Impact of Diet in Shaping Gut Microbiota Revealed by a Comparative Study in Infants During the First Six Months of Life

  • The development of the gut is controlled and modulated by different interacting mechanisms, such as genetic endowment, intrinsic biological regulatory functions, environment influences and last but no least, the diet influence. In this work, we compared the fecal microbiota of breast-fed (BF), formula-fed (FF), and mixed-fed (MF) infants from Hebei Province, China. By using high-throughput 16S rDNA sequencing analyses, we found some differences in gut microbiota in the three groups. Firmicutes and Proteobacteria were the dominant bacteria at the phylum level in the three groups, where FF infants showed a significant depletion in Bacteroidetes (p < 0.001) and Actinobacteria (p < 0.05). Enterobacteriaceae was the dominant bacteria at the family level in the three groups, but FF infants showed higher Enterobacteriaceae enrichment than BF and MF infants (p < 0.05); the abundance of the Bifidobacteriaceae was only 8.16% in the feces of BF infants, but higher than in MF and FF infants (p < 0.05). The number of genera detected (abundance >0.01%) in BF, MF, and FF infants was only 15, 16, and 13, respectively. This study could provide more accurate and scientific data for the future study of infant intestinal flora.

Ten years of experience in the prevention of mother-to-child human immunodeficiency virus transmission in a university teaching hospital

  • Purpose: Administration of antiretroviral drugs to mothers and infants significantly decreases mother-to-child human immunodeficiency virus (HIV) transmission; cesarean sections and discouraging breastfeeding further decreases this risk. The present study confirmed the HIV status of babies born to mothers infected with HIV and describes the characteristics of babies and mothers who received preventive treatment. Methods: This study retrospectively analyzed medical records of nine infants and their mothers positive for HIV who gave birth at Korea University Ansan Hospital, between June 1, 2003, and May 31, 2013. Maternal parameters, including HIV diagnosis date, CD4+ count, and HIV ribonucleic acid (RNA) copy number, were analyzed. Infant growth and development, HIV RNA copy number, and HIV antigen/antibody test results were analyzed. Results: Eight HIV-positive mothers delivered nine babies; all the infants received antiretroviral therapy. Three (37.5%) and five mothers (62.5%) were administered single- and multidrug therapy, respectively. Intravenous zidovudine was administered to four infants (50%) at birth. Breastfeeding was discouraged for all the infants. All the infants were negative for HIV, although two were lost to follow-up. Third trimester maternal viral copy numbers were less than 1,000 copies/mL with a median CD4+ count of $325{\mu}L$ ($92-729{\mu}L$). Among the nine infants, two were preterm (22.2%) and three had low birth weights (33.3%). Conclusion: This study concludes that prophylactic antiretroviral therapy, scheduled cesarean section, and prohibition of breastfeeding considerably decrease mother-to-child HIV transmission. Because the number of infants infected via mother-to-child transmission may be increasing, studies in additional regions using more variables are necessary.

Parental concerns about their premature infants' health after discharge from the neonatal intensive care unit: a questionnaire survey for anticipated guidance in a neonatal follow-up clinic

  • Purpose: The aim of this study was to develop an appropriate nursing information guideline according to corrected age, after investigating parents' concerns about the growth, development, and diseases of their premature infants after discharge from the neonatal intensive care unit (NICU). Methods: The parents of premature infants (birth weight, <2,500 g; gestational age, <37 weeks) who went to a neonatal follow-up clinic after NICU discharge at Seoul St. Mary's Hospital from January 2005 to December 2009, were asked with regard to their concerns about their infants through a questionnaire survey. The results of physical examinations, including body measurements and neurodevelopmental status at 4, 8, 12, and 18 months of corrected age, were retrospectively reviewed in 390 infants. Results: The most common parental concerns were developmental delay, poor growth, and feeding and nutritional problems. Parental concerns about developmental delay, growth failure in improvement in body weight and length, and overweightness were high in specificity but very low in sensitivity. After NICU discharge, 30% of premature infants experienced infectious diseases before 18 months of corrected age, the most common of which was respiratory tract infection. Conclusion: For guiding of premature infants in outpatient day clinics after NICU discharge, it is necessary to identify the parents' highest concerns, to educate them about the possibilities of growth and neurodevelopmental disabilities in their infants and to provide them with handouts containing guidelines on the management of infectious diseases, especially respiratory infections.

