Analysis of the Correlation between Activity of the Suprahyoid Muscles, Infrahyoid Muscles and the New VFSS Scale in Stroke Patients with Dysphagia

  • PURPOSE: This study was conducted to investigate the correlation between the activity of suprahyoid muscles, infrahyoid muscles and swallowing ability in stroke patients with dysphagia. METHODS: The subjects of this study were 120 patients who were diagnosed with CT or MRI stroke and had swallowing disorder at Daegu Metropolitan General Hospital from August 2014 to February 2017. The suprahyoid and infrahyoid muscle activity was measured in patients with dysphagia and the new videofluoroscopy swallowing studies scale (new VFSS scale) was used for evaluation of swallowing function. Correlation analysis was conducted using the measured data. RESULTS: The activities of suprahyoid muscles were 325.8 (%RVC) on average, while the average infrahyoid muscle activity was 302.65 (%RVC) and the average value of the new VFSS scale was 31.52. The total for oral organs was 3.62 on average and that for pharynx organs was 28.30 on average. The activity of the suprahyoid muscles showed a significant positive correlation with the activity of the infrahyoid muscles, but a significant negative correlation with the total oral phase, total pharyngeal phase and total new VFSS scale (p<.01). The activity of the infrahyoid muscles showed a significant negative correlation with the total oral phase, total pharyngeal phase, and total new VFSS (p<.01). CONCLUSION: Based on the results of this study, it is necessary for researchers to consider the infrahyoid and suprahyoid muscles when conducting swallowing rehabilitation.

성악전공자와 비전공자에서의 음도에 따른 성대외근의 표면근전도 변화 (Surface EMG Activity of the Suprahyoid and Infrahyoid Muscles along the Pitch Changes in Trained and Untrained Singers)

  • Extrinsic laryngeal muscles are well known to be important for the classical singers. We tried to elucidate any differences in the function of above muscles between trained and untrained singers by non-invasive surface electromyography(EMG). Four trained sopranos and four untrained singers sang vowel /i/ at different pitch(E3, G3, C4, E4, G4, C5, E5, G5, C6). The EMG activities of the suprahyoid, infrahyoid and omohyoid muscles were measured using surface electrodes. In trained singers, infrahyoid muscle activities increased more than those of suprahyoid in most of pitch. To the contrary, in untrained singers, the pattern of EMG activities were variable among each subjects and the EMG activities of suprahyoid muscles were relatively greater than those of infrahyoid.

  • Obstructive sleep apnea syndrome(OSAS) is a complex sleep disorder characterized by intermittent apnea secondary to sleep-induced obstruction of the upper airway. It occurs because of an airway obstruction anywhere between the trachea and the oronasal apparatus. The hallmark of OSAS is snoring, which is caused by vibration of the tissues of the pharynx as the airway narrows. The consequences of OSAS have focused on excessive daytime sleepiness resulting from sleep fragmentation and the cardiovascular derangements producing hypertension and arrhythmias. The primary method of controlling OSAS has been surgery. The current surgical procedures used for OSAS are tracheostomy, tonsillectomy, nasal septoplasty, uvulopalatopharyngoplasty, anterior mandibular osteotomy with hyoid myotomy and suspension, and maxillary, mandibular and hyoid advancement. We report a case of OSAS that was improved by genial advancement with infrahyoid myotomy and suspension. The patient was objectively documented by polysomnography, cephalometric analysis, and physical examination before the surgical procedure. The patient underwent genial advancement with infrahyoid myotomy and suspension. Patient had a good response from surgery.

진행된 성문상부암의 발생부위별 임상적 특성에 관한 연구 (Clinical Characteristics of Advanced Supraglottic Cancer by Subsites)

  • The prognosis of supraglottic cancer is worse than that of glottic cancer. Supraglottic cancers by subsites have different microenvironment of cancer cells, locoregional spread patterns. Therefore we presume that high therapeutic efficacy, while preserving the organ, can be obtained when supraglottic cancer is treated effectively according to its biological behaviors. For the purpose of determination of clinical characteristics and causes of treatment failures by subsites of supraglottis, the authors analyzed 24 cases(stage III 14 cases, stage IV 10 cases) of supraglottic cancer which were managed mainly by surgery in our institute. The results were as follows; 1) The suprahyoid group had worse pathologic grades, more frequent spread to hypopharynx, more freguent recurrence at primary site, and better three-year survival rate than the infrahyoid group. 2) The infrahyoid group had more frequent spread to glottis, understaging, recurrence at cervical nodes than the suprahyoid group. 3) There was no differences in nodal metastasis by sub sites. These results suggest that the suprahyoid group may have more aggressive spread pattern but better prognosis than the infrahyoid group.

