Clinical and radiographic features of facial cosmetic materials: A systematic review

  • Purpose: The aim of this study was to systematically screen the literature for studies reporting cosmetic material in the oral and maxillofacial complex to shed light on the types of cosmetic materials, their radiographic appearance, and possible complications. Materials and Methods: Five electronic databases were reviewed for eligible studies. The general search terms were "cosmetic," "filler," "face," and "radiograph." Demographics, material types, clinical and radiographic presentation, and complications were recorded. Results: Thirty-one studies with 53 cases met the inclusion criteria. The mean age was 52.6±15.4 years with a 4 : 3 female-to-male ratio. The most common material was calcium hydroxyapatite (CaHa) (n=14, 26.4%), found incidentally. The materials were generally located within the upper cheek and zygoma (n=35, 66.0%), radiographically well-defined (n=44, 83%), and had no effects on the surrounding structures (n=27, 50.9%). The internal structure was radiopaque (calcification, hyperdensity) for gold wires, CaHa, bone implants, and secondary calcification or ossification. Outdated cosmetic materials or non-conservative techniques were infiltrative, had effects on the surrounding structures, and presented with clinical signs, symptoms, or complications. Conclusion: Conventional radiography, cone-beam computed tomography, and multi-detector computed tomography are useful to differentiate several cosmetic materials. Their magnetic resonance imaging appearance was highly variable. The infrequent inclusion of cosmetic materials in the differential diagnosis implies that medical and dental specialists may be unfamiliar with the radiographic appearance of these materials in the face.

A Green Preparation of Drug Loaded PAc-β-CD Nanoparticles from Supercritical Fluid (초임계 유체를 이용한 약물이 담지된 PAc-β-CD 나노 입자의 친환경적인 제조)

  • Rapid expansion of supercritical solution (RESS) process was used to make molsidomine (MOL) loaded peracetyl-β-cyclodextrin (PAc-β-CD) nanoparticles, which were collected into the air. The effect of the concentration of the drug PAc-β-CD (0.5 and 1 wt%), extraction temperature (45 ~ 60 ℃), nozzle length (5 ~ 20 mm) and internal diameter (ID) (50 ~ 150 μm) of a capillary, and spray distance on the particle size and morphology of the resulting particles were investigated. The interaction of a drug and PAc-β-CD was confirmed by 1H-NMR spectroscopy while the particle size was measured by means of a scanning electron microscope. It was found that increasing the temperature from 45 ℃ to 60 ℃ and decreasing the nozzle diameter from 150 μm to 50 μm had an increasing effect on the average particle size, while increasing the spray distance led to a decrease in the average particle size at a constant pressure of 34.5 MPa and temperature of 45 ℃. With 0.5 wt% of PAc-β-CD, the capillary nozzle of short length (5 mm) and small ID (50 μm) gave the smallest size (165 nm). The obtained nanoparticles showed increased dispersity and solubility in oil. The oil suspension of the inclusion complex showed increased sustainability, which can increase the in-vitro controlled release time of the drug.

Gold and Silver Mineralization of the Soowang Ore Deposits in Muju, Korea (무주 수왕광상의 금-은 광화작용)

  • The Soowang Au-Ag deposits occur as quartz veins which filled fissures in middle Cretaceous porphyritic granite an/or gneiss of the Precambrian Sobaegsan gneiss complex. The paragenetic studies suggest that vein filling can be divided into four identifiable stages (I to IV). Stage I is the main sulfide stage, characterized by the deposition of base-metal sulfide and minor electrum. Stage II is the electrum stage, whereas stage III represents a period of the deposition of silver-bearing sulfosalts and minor electrum. Stage IV is the post ore stage. Mineralogical and fluid inclusion evidences suggest that mineralization of the Soowang deposits were deposited by the cooling of the fluids from initial high temperatures 300$^{\circ}C$ to later low temperatures 150$^{\circ}C$. The salinity of the fluids were moderate, ranging from 10.4wt.% equivalent NaCl in sphalerite to 3.1wt.% equivalent NaCl in barite. The gold-silver mineralization of the Soowang mine occurred at temperatures between 140 and 250$^{\circ}C$ from fluids with log $fs_2$ from -12 to -18 atm. A consideration of the pressure regime during ore deposition, based on the fluid inclusion evidence of boiling, suggests lithostatic pressure of less than 210 bars. This pressure condition indicates that vein system of the Soowang deposit formed at depth around 800 m below the surface at the time of gold-silver mineralization.

