Clinical use of cold blood potassium cardioplegia (냉혈 K 심정지액의 임상적 이용)

  • Cold blood potassium cardioplegia, by two types of the infusion systems, was used in 45 patients who were undergone the open cardiac operation. Method A [in 25 patients] was the syringe infusion system and method B [in 20 patients] was the pump infusion system. Cold blood potassium cardioplegic solution was used less amount on method B than method A. Serum potassium was often increased significantly on method A as a result of excessive infusion of cold blood potassium cardioplegic solution. But method B, excessive infusion of cold blood potassium cardioplegic solution was prevented by reperfusion of the previous infused cold blood potassium cardioplegic solution through the recirculation system. Alteration of infused rate and concentration of potassium in cold blood potassium cardioplegic solution during infusion [which might be suggested on the method A] could be controlled on method B.

  • The resin infusion method is the latest technology of FRP laminating and cleaning to improve FRP hull quality. This method is focused on how to arrange infusion channels for the laminiating strategy. The laminating strategy using the resin infusion method has been utilized to complete the infusion work and remove the cavities on the FRP surface within the curing time. It is resulting from the arrangement of infusion and vacuum channels, the resin property, and the combination of FRP. This strategy has been depended on the field experience for manufacturing FRP without the resin infusion simulation. This study can help to improve the efficiency of FRP fabrication with the laminating strategy including the resin infusion simulation instead of the field experience.

Performance evaluation study of a commercially available smart patient-controlled analgesia pump with the microbalance method and an infusion analyzer

  • Background: Patient-controlled analgesia (PCA) has been widely used as an effective medical treatment for pain and for postoperative analgesia. However, improper dose errors in intravenous (IV) administration of narcotic analgesics from a PCA infusion pump can cause patient harm. Furthermore, opioid overdose is considered one of the highest risk factors for patients receiving pain medications. Therefore, accurate delivery of opioid analgesics is a critical function of PCA infusion pumps. Methods: We designed a microbalance method that consisted of a closed acrylic chamber containing a layer and an oil layer with an electronic balance. A commercially available infusion analyzer (IDA-5, Fluke Co., Everett, WA, USA) was used to measure the accuracy of the infusion flow rate from a commercially available smart PCA infusion pump (PS-1000, UNIMEDICS, Co., Ltd., Seoul, Korea) and compared with the results of the microbalance method. We evaluated the uncertainty of the flow rate measurement using the ISO guide (GUM:1995 part3). The battery life, delay time of the occlusion alarm, and bolus function of the PCA pump were also tested. Results: The microbalance method was good in the short-term 2 h measurement, and IDA-5 was good in the long-term 24 h measurement. The two measurement systems can complement each other in the case of the measurement time. Regarding battery performance, PS-1000 lasted approximately 5 days in a 1 ml/hr flow rate condition without recharging the battery. The occlusion pressure alarm delays of PS-1000 satisfied the conventional alarm threshold of occlusion pressure (300-800 mmHg). Average accuracy bolus volume was measured as 63%, 95%, and 98.5% with 0.1 ml, 1 ml, and 2 ml bolus volume presets, respectively. A 1 ml/hr flow rate measurement was evaluated as 2.08% of expanded uncertainty, with a 95% confidence level. Conclusion: PS-1000 showed a flow accuracy to be within the infusion pump standard, which is ± 5% of flow accuracy. Occlusion alarm of PS-1000 was quickly transmitted, resulting in better safety for patients receiving IV infusion of opioids. PS-1000 is sufficient for a portable smart PCA infusion pump.

Strength Characteristics of FRP Composite Materials for Ship Structure (선체구조용 FRP 복합재료의 강도 특성)

  • In this study, various types of composite materials and adhesives that are actually used in the shipbuilding field for small ships, leisure boats, and fishing boats were applied in the hand lay-up method and vacuum infusion method to manufacture specimens. Then the tensile strength, tensile modulus, flexural strength, and flexural modulus values of these specimens were obtained. In addition, the barcol hardness and fiber content were obtained from the specimens. The results showed that the strengths of the specimens manufactured using the vacuum infusion method were higher than those manufactured using the hand lay-up method. Moreover, the barcol hardness and fiber content were also higher in those manufactured using the vacuum infusion method. The specimens manufactured using the vacuum infusion method were thinner despite their large fiber content.

  • The purpose of this study is to explore the process of knowledge creation at the infusion stage in IT adoption. Because of the lack of the prior research on the infusion and the process of individual knowledge creation we employed Grounded Theory Method which is a qualitative method for building a theory inductively. Collecting and analyzing data from 13 knowledge workers including medical doctors, project managers, and bankers, a process model is developed through the serial process of open, axial and selective coding. We conceptualized the knlowledge creation at the infusion stage as a social process where knowledge is expressed through private and public justifications. This study also found that organizational politics, top management supports, and organization strategy are important factors facilitating the knowledge creation process.

