Gallbladder wall thickness adversely impacts the surgical outcome

  • Methods: Patients who underwent cholecystectomy were classified into two groups according to their GBWT status (GBWT+ vs. GBWT-). Results: Among 1,211 patients who underwent cholecystectomy, GBWT+ was seen in 177 (14.6%). The GBWT+ group was significantly older with more males, higher ASA score, higher alkaline phosphatase level, higher international normalized ratio, and lower albumin level than the GBWT- group. On ultrasound, GBWT+ patients had larger stone size, more pericholecystic fluid, more common bile duct stone, and more biliary pancreatitis. Compared with the GBWT- group, the GBWT+ group had more urgent surgeries (12.4% vs. 3.2%, p = 0.001), higher conversion rate (4.5% vs. 0.3%, p = 0.001), prolonged operative time (67 ± 38 vs. 54 ± 29 min; p = 0.001), more bleeding (3.4% vs. 0.5%, p = 0.002), and more need of drain (21.5% vs. 10.5%, p = 0.001). By multivariate analysis, factors associated with increased length of hospital stay were GBWT+ (HR: 1.97, 95% CI: 1.19-3.25, p = 0.008), urgent surgery (HR: 10.2, 95% CI: 4.07-25.92, p = 0.001), prolonged surgery (HR: 1.01, 95% CI: 1.0-1.02, p = 0.001), and postoperative drain (HR: 11.3, 95% CI: 6.40-20.0, p = 0.001). Conclusions: Variables such as GBWT ≥ 5 mm, urgent prolonged operation, and postoperative drains are independent predictors of extended hospital stay. GBWT+ patients are twice likely to stay in hospital for more than 72 hours and more prone to develop complications than GBWT- patients.

Factors Influencing Nursing Students' Intention of Childbirth (간호대학생의 출산의지에 영향을 미치는 요인)

  • This study investigated how the awareness about marriage, the value of children, and the motivation for childrearing influences childbirth will in nursing students. The data was collected from May 1 to May 30, 2019, and 248 data samples were finally used for analysis. The collected data was analyzed using the SPSS /WIN 22.0 program. Descriptive statistics, Pearson's correlation coefficients, t-tests, ANOVA, Scheffe's test and multiple regression analysis were all calculated. The average of awareness about marriage was 3.39±.38, the average of the value of children was 2.72±0.39, the average motivation of childrearing was 2.73±0.78 and the average of childbirth will was 3.12±0.48. The childbirth will according to the general characteristics, was significantly different according to age (F=6.002, p=.003). Among the awareness about marriage, conservative view on marriage(r=.207, p=.001), active view on marriage(r=.226, p<.001), instrumental view on marriage(r=.181, p=.004), and motivation of childrearing(r=.283, p<.001) showed positive correlation ed with childbirth will. Childbirth will was negatively correlated with passive view on marriage (r=-.292, p<.001), and the value of children (r=-.226, p<.001). The factors affecting childbirth will of the study subjects were age(β=.155, p=.007), passive view on marriage(β=-.383, p<.001), motivation of childrearing (β=.315, p<.001), and the explanatory power was 23.6%. According to this study, it is necessary to confirm the effects of developing an intervention program and applying this program to improve the decisions of nursing students concerning marriage and having children.

Effects of Physical Activity Practice Rates and Knowledge Related to Cardiocerebrovascular Disease Prevention on Health Behavior Case Study Focusing on Middle Aged Women with Risk of Central Obesity (중심비만 위험인자를 가진 중년여성의 신체활동 실천율, 심뇌혈관질환 예방관련 지식이 건강행위에 미치는 영향)

