The Effects of Institutional and Market Factors on Nurse Staffing in Acute Care Hospitals (의료기관과 시장특성이 간호사 확보수준에 미치는 영향)

  • Nurse staffing level is an important factor that influences the quality of health service and patient outcomes. This study was carried out to examine the current state of acute hospital nurse staffing and find out factors that affect the nurse staffing level. Nurse staffing of individual hospitals was measured using the number of registered nurses per 100 beds. Descriptive and multiple regression analyses were conducted using 592 acute care hospitals' data. Regression model included structure factors such as referral level, ownership, medical and general staffing, and financial outcome factors such as occupancy rate, inpatient and outpatient revenues. Market characteristics included strength of competition, supply of nurses, and income and health status level of consumers. The average number of nurses per 100 beds was 28 and showed a great variation according to the referral level. Regression model explained this variation as much as 76.87%. Hospital structure variables which affecting the hospital nurse staffing level positively were ICU bed ratio, the staffing level of specialist, training doctor and employees except doctor and nursing personnel, while the negative factor was nurse aid staffing level. General hospitals employed more nurses than hospitals. Among outcome characteristics, occupancy rate and the amount of health insurance inpatient revenue affected positively on the hospital nurse staffing level. The more supply of the new nurse and the higher consumer income and health status in the medical service markets, the more nurses were employed by the medical institutes. According to the study result, hospitals employed more nurses when they had more financial incentive by increasing nurses. This means appropriate hospital incentive policy and regulation policy, which hospital violate nurse staffing level have to pay penality, should be needed. Clarifying job description between nurses and nurse aids and the reentry program for unemployed experienced nurses will be helpful to increase nurse staffing level.

A Study on the Characteristics of Enterprise R&D Capabilities Using Data Mining (데이터마이닝을 활용한 기업 R&D역량 특성에 관한 탐색 연구)

  • As the global business environment changes, uncertainties in technology development and market needs increase, and competition among companies intensifies, interests and demands for R&D activities of individual companies are increasing. In order to cope with these environmental changes, R&D companies are strengthening R&D investment as one of the means to enhance the qualitative competitiveness of R&D while paying more attention to facility investment. As a result, facilities or R&D investment elements are inevitably a burden for R&D companies to bear future uncertainties. It is true that the management strategy of increasing investment in R&D as a means of enhancing R&D capability is highly uncertain in terms of corporate performance. In this study, the structural factors that influence the R&D capabilities of companies are explored in terms of technology management capabilities, R&D capabilities, and corporate classification attributes by utilizing data mining techniques, and the characteristics these individual factors present according to the level of R&D capabilities are analyzed. This study also showed cluster analysis and experimental results based on evidence data for all domestic R&D companies, and is expected to provide important implications for corporate management strategies to enhance R&D capabilities of individual companies. For each of the three viewpoints, detailed evaluation indexes were composed of 7, 2, and 4, respectively, to quantitatively measure individual levels in the corresponding area. In the case of technology management capability and R&D capability, the sub-item evaluation indexes that are being used by current domestic technology evaluation agencies were referenced, and the final detailed evaluation index was newly constructed in consideration of whether data could be obtained quantitatively. In the case of corporate classification attributes, the most basic corporate classification profile information is considered. In particular, in order to grasp the homogeneity of the R&D competency level, a comprehensive score for each company was given using detailed evaluation indicators of technology management capability and R&D capability, and the competency level was classified into five grades and compared with the cluster analysis results. In order to give the meaning according to the comparative evaluation between the analyzed cluster and the competency level grade, the clusters with high and low trends in R&D competency level were searched for each cluster. Afterwards, characteristics according to detailed evaluation indicators were analyzed in the cluster. Through this method of conducting research, two groups with high R&D competency and one with low level of R&D competency were analyzed, and the remaining two clusters were similar with almost high incidence. As a result, in this study, individual characteristics according to detailed evaluation indexes were analyzed for two clusters with high competency level and one cluster with low competency level. The implications of the results of this study are that the faster the replacement cycle of professional managers who can effectively respond to changes in technology and market demand, the more likely they will contribute to enhancing R&D capabilities. In the case of a private company, it is necessary to increase the intensity of input of R&D capabilities by enhancing the sense of belonging of R&D personnel to the company through conversion to a corporate company, and to provide the accuracy of responsibility and authority through the organization of the team unit. Since the number of technical commercialization achievements and technology certifications are occurring both in the case of contributing to capacity improvement and in case of not, it was confirmed that there is a limit in reviewing it as an important factor for enhancing R&D capacity from the perspective of management. Lastly, the experience of utility model filing was identified as a factor that has an important influence on R&D capability, and it was confirmed the need to provide motivation to encourage utility model filings in order to enhance R&D capability. As such, the results of this study are expected to provide important implications for corporate management strategies to enhance individual companies' R&D capabilities.

