Personal Protective Equipment Availability and Utilization Among Interventionalists

  • Objective: This study explored personal protective equipment (PPE) availability and PPE utilization among interventionalists in the catheterization laboratory, which is a highly contextualized workplace. Methods: This is a cross-sectional study using mixed methods. Participants (108) completed a survey. A hyperlink was sent to the participants, or they were asked to complete a paper-based survey. Purposively selected participants (54) were selected for individual (30) or group (six) interviews. The interviews were conducted at conferences, or appointments were made to see the participants. Logistic regression analysis was performed. The qualitative data were analyzed thematically. Results: Lead glasses were consistently used 10.2% and never used 61.1% of the time. All forms of PPE were inconsistently used by 92.6% of participants. Women were 4.3 times more likely to report that PPE was not available. PPE compliance was related to fit and availability. Conclusions: PPE use was inconsistent and not always available. Improving the culture of radiation protection in catheterization laboratories is essential to improve PPE compliance with the aim of protecting patients and operators. This culture of radiation protection must include all those involved including the users of PPE and the administrators and managers who are responsible for supplying sufficient, appropriate, fitting PPE for all workers requiring such protection.

Leading for Safety: A Question of Leadership Focus

  • Background: There is considerable evidence that leadership influences workplace safety, but less is known about the relative importance of different leadership styles for safety. In addition, a leadership style characterized by an emphasis and a focus on promoting safety has rarely been investigated alongside other more general leadership styles. Methods: Data were collected through a survey to which 269 employees in a paper mill company responded. A regression analysis was conducted to examine the relative roles of transformational, transactional (management-by-exception active; MBEA), and safety-specific leadership for different safety behavioral outcomes (compliance behavior and safety initiative behaviors) and for minor and major injuries. Results: A safety-specific leadership contributed the most to the enhanced safety of the three different kinds of leadership. Transformational leadership did not contribute to any safety outcome over and above that of a safety-specific leadership, whereas a transactional leadership (MBEA) was associated with negative safety outcomes (fewer safety initiatives and increased minor injuries). Conclusion: The most important thing for leaders aiming at improving workplace safety is to continuously emphasize safety, both in their communication and by acting as role models. This highlights the importance for leadership training programs aiming to improve safety to actually focus on safety-promoting communication and behaviors rather than general leadership. Furthermore, an overly monitoring and controlling leadership style can be detrimental to attempts at achieving improved workplace safety.

뉴질랜드 어획량 균형 제도: 시행전략모델 (New Zealand's Catch Balancing Regime: A Model of Enforcement Strategies)

  • 본 연구는 양도성개별할당량(individual transferable quota:ITQ)제도하에서 어획량 보고 시스템과 관련된 뉴질랜드의 독특한(unique) 어획량균형제도(Catch Balancing Regime)인 추정가치(deemed value) 시스템의 시행전략에 관한 한 이론적 모델을 제시한다. 본 논문에서 제시된 시행전략모델은 현시원칙(revelation principle)과 유인합치성(incentive compatibility)이론에 근거한 이론적 실증 분석에 기초한다. 분석 결과, 뉴질랜드에서 시행되고 있는 추정가치 시스템은 ITQ 제도하에서 지금껏 규제의 사각지대로 여겨졌던 부수어획(bycatch)과 불법투기(discarding), 그리고 남획(overfishing)과 같은 난제들을 통제 조정할 수 있는 가치 있는 제도적 메커니즘을 가지고 있는 것으로 나타났다. 다시 말해, 분석된 추정가치 시스템에 의한 어획량균형제도의 시행은 우선, 쿼타 보유자들의 어획보고에 대한 규제순응(regulatory compliance)을 향상시키고, 둘째, 어업의 감시 감독(surveillance monitoring)에 대한 비용을 안정화시키며, 셋째, 어자원의 지속적 효율적 관리에 대한 긍정적 유인을 제공한다.

