Alveolar bone thickness and fenestration of incisors in untreated Korean patients with skeletal class III malocclusion: A retrospective 3-dimensional cone-beam computed tomography study

  • Purpose: The purpose of this study was to evaluate vertical bone loss and alveolar bone thickness in the maxillary and mandibular incisors of patients with skeletal class III malocclusion. This study also aimed to evaluate the periodontal condition of class III malocclusion patients who had not undergone orthodontic treatment. Materials and Methods: The sample included cone-beam computed tomography scans of 24 Korean subjects (3 male and 21 female). Alveolar bone thickness (ABT), alveolar bone area (ABA), alveolar bone loss (ABL), and fenestration of the maxillary and mandibular incisors were measured using 3-dimensional imaging software. Results: All incisors displayed an ABT of less than 1.0 mm from the labial surface to root level 7 (70% of the root length). A statistically significant difference was observed between the mandibular labial and lingual ABAs and between the maxillary labial and mandibular labial ABAs. The lingual ABA of the mandibular lateral incisors was larger than that of the mandibular central incisors. ABL was severe on the labial surface. A statistically significant difference was observed between the maxillary and mandibular labial ABL values(21.8% and 34.4%, respectively). Mandibular lingual ABL (27.6%) was significantly more severe than maxillary lingual ABL (18.3%) (P<0.05). Eighty-two fenestrations were found on the labial surfaces of the incisors, while only 2 fenestrations were observed on the lingual surfaces. Fenestrations were most commonly observed at root level 6. Conclusion: Careful evaluation is needed before orthodontic treatment to avoid iatrogenic damage of periodontal support when treating patients with class III malocclusion.

Alveolar bone thickness around maxillary central incisors of different inclination assessed with cone-beam computed tomography

  • Objective: To assess the labial and lingual alveolar bone thickness in adults with maxillary central incisors of different inclination by cone-beam computed tomography (CBCT). Methods: Ninety maxillary central incisors from 45 patients were divided into three groups based on the maxillary central incisors to palatal plane angle; lingual-inclined, normal, and labial-inclined. Reformatted CBCT images were used to measure the labial and lingual alveolar bone thickness (ABT) at intervals corresponding to every 1/10 of the root length. The sum of labial ABT and lingual ABT at the level of the root apex was used to calculate the total ABT (TABT). The number of teeth exhibiting alveolar fenestration and dehiscence in each group was also tallied. One-way analysis of variance and Tukey's honestly significant difference test were applied for statistical analysis. Results: The labial ABT and TABT values at the root apex in the lingual-inclined group were significantly lower than in the other groups (p < 0.05). Lingual and labial ABT values were very low at the cervical level in the lingual-inclined and normal groups. There was a higher prevalence of alveolar fenestration in the lingual-inclined group. Conclusions: Lingual-inclined maxillary central incisors have less bone support at the level of the root apex and a greater frequency of alveolar bone defects than normal maxillary central incisors. The bone plate at the marginal level is also very thin.

구내용 분광광도계를 이용한 자연치의 색상분석 (COLOR ANALYSIS OF THE NATURAL TEETH WITH A MODIFIED INTRAORAL SPECTROPHOTOMETER)

  • The recognition of the esthetics as well as their function of the teeth in the dental field is growing. Evaluating the esthetics of a tooth concerns shape, texture, and color. And among these, color has been the most difficult element to make natural expression. Color is a very subjective element affected by many different factor like light source, object, observer. Numerous efforts to quantifying color has been made. Currently the use of colorimeter and spectrophotometercolor that the color measuring device has been suggested. In this study, the normal range of the tooth color in canines, lateral incisors and central incisors for Koreans was evaluated with a modified spectrophotometer for intraoral use. The color differences in terms of teeth positions, age and area within the tooth was evaluated and comparatively analyzed. Results were as follow ; 1. In all age groups, the central incisors are most bright and least yellowish opposit to the canines are most dark and yellowish. The central incisors and lateral incisors showed the similar color patterns. 2. In the comparison between gender, it showed that in the 10' s group male have more yellowish, the 20's female have lighter and more redness, the 30's indicated more redness in female and in the 40's the lightness is more in female. 3. With aging, there was no significant change in $L^*$ and $a^*$ coordinates but a shift was in $b^*$ to yellowish in the 30' sand 40's in central incisors. Lateral incisors and canines showed a lighter and more yellowish tooth color with an irregular change of color coordinate in $a^*$. 4. In all teeth the middle and cervical portion showed close correlation with over 0.7(0.05>p) but color ($a^*$, $b^*$) showed higher correlation than lightness($L^*$). 5. It has been shown that through the regression test the cervical color could be calculated by the middle color value.

