The benign tumor of smooth muscle, the leiomyoma, may arise anywhere in the body, in such tissues as the muscularis of the gut and the media of blood vessels, although by far its most common location is the uterus. Indeed, the mediastinal leiomyomas are extremely rare, especially when one excludes those arising in the esophagus. No dogmatic statements can be made with regard to age, sex distribution, symptoms, or clinical course because of the extreme paucity of cases available for evaluation. Although the majority of the reported cases are in the posterior mediastinum, thus suggesting esophageal origin. We have experienced a right inferior mediastinal leiomyoma in 36 years old housewife. She complained hemoptysis and right lower chest pain associated with intermittent low-graded fever and chillness. Routine X-ray film revealed a large irregular cystic mass density in the right lower thoracic cavity. On exploratory thoracotomy, a huge round yellow-gray colored solid mass, measured about 2.5kg in weight, was located in the right inferior mediastinum just above right hemidiaphragm, medially. The tumor mass was removed, being proved to be leiomyoma on histopathologic examination. Her postoperative course was uneventful and discharged without complication.

This is the third in a series on coronary artery bypass which reviews three alternative arterial conduits. The radial artery has become the most widely used of the three and accumulating experience demonstrates better patency at 10 years versus saphenous vein. Drawbacks are a long incision on the forearm, the propensity for spasm and persistent sensory disturbance in about 10%. The first is answered by endoscopic harvest which may yield a shorter conduit but reduces sensory nerve injury. Spasm is managed pharmacologically and by less harvest trauma. The gastroepiploic artery is used in situ and free and although the abdominal cavity is entered complications are minimal and patency compares favorably with the radial artery. Use of the inferior epigastric artery remains minimal and its similar length often requires composite use but limited patency data are supportive. Other arteries have had rare use and this is unlikely to change because the three presented here have significant advantages and acceptance.

In this paper, we survey various notions and results related to statistical convergence of a sequence of fuzzy numbers, in which statistical convergence for fuzzy numbers was first introduced by Nuray and Savas in 1995. We will go over boundedness, convergence of sequences of fuzzy numbers, statistically convergence and statistically Cauchy sequences of fuzzy numbers, statistical limit and cluster point for sequences of fuzzy numbers, statistical mono-tonicity and boundedness of a sequence of fuzzy numbers and finally statistical limit inferior and limit inferior for the statistically bounded sequences of fuzzy numbers.

Recently, the system of high speed milling(HSM) is widely used for reducing the operation time and maximizing efficiency of work. The most research of high speed milling system is still leaves much to be desired. Specially the research of mass pocket high speed processing with high precision is the first and probably the last. So this paper showed mass pocket processing of high precision with a duralumin and then confirmed a cause of inferior goods through the CAD/CAM pattern simulation and experimentation. And this paper showed high speed processing system reduce the rate of inferior from this optimal pattern.

6 Sigma movement is the quality innovation activity which Motorola, a nineteen eighties' top-ranking American company of electronics and communication, has started to survive the Japanese companies' frightening gami-gaze. In early days, it's just applicated for reducing customers' complain by eliminations inferior goods centering round producing activity and filling up the competitive market's demand for price and delivery deadline. Nowaday, 6 Sigma application has created a boom extending to a company's most activity fields without any exact scrutiny of 6 Sigma. Although 6 Sigma movement starts from the question of what customers want not by cutting off the inferiority rate but by cutting out causes of producing inferior goods, the service industry has a special character that makes it difficult to compute the level of Sigma. Thus this research examines suitability for introduction of 6 Sigma to the Service Industry.

The occurrence of traumatic hernia is rare. However, traumatic lumbar hernias are the most frequently occurring traumatic hernias. Superior lumbar hernias occur more frequently than inferior lumbar hernias, but for anatomical reasons, among traumatic lumbar hernias, inferior lumbar hernias occur more frequently than superior lumbar hernias. Repair of a lumbar hernia is very difficult. Mesh fixation to the bony part and general weakness of surrounding tissue make repair of a lumbar hernia tricky. A traumatic lumbar hernia was repaired using transfascial fixation around the defect to secure the mesh. This technique is another choice for a lumbar hernia technique.

Oculomotor nerve is the third cranial nerve, controlls four of the six extraocular muscles(superior rectus muscle, medial rectus muscle, inferior rectus muscle and inferior oblique muscle), levator palpebrae superioris muscle, cilliary muscle and muscle sphincter pupillae. In the oculomotor nerve palsy, limited oculogyration, diplopia, blepharoptosis, accomodation paralysis and mydriasis can be occured. We experienced an improved case of the oculomotor nerve palsy patient treated with oriental medicine for 25days. We used herbal medicine and acupuncture. Based on this experience, it is considered that oriental medicine can be applied to the treatment of the oculomotor nerve palsy.

Two patients with chronic Budd-Chiari syndrome resulting from segmental obstruction of the inferior vena cava underwent operation. There were 1 man and 1 woman. The obstructed segment was directly visualized by a transthoracic, transdiaphragmatic, retroperitoneal approach. In these two cases, severe segmental obstruction of the inferior vena cava was observed just above the right hepatic vein. These patients underwent successful retrohepatic cavoatrial bypass with a polytetrafloroethylene [PTFE] graft [ 16mm plain and 16mm ringed graft ]. There were no operative mortality and postoperative complication. These patients have been followed up for 6months and 36months without evidence of re-obstruction. When there is a severe stricture of the IVC with hepatic veins draining freely into the obstructed segment of the IVC, a dorsal cavoatrial bypass with a PTFE graft, preferably ringed, is the method of choice.

Membranous obstruction of the inferior vena cava[IVC] is a rare congenital anomaly that may present clinical features of Budd-Chiari syndrome caused by chronic obstruction of hepatic drain. We have experienced a case of IVC obstruction caused by hour-glass constriction and membrane in its center. Operative correction was accomplished using profound hypothermia [20%] and total circulatory arrest of 26 minutes. This technique permitted resection of membrane with direct vision and removal of thrombus of IVC and hepatic vein. After then constricted IVC was repaired with autologous pericardial patch. Total circulatory arrest was used intermittently for good visual field. Postoperative course was smooth and postoperative angiography showed unobstructed flow through the IVC in spite of slight constriction of cavoatrial junction and nearly complete disappearance of collateral vessels.

The authors report a case of unsuspected myocardial ischemia detected during CoDe FDG PET (coincidence detection fluorodeoxyglucose positron emission tomogram) which was performed for the evaluation of a solitary pulmonary nodule. Camera-based FDG PET without attenuation correction often reveals false defect in the inferior wall of the left ventricle in normals due to excessive attenuation. However, this asymptomatic patient had increased uptake in the inferior wall suggesting ischemic myocardium. The scan finding was confirmed by Tl-201 myocardial SPECT and coronary angiogram. The patient then underwent successful PTCA of mild RCA and right ventricular branch followed by right upper lobectomy for small cell lung cancer.