영아의 기질에 따른 영아-교사의 어린이집 일과 내 상호작용 (Interactions between Infants of Different Temperament and Teachers at a Nursery)

  • 본 연구의 목적은 어린이집 현장에서 기질이 다른 영아들이 교사와 어떤 형태의 상호작용을 하는지 알아보고, 그 차이가 시사하는 점에 대해 생각해보며 바람직한 양육 및 교육의 의미를 밝혀보고자 하는데 있다. 본 연구는 2011년 6월부터 8월까지 13주에 걸쳐 진행되었으며 만 1, 2세 학급의 영아 8명과 담임교사 2인을 대상으로 진행되었다. 자료 수집은 참여관찰과 교사와의 면담, 동영상촬영, 음성녹음 및 각종 문서자료 등을 통해 이루어졌고 관찰 기간동안 수집된 자료를 토대로 기질별 상호작용 유형을 범주화 하고 각 일과시간별로 어떤 상호작용이 이루어졌는지 그 특성과 차이점을 살펴 보았다. 본 연구의 주요 결과는 다음과 같다. 첫째, 기질이 다른 영아들은 각각의 기질에 따라 같은 상황에서도 서로 다른 반응을 보이며 이는 영아-교사의 상호작용에 영향을 미친다. 둘째, 기질이 다른 영아들의 일과시간별 상호작용 유형은 기질에 따라 분명한 차이를 보이며 이때의 상호작용은 기질 이외의 다른 변인에 의해 영향을 받기도 한다. 따라서 교사는 영아들에게 적절한 지원과 환경을 제공하여 기질의 긍정적인 영향을 높이고 부정적인 영향은 최소화시켜 건강한 영아발달과 원만한 또래관계 형성을 위해 노력해야 할 것이다.

유헬스케어 패치형 무선 체온 모니터링 시스템 구현 (U-Healthcare Patch Type Wireless Body Temperature Monitoring System)

  • Recently, there have been many research on fever management using u-healthcare technology. Especially, fever of infants requires continuous monitoring of body temperature by parents. For infants between 4 weeks and under 5 years old, it is recommended to use an electronic thermometer or chemical thermometer in the axilla, or to use an infrared thermometer. However, in order to overcome the reality of not being able to waste significant time on continuous monitoring, there have been demands of patch type thermometers with the internet of things (IoT) and wireless communication technologies. Existing IoT thermometers are difficult to attach to infants' body because they do not take into account its size, and their interoperability is not guaranteed because they do not comply with standards in communication. Therefore, in this study, a patch-type thermometer with a diameter of 20 mm and a weight of 2.9 g was developed to manage the fever of infants, while it communicates wirelessly with Bluetooth Low Energy (BLE) communication protocol and complies with IEEE 11073 PHD(Personal Health Device) at the same time. We verified its performance under the requirements of thermometers regulated by the Korean Ministry of Food and Drug Safety.

어머니-영아간의 상호작용방식이 영아발달에 미치는 영향 (Mother-Infant Interaction Styles Associated with Infant Development)

  • The subjects of this study were 31 mothers and their first-born infants from middle class families living in Seoul. Mother-infant interactions were filmed at 5 and 13 months of age during naturalistic play situations at home. Questionnaire data were also collected. Results revealed that both maternal didactic and social interactions decreased over the 5 to 13 month time period but (only for infants) object-oriented interaction increased over time. Infant object-oriented interaction at 13 months was predicted by cumulative effects of both mother's social stimulation at 5 months and infant social interaction at 13 months. Infant's social interaction at 13 months was predicted by infant's object-oriented interaction at 13 months. Infant language development was predicted by mother's didactic stimulation.

영아의 기질과 발달수준 및 어머니의 양육 특성 변인이 양육효능감에 미치는 영향 (Effects of Infant Temperament, Development, and Maternal Parenting Variables on Parenting Efficacy)

  • The purpose of this study was to investigate which variables predicted parenting efficacy. The variables of interest were demographic variables regarding both the infants and mothers, infant temperament and development, maternal parenting knowledge, parenting belief, and parenting stress. The subjects consisted of 260 infants and mothers. Data on infant's temperament, parenting knowledge, parenting belief, parenting stress and parenting efficacy were gathered through maternal self-reported questionnaires. Furthermore, infant development was assessed by classroom teacher. Data were analyzed by descriptive statistics, correlation and regression analyses. Our results indicated that infant's sociability and activity, parenting knowledge about emotional development, parenting beliefs emphasizing the role of nature in infant development, low parenting stress all predicted parenting efficacy.