  • This study was conducted to evaluate the effects of physical activity practice rates and knowledge related to cardiocerebrovascular disease prevention on the health behavior of middle aged women. Data were collected from Oct to Nov 2017 from 142 middle-aged women living in 24 Eup, Myeon, and Dong areas in North Gyeongsangbuk-do Province using a structured questionnaire. The obtained data were analyzed using descriptive statistics, t-tests, ANOVA, Pearson's correlation, and stepwise multiple regression analysis. The major factors influencing health behavior were found to be alcohol consumption (${\beta}=0.15$, p=0.009), diet (${\beta}=0.16$, p=0.003), vigorous intensity (${\beta}=0.14$, p=0.011), marriage (${\beta}=0.19$, p<0.001), interest in one's own health (${\beta}=0.23$, p<0.001), and health recognition (ill: ${\beta}=0.31$, p<0.001). Alcohol consumption and diet were factors of cardiocerebrovascular knowledge, vigorous intensity was a factor of physical activity practice rate, marriage and interest in one's own health were factors of general characteristics, and health recognition was a factor in health-related characteristics. Health-promotion activity was positively correlated with knowledge regarding cardiocerebrovascular disease prevention (r=0.41, p<0.001) and physical activity practice rate (r=0.44, p<0.001). It will be necessary to develop and apply practical intervention programs based on disease prevention knowledge and physical activity to enhance the health behavior of middle aged women.

Relationship between the Lipid Accumulation Product Index and Alanine Aminotransferase in Korean Adult Men (한국 성인남성에서 Lipid Accumulation Product 지수와 Alanine Aminotransferase와의 관련성)

  • The LAP (lipid accumulation product) index is an indicator of excessive fat accumulation in combination with the fasting triglyceride concentration and waist circumference. This study examined the relationship between the LAP index and ALT (alanine aminotransferase) level and the effect of the LAP index on the ALT level. Cross-sectional studies were conducted on healthy adult males without a history of liver disease. From January 2015 to June 2017, 13,854 adults between 20 and 70 years of age, who underwent health screening at a general hospital in Gyeonggi-do, were enrolled in this study. The LAP index was calculated as [waist circumference (cm)-65]${\times}$[triglyceride (mmol/L)] for males. Serum ALT abnormalities were set at 40 IU/L or more in males. All subjects underwent measurements of the anthropometric indicators and physiological examinations. A significant increase in ALT with increasing LAP quintiles was observed (p<0.001). The LAP index correlated with ALT (r=0.238, p<0.001). Insulin (r=0.449, p<0.001) and HDL-cholesterol (r=-0.369, p<0.001) were strongly correlated with LAP. In addition, the LAP index is a factor affecting the ALT level (p<0.001). As the LAP index increased, the risk of an elevated ALT was higher (p<0.001). In conclusion, the LAP index was a predictor of an ALT elevation in Korean men.

The Influence of Tangible Clues of Raw-fish Restaurants on Consumers' Emotional Responses & Repurchasing Behavior in East-northern Area of Kangwon-Province (강원도 영동권 지역 생선회 전문점의 유형적 근거가 고객의 감정반응과 재구매 행동에 미치는 영향)

  • The purpose of this article was to study the influence of tangible clues of raw-fish restaurants on consumers' emotional responses and repurchasing behavior in eastern area of Kangwon-province. In this study frequency analysis, reliability analysis, factor analysis and path analysis(SEM) were applied to analyze the data. Through path analysis tangible clues' performances had generally significant influences upon consumers' emotional responses to buy products of raw-fish restaurants. According to the results, exterior facility had positive influence on pleasure(p<0.001) and arousal(p<0.01). Interior facility had positive influence on pleasure(p<0.001) and arousal(p<0.001), but negative on dominance(p<0.01). And the others had positive influence on pleasure(p<0.05). Press copy had positive influence on pleasure(p<0.001) and arousal(p<0.001). And Pleasure and arousal had positive influence on repurchasing behavior(p<0.001). But Dominance didn't have significant influence on repurchasing behavior. As a result, we studied that the tangible clues was an effective marketing tactic to maintain and increase consumers' positive emotional reponses to repurchase at restaurants. Therefore it is finally needed that dining-out corporations manage tangible clues in a customer-maintaining without additional costs, and especially to use mixing tangible clues each other in some suitable condition.