Fluid Inclusion Studies of the Fluorite Deposits in Korea (우리나라 형석광상(螢石鑛床)의 유체포유물(流體包有物) 연구(硏究))

  • The flourite in Hwacheon, Hwanggangri and Keumsan district are major fluorite producing areas in Korea. The fluorite deposits of Hwacheon district are wholly fissure filling hydrothermal veins embedded in Precambrian gneiss and schists and Jurassic granites. Also some fluorite deposits are emplaced in felsite whose age is unknown. Emplacement of most fluorite veins of the district are controlled by EW fracture system. Fluorites are generally accompanied to chalcedonic quartz and also kaolinite, montmorillonite, dickite and calcite in parts. Vertical and lateral mineral zonings are not distinct. The fluorite deposits in the Hwanggangri district are wholly embedded in limestone and other calcareous sediments of Paleozoic Yeongweol Group. Most of the fluorite deposits belong to one of two categories which are steeply. dipping veins and gently dipping replacement deposits adjacent to Late Cretaceous(83-90mys) granite bodies. The strikes of fluorite veins of Hwanggangri district mostly occupy the fractures of $N30^{\circ}-40^{\circ}E$ and $N30^{\circ}-40^{\circ}W$ system. Fluorites are accompanied to calcite, milky quartz, chalcedonic quartz, and also montmorillonite, kaolinite in parts. But in some deposits, scheelite, various sulfide minerals and barite are accompanied. Emplacement of fluorite deposits are largely controlled by lithology and structures of this district. In some deposits fluorite veins gradate to scheelite veins and also telescoping of the mineral zones are found in this district. In the Keumsan district, fissure-filled fluorite veins and replacement deposits are mostly emplaced in limestone of Paleozoic Yeongweol Group, late Cretaceous quartz-porphyry, granite and sandstone. Some deposits are emplaced in Precambrian metasediments. Mineralogy and other characteristics of the deposits in this district is similar to those of Hwanggangri district. Fluid inclusion studies reveal the difference of salinities, $CO_2$ contents of ore fluid and temperatures during fluorite mineral deposition in the these districts. In Hwacheon district, ore-fluids were comparatively dilute brine and low $CO_2$ content. Filling temperatures ranges $104^{\circ}C$ to $170^{\circ}C$. In the Chuncheonshinpo mine, most deeply exploited one in this district, salinitles range 0.5-2. 2wt. % NaCl and filling temperatures range from $116^{\circ}C$ to $143^{\circ}C$. In the Hwanggangri district, ore fluids were complex and filling temperature ranges very widly. In the contact metasomatic fluorite deposits, ore fluid were NaCl rich brines with moderate $CO_2$ content and filling temperatures range from $285^{\circ}C$ to above $360^{\circ}C$. Fluids inclusions in tungsten and sulfide minerals bearing fluorite veins show high $CO_2$ content up to 31wt. %. Filling temperature ranges from $101^{\circ}C$ to $310^{\circ}C$. Fluids inclusions In mainly fluorite bearing veins were more dilute brine and low $CO_2$ contents. Filling temperatures range from $95^{\circ}C$ to $312^{\circ}C$. Filling temperature of fluid inclusions of Keumsan district are between $95^{\circ}C$ and $237^{\circ}C$. Data gathered from geologic, mineralogic and fluid inclusion studies reveal that fluorite mineralization in H wacheon district proceeded at low temperature with dilute brine and low $CO_2$ content. In Hwangganri district, fluorite mineralization proceeded by several pulse of chemically distinct ore fluids and formed the mineralogically different type of deposits around cooling granite pluton which emplaced comparatively shallow depth.

Effects of Sulfobutyl Ether $\beta$-Cyclodextrin on Physicochemical Properties of Dexamethasone Dipropionate