Comparative evaluation of nasal and alveolar changes in complete unilateral cleft lip and palate patients using intraoral and extraoral nasoalveolar molding techniques: randomized controlled trial

  • Objectives: Cleft lip and palate is a common congenital anomaly that impairs the aesthetics, speech, hearing, and psychological and social life of an individual. To achieve good aesthetic outcomes, presurgical nasoalveolar molding (NAM) has become important. Currently, the intraoral NAM technique is widely practiced. Numerous modifications have been made to intraoral NAM techniques, but the original problem of compliance leading to discontinuation of treatment remains unsolved. Therefore, the present study compared an extraoral NAM technique with the intraoral NAM technique. Materials and Methods: Twenty infants with complete unilateral cleft lip and palate were included and divided into two equal groups. Group A received the intraoral NAM technique, and Group B received the extraoral NAM technique. Pre- and postoperative extraoral and intraoral measurements were recorded. Results: Groups A and B did not differ significantly in any extraoral or intraoral parameter. Conclusion: The extraoral NAM technique is as effective as the intraoral NAM technique in achieving significant nasal and alveolar changes in complete unilateral cleft lip and palate patients. Additionally, it reduces the need for frequent hospital visits for activation and the stress associated with the insertion and removal of the intraoral NAM plate, thereby improving compliance.

직불제 이행점검 모니터링을 위한 원격탐사 기법 활용성 평가 (Assessment of the Utility of Remote Sensing Techniques for Monitoring Compliance with Direct Payment Programs)

  • 공익직불제란 농업활동을 통해 환경보전, 식량의 안정적 공급, 농촌 공동체 유지 등 공익기능을 수행하는 것을 전제로 전부 재정을 통해 농업생산자와 농촌 주민들에게 직불금을 지급하는 제도를 의미한다. 작물 재배면적과 생산량 등 작황을 과학적으로 추정하는 것은 농산물 수급 조절과 연계된 농업정책 수립에 기반이 되므로 국가 경제에 미치는 영향으로 인해 중요성이 점차 증가하고 있다. 공익직불제 이행점검과 관련하여 현장조사를 통한 전수조사는 예산 및 소요시간 대비 매우 낮은 효율성을 보여준다. 이러한 직불제 이행점검 대상 면적의 증가와 현장 조사의 다양한 어려움은 현행 이행점검의 효율화와 대응책 마련 필요성을 요구하고 있다. 이에 대한 방안으로 전체적인 현장 방문 없이도 정책에 필요한 정보를 신속하게 취득하게 해 주는 원격탐사 기술의 적용과 공간정보 활용 방법은 효율적인 이행점검 방 법으로 논의되고 있다. 따라서 본 연구는 공익직불사업 이행점검 시 업무효율성 향상을 위한 위성영상 활용 가능성을 평가하였다. 김제시(평지)와 합천군(산지)을 대상으로 영농기에 촬영된 위성영상의 식생지수와 공간변이를 활용하여 농경지 경작 유무, 혼재지를 판별하고 그 결과를 현장조사 자료를 통해 검증하였다. 향후 발사 예정인 차세대 중형위성 4호(농림위성)의 경우 한반도를 3일에 촬영할 수 있는 높은 시간해상도와 공간해상도(5 m)를 바탕으로 공익직불제 이행점검 및 시계열 영상을 활용한 전략작물직불제(동계작물, 콩 등) 적용이 가능할 것으로 기대된다.

The Comparative Analysis of the Internal Control According to Economic Changes in Korean Companies

  • Prior to the 2000s, internal control had not been among the high priority issues in the management's agenda. Since then, however, it has become one of the hottest issues, and has received a significant attention as the means of improving the transparency, sustainability, and competitiveness of a company. The objectives of this paper are to examine if there has been any noticeable changes in the level of internal controls of Korean companies before and after the 2010, and to analyze the underlying drivers and issues thereto. Accounting manipulation and moral hazard were among the factors to cause the Korean financial crisis in 1997 and 2008. Since then, the capital market has had a strong pressure on Korean companies to enhance the transparency of management and accounting while the government has made the laws, requirements, and recommendations to alleviate the moral hazard problems of management and enhance the accounting transparency. Both market and government have driven companies to put more priority on the reliability of financial reporting and the compliance of applicable laws and regulations. Thereby, the market and governmental forces has led companies to enhance the level of internal controls which contribute to the reliability of financial reporting and the compliance The pressure on companies to enhance the level of internal controls may be different across industries. The capital market and government experiencing the severe financial crisis in 1997 and 2008 put even more pressure on financial companies such as banks to upgrade the reliability of financial reporting and the compliance of regulations to the global level than on non-financial companies. A survey is performed on the changes in the level of internal controls of 54 major companies consisting of 10 financial and 44 non-financial companies in Korea. The survey results show that the average level of internal controls of Korean companies has noticeably improved and that the change in the level of control environment factor is higher than that of IT control factor. The analysis on the industry differences shows that financial companies increased the level of control environment factor more than non-financial companies did while non-financial companies upgraded the level of IT control factor more than financial companies did relatively. Among internal control categories, the most improved area since the economic crisis is "Risk Assessment." The global best practices for risk management have been developed primarily in the financial industry and then spread to other industries. The general level of control practices of Korean companies has been improving significantly, but still appears below the global advanced practices.