The Effect of Increased Dilution Volume and Prolonged Infusion Time of Vancomycin on Incidence of Adverse Reactions through Peripheral Venous Cannulae (말초정맥을 통한 반코마이신희석과 주입시간연장이 부작용발생에 미치는 영향)

  • The purpose of the study was to explore the effect of 2 hour infusion of vancomycin(1g) in 200ml of isotonic saline every 12 hour on the frequency of "red man syndrome", phlebitis and length of peripheral catheter placement of infected patients, in order to provide safe infusion method for reducing vancomycinin-duced RMS and phlebitis. The subjects of the study consisted of 16 hospitalized patients; 3 oncology and gastro-intestinal patients, 1 neurological patient, 6 thoracic surgical patients and 6 orthopedic patients, who had received vancomycin from July to October in 1999 at S-hospital. The dependent variables were the incidence of RMS, phlebitis and the length of peripheral catheter placement. The incidence of RMS was checked by an inspector at the first night whenever the infusion method of vancomycin was changed. RMS was observed every 15 minutes during an hour for symptoms of RMS such as itching, erythema, chest pain and systolic blood pressure. Incidence of phlebitis was assessed by inspector twice a day from the insertion of peripheral catheter to the removal of the catheter. The data were analyzed by percentage, mean, $X^2$-test, t-test, repeated ANOVA, and logistic regression analysis using the SPSSWIN program. The results are summarized as follows; 1. No significant difference was identified in frequency of RMS between the experimental group and control group. 2. There was no significant difference in the change of systolic blood pressure as the time goes on between the experimental group and control group. 3. The incidence of phlebitis was significantly lower in the experimental group than in the control group. 4. The length of peripheral catheter placement was significantly longer in the experimental group than in the control group. 5. Other drugs administrated with vancomycin didn't influence the occurrence of phlebitis. However, the infusion method of vancomycin influenced the occurrence of phlebitis. The results suggest that 2 hour infusion of vancomycin(1g) in 200ml of isotonic saline every 12 hours may decrease the incidence of phlebitis and increase the length of peripheral catheter placement compared to 1 hour infusion of vancomycin(1g) in 100ml of isotonic saline every 12 hours. However, it does not reduce the incidence of RMS.

  • Recently, the applied frequency of composite materials was increased from the viewpoint of lightweight, high strength and low cost when a leisure boat and a fishing boatwere built. However, studies on the mechanical properties of composite material with ship are rare. Specially, a leisure boat and fishing boat with FRP had been built by hand lay-up method. However, the vacuum infusion method is rising recently for ship building. The manufactured these FRP plates were combined by using the adhesive. Therefore, in this study Cleavage peel strength, Shear strength and fatigue limit of adhesive bonds by tensileloading were estimated. From test results, the strengths of FRP specimens made by the vacuum infusion method are higher than that of the hand lay-up method.

  • This study attempted to find a new manufacturing method through Basalt fiber and resin infusion method to expand the range of expressions of Contemporary jewelry produced in studios. To this end, the concept of fiber introduction and infusion method was examined through analysis of used cases, and based on this, flat plates and color samples were produced, and then the samples were made to estimate the possibility of direct use in jewelry. As a result, the infusion method process presented certain results that could be used as a production process in the studio, and Basalt fibers were able to express various colors of spray and porselin, especially fiber its painted with porselin, which have high strength and they can be expressed intentionally, it are very valuable as jewelry's material, it is expected to be an opportunity to expand the practical production methods and expressions of Contemporary jewelry.

  • The importance of the infusion pump monitoring system has become amplified in accordance with the growth of IoT(Internet of Things) technology and medical devices. A monitoring system can be described as an essential part of infusion pump system because the patient must be observed all the time. Infusion pump monitoring system is significant to have better safety and efficiency. In this paper, we propose an efficient algorithm and scheme in the infusion monitoring system. In particular, the proposed algorithm based on the time of the database interlocking part was found to have a 30% higher efficiency than the conventional method.

  • The author experienced of four patients with intractable pain who were treated by continuous intraventricular infusion of morphine through an implanted port system. One suffered from tongue cancer and the others from bone metastasis or distant metatasis of abdominal cancer which were ineffectively to managed through an epidural route. Our experience is that this is a safe and effective method of pain management in patients with head and neck cancer. It is useful as well in patients who have intractable pain that cannot be managed through an intrathecal or epidural route.