물의 양과 온도가 목뿔위근육과 목뿔아래근육의 근활성도에 미치는 영향 (Effect of Water Volume and Temperature on Muscle Activity of the Supra and Infrahyoid Muscles)

  • 목적 : 본 연구는 정상 성인을 대상으로 삼킴 시 물의 온도와 양에 따른 목뿔위근육과 목뿔아래근육의 활성도를 측정하여, 임상에서 삼킴장애를 평가하기 위한 기초자료로 활용하고자 하였다. 연구 방법 : 건강한 성인 27명을 대상으로 물의 양과 온도에 따른 목뿔위근육과 목뿔아래근육의 근활성도를 비교하기 위하여 양은 3 mL, 20 mL로, 온도는 $4^{\circ}C$, $22^{\circ}C$, $40^{\circ}C$로 각각 설정하였고, 대상자별로 기준값(reference voluntary contraction, %RVC)을 설정하기 위하여, 침을 삼키는 동안 오른쪽 및 왼쪽 목뿔위근육과 목뿔아래근육의 활성도를 3회 측정하여 평균값을 사용하였다. 연구 장비는 안드로이드 기반 근전도 측정 장비인 2EM(4D-MT, Relive, Gimhae, Korea)을 이용하여 자료를 수집, 처리하였다. 결과 : 양을 기준으로 비교했을 때 왼쪽 목뿔아래근육은 $4^{\circ}C$, $22^{\circ}C$, $40^{\circ}C$에서 (p=.00; p=.00; p=.00), 오른쪽 목뿔아래근육은 $22^{\circ}C$$40^{\circ}C$에서 (p=.01; p=.01), 왼쪽 목뿔위근육은 $4^{\circ}C$에서 (p=.03), 3 mL 물보다는 20 mL 물을 삼킬 때 근활성도가 증가하였다. 온도를 기준으로 분석한 결과 20 mL의 물을 삼킬 때 $4^{\circ}C$, $22^{\circ}C$ 보다는 $40^{\circ}C$ 조건에서 오른쪽 목뿔위근육의 근활성도가 유의하게 감소하였다 (p=.04). 결론 : 20 mL가 3 mL보다 오른쪽 및 왼쪽 목뿔아래근육, 왼쪽 목뿔위근육에서 근활성도가 유의하게 증가하였고, $40^{\circ}C$$4^{\circ}C$, $22^{\circ}C$보다 오른쪽 목뿔위근육에서 근활성도의 유의한 감소를 보였으나, 양과 온도를 모두 고려한 일부조건에서만 의미가 있었다. 추후에는 점도, 맛 등의 다른 변수를 고려하여 건강한 대상자 뿐 아니라 신경계 손상환자를 대상으로 한 임상연구가 더 필요할 것이다.

정중선 이소성 갑상선에서 주사상의 변화 추적 - 증례보고 - (A Changing Pattern of Isotope Scan of the Midline Ectopic Thyroid - A Case Report -)

  • A 17 month-old girl presented in the pediatric clinic on November 27th, 1990 with a neck mass. The mass was 2 cm in diameter, firm in consistency and movable on the upper pole of the thyroid cartilage in the midline. The technetium thyroid scan showed a hot reactivity at the compatible site of the mass, but no other radioactivity in either site of the normal thyroid positions. At her second visit on January 23th, 1996, the mass had enlarged up to 3.5cm in diameter in the same location of the neck. The follow up thyroid scan revealed a walnut sized, snowman-like radioactivity. One of the snowman-like double images seemed to be a lingual ectopic thyroid and the other a midline ectopic thyroid remnant in the infrahyoid level. This interpretation was supported by the computed tomography of the neck, which showed a ligual mass in the foramen cecum area and an another mass in the anterior comis-sure of the larynx in the mildline. Thyroid function test was normal except a slightly increased TSH. As a result of this changing pattern of thyroid radioactive images, a case of a lingual thyroid as well as another midline ectopic thyroid tissue at the infrahyoid level is reported.