실험적 급성호흡부전에서 호기말양압에 의한 폐유순도와 산소운반의 변화 및 상관관계 - 호흡부전의 기전에 따른 차이 - (Relationship of Compliance and Oxygen Transport in Experimental Acute Respiratory Failure during Positive End-Expiratory Pressure Ventilation)

  • 연구배경 : 성인성 호흡곤란증후군에서 호기말양압(PEEP)이 저산소혈증을 호전시키는 기전은 감소된 기능적잔기량의 증가에 따른 생리적단락의 감소이며 어느 수준 이상의 PEEP에서는 심박출량이 유의하게 감소해 산소운반은 오히려 감소하는 것으로 알려져 있다. 산소운반이 최대인 PEEP 수준을 결정하는 비관혈적 지표로 폐유순도(compliance)가 보고되었으나 이의 유용성에 대해서는 논란이 많다. 한편 급성 폐동맥색전증은 성인성 호흡곤란증후군과는 달리 기능성 잔기량의 감소가 유의하지 않은 질환이며 따라서 PEEP의 적용에 따른 환기 및 혈류역학의 변화도 다를 것으로 추측된다. 저자들은 성인성 호흡곤란증후군과 급성 폐동맥색전증에서 총정적유순도(total static compliance)가 산소운반을 최대로하는 PEEP 수준의 결정에 유용한 지표로 이용될 수 있는가를 검정해보고, 성인성 호흡곤란증후군과는 저산소증의 기전이 다른 급성 폐동맥색전증에서 PEEP 적용에 따른 환기 및 혈역학의 변화에 대한 기초자료를 제공하고자 본 연구를 시행 하였다. 방법 : 한국산 잡견 14마리중 7마리에는 oleic acid 0.07ml/kg를 정주해 성인성 호흡곤란증후군을 유발하고, 나머지 7마리에는 자가혈병을 이용해 급성 폐동맥색전증을 유발한 후 PEEP 수준을 0, 5, 8, 12, 15 cm$H_2O$로 증가시켜가며 환기 및 혈역학의 제지표를 측정하였다. 결과: 1) PEEP수준의 증가에 따른 환기 및 혈역학의 변화 ARDS군에서 PEEP수준의 상승에 따라 동맥혈 산소분압및 동맥혈 이산화탄소분압은 증가하였고, 생리적단락과 심박출량은 감소하였다 (p<0.05). 급성 폐동맥색전증군에서도 PEEP수준의 상승에 따라 생리적단락 및 심박출량은 감소하였고, 동맥혈 산소분압 및 동맥혈 이산화탄소분압은 증가하였다 (p<0.05). PEEP수준 1 cm$H_2O$ 상승에 따른 변화로 심박출량의 감소는 급성 폐동맥색전증군에서 ARDS 군에 비해 유의하게 컸고, 동맥혈 산소분압, 동맥혈 이산화탄소분압, 생리적단락 및 폐혈관저항의 변화는 두군간에 유의한 차이가 없었다. 2) 산소운반과 총정적유순도 ARDS군에서 총정적유순도와 산소운반은 PEEP 적용후 증가해 5 cm$H_2O$에서 가장 컸으며 (p<0.05), 급성 폐동맥색전증군에서는 PEEP 적용후 총정적유순도와 산소운반이 감소하였다(p<0.05). 양군 모두 산소운반이 최대인 PEEP 수준에서 총정적유순도도 최대치를 기록하였다. ARDS군에서 산소운반의 변화와 총정적유순도의 변화사이에는 유의한 상관관계가 있었고 (r=0.86, p<0.01), 급성 폐동맥색전증군에서도 산소운반의 변화와 총정적유순도의 변화사이에 약한 상관관계가 있었다(r=0.60, p<0.01). 결론 : 이상의 결과로 ARDS와 급성 폐동맥색전증에서 PEEP의 적용시 산소운반을 최대로 유지하기 위한 PEEP수준의 결정에 총정적유순도가 유용한 지표로 이용될 수 있을것으로 생각된다.

