귀속요인이 노인들의 삶의 질에 미치는 영향에 대한 비교 연구 - 한국노인과 미국이민 한국노인을 중심으로 - (Attributions to Quality of Life Between Korean and Korean-American Elderly)

  • Purpose: The present study was designed to explore and compare attributes of the quality of life between Korean elderly and Korean-American elderly. Method: The research design was a descriptive design and questionaries were used. Studies were done using questionnaires. Data were collected from convenient sample of 163 Korean elderly and 151 Korean-American elderly recruited from senior centers and churches. Collected data were analyzed by using SPSS/WIN 10.0. Results: There were significant difference between Korean elderly and Korean-American elderly in education and income. Korean-American elderly showed higher selfesteem, self-efficacy, and quality of life than those of Korean elderly. The influencing factors on quality of life in Korean elderly were defined as self-esteem and self-efficacy. And the influencing factor on quality of life in Korean-American elderly was defined as self-efficacy. Conclusion: In conclusion, the results of this study showed that self-efficacy was a very important factor as attribution to the quality of life for both elderly group. Therefore, self-efficacy should be promoted in nursing intervention in order to enhance the quality of life for both Korean and Korean-American elderly.

고령자 가구의 소비특성 및 소비패턴 결정요인 (The Determinants of Consumption Characteristics and Patterns of Elderly Households)

  • 고령에 대한 개념은 학자와 법령 등에 따라 다양하게 정의되고 있으나, 본 연구의 특성상 소비지출이 소득과 관련이 깊으므로, 고령자고용촉진법에서 기준으로 하고 있는 55세를 고령자의 기준연령으로 설정하였으며, 고령자 가구는 고령자 1인가구와 고령자부부가족만으로 제한하여 연구를 진행하였다. 소비특성은 욕구의 반영이라는 표출된 욕구로 파악될 수 있어 사회복지 측면에서도 의미 있는 분석 대상이라 사료된다. 따라서 본 연구는 고령자 가구의 소비형태를 유형화해서 소비특성을 파악하고, 소비패턴을 결정하는 요인을 찾아 고령자 가구의 표출된 욕구를 통해 관련 정책 수립에 기여하고자 하였다. 조사대상인 고령자 가구 소비 패턴의 내재적 구조 유형을 살펴보기 위해 고령자 가구의 소비지출 항목을 투입하여 군집분석(Cluster analysis)의 K-means법을 실시하였으며, 결과 4개의 군으로 유형화 되어 각각 '보건의료 중심형', '저축 중심형', '생계 중심형', '식비 중심형'으로 명칭을 부여하였다. 고령자 가구의 소비패턴 결정에 영향을 미치는 요인을 분석하기 위하여 이항로지스틱 분석을 사용하였다. 연구결과 고령자 가구는 서로 다른 욕구와 문제에 직면해 있으며, 이를 해결하기 위한 접근방법도 다양해야 할 필요성이 있었다. 특히 지금까지 노인하면 경제적 빈곤자로 인식되어 왔으나, 연구에서는 저축을 통해 준비된 가구들도 있다는 것을 알 수 있었다. 전반적으로 생계 중심형이 가장 많았으며, 이에 영향을 주는 요인으로 혼인여부와 가계소득이 중요한 역할을 하고 있었다. 따라서 고령자 가구의 소득확대에 대한 노력이 필요함을 시사하고 있다. 또한 연령, 주택소유, 주관적 건강상태 등도 유의미한 영향력이 있는 것으로 나타났다. 이러한 연구 결과를 통해 결론 부분에서는 보건의식에 대한 고령자 스스로의 인식 개선, 노년기의 건강 상태에 대한 표준화된 기준 제시, 고령자의 문화생활에 대한 접근성 확보, 삶의 질을 높이기 위한 재정관리 코디, 고령자에게 맞는 일자리 개발과 보급, 협동주거형태인 공동생활가정 보급 등을 제도적 과제로 제언하였다.

