12세 여아에게서 활동성 폐결핵에 동반된 장결핵 1례 (A Case of Tuberculous Enteritis with Active Pulmonary Tuberculosis in a 12-Year-Old Girl)

  • 결핵은 다양한 임상양상을 보이는 감염병으로 과거에 비해 많이 감소하였고 소아 결핵도 점차 줄어들고 있으나 여전히 중요한 감염병이다. 소아의 결핵은 성인과는 다른 임상적 특징을 가지며 폐외 결핵의 빈도가 성인보다 높고, 청소년의 결핵은 결핵균 감염에서 실제 질병으로 발전될 위험이 크다. 폐외 결핵 중 장결핵은 증상이 비특이적이고 다양하여 진단이 늦어질 경우 사망률 및 이환율이 높아질 수 있다. 크론병 또는 만성적인 소화기 질환과의 감별이 중요하며, 최근 대장 내시경의 발달로 감별 진단이 용이해지고 있다. 저자들은 6개월간 지속된 설사와 복통, 체중 감소와 간헐적이고 반복적인 $37.5-38^{\circ}C$의 발열로 내원한 12세 여아에게서 대장 내시경 검사를 하여 시행한 생검 조직의 PCR 검사와 조직학적 소견으로 장결핵을 진단하였고, 연이어 촬영한 흉부 방사선 상 활동성 폐결핵으로 진단되어 11개월간의 항결핵제 치료로 완치된 증례를 경험하였다.

Enhanced Anti-inflammatory Effects of γ-irradiated Pig Placenta Extracts

  • Porcine placenta extract (PPE) is known to possess anti-inflammatory properties owing to its high concentration of bioactive substances. However, the need to eliminate blood-borne infectious agents while maintaining biological efficacy raises concerns about the optimal method for sterilizing PPE. Therefore, the objective of this study was to compare the effects of the standard pressurized heat (autoclaving) method of sterilization with γ-irradiation on the anti-inflammatory effects of PPE. The anti-inflammatory actions of these two preparations of PPE were evaluated by measuring their inhibitory effects on the production of NO, the expression of iNOS protein, and the expression of iNOS, COX2, TNF-α, IL-1β, and IL-6 mRNA in lipopolysaccharide-stimulated RAW 264.7 cells. Compared with autoclaved PPE, γ-irradiated PPE showed significantly greater inhibition of NO production and iNOS protein expression, and produced a greater reduction in the expression of iNOS, COX2, TNF-α, IL-1β, and IL-6 mRNA. These results provide evidence that the sterilization process is crucial in determining the biological activity of PPE, especially its anti-inflammatory activity. Collectively, our data suggest that γ-irradiated PPE acts at the transcriptional level to effectively and potently suppresses the production of NO and the expression of pro-inflammatory cytokines.

Bactericidal and wound disinfection efficacy of nanostructured titania

  • Infections are caused due to the infiltration of tissue or organ space by infectious bacterial agents, among which Staphylococcus aureus bacteria are clinically most relevant. While current treatment modalities are in general quite effective, several bacterial strains exhibit high resistance to them, leading to complications and additional surgeries, thereby increasing the patient morbidity rates. Titanium dioxide is a celebrated photoactive material and has been utilized extensively in antibacterial functions, making it a leading infection mitigating agent. In view of the property amelioration in materials via nanofication, free-standing titania nanofibers (pure and nominally doped) and nanocoatings (on Ti and Ti6Al4V implants) were fabricated and evaluated to assess their efficacy to mitigate the viability and growth of S. aureus upon brief (30 s) activation by a portable hand-held infrared laser. In order to gauge the effect of exposure and its correlation with the antibacterial activities, both isolated (only titania substrate) and simultaneous (substrate submerged in the bacterial suspension) activations were performed. The bactericidal efficacy of the IR-activated $TiO_2$ nanocoatings was also tested against E. coli biofilms. Toxicity study was conducted to assess any potential harm to the tissue cells in the presence of photoactivated materials. These investigations showed that the photoactivated titania nanofibers caused greater than 97% bacterial necrosis of S. aureus. In the case of titania-coated Ti-implant surrogates, the bactericidal efficacy exceeded 90% in the case of pre-activation and was 100% in the case of simultaneous-activation. In addition to their high bactericidal efficacy against S. aureus, the benignity of titania nanofibers and nanocoatings towards tissue cells during in-vivo exposure was also demonstrated, making them safe for use in implant devices.

No Detection of Xenotropic Murine Leukemia Virus-Related Viruses in Prostate Cancer in Sanandaj, West of Iran

  • Background: Multiple etiologies have been hypothesized for prostate cancer, including genetic defects and infectious agents. A recently reported gamaretrovirus, xenotropic murine leukemia virus-related virus (XMRV) has been reported to be detected in prostate cancer. However, this virus has not been detected in similar groups of patients in other studies. Herein, we sought to detect XMRV in prostate cancers and benign controls in Sanandaj, west of Iran. Materials and Methods: In a case-control study, genomic DNA was extracted from formalin fixed and paraffin embedded prostate tissues from a total of 163 Iranian patients. We developed a conventional and a nested PCR assay using primers targeting to an env specific sequence of XMRV. PCR assays were carried out on 63 prostate cancers and 100 benign prostate hyperplasias. Results: Beta-actin sequences were successfully detected in the DNA extracts from all prostate tissues, confirming DNA extraction integrity. We did not detect XMRV in samples either from prostate cancers or benign prostate hyperplasias using XMRV specific primers. Conclusions: We conclude that in our population XMRV does not play a role in genesis of prostate cancer.