Prediction Model for Reduced Bone mass in Women using Individual Characteristics & Life Style Factors (여성의 개인적 특성과 생활양식요인을 이용한 골량감소 예측모형)

  • This study was carried out to identify the Important modifiable risk factors for reduced bone mass and to construct prediction model which can classify women with either low or high bone mass. Through the literature review, individual characteristics such as age, body weight, height, education level, family history, age of menarche, postmenopausal period, gravity, parity, menopausal status, and breast feeding period were identified and factors of life style such as past milk consumption, past physical activity, present daily activity, present calcium intake, alcohol intake, cigarette smoking, coffee consumption were identified as influencing factors of reduced bone mass in women. Four hundred and eighty women aged between 28 and 76 who had given measurement bone mineral density by dual energy x-ray absortiometry in lumbar vertebrae and femur from July to October, 1997 at 4 general hospitals in Seoul and Pusan were selected for this study. Women were excluded if they had a history of any chronic illness such as rheumatoid arthritis, diabetes mellitus, hyperthroidism, & gastrointestinal disorder and any medication such as calcium supplements, calcitonin, estrogen, thyroxine, antacids, & corticosteroids known affect bone. As a result of these exclusion criteria, four hundred and seventeen women were used for analysis. Multiple logistic regression model was developed for estimating the likelihood of the presence or absence of reduced bone mass. A SAS procedure was used to estimate risk factor coefficient. The results are as follows For lumbar spine, the variables significant were age, body weight, menopause status, daily activity, past milk consumption, and past physical activity(p<0.01), while for femoral Ward's triangle, age, body weight, level of education, past milk consumption, past physical activity(p<0.001). Past physical activity, present daily activity and past milk consumption are the most powerful modifiable predictors in vertebrae and femur among the predictors. When the model performance was evaluated by comparing the observed outcome with predicted outcome, the model correctly identified 74.1% of persons with reduced bone mass and 84.5% of persons with normal bone mass in the lumbar vertebrae and 82.9% of persons with reduced bone mass and 75.0% of persons with normal bone mass in the femoral Ward's triangle. On the basis of these results, a number of recommendations for the management of reduced bone mass may be made : First, those woman who are classified as high risk group of the reduced bone mass in the prediction model should examine the bone mineral density to further examine the usefulness of this model. Second, the optimal amount of milk consumption and a regular weight bearing exercise in childhood, adolescence, and early adult should be ensured.

The Effect of the Passion of Preliminary Entrepreneurs on Entrepreneurial Intention (예비창업가의 열정이 창업의도에 미치는 영향)