신규면허간호사 공급량 증가가 의료기관 간호사 확보수준에 미친 영향과 정책적 함의 (Impact of Increased Supply of Newly Licensed Nurses on Hospital Nurse Staffing and Policy Implications)

  • Purpose: This study aimed to analyze the impact of increasing the supply of newly licensed nurses on improving the hospital nurse staffing grades for the period of 2009~2014. Methods: Using public administrative data, we analyzed the effect of newly licensed nurses on staffing in 1,594 hospitals using Generalized Estimating Equation (GEE) ordered logistic regression, and of supply variation on improving staffing grades in 1,042 hospitals using GEE logistic regression. Results: An increase of one newly licensed nurse per 100 beds in general units had significantly lower odds of improving staffing grades (grades 6~0 vs. 7) (odds ratio=0.95, p=.005). The supply of newly licensed nurses increased by 32% from 2009 to 2014, and proportion of hospitals whose staffing grade had improved, not changed, and worsened was 19.1%, 70.1%, and 10.8% respectively. Compared to 2009, the supply variation of newly licensed nurses in 2014 was not significantly related to the increased odds of improving staffing grades in the region (OR=1.02, p=.870). Conclusion: To achieve a balance in the regional supply and demand for hospital nurses, compliance with nurse staffing legislation and revisions in the nursing fee differentiation policy are needed. Rather than relying on increasing nurse supply, retention policies for new graduate nurses are required to build and sustain competent nurse workforce in the future.

전화모니터링이 심부전 환자의 증상경험과 자기관리 이행정도에 미치는 효과 (Effect of a Telephone Monitoring on Self-management & Symptom Experiences in Patients With Heart Failure)

  • Purpose: The main purpose of this study was to examine the effectiveness of a standardized telephone monitoring intervention in addressing the symptom experience and improving self-management ability in patients with heart failure. Methods: A non-equivalent control group pre-post test design was used. There were 17 patients in the experimental group, and 16 in the control group. According to the protocol, patients in the experimental group received 15 to 30 minute-telephone monitoring four times, once a week for 4 weeks. Data were analyzed by ${\chi}^2$-test, Mann-Whitney U test. Results: 1) The experimental group showed a significant increase in compliance with self-management compared to the control group. 2) There was a significant decrease in degree for 3 symptoms(DOE, PND, & continuing fatigue) in the experimental group, after telephone monitoring. However, the experimental group did not show significant decrease in the degree of the total symptom experiences. Conclusions: The results of this study provide evidence that standardized telephone monitoring is effective in relieving symptom experience and improving self-management in patients with heart failure over the course of telephone monitoring.

외국인 근로자의 취업생활 만족도 제고를 통한 기업 내 정착지원방안 (Establishing Employee Support Program for Foreign Laborers at Company Level by Improving Working Life Condition)

  • The purpose of this study is to draw employee support program at the company level through field survey of foreign laborers' satisfaction level of working life. To analyze the factors that determine working life satisfaction, regression analysis was conducted. Satisfaction level of working hours(t=2.469), living environment(t=2.140), post offices/banks (t=3.024) showed a statistically significant. Also these variables, working hours, living environment, post office/banks are verified to be very important for improving the level of their working life. Through these findings we can understand that employee support activities, providing user-guide of bank and post office, compliance with standards for working hours, improvement of the living conditions (accommodation, meals, etc.), the leisure time program support, the children care support and active utilization of foreign workers and strengthen the educational program, are necessary to improve the satisfaction level of foreign laborers' working life in Korea. It is recommended that managers need to recognize that careful consideration of living and working condition could contribute to stable settlement and increase working life satisfaction for the foreign workers at company level.