  • Complex formation of practically insoluble dexamethasone dipropionate (DDP) with ${\beta}$-cyclodextrin (${\beta}$-CD), dimethyl-${\beta}$-cyclodextrin (DMCD), trimethyl-${\beta}$-cyclodextrin (TMCD), 2-hydroxypropyl-${\beta}$-cyclodextrin (HPCD) and sulfobutyl ether ${\beta}$-cyclodextrin (SBCD) in water was investigated by solubility method at various temperatures. Water solubility of DDP was found to be 1.78 $\mu\textrm{g}$/$m\ell$ at 37$^{\circ}C$. Propylene glycol (PG)-water cosolvent increased the solubility of DDP, but the solubilization was not sufficient (8.93 $\mu\textrm{g}$/$m\ell$ in 20% PG). The addition of CD markedly increased the solubility of DDP in water, and A$\sub$L/ type phase solubility diagrams were obtained with ${\beta}$-CD, TMCD, HPCD and SBCD, where the apparent stability constants of the soluble complexes at 25$^{\circ}C$ were determined to be 1388, 216, 1054, and 1992 M$\^$-1/, respectively. However, DMCD remarkably increased the solubility of DDP, and showed an A$\sub$P/ type diagram, suggesting that DMCD forms a soluble complex of high order with DDP. The stability constant for the DDP-DMCD complex at 25$^{\circ}C$ was determined to be 19132 M$\^$-1/. The thermodynamic parameters were calculated for the inclusion complex formation in aqueous solution. CD (1${\times}$10$\^$-2/M) remarkably decreased the partition coefficients of DDP between isopropyl myristate/water in the order of TMCD < ${\beta}$-CD < HPCD < SBCD < DMCD, and in squalane/water system in the order of HPCD < TMCD < ${\beta}$-CD < DMCD < DMCD $\leq$ SBCD. This finding represents that, in a o/w type cream, cyclodextrin complexation with DDP may result in high concentration of DDP in aqueous phase. The permeation of DDP through a cellophane membrane was highly suppressed by the addition of CD, and the degree of suppression was different among CDs, indicating that CD may control the skin permeation of DDP. The dissolution rates of solid dispersions with CDs were much faster than those of drugs alone and corresponding physical mixtures. All DDP-CD solid dispersions exceeded the equilibrium solubility. Consequently these results suggest that complex formation of DDP with CDs may provide useful means to markedly enhance the solubility, and CDs are useful in the semi-solid preparations such as creams and gels for topical application.

Outcomes of Primary Unilateral Cheiloplasty in Same-Day Surgical Settings

  • Background Financial, clinical, and psychological considerations have made same-day surgery an attractive option for a variety of procedures. This article aimed to analyse the postoperative results of same-day primary unilateral cleft nasolabial repair. Methods This study was performed from 2011 to 2014. Unilateral cleft lip patients fulfilling the inclusion criteria were preoperatively classified as mild, moderate, and severe. All patients underwent same-day surgery and were discharged after satisfying the appropriate clinical criteria, receiving thorough counselling, and the establishment of a means of communication by phone. Postoperative outcomes were assessed and stratified according to preoperative severity and the type of repair. Results A total of 423 primary unilateral cleft lip patients were included. Fisher's anatomical subunit approximation technique was the most common procedure, followed by Noordhoff's technique. The postoperative outcome was good in 89.8% of cases, fair in 9.9% of cases, and poor in 0.2% of cases. The complication rate was 1.18% (n=5), and no instances of mortality were observed. The average hospital stay was 7.5 hours, leading to a cost reduction of 19% in comparison with patients who stayed overnight for observation. Conclusions Mild unilateral cleft lip was the most common deformity for which Fisher's anatomical subunit approximation technique was performed in most of the cases, with satisfactory postoperative outcomes. Refinements in the cleft rhinoplasty techniques over the course of the study improved the results regarding cleft nasal symmetry. Single-day primary unilateral cleft cheiloplasty was found to be a cost-effective procedure that did not pose an additional risk of complications.

Systematic Review of Randomized Controlled Trials on Herbal Medicine for Treatment of Human Obesity (비만 치료 한약의 무작위 대조임상시험에 대한 체계적 분석)

  • Objectives : The objective of the study was to summarize randomized clinical trials(RCTs) that have assessed the effectiveness of herbal medicine on treatment of obesity and to propose better process of study. Methods : NLM Medline(pubmed), EMBASE, the Cochrane library, Science Direct, EBSCO, 4 Korean medical databases were systematically searched and 4 Korean medical journals were manually searched for clinical trials investigating the efficacy of herbal medicines on treatment of overweight or obese people from 1998 to 2008. The methodological quality was assessed using a Jadad score and validity was assessed using Oxford Pain Validity Scale(OPVS). Results : 14 RCTs met all the inclusion criteria. The methodological and ethical quality of the trials was generally low. The mean score by Jadad was 2.6 and the mean score of validity was 11.2. Complex herbal medicine was used in 8 RCTs and single herbal medicine was used in 6 RCTs. Except 1 RCT, the other RCTs reported positive effects of herbal medicine on treatment of obesity. Herbal medicines didn't seem to affect toxicity. In general adverse events relevant with the therapy were minor, but more than half of RCTs did not report about the safety or adverse events of herbal medicine, questioning their reliability. Conclusions : Although most RCTs concluded the efficacy and safety of herbal medicines on treatment of obesity, the quality of trials was low in general. Further rigorous clinical trials using complex herbal medicine should be performed.

