  • Present study was undertaken to establish the modified Broca's indices to estimate standard body weight by using a total of 5,496 insured persons who were medically examined at the Honam Medical Room of Dong Bang Life Insurance Company Ltd. from January, 1983 to January, 1986. The results were as follows: 1. Serum 2,3,7,8-Tetrachlorodibenzo-p-dioxin Levels and Their Association With Age, Body Mass Index, Smoking, Military Record-based Variables, and Estimated Exposure to Agent Orange in Korean Vietnam Veterans

  • Objectives: The aim of this study was to examine the levels of serum 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) and evaluate their association with age, body mass index, smoking, military record-based variables, and estimated exposure to Agent Orange in Korean Vietnam veterans. Methods: Serum levels of TCDD were analyzed in 102 Vietnam veterans. Information on age, body mass index, and smoking status were obtained from a self-reported questionnaire. The perceived exposure was assessed by a 6-item questionnaire. Two proximitybased exposures were constructed by division/brigade level and battalion/company level unit information using the Stellman exposure opportunity index model. Results: The mean and median of serum TCDD levels was 1.2 parts per trillion (ppt) and 0.9 ppt, respectively. Only 2 Vietnam veterans had elevated levels of TCDD (>10 ppt). The levels of TCDD did not tend to increase with the likelihood of exposure to Agent Orange, as estimated from either proximity-based exposure or perceived self-reported exposure. The serum TCDD levels were not significantly different according to military unit, year of first deployment, duration of deployment, military rank, age, body mass index, and smoking status. Conclusions: The average serum TCDD levels in the Korean Vietnam veterans were lower than those reported for other occupationally or environmentally exposed groups and US Vietnam veterans, and their use as an objective marker of Agent Orange exposure may have some limitations. The unit of deployment, duration of deployment, year of first deployment, military rank, perceived self-reported exposure, and proximity-based exposure to Agent Orange were not associated with TCDD levels in Korean Vietnam veterans. Age, body mass index and smoking also were not associated with TCDD levels.

A Policy Intervention Study to Identify High-Risk Groups to Prevent Industrial Accidents in Republic of Korea

  • Background: The objective of this study is to identify high-risk groups for industrial accidents by setting up 2003 as the base year and conducting an in-depth analysis of the trends of major industrial accident indexes the index of industrial accident rate, the index of occupational injury rate, the index of occupational illness and disease rate per 10,000 people, and the index of occupational injury fatality rate per 10,000 people for the past 10 years. Methods: This study selected industrial accident victims, who died or received more than 4 days of medical care benefits, due to occupational accidents and diseases occurring at workplaces, subject to the Industrial Accident Compensation Insurance Act, as the study population. Results: According to the trends of four major indexes by workplace characteristics, the whole industry has shown a decreasing tendency in all four major indexes since the base year (2003); as of 2012, the index of industrial accident rate was 67, while the index of occupational injury fatality rate per 10,000 people was 59. Conclusion: The manufacturing industry, age over 50 years and workplaces with more than 50 employees showed a high severity level of occupational accidents. Male workers showed a higher severity level of occupational accidents than female workers. The employment period of < 3 years and newly hired workers with a relatively shorter working period are likely to have more occupational accidents than others. Overall, an industrial accident prevention policy must be established by concentrating all available resources and capacities of these high-risk groups.

  • In the chemical process industries, accidents have a high potential and large effects on catastrophic results. Therefore the safety management for accident prevention plays a crucial role to guarantee the process safety. For these reasons, many systematic methods for safety management system have been widely employed in the fields of chemical processes. PSM (Process safety management) is one of most representative methods. The audit system, which is one of PSM system components, evaluates the performance of PMS system. However, most existing safety audit systems are not systematic and these are performed based on knowledges and experiences of various specialist. Moreover, the safety audit is only performed based on each independent technical component. So, the results of safety audit are not a quantitative index but only a series of commentaries. Finally, it is very difficult to obtain the comparison with other plants or industries. In this study, the novel systematic method and index-based accident database of auditing safety management systems for quantitative assessment are proposed. First, the elements of safety audit replace technical methods to categories of accident database. The F-N curve of each category for accident database is employed to derive the index for quantitative assessment. The Accidental Factor Risk Index (AFRI) is suggested for evaluating the effect of each element in accident database and safety audit system. The safety audit can be modified according to the proposed index.

Relationship between Local Extinction Index and Medical Service Uses of Chronic Diseases (지역 소멸위험지수와 지역의 만성질환 의료이용의 관계)

  • Background: This study purposed to analyze the relationship between the local extinction index and medical service uses of chronic diseases. The local extinction index is an indicator of the demographic structure and population aging of the region. Methods: The 2014-2018 statistics of National Health Insurance Corporation and Korean Statistical Information Service data were used for the analysis. First, descriptive statistics were used to analyze the general status of research variables. Second, a panel analysis was performed to analyze the relationship between the local extinction index and medical service uses of chronic diseases (hypertension, diabetes mellitus, periodontal disease, arthritis, mental health, epidemic disease, liver disease). Medical service uses were measured by the number of visits/inpatient days and medical charges of seven chronic diseases. Results: Panel analysis results showed that higher local extinction risks (meaning lower local extinction index) had a positive relationship with the number of visits/inpatient days and medical charges of chronic diseases. But the relationships were varied when the seven chronic diseases were analyzed separately. Conclusion: This study showed a significant relationship between the local demographic structure and medical service uses of chronic disease. Analyzing the local demographic structure will be an essential prerequisite step for implementing appropriate regional health care policies.