Surgical endodontic management of infected lateral canals of maxillary incisors

  • This case report presents surgical endodontic management outcomes of maxillary incisors that were infected via the lateral canals. Two cases are presented in which endodontically-treated maxillary central incisors had sustained lateral canal infections. A surgical endodontic treatment was performed on both teeth. Flap elevation revealed vertical bone destruction along the root surface and infected lateral canals, and microscopy revealed that the lateral canals were the origin of the lesions. After the infected lateral canals were surgically managed, both teeth were asymptomatic and labial fistulas were resolved. There were no clinical or radiographic signs of surgical endodontic management failure at follow-up visits. This case report highlights the clinical significance and surgical endodontic management of infected lateral canal of maxillary incisor. It is important to be aware of root canal anatomy variability in maxillary incisors. Maxillary central incisors infected via the lateral canal can be successfully managed by surgical endodontic treatment.

  • This study was performed to investigate the changes of dento-alveolar relationship in relation to the eruption of in cisors using roentgenocephalometry. The Subjects consisted of 76 females from 4 to 8 years with normal occlusion. The following conclusions were obtained: 1. When maxillary and mandibular permanent central incisors were erupted, maxillary alveolar prognathism was increased, while mandibular alveolar prognathism was not changed. 2. Axial inclination of permanent incisors was greater than that of primary incisors.

  • The author has performed a statistical study on the vertical and horizontal distance between maxillary central incisors and incisive papilla, the maxillary intercanine width, and the form of maxillary dental arch. Stone models of maxillary dental arch were made in 403 Korean adults (300 male, 103 female) from 20 to 30. years of age. The results were as follows; 1. The vertical distance between maxillary central incisors and incisive papilla was 6.0mm.. 2. The horizontal distance between maxillary central incisors and incisive papilla was 9.0mm.. 3. The maxillary intercanine width was 37.7mm.. 4. The U type of maxillary dental arch was the greatest percentage with 68.2 %, the percentage for O type was about 21.4% and the V type 10.4%.

  • In order to evaluate the effect of signs and symptoms of temporomandibular disorders on the bite force of anterior teeth, the author estimated a feature of maintaining the maximum bite force on central incisors in Korean 34 adult females with signs and symptoms of temporomandibular disorders (TMD group) and in Korean 31 adult females within normal masticatory function far from any sign or symptom of TMD (control group), and analyzed the data statistically. The obtained results were as follows: 1. There was not a significant difference of the maximum bite force on central incisors between TMD group and control group (P>0.05). 2. The duration of maintaining the maximum bite force on central incisors in TMD group was shorter than it in control group (P<0.01).

  • The esthetic demands are growing in treatment of primary maxillary incisors as well as permanent ones. There is something dissatisfying about in the keen eyes of the pediatric dentists on their esthetic results of efforts because current tooth color system is created for the purpose of the application to the permanent incisors. In this study using the scientific color measurement device, I would like to introduce a color scheme for the primary maxillary incisors and suggest clinical tips for the better results of resin composite restorations.

교정치료시 하악 전치부 치은퇴축의 고려 (Gingival recession of lower anterior incisors in orthodontic treatment)

  • Purpose: These case reports show the orthodontic treatment of lower anterior incisors with gingival recession. Materials and Methods: Three cases were treated by an orthodontist and a periodontist. Each case had lingually tilted lower anterior incisors, anterior crossbite and skeletal Cl III pattern. Results: A variety of etiological factors were thought to cause gingival recession: aging, oral hygiene, tooth malpositioning, occlusal trauma. Conclusion: Due to the interaction among many possible contributing factors, it is difficult to predict whether further gingival recession may occur at a given site. The position and the movement of the lower anterior incisors with gingival recession are important factors in diagnosis and orthodontic treatment planning.

미취학 아동의 우유병 우식증 유병률에 관한 조사 연구 (PREVALENCE OF NURSING CARIES IN PRESCHOOL CHILDREN)

  • The purpose of this study was to investigate the prevalence rate of the nursing caries in preschool children by retrospective epidemiological survey. 500 preschool children in Iksan city were examined for their dental caries experience (dt, mt, ft). Subjects were divided stepwise by the pattern of caries experience: Group 1-no caries, Group 2-no caries in the upper arch, Group 3-no caries among the upper incisors, Group 4-dmft in the lower arch $\geq$ mft in the upper arch, Group 5-dmft among the upper molars > dmft among the upper incisors, Group 6-dmft among the upper molars < dmft among the upper incisors+caries among the lower incisors, and Group 7-the rest. Groups 6 was regarded as the group which had experienced rampant caries, and the Group 7 was regarded as the group which had the typical pattern of nursing caries. The prevalence rate of nursing caries (Group 7) was 16.0%.