Efficacy and Tolerance of a New Anti-Regurgitation Formula

  • Purpose: Regurgitation is a common physiological phenomenon in infants. The aim of the present study was to evaluate the efficacy of a new anti-regurgitation (AR) formula (Novalac), thickened with an innovative complex including fibres, on the daily number of regurgitations and to assess its impact on stool consistency and frequency. Methods: Infants younger than five months, presenting at least 5 regurgitations per day were recruited in this trial. The efficacy of the new formula on regurgitation (daily number and Vandenplas score), stool frequency and consistency were assessed at day 14 and 90. Growth data were recorded at each study visit. Results: Ninety babies (mean age $9.6{\pm}5.8weeks$) were included in the full analysis data set. The mean number of regurgitation episodes at inclusion was $7.3{\pm}3.4$. In all infants, regurgitations improved after 2 weeks. The daily number of regurgitations decreased significantly ($-6.3{\pm}3.3$, p<0.001) including in those previously fed a thickened formula ($-6.2{\pm}3.0$, p<0.001). There was no significant change in stool consistency at day 14. After 3 months, 97.5% of infants had formed or soft stools. Growth was appropriate with a slight increase of weight-for-age z-score (from $-0.5{\pm}1.0$ to $-0.1{\pm}0.9$) and no change of weight-for length z-score ($-0.1{\pm}1.1$ to $-0.1{\pm}-1.1$). Conclusion: The new AR formula thickened with an innovative complex is very effective in reducing the daily number of regurgitations without having a negative impact on stools consistency.

Effects of dietary copper sources and levels on growth performance, copper digestibility, fecal and serum mineral characteristics in growing pigs

  • This experiment was conducted to investigate the effects of three different copper (Cu) sources (one inorganic and two organics) and levels (0, 50, and 100 mg/kg) on the growth performance, Cu digestibility, fecal mineral excretion, serum mineral concentration, jejunal morphology, and serum biochemical profile of growing pigs. A total of 42 male, growing pigs (31.08 ± 1.82 kg) were randomly assigned to seven treatments consisting of one negative control (0 mg/kg of added Cu level) and treatments with copper sulfate (CuSO4), Cu-amino acid complex (CuAA), and Cu-hydroxy-4-methylthio butanoate chelate complex (CuHMB) at 50 and 100 mg/kg each for 28 d. Pigs fed 50 or 100 mg/kg of Cu showed improved (p < 0.05) average daily gain and feed intake. Although Cu excretion decreased (p < 0.01) in pigs fed 100 mg/kg of organic Cu sources compared to those fed CuSO4, there was no difference between the Cu sources in pigs fed 50 mg/kg. However, the apparent total tract digestibility of Cu increased (p < 0.01) in pigs fed organic Cu sources compared with that in pigs fed CuSO4. The addition of CuHMB increased (p < 0.01) serum phosphorus and sulfur concentrations; however, there were no effects of source and level on jejunal morphology and serum biochemical profile. These results suggest that the inclusion (50 mg/kg) of organic Cu sources (CuAA and CuHMB) in the growing pig diet could be beneficial for growth performance and Cu availability and may reduce environmental pollution.

Simple and Sensitive Electrochemical Sandwich-type Immunosensing of Human Chorionic Gonadotropin based on b-cyclodextrin Functionalized Graphene

  • The effective detection of human chorionic gonadotropin (HCG) is considerably important for the clinical diagnosis of both of early pregnancy and nonpregnancy-related diseases. In this work, a simple and sensitive electrochemical sandwich-type immunosensing platform was designed by synthesizing b-cyclodextrin (CD) functionalized graphene (CD/GN) hybrid as simultaneously sensing platform and signal transducer coupled with rhodamine b (RhB) as probe. In brief, GN offers large surface area and high conductivity, while CD exhibits superior host-guest recognition capability, thus the primary antibody (Ab1) of HCG can be bound into the cavities of CD/GN to form stable Ab1/CD/GN inclusion complex; meanwhile, the secondary antibody (Ab2) and RhB can also enter into the cavities, producing RhB/Ab2/CD/GN complex. Then, by using Ab1/CD/GN as sensing platform and RhB/Ab2/CD/GN as signal transducer (in which RhB was signal probe), a simple sandwich-type immunosensor was constructed. Under the optimum parameters, the designed immunosensor exhibited a considerable low analytical detection of 1.0 pg mL-1 and a wide linearity of 0.002 to 10.0 ng mL-1 for HCG, revealing the developed sandwich-type electrochemical immunosensing platform offered potential real applications for the determination of HCG.