  • As part of the recent economic revitalization measures, the national level of encouragement for young people's entrepreneurship continues, and investment in young people's entrepreneurship and support from young people are increasing. Entrepreneurship intention is a direct factor leading to entrepreneurship, and many studies have studied the individual characteristics of entrepreneurs that influence entrepreneurial intention. In the same context, the purpose of this study was to investigate the entrepreneur's passion as a factor affecting entrepreneurial intention, divided into harmony passion and obsessive passion. In addition, the pass to need for achievement, emotion, and subjective norms, which are the individual characteristics of entrepreneurs that influence passion, were verified. To this end, an online survey was conducted for prospective entrepreneurs in universities, and data of 203 persons were collected and structural equations were analyzed. As a result of the analysis, it was found that the need for achievement and positive emotions of preliminary entrepreneurs had a positive (+) effect on the passion for harmony, while the negative emotions and subjective norms were not significant for the passion for harmony. The need for achievement had a negative (-) effect on the obsessive passion, and the positive and negative emotions had a positive (+) effect on the obsessive passion, but the subjective norm was not significant on the obsessive passion. It was confirmed that harmony passion and obsessive passion showed positive (+) effect on entrepreneurial intention. It also confirmed whether harmonious passion and obsessive passion have a mediating effect between need for achievement, positive emotion, negative emotion, and entrepreneurial intention. As a result, it has a mediating effect between need for achievement, positive emotion and entrepreneurial intention, especially need for achievement has a complete mediating effect. Obsessive passion was also confirmed to have a mediating effect in the relationship between positive emotions and intention to start a business.

A Study on the Factors Affecting the Low Back Pain of Workers in Hospital (병원 근무자의 요통 발생 요인에 관한 연구)

  • Background: The purpose of this research was to investigate the factors affecting the low back pain of workers in hospital. 214 subjects waking at two general hospitals in Yosu city participated in this survey. Subjects consisted of doctors, nurses, medical engineers, officers and general laborers. The survey data were collected by a written questionnaire which made out by themselves for 25 days, from fourth August to 29th August, 2008. Methods: The questionnaire consisted of four categories, general, occupational, working habitual and the daily living characteristics. The collected data were analyzed by Chi-square test based on the present or absent of low back pain. Results: 1. In the general characteristics, low back pain had no significant relationship to all factors, sex, ago, body mass index, weight and height. 2. In the occupational characteristics, the phase of distribution of low back pain had statistical significant differences in the working hours a week, satisfaction of pay, satisfaction of occupation(p<0.05). However low back pain did not significantly related to the kind of occupation, period of work and degree of stress. 3. In the habitual characteristics, low back pain was significantly influenced by working posture, frequency of using lumbar and heavy material lifting, monotonous repetition of working operation and noise(p<0.05). No significant difference was shown in the factor of convenience of chair. 4. In the daily living characteristics, low back pain shown the significant differences in walking time a day, status of health and smoking pattern(p<0.05). there were, however, no significant differences in the aspect of the kind of house and bed, sleeping attitude, driving, riding time on the vehicle, exercising, frequency of cultural life and drinking alcohol. Conclusion: when I see above resultants totally, it appears a higher incidence caused by working environment rather than living habit and then consequently compared to hospital workers, they also have high incidence like others. In order to reduce incidence of low back pain and enjoy the our life we need to educate ourselves preventing program for low back pain and try to effort for preventing of low back pain on each department and individual.

  • This paper aims to analyze the effects of the residential area and personal characteristics of households on the preference of residential location factors. The main results of this study are as follows. Firstly, considering the results of multiple regression analysis, it can be seen that preference of location factors is differentiated according to the residential area and personal characteristics of households by individual location factors. Secondly, considering the results of binomial logistic regression analysis, it can be also elucidated that the preferred location factors are differentiated according to the residential area and personal characteristics of the households. Therefore, it is judged that the residential area and personal characteristics of household and the location factors of the household are mutually influencing each other.

  • Purpose: In this study, we investigated effects of employees characteristics, expertise, customer satisfaction, devotion of employees to their company, business performance of the optician. Methods: For two months, from August to October 2011, employees participated in individual questionnaires. And the SPSS v14.0 was applied for statistical analysis. Results: This study has shown that the employees characteristics have an effect on the financial performance and satisfaction performance of opticians. In particular, the 'Customers satisfaction' a was important characteristics of employees affecting finance performance, and the 'Devotion to their company' was important factor that affects satisfaction performance. Conclusions: From the result found in this study, it is recommended that further efforts for inspiring the employees devotion to their company and for improving customer satisfaction are required. Above all, employer should pay attention on more suitable ways to encourage work of his employee with considering different situation or circumstance of each optician.