Effect of Astaxanthin on Anti-Inflammatory and Anti-Oxidative Effects of Astaxanthin Treatment for Atopic Dermatitis-induced Mice

  • Background: This study sought to determine whether the antioxidant effects of astaxanthin (AST) could have an anti-inflammatory effect to reduce inflammation caused by atopic dermatitis (AD). Methods: Using a mouse model of AD induced by phtalic acid (PA), the levels of inflammation, inflammatory agents, and evidence of antioxidant activity were examined in PA treated mice (n = 3), PA-AST treated mice (n = 3), and a control group of mice (n = 3). This included measurements of ear thickness, levels of mast cells, IgE, inflammatory cytokine, malondialdehyde (MDA), hydrogen peroxide, HO-1, and GPx-1. Results: AST treatment significantly prevented inflammation as measured by ear thickness (p < 0.05), mast cell count (p < 0.001), and IgE concentration in the blood (p < 0.001). Levels of TNF-α (p < 0.001), IL-1β (p < 0.001), IL-6 (p < 0.001), and MDA (p < 0.05) were also significantly lower. In addition, GSH levels increased significantly (p < 0.001), and the level of hydrogen peroxide significantly reduced (p < 0.01). The expression of HO-1, GPx-1 increased. Conclusion: In this small experimental study, AST acted on inflammatory mechanisms that induced AD, through anti-inflammatory and antioxidant mechanisms, and is a candidate of interest in the clinical treatment of AD.

In Situ Dry Matter, Nitrogen and Phosphorous Disappearance of Different Feeds for Ruminants

  • Four feeds, three concentrates (rice bran, soybean meal and flaked corn) and one forage (corn silage) were incubated in four ruminally fistulated Holstein steers over three one week periods in a 3${\times}$4 incomplete latin square design where steers served as blocks and feeds as treatment. The objectives of the study were to investigate in situ DM, N and P degradability characteristics of feeds in order to assess availability of these nutrients by ruminants. In each period, all feeds were incubated in quadruplets (corn silage in triplicates) in the rumen of each steer in a reverse order for 3, 6, 9, 12, 18, 24 and 48 h. The DM 'a' fraction was higher and lower (p<0.001) in corn silage and rice bran respectively. Although corn silage contained the lowest (p<0.01) DM 'b' fraction, flaked corn contained the highest. Rate of DM degradation of flaked corn and corn silage were half (p<0.05) of the rate of DM degradation of either rice bran or soybean meal. Potential or effective DM degradability (p<0.05 to 0.001) at various passage rates were the lowest for rice bran and the highest for soybean meal. Corn silage N 'a' and 'b' was the highest and lowest, respectively (p<0.01). N 'c' of corn silage and rice bran was higher (p<0.001) than other feeds. Potential N degradability was the lowest in flaked corn (p<0.05). P 'a' was high (p<0.01) for corn silage and rice bran. P 'b' fraction was very high (p<0.001) in soybean meal but was absent in corn silage. Availability of DM (p<0.01 or 0.001), N (p<0.001) and P (p<0.05) differed between feeds at various passage rates except P availability at k=0.02 per h (p>0.05). The results demonstrate that the availability of DM, N and P by ruminants depends on feed as well as categories of animal.

The Influence of Emotional Labor, Job Stress, and Burnout on Turnover Intention of Care Worker's at Long-Term Care Hospitals (요양병원 요양보호사의 감정노동, 직무스트레스 및 소진이 이직의도에 미치는 영향)

  • The purpose of this study was to identify the extent of and relationships among emotional labor, job stress, burnout and turnover intention and the factors influencing the turnover intention of care workers at long-term care hospitals. The study was approved by KUIRS, and the data were collected from August 12 to September 30, 2015, and analyzed using the SPSS WIN 18.0 program. The participants of this study were 200 care workers at ten different long-term care hospitals in B city and G province. Data were collected through self-reported questionnaires and analyzed by descriptive statistics, the t-test, ANOVA, Pearson's correlation coefficients, and Stepwise multiple regression. The turnover intention was significantly correlated with emotional labor (r=.372, p<.001), job stress (r=.471, p<.001), and burnout (r=.411, p<.001). In the multiple regression, job stress (${\beta}=.290$, p<.001), burnout (${\beta}=.239$, p<.001), type of employment (${\beta}=.185$, p<.001), and emotional labor (${\beta}=.157$, p=.017) were identified as factors influencing the turnover intention. These factors explained 32.6% of the total variance in the turnover intention. Based on the results, in order to reduce the turnover intention, the improvement of work environment and clear job-descriptions are needed to reduce the job stress, and the development and implementation of proactive psycho-social interventions are necessary to reduce burnout and emotional labor in care workers at long-term care hospitals.