Unemployment Insurance Take-up Rates in Korea (한국의 구직급여 수급률 결정요인 분석)

  • This paper investigates the cyclical behavior of UI benefit take-up rate, the share of unemployed persons who are eligible for job seekers' allowances(JSA) and actually receive them. Using Korea's Employment Insurance DB, it also identifies the factors linked to the decision to take up job seekers' allowances. The results show that the take-up rate is countercyclical and leads both unemployment rate and Coincident Composite Index cyclical component by 6 months and is positively correlated with replacement rate and benefit duration, suggesting that extending benefit duration and raising benefit level can boost benefit claims to increase take-up rates in Korea.

Relationship between Medical Service Specialization and Operational Performance in Hospitals: Focusing on Length of Stay and Medical Expense (병원 진료의 전문화와 운영 성과 간의 관계: 재원일수와 급여비용을 중심으로)

  • Objectives : Medical service specialization could have positive effects on their profits and medical service quality. This study was to examine the relationship between medical service specializations and operational performance in hospitals. Methods : We used the National Inpatient Sample data provided by the Health Insurance Review and Assessment Service from 2010 to 2013. The hospital operational performance was determined by measuring the specialization level of the hospital based on DRGs. Results : The information theory index was 2.38 in 2010, 2.38 in 2011, 2.37 in 2012, and 2.37 in 2013. A multiple regression model was constructed which showed that if the specialization level becomes higher, it decreases the length of stay per case with an increase in medical expense. Conclusions : Differentiation and concentrated medical service specialization strategy have had a positive effect on the operational performance of hospitals.

  • The damageability and repairability of similar platform type vehicles could be very concerned with design optimization. In all the vehicles crash tested, small size passenger vehicles were weakness in aspect of damageability and repairability. The most critical area appears to be repair cost considering that parts cost is the largest portion of total repair cost segments. Besides repair cost, attaching method of front sidemember and subframe are placed special importance for impact energy absorption and damageability and repairability. So in order to improve damageability and repairability of vehicle structure and body component of the monocoque type passenger vehicles, the end of front side member and front back beam should be designed with optimum level and to supply the end of front side member as a partial condition approx 300mm. The effectiveness of design concept on the 40% offset frontal impact characteristics of the passenger vehicle structure is investigated and summarized.

Association between breastfeeding and early childhood caries: analysis of National Health Insurance Corporation's oral examination data for infants and toddlers (모유수유와 유아기 우식증과의 관련성: 국민건강보험공단 영유아 구강검진 자료 분석)

  • Objectives: The aim of this study was to investigate the effect of breastfeeding on the occurrence of early childhood caries in Korean infants and toddlers. Methods: Data on oral examinations of infants and toddlers of the National Health Insurance Service were analyzed. The study subjects were children who participated in both the first, second, and third oral examinations and the first general health examination in 2008-2017 (n=142,185). Based on the responses to the questionnaire, the subjects were classified into breastfeeding, formula feeding, and mixed feeding groups. The participants were monitored for the development of early childhood caries in three sequential oral examinations. Results: Based on the oral examination results conducted at 54-65 months old, the decayed-filled teeth index of the breastfeeding group was the highest (2.03±3.08), followed by the mixed (1.96±3.03) and the formula feeding groups (1.82±2.91). The Cox proportional hazard regression model including all the variables showed that the risk of developing dental caries was significantly lower in the formula (hazard ratio [HR], 0.85) and mixed feeding groups (HR, 0.91) than in the breastfeeding group. Conclusions: Breastfeeding children have a higher risk of early childhood caries; therefore, oral hygiene education and regular dental check-ups are necessary.

A qualitative research on the needs for oral care according to the subjective oral health status of the elderly (노인의 주관적인 구강건강상태에 따른 구강관리 요구도에 관한 질적연구)

  • Objectives: The purpose of this study was conducted an in-depth analysis of the subjective oral status and oral care needs according and problems of the elderly. Methods: A phenomenological research method was from April 13 to 30, 2023, 15 elderly people aged 65 or older in Gwangju and Jeolla regions were surveyed. Results: He was experiencing oral changes such as difficulty chewing, dry mouth and indigestion, sensitive teeth, smell of fear and feeling sensitive when eating sweet or cold food. They were burdened by the financial difficulties of dental treatment costs, the inconvenience caused by frequent visits, and the pain experienced during treatment. Realized the need for necessity of oral care education, and their confidence was restored through dental treatment. It was necessary the image recovery of dentistry, and they wanted to maintain oral health through the expansion of treatment health insurance. Conclusions: Consequently, it is necessary to develop a practical oral health management program for the elderly based on social communication regarding of the elderly and to expand health insurance coverage.