교차오염방지를 위한 공기정화제품에서 미생물 오염도 분석 (Monitoring of Microbial Contamination in Air Purifier for Preventing Cross-contamination)

  • 실내 공기질 관리의 중요성이 대두되면서 쾌적한 실내 환경에 도움을 주는 공기청정 기능과 습도 조절 기능을 동시에 갖춘 제습기와 공기청정기가 각광받고 있다. 하지만 오랜 기간 동안 공기정화제품을 사용하게 될 시에는 필터가 오염되어 본연의 기능을 상실하게 되는 것으로 알려져 있지만 이에 대한 구체적인 연구나 보고는 드문 편이다. 이에 본 연구에서는 가정과 사무실 등에서 사용한 공기정화제품을 수거하여 주요 부위별 미생물 오염도 및 주요 오염 미생물들을 분석하였다. 그 결과, 4 종류의 공기정화제품에서 오염도가 높은 부위는 필터부위, 물이 직접 닿는 부위 및 공기가 외부로부터 직접적으로 들어오는 입구부위 등에서 미생물학적 오염도가 가장 높았다. 하지만 공기정화제품은 사용하는 환경과 조건에 따라서 미생물학적 오염도 및 오염 미생물의 성상은 각각 다르게 나타났다. 하지만 이들 공기정화제품들에는 공통적으로 Staphylococcus sp., Micrococcus sp. 그리고 Bacillus sp.의 세균과 Cladosporium sp. 및 Penicillium sp.의 진균이 공통적으로 오염되어 있는 우점종인 것으로 분석되었다.

Association of Viral Infections with Risk of Human Lymphomas, Egypt

  • Background: The aim of this study was to determine and evaluate the association of different viral infections, with hepatitis B and C viruses, Epstein-Barr virus, cytomegalovirus and human herpes virus-8 (HBV, HCV, EBV, CMV, HHV-8) with the risk of lymphomas (Hodgkin and non-Hodgkin) among Egyptian patients, and correlate with the histopathological staging and typing as well as the prevalence of combined infections. Materials and Methods: A total of 100 newly diagnosed lymphoma patients with 100 healthy age and sex matched normal controls were assayed for viral infection using enzyme linked immunosorbant assay (ELISA) followed by real time polymerase chain reaction (RT-PCR). Results: Our results showed a high statistical significant difference between cases and controls as regards clinical and laboratory findings (P<0.001 and=0.003). A high statistical difference was seen for the association of most viruses and lymphoma cases (p<0.001) except for positive HBs Ag, positive CMV IgG and HHV-8 (p=0.37, 0.70 and 1.0 respectively). No statistical significant difference was found between Hodgkin (HL) and non-Hodgkin (NHL) as regards viral prevalence except HCV antigen, 57.1% for HL and 26.5% for NHL (p = 0.03). Only, HBV DNA showed a high significant value among infiltrated bone marrow cases (p=0.003) and finally, a high significant association of 2 combined viral infections with infiltrated bone marrow lymphoma cases (p=0.04). Conclusions: Our results showed that infection with HBV, HCV, CMV and EBV were associated with increased risk of lymphoma among the Egyptian population. Detection of new associations between infectious agents and risk of cancer development will facilitate progress in elaboration of prophylactic measures, early diagnostic methods and, hopefully, novel therapy of malignant tumours.

High-throughput Gene Expression Analysis to Investigate Host-pathogen Interaction in Avian Coccidiosis

  • Poultry products including meat and eggs constitute a major protein source in the American diet and disease-causing pathogens represent major challenges to the poultry industry. More than 95% of pathogens enter the host through the mucosal surfaces of the respiratory, digestive and reproductive tracts and over the past few decades, the two main mechanisms used to control diseases have been the use of vaccines and antibiotics. However, in the poultry industry, there are mounting concerns over the ability of current vaccines to adequately protect against emerging hyper-virulent strains of pathogens and a lack of suitable, cost effective adjuvants. Thorough investigation of the immunogenetic responses involved in host-pathogen interactions will lead to the development of new and effective strategies for improving poultry health, food safety and the economic viability of the US poultry industry. In this paper, I describe the development of immunogenomic and proteomic tools to fundamentally determine and characterize the immunological mechanisms of the avian host to economically significant mucosal pathogens such as Eimeria. Recent completion of poultry genome sequencing and the development of several tissue-specific cDNA libraries in chickens are facilitating the rapid application of functional immunogenomics in the poultry disease research. Furthermore, research involving functional genomics, immunology and bioinformatics is providing novel insights into the processes of disease and immunity to microbial pathogens at mucosal surfaces. In this presentation, a new strategy of global gene expression using avian macrophage (AMM) to characterize the multiple pathways related to the variable immune responses of the host to Eimeria is described. This functional immunogenomics approach will increase current understanding of how mucosal immunity to infectious agents operates, and how it may be enhanced to enable the rational development of new and effective strategies against coccidiosis and other mucosal pathogens.