두부, 경부, 견부의 근육불균형에 관한 연구 (A Study of Muscle Imbalance of Head, Cervical and Shoulder Region)

  • This study were to review of muscle imbalance of head, cervical and shoulder region. Head, cervical and shoulder region is a complicated mechanical unit. interconnected by numerous soft tissue links. These links, or articulation are functionaly and reflexly interdependent on one another. The line of gravity falls anterior to the transverse axis of rotation for flexion and extension of the head and creates a flexion moment. which tends to tut the head forward, is counteracted by tension in the tectorial membrane, and ligamentum nuchae, and by activity of the neck extensors. Therefore, the flexion moment equilibrate with the extension moment. Changing of the equilibrium will make mid cervical straight. It will make forward head posture(FHP) also. FHP makes imbalance of suboccipital muscles, suprahyoid muscles and infrahyoid muscles. It has some relationship with temporomandibular joint, spine and equilibrium of pelvis.

종격동내 위치한 갑상선 내 갑상설관 낭종: 증례 보고 및 문헌 고찰 (Intrathyroid Thyroglossal Duct Cyst Extending into the Mediastinum: A Case Report and Review of Literature)

  • Thyroglossal duct cyst (TGDC) is the most common congenital cyst in the neck and usually presents in children. It is most commonly located at the infrahyoid level. However, intrathyroid TGDC in the adult is extremely rare and its pathogenesis remains unclear. Curiously, an intrathyroid TGDC was found on the lateral aspect of the inferior pole of the thyroid gland. Given the unique position of this lesion, and its capacity to stimulate other nodules in the thyroid, it should be differentiated from other lateral neck masses, such as: thymic cysts, branchial cleft cysts, cystic hygroma, lipomas, lymphadenopathies, cervical teratomas, and lymphomas. In this case, a separate cystic mass was found in the right inferior pole of the thyroid gland and extended to the mediastinum. We confirmed it as TGDC by pathology and histological examination.

갑상선 종양을 제외한 경부중앙 종물에 대한 임상적 고찰 (Evaluation of Midline Neck Masses Except Thyroid Tumors)

  • Midline neck masses have numerous origin and it is important to diagnose correctly for management. A clinical analysis of 29 cases of midline neck masses confirmed by histopathological examination was done retrospectively during the last 5 years. The results were followings; 1) Of 29 cases, thyroglossal duct cyst was most frequent(17 cases, 58.6%) and non-specific lymphadenopathy was the next(4 cases, 13.8%). 2) Midline neck masses were most frequent on the suprahyoid area(12 cases, 41.4%) and hyoid area was the next(7 cases, 24.1%). 3) Two thyroglossal duct carcinoma was included in 17 thyroglossal duct cyst. 4) Seventy percent of thyroglossal duct cyst was present on hyoid and infrahyoid area.

Influence of Forward Head Posture on Electromyography Activity of Hyoid Muscles During Mouth Opening

  • Although the relationship between temporomandibular disorder and forward head posture (FHP) is controversial, it is generally accepted that altered head posture can affect mandible position and masticatory muscles activity. Because suprahyoid (SH) and infrahyoid (IH) muscles are stretched by increased passive tension in FHP, this study investigated their activity during mouth opening in FHP compared to neutral head posture (NHP). Twenty healthy subjects (10 males and 10 females) participated in this study. Head postures were evaluated with a cervical range of motion instrument. Electromyography (EMG) activity of bilateral SH and IH muscles was measured while an open mouth was maintained at each head posture. Paired t-test was used to identify significant differences in normalized EMG activity between head postures. Statistical significance was set at .01. Results showed the normalized EMG activity of SH and IH muscles were significantly lower in FHP compared to NHP. This finding indicates that FHP affects the EMG activity of hyoid muscles when they are stretched.
