A Study on the Development of the Questionnaire for Pattern Identification of Liver-qi Stagnation Infertility)

  • Objectives: The aim of this study is the development of the questionnaire for pattern identification of Liver-qi stagnation infertility. Methods: We conducted a literature review and retrieved the symptoms and diagnosis from Korean and Chinese literatures which mentioned pattern identification of Liver-qi stagnation infertility. Based on the findings, We conducted three times expert Delphi surveys on selection of question items and determination of weight. Results: 12 questionnaire items for subjects and 4 questionnaire items for researchers were selected from 19 references. From expert delphi surveys, we finally determined 14 questionnaire items (10 items for subjects, 4 items for researchers) which are clinically significant and obtained weight of question items. Conclusions: Questionnaire for pattern identification of Liver-qi stagnation infertility was developed through experts' discussion. Further study is required to identify the validity and reliability of this pattern identification instrument for Liver-qi stagnation infertility.

화병 한의 평가도구 개발을 위한 기초 연구 (Preliminary Study to Develop the Instrument of Oriental Medical Evaluation for Hwa-Byung)

  • Objectives : This study was performed to develop a standard instrument of oriental medical evaluation for hwa-byung. Methods : The advisor committee on this study was organized by 17 neuropsychiatry professors of oriental medical colleges. The items and structure of the instrument were quoted from the instrument of pattern identification for hwa-byung. We took consultation twice from the advisor committee and we also took additional advices by e-mail. Results : We discriminated between bian-zheng and su-zheng from the answers of the advisor committee. We got the mean weight of each symptom and sign from the answers of the advisor committee. We got the final weight from the combination of the ratio of bian-zheng to the number of all answers of the advisor committee and mean weight. Conclusions : The instrument of oriental medical evaluation for hwa-byung was developed through experts' discussion. If the validity and reliability of this instrument is confirmed through additional clinical trial, the instrument of oriental medical evaluation for hwa-byung is expected to be applied to the subsequent research.

비기허증(脾氣虛證) 진단평가도구의 신뢰도 및 타당도 예비 평가: 만성 소화불량 환자 대상 설문지 검증 임상시험 (Reliability and Validity Analysis of a Standard Instrument of Diagnosis and Assessment for Spleen Qi Deficiency Pattern in Chronic Dyspepsia Patients)

  • Objectives: This study is aimed at assessing the reliability of a standard instrument of diagnosis and assessment for Spleen Qi deficiency pattern questionnaire (SQDQ) and examining the validity of the SQDQ by comparing the pattern identification scores of different groups. Methods: We conducted a survey of 72 participants (60 patients with chronic dyspepsia and 12 healthy subjects) using self-reported questionnaire. Participants were given written consent and this study was performed under the permission of institutional review board of Kyung-Hee university Korean medicine hospital. Results: The reliability and the validity of the questionnaire were inspected. Internal consistency of the SQDQ was excellent. Construct validity analyzed by exploratory factor analysis produced 4 factors, which were selected from eigenvalues that are greater than 1.0. The factor 1, 2, 3 and 4 showed 'fatigue', 'meal', 'diagnosis' and 'figure' respectively. For most of SQDQs' items, there were significant differences observed between the Spleen Qi and the non-Spleen Qi groups. However, the 'emaciation', 'tongue diagnosis' and 'pulse diagnosis' showed no significant differences. Conclusions: The SQDQ restructured in this study may provide a fundamental questionnaire and a further study is required for a more advanced, standardized and statistically proven questionnaire.