입효산(立效散)의 Methicillin-Resistant Staphylococcus aureus에 대한 항균활성에 관한 연구 (Antibacterial Effect of Ipyo-san against Methicillin-Resistant Staphylococcus aureus)

  • Objectives : Methicillin-resistant Staphylococcus aureus (MRSA) has a cephalosporin and beta-lactam antibiotic-resistant strains. MRSA is one of the major pathogens causing hospital infection and the isolation ratio of MRSA has gradually increased. Consequently, increased resistance to antibiotics is causing serious problems in the world. Therefore, there is a need to develop alternative antimicrobial drugs for the treatment of infectious diseases. Methods : The antibacterial activities of Ipyo-san were evaluated against 2 strains of MRSA and 1 standard Methicillin-susceptible staphylococcus aureus (MSSA) strain by using the disc diffusion method, minimal inhibitory concentrations (MIC) assay, colorimetric assay using MTT test, checkerboard dilution test and time-kill assay performed under dark. Results : The MIC of Ipyo-san water extract against S. aureus strains ranged from 1000 to $2,000{\mu}g/ml$, so we confirmed that it had a strong antibacterial effect. Also, the combinations of Ipyo-san water extract and conventional antibiotics exhibited improved inhibition of MRSA with synergy effect. We suggest that Ipyo-san water extract against MRSA has antibacterial activity so it has potential as alternatives to antibiotic agents. For the combination test, we used Triton X-100 (TX) and DCCD for measurement of membrane permeability and inhibitor of ATPase. As a result, antimicrobial activity of Ipyo-san water extract was affected by the cell membrane. Conclusions : We suggest that the Ipyo-san water extract lead the treatment of bacterial infection to solve the resistance and remaining side-effect problems that are the major weak points of traditional antibiotics.

Evaluation of Anti-Toxoplasma IgG, IgM, and IgA in Mothers with Spontaneous Abortion in Zanjan, Northwest Iran

  • Toxoplasma gondii is one of the major agents of infectious abortions and due to its worldwide distribution can threat healthy pregnant women who had no previous exposure to this parasite. The present study was designed to investigate the contribution of T. gondii to spontaneous abortions in Zanjan, Northwest of Iran, using ELISA method. Blood Samples were collected from 264 mothers referred to the provincial hospitals of Zanjan due to spontaneous abortion. The sera were isolated and subjected to evaluate the anti-Toxoplasma IgG, IgM and IgA antibodies. The results showed IgG positive ($IgG^+$) in 99 cases (37.5%). A total of 68 women (25.8%) showed seroconversion with IgM or IgA or both IgM and IgA. They included: $IgM^+$ in 21 (8.0%), $IgA^+$ in 23 (8.7%) and both $IgM^+$ and $IgA^+$ in 24 (9.1%) subjects. In 23 cases, positive titers of IgM and IgG were accompanied. In general, the analysis of anti-Toxoplasma antibody patterns, showed that about 17% of the spontaneous abortions were associated with serological patterns of acute infection. According to these findings, a considerable proportion of spontaneous abortions can be attributed to T. gondii in the study area.

제주지역 도축돈의 간질성 신염 (Multifocal interstitial nephritis of pigs slaughtered in Jeju)

  • Total 160 head of porcine kidneys were examined for gross and histopathological lesions and polymerase chain reaction (PCR) for porcine circovirus type 2 (PCV-2), porcine parvovirus (PPV), Leptospira species and porcine reproductive and respiratory syndrome virus (PRRSV). Grossly, 137 kidneys (85.6%) had lesions characterized by the presence of the scattered white foci. Microscopically, multifocal interstitial nephritis, which classified into 4 grades such as, no lesion (Score 0), mild lesion (Score 1), moderate lesion (Score 2) and severe chronic lesion (Score 3) with fibrosis, was observed in 159 cases (99.4%). The histopathologic mean score for multifocal interstitial nephritis was significantly different (P<0.05) between the cases of PCV-2 single infection and the cases of co-infection with PCV-2 and PPV. According to PCR evaluation, PCV-2 were detected in 73.8% (118 cases), PPV were in 66.9% (107 cases), however Leptospira spp. and PRRSV were negative in all kidneys. Both PCV-2 and PPV were detected in 52.5% (84 cases). In 84 cases co-infected with PCV-2 and PPV, the occurrence of lymphoid follicle and vasculitis were observed as 65.5% (55 cases) and 26.2% (22 cases), respectively. These results revealed that PCV-2 and PPV were major infectious agents for interstitial nephritis in slaughtered pigs, Jeju. And the histopathologic lesions of multifocal interstitial nephritis were more severe in the case co-infected with PCV-2 and PPV.