영아(18-24개월)의 언어발달능력 지표에 관한 조사 연구 (A Study on the Indicators of Language Development Ability in Infants 18-24 Months)

  • 본 연구는 언어가 가장 많이 발달하는 18-24개월 시기의 영아의 발달에 대해 세부적으로 파악하고, 빠른 시기에 적절한 중재 설계를 촉진 시킬 수 있는 발달 평가의 새로운 규준 마련에 목적이 있다. 따라서 우리는 18-24개월 영아의 언어발달 능력 지표를 알 수 있는 선별검사, 표준화된 검사도구 등의 하위 항목들을 구문/음운, 의미, 화용의 영역으로 나누어 각 문항들을 분류하고 5점 척도로 체크리스트를 만들어 18-24개월 영아를 둔 부모를 대상으로 설문조사를 실시하였다. 현재 우리나라에서 국가적으로 시행하는 영유아 건강검진에서 언어 지연이 있는 영아 선별에 낮은 민감도, 국내 검사도구들이 표준화 된지 최소 6년에서 많게는 10년 이상으로 또래집단과 비교하였을 때 발달수준의 격차가 크다는 부모 또는 교사의 보고가 많은 실정이다. 연구 결과에서도 수용 및 표현 관련 설문 문항에서 '항상 가능하다.'와 '하기 시작했다'의 비율이 50~60% 이상이었다. 따라서 본 연구의 결과를 바탕으로 빠른 시기에 세부적으로 적절한 중재 설계를 촉진 시킬 수 있는 발달 평가의 새로운 규준이 마련되어 발달 및 언어 지연이 있는 영아의 선별에 활용되기를 바란다.

Cerebral Hemodynamics in Premature Infants

  • Extremely low birth weight infants remain at increased risk of intraventricular hemorrhage from the fragile vascular bed of the germinal matrix; the roles of hypotension (ischemia) and reperfusion (hyperemia) in the development of intraventricular hemorrhage are still debated. Cerebrovascular pressure autoregulation protects the brain by maintaining constant cerebral blood flow despite changes in blood pressure. The ontogeny of cerebrovascular pressure autoregulation has not been well established and uncertainty remains about the optimal arterial blood pressure required to support brain perfusion. Another important aspect of premature cerebral hemodynamics is the critical closing pressure--the arterial blood pressure at which cerebral blood flow ceases. Interestingly, in premature infants, the critical closing pressure approximates the mean arterial blood pressure. Often in this unique population, cerebral blood flow occurs only during systole when the diastolic arterial blood pressure is equal to the critical closing pressure. Moreover, the diastolic closing margin, a metric of cerebral perfusion that normalizes diastolic arterial blood pressure to the critical closing pressure, may be a better measure than arterial blood pressure for defining cerebral perfusion in premature infants. Elevated diastolic closing margin has been associated with intraventricular hemorrhage. This review summarizes the current state of understanding of cerebral hemodynamics in premature infants.

이유식의 조리법과 월령별 이유 급식 계획 (Recipes for the supplementary foods and monthly feeding plans for infants)

  • This study was carried out to develop supplementary foods for infants and monthly feeding plans of five steps. The developed supplementary foods were divided into 3 stages. First stage was designed for the babies just beginning eat pureed vegetables and fruits. Second stage was combined the nutritio-nal attributes of both vegetables and meat. Third stage was featured bite-sized pieces of tender meat and vegetables that appealed to the most mature tastes of babies. Feeding plans were divided into 5 steps according to their development. At 3 months, small amount of supplementary of smooth porri-dge can be introduced after a milk feeding each day. At 5 months, the quantity of supplementary foods can be gradually increased, and it can be given after 2 or 3 times of milk feedings. By being given different foods, infants will learn to become accustomed to new flavors at 7 months. At 9 months, the same types of additional foods are given at least 3 times a day after the milk feeds. Infants will be able to take most of the foods at 12 months, and it is important to give the infants solid foods after milk feedings at least 3 times a day.