독거노인의 노인돌봄서비스 만족도에 영향을 미치는 요인 (Factors Affecting the Satisfaction of Elderly Care Service in the Elderly Living Alone)

  • 본 연구의 목적은 독거노인의 노인돌봄서비스 만족도에 영향을 미치는 요인을 살펴보고, 노인돌봄서비스의 만족도 증진을 위한 기초 자료를 제공하고자 한다. 연구대상은 충북 J시의 노인돌봄기본서비스를 받고 있는 65세 이상 독거노인 301명을 대상으로 하였으며 영향요인을 살펴보기 위해 다중회귀분석을 실시하였다. 본 연구결과 독거노인의 노인돌봄서비스 만족도에 영향을 미치는 요인은 관리사 요인, 서비스 요인, 기관 요인, 소득수준이 유의미한 것으로 나타났다. 이는 생활관리사, 서비스 요인, 기관 요인이 높을수록, 소득수준이 낮은 독거노인 일수록 노인돌봄서비스 만족도가 높은 것으로 나타났다. 특히 생활관리사 요인이 노인돌봄서비스 만족도에 가장 큰 영향을 미치는 것으로 나타났으며, 다음은 기관요인, 서비스요인, 소득수준 순으로 나타났다. 이러한 연구결과를 근거로 정책제언을 하면 첫째, 생활관리사들의 전문성 증진과 처우개선, 둘째, 기관의 전문적이고 체계적인 서비스 지원과 사회안전망 강화, 셋째, 노인돌봄서비스의 품질향상을 위한 적절성·접근성·지속성 강화가 필요하다.

노인의 정신건강과 주관적 구강건강상태가 치주질환에 미치는 영향 (The effects of mental health status and subjective oral health status on periodontal disease for the elderly)

  • Objectives: The purpose of this study was to investigate how mental health status affects oral health in the elderly. Methods: The subjects of the study provided the data for the sixth Korea National Health and Nutrition Examination Survey from 2013 to 2015. Four-hundred and twenty-one subjects were selected for the study. All data were analyzed by complex sampling frequency, chi-square test, and composite multiple regression analysis using SPSS 21.0 version. In addition, a significance level of 0.05 was considered. Results: The general factors affecting cases of periodontal disease were general characteristics, mental health status, oral health status, elderly males, household income, education level, daily brushing frequency, speaking, and stress. The effect of periodontal disease was 1.64 times higher in the male group than in the female group (p<0.001). In the household income level, the 'low' group had 1.91 times more periodontal disease, while the 'mid low' group had 1.64 times more periodontal disease than the 'high' group (p<0.05). Periodontal disease was found to be lower in the group that recognized subjective oral health status as 'good' (p<0.05). The study subjects had low levels of periodontal disease when there was no speaking difficulty (p<0.05). Conclusions: Therefore, in order to improve oral health of the elderly, it is necessary to understand the mental health condition of the elderly and prepare proper oral health education programs accordingly. Institutional devices for various oral health projects should be prepared, as well.

An Analysis of the Senior Employment Programs for Wellness in Changwon City based on Blended Return On Investment

  • The purpose of this study is to evaluate the economic and social impact of the SEP in which the elderly participate by using the blended return on investment (BROI), economic return on investment (EROI), and social return on investment (SROI) research methods. And the sustainable conditions of SEP were confirmed. This study was conducted with one market-type SEP approved by the Korea Elderly Labor Force Development Institute (KLFDI), one preliminary social enterprise approved by Changwon City Hall, and one market-type SEP operated by a social welfare center for the elderly. As a result of the study, it was found that EROI, SROI, and BROI were the highest in the SEP of preliminary social enterprises operated by subsidies in Changwon.However, the difference between EROI and SROI was greatest in the market-type SEP operated by the elderly social welfare center. There was a big difference between economic and social impacts. The social influence of the elderly was evaluated to be higher than the income of the elderly.

노인 재무교육프로그램의 개발과 실시 (A study on a Financial Education Program for the Elderly)

  • The purpose of this study is to develop and test the effectiveness of a financial education program that make the elderly recognize financial problems and find available strategy to improve their financial situation. Based on the literature review and economic structure of the elderly, a financial education program was developed with three parts: 'My elderly lift'; 'Doubling my income'; and 'Safe investment strategy.' 42 elderly people in Senior academy participate in three-session program and 39 people completed pre and post-test and satisfaction survey. In order to examine the effectiveness of the program paired t-test was used. According to evaluation results, the program was found to have a positive effect on enhancing knowledge in elderly lift and financial management, but not on changing attitude. Most of participants show high level of satisfaction with the program.

거동불편노인가족 간병인과 일반노인가족 부양자의 스트레스 영향변인에 관한 연구 (The Infuluential Variables on the Stress of Disordered Elderly Family Caregivers and General Elderly Family Supporters.)