A Study on the Knowledge, Attitude, and Practice of Public Health Management In Industry in Kyeongnam Province (경남지역 일부 근로자들의 산업장에서의 건강관리에 대한 지식, 태도, 실천)

  • The purpose of the study is to provide basic data on the public health management in the industrial work places, and some implications on the public health education related to workers self care capacity of their own health. To achieve this purpose a questionnaire was provided to 332 workers during February 11-28, 1993. This was do in Changwon-shi Geojeoi-kun, Chungmoo-shi, Jinjoo-shi, of Kyeongnam province. Through the analysis of the survey results, a relationship was deduced between worker's general characteristics and basic elements of health management in the work place. The main results of this study can be summarized as follows : 1. In the knowledge area special health screening received the highest score 3.18. While the educational program scored the lowest .85. In the Attitude area the highest score was achieved in the affirmative attitude to the time consumed by health checks (3.28). The lowest by the management of health checks(1.53). In the Practice area, participation in health checking is the most active (3.44) , and public health education is the least active (0.95). 2. The differences of knowledge by workers' general characteristics is statistically significant in the work sector (P<.05), age (P<.05), sex (p<.001), marital status (P<.001), and prior career(P<.01). But in Attitude general characteristics are not significant factors. In the Practice area, did have a statistical significance, work sector (p<.001), age (p<.001), sex (P<.05), marital status (p<.001) and work career (P<.001). 3. The total difference by workers' general characteristics has a statistical significance only in the case of marital status (P>05). The note of married workers is higher than that of single workers. 4. The correlation between any two of Knowledge, Attitude, and Practice do have a statistical significance (p<.001).

A Study on Quality Characteristics of Doraji (Platydodon grandiflorum) Yanggeng using by Different Pre-treatment Methods and Amounts Adding Levels of Doraji (전처리 방법과 비율을 달리하여 제조한 도라지 양갱의 품질 특성에 관한 연구)

  • The mechanical characteristics (texture and color) and consumer preference of Doraji Yanggengs using different pre-treatments and added levels of Doraji were presently evaluated. Three kinds of Doraji Yanggengs were made with differing amounts of Doraji powder (DPY; 30, 50, or 70 g), ground raw Doraji (GRDY; 100, 200, or 300 g), and Doraji juice (DJY; 100, 200, or 300 g). Texture characteristics (hardness, adhesiveness, springiness, chewiness, gumminess, cohesiveness) and color values (lightness, redness, yellowness) were measured three times in three experiments conducted with each formulation. Preference was measured with a nine-point hedonic scale for Yanggengs once every three experiments. Consumer preference was measured with color, flavor, taste, appearance, texture, and overall acceptability. Significant differences according to the amount of Doraji powder were evident with DPY for hardness (p<0.001), adhesiveness (p<0.05), and cohesiveness (p<0.001). Hardness (p<0.001), adhesiveness (p<0.001), chewiness (p<0.01), and gumminess (p<0.01) of 100 g GRDY displayed higher scores than 200 g and 300 g GRDY, but displayed significantly lower scores in cohesiveness (p<0.01). Hardness (p<0.001), adhesiveness (p<0.001), chewiness (p<0.001), gumminess (p<0.01), and cohesiveness (p<0.05) of DJY showed significant differences according to amount of Doraji juice. The highest score in a preference test among DPYs was evident for 30 g DPY. There were no significant differences in preference among GRDYs and DJYs. Color, flavor, taste, appearance, texture, and overall acceptability were significantly (p<0.01) correlated in all Yanggengs. DJY was preferred more than DPY and GRDY. Women in their forties and fifties preferred Yanggeng more when compared to women in their twenties and thirties.