Classification of Elderly Men's Foot Side Type from 3D Scan Data (3차원 스캔 데이터에 의한 노년 남성의 발 측면유형 분류)

  • This study identifies the foot side shapes of elderly men by classifying foot types according to 3D foot shapes and analyzing individual characteristics. The subjects were 284 elderly men over 60 years of age who lived in Gwangju and did not have foot related diseases. They were measured with a scanner (Nexcan$^{(R)}$ of K&I Technology) to obtain three dimensional feet shapes. Anthropometric measuring items consisted of 28 items estimated on the right foot of each subject. 3D scan data were analyzed by various statistical methods such as factor analysis, ANOVA and cluster analysis using the statistical program SPSS 19.0. A total of 7 factors were extracted through a factor analysis and these factors represent 77.56% of total variance. The 8 factors were: inside height and side gradient, ankle thickness, size from foot center to ankle, lateral malleolus height, forefoot height, instep and heel height and gradient. A total of 3 clusters (as foot type) were categorized using 7 factor scores by cluster analysis. Type 1 was classified as high forefoot and low midfoot compared to the length. Type 2 was classified as low forefoot and high midfoot, and type 3 was classified as low forefoot and low midfoot.

The Relationship between Affinity of Membrane and Optimum Operation Conditions in the Pervaporation of Aqueous Ethanol (에탄올 수용액의 투과증발에 있어서 막의 친화성과 최적 조업조건의 관계)

  • The relationships between affinity of membranes and optimum operation conditions were investigated in the pervaporation of water(1)/ethanol(2) mixture through cellulose acetate(CA) membranes having more affinity to water and silicone rubber(SR) membranes having more affinity to ethanol. CA and SR membranes were prepared and amount of sorption, sorption selectivity, pervaporation separation factor and pervaporation rate in both of membranes were determined and compared. The effects of downstream pressure were analyzed using Thompson diagram and the sorption and pervaporation characteristics with composition of feed and operation temperature were examined in terms of affinity, activity coefficient, plasticizing effect and activation energy of individual species. In the separation of water through CA membranes, high performance of both pervaporation separation factor (water to ethanol, $[\alpha^2_1]_{PV}$) and pervaporation rate was obtained in the conditions of low downstream pressure, middle range of feed concentration and high temperature. In the separation of ethanol through SR membranes, pervaporation separation factor(ethanol to water, $[\alpha^2_1]_{PV}$) increased with downstream pressure and decreased with concentration of ethanol in feed and operation temperature, while pervaporation rate showed opposite trends to those of ($[\alpha^2_1]_{PV}$).

Controlled Release Behavior of Bioactive Molecules from Photo-Reactive Hyaluronic Acid-Alginate Scaffolds

  • There are three important components in tissue engineering: the cells, signaling factors (cytokines and growth factors), and scaffolds. To obtain finely engineered tissue, all three components should perform their individual functions and be fully integrated with each other. For the past few years, we have studied the characteristics of photodimerizable HA (CHA)/alginate (CA) composite materials. CHA/CA complex hydrogels, which were irradiated under UV light and, then treated with calcium ions, were found to have good biocompatibility, mechanical properties and water resistance for implantable tissue scaffolds. In this study, we introduced a cell growth factor (basic fibroblast growth factor; bFGF) into the CHA/CA scaffolds and studied its release behavior. We also introduced tetracycline hydrochloride and flurbiprofen into the same scaffolds as model activation factors and evaluated their release behaviors from the scaffolds. The drug release rate from the materials was influenced by various parameters, such as the degree of crosslinking, the cross linker type, the physico-chemical properties of the drug, and the amount of the drug in the polymer. The results indicated that the negatively charged CHA/CA composite materials showed sustained release behavior and that HA has a particularly strong negative charge, making it attractive toward tetracycline hydrochloride and bFGF, but repulsive toward flurbiprofen.