  • The purpose of this study was to investigate the infuluential variables between general character, social support and stress of family caregivers for disordered elderly family and family supporters for general elderly. The study was conducted through interviews with 177 family caregivers and questionaires with 218 supporters in Taegu and Kyeongbuk province. The data were analysed with the spss $pc^+$ statistical package using frequency, correlation, multiple regression, and path anaysis. As results of this study were as follows: Disordered elderly family; degree of physical and psychological disorder, caring-time per day, social support are important factors affecting the stress. General elderly family; digree of physical and psychological disorder, monthly family income, social support are important factors affecting the stress.

Metabolic Syndrome and Its Related Factors among Korean Elderly in Urban and Rural Areas

  • The accelerated aging population may lead to the prevalence of metabolic syndrome and increase in medical costs as well. The aim of this study is to investigate the association with prevalence of metabolic abnormalities and its components in urban/rural area among Korean elderly. We examined the association between metabolic abnormalities and urban/rural area with data from the 2009 Korean National Health and Nutrition Examination Survey. The subjects were 1,622 elderly aged 65 years or over. The sixty percent of Korean elderly reside in urban and 40 % in rural. Rural residents were significantly lower income and less educated than urban residents. But there was no significant different in nutritional factors. They showed significantly 26% lower likelihood of having metabolic syndrome (0.578-0.950, p=0.018) compared with urban residents. Urban residents were associated with higher prevalence of metabolic syndrome than rural residents. This results will contribute to develop strategy for prevention metabolic syndrome for Korean elderly according to urban/rural area.

Effect of Work on Medical Expenditures by Elderly: Findings From the Korean Health Panel 2008-2013

  • Background: This study was conducted to investigate the effects of work on medical expenditures by the elderly. Methods: Data pertaining to individuals aged 65 or older collected by the Korean Health Panel 2008-2013 were used. The effects of work on medical expenditures were analyzed in a panel tobit model adjusted for several variables of demographic factors, socioeconomic factors, and health factors for health care. Data were also analyzed based on age groups (65-74, $75{\leq}$), type of work (waged or self-employed), and working time (daytime work or night time work). Results: Among the elderly older than 65 years, 34-37% were workers. Work among the elderly reduced medical expenditures relative to nonworking elderly. Specifically, medical expenditures were lower in individuals older than 75 years, as well as among those who were self-employed insured and had medical aid insurance and those who exercised. However, medical expenditures were higher among females, married individuals, those with a higher household income, and those with a chronic disease. Elderly wageworkers showed reduced medical expenditures than nonworking elderly and elderly daytime workers did. Conclusion: The elderly population's work, especially wage work and daytime work, reduced medical expenditures relative to no work. These results provide valuable information for policymakers by indicating that work was associated with lower medical expenditures than no work. If elderly work is to be encouraged, it is necessary to provide a variety of high-quality wage work.

충북지역 노인들의 약물복용 및 영양상태 -III. 심리적요인이 약물복용 및 영양상태에 미치는영향 - (Drug Consumption and Nutritional Status of the Elderly in Chung-buk Area -III. Psychological Effect on Drug Consumption and Nutritional Status-)

  • The influences of depression and health anxieth on the elderly's drug use and nutritional status were evaluated by interviews with questionaire from August to October in 1996. One hundred and thirty-one male and 231 female elderly in Chung-buk area were the sample for this study. Men's depression score was 22.3 and 25.2 for women, respectively out of 27. Women showed a significantly higher score for depression and health anxiety than men. Gender, age, marital status, number of family, education, income, medical insurance, and mobility and region significantly affected the health anxiety score. The higher depression score the elderly had, the more frequently they took drugs. Conversely. the higher depression score the elderly had, the less frequently they took nutritional supplements. For women, the higher depression score the elderly had, the more they smoked. More depressed elderly showed a significantly smaller BMI compared to the less depressed ones. A negative correlationship existed between the depression score and the elderly's nutrient intakes, especially energy intake which showed a significant negative correlation. There were no significant differences between health anxiety score and nutrient intakes of men. Women who had a higher health anxiety score consumed more energy significantly. The depression score did not affect the elderly's blood biochemical indices. Women who had a low health anxiety score showed a significantly higher HDL-C level.
