Updates of Evidence-Based Nursing Practice Guideline for Prevention of Venous Thromboembolism (근거기반 정맥혈전색전증 예방 간호실무지침 개정)

  • Purpose: This study aimed to update the previously published nursing practice guideline for prevention of venous thromboembolism (VTE). Methods: The guideline was updated according to the manuals developed by National Institute for Health and Care Excellence (NICE) and Scottish Intercollegiate Guidelines Network (SIGN), and a Handbook for Clinical Practice Guideline Developer Version 10. Results: The updated nursing practice guideline for prevention of VTE was consisted of 16 domains, 46 subdomains, and 216 recommendations. The recommendations in each domain were: 4 general issues, 8 assessment of risk and bleeding factors, 5 interventions for prevention of VTE, 18 mechanical interventions, 36 pharmacological interventions, 36 VTE prevention starategies for medical patients, 25 for cancer patients, 13 for pregnancy, 8 for surgical patients, 7 for thoractic and cardiac surgery, 16 for orthopedic surgery, 10 for cranial and spinal surgery, 5 for vascular surgery, 13 for other surgery, 3 educations and information, and 2 documentation and report. For these recommendations, the level of evidence was 32.1% for level I, 51.8% for level II, and 16.1% for level III according to the infectious diseases society of America (IDSA) rating system. A total of 112 new recommendations were developed and 49 previous recommendations were deleted. Conclusion: The updated nursing practice guideline for prevention of VTE is expected to serve as an evidence-based practice guideline for prevention of VTE in South Korea. It is recommended that this guideline will disseminate to clinical nursing settings nationwide to improve the effectiveness of prevention of VTE practice.

  • In paddy rice farming, water management is a critical task. To suppress weed emergence during the early stages of growth, fields are deeply flooded, and after transplantation, the water level is reduced to promote rooting and stimulate stem generation. Later, water is drained to prevent the production of sterile tillers. The adequacy of water supply is influenced by various factors such as field location, irrigation channels, soil conditions, and weather, requiring farmers to frequently check water levels and control the ingress and egress of water. This effort increases if the fields are scattered in remote locations. Automated irrigation systems have been considered to reduce labor and improve productivity. However, the net income from rice production in 2022 was about KRW 320,000/10a on average, making it financially unfeasible to implement high-cost devices or construct new infrastructure. This study focused on developing an IoT-Based irrigation valve that can be easily integrated into existing agricultural infrastructure without additional construction. The research was carried out in three main areas: Firstly, an irrigation valve was designed for quick and easy installation on existing agricultural pipes. Secondly, a power circuit was developed to connect a low-power Cat M1 communication modem with an Arduino Nano board for remote operation. Thirdly, a cloud-based platform was used to set up a server and database environment and create a web interface that users can easily access.

An Exploratory Study on Educational Significance and Environment of Flipped Learning (Flipped learning의 의의 및 교육환경에 관한 탐색적 연구)

  • Recently, there has been an effort to convert standardized educational system to customized one considering students' individual differences and aptitude. Flipped learning has been regarded as a teaching method fitting to this movement and has gained much interest by educators. Despite its increasing interest, its studies fall short in the quantity above all. This study investigated the concept of flipped learning and explored its educational value in terms of student' and teacher' perspectives. The study also examined the requirements to apply flipped learning to its practice. Flipped learning was found to accomplish mastery learning and to promote interaction between teacher and students and also peer-to-peer interaction. It was expected to reduce the burden of teacher's duties and secure his or her interest in students and the tutorial time for individual students. Essential elements in its application in practice included students' voluntary participation, the consent of their parents and support from their school and government. In addition, teachers' professional development was found to be important to improve their understanding and application of flipped learning in practice.

  • Education is essential for specialists in charge terminal care of cancer patients. In the second 10-year plan to conquer cancer under the Cancer Control Act, the Korean government announced a plan to train more hospice and palliative care (HPC) specialists based on quantitative HPC expansion. Specifically, the government aims to develop e-learning programs to foster social workers in HPC, following those offered to doctors and nurses. In HPC, social workers have served a vital role in helping patients overcome psychological and social issues. As professionals, they have carried out their responsibilities and played their part in the field to help HPC to take root and be institutionalized in Korea. To date, it has been difficult to obtain practical knowledge and skills for social workers due to the lack of systematic training program. Development of an e-learning program for social workers, as proposed in this study, should strengthen social workers in charge of terminally-ill cancer patients in terms of their identity, expertise, and practical skills in clinical setting and improve their access to education. We also hope the program to be further developed by the government by introducing an education system that offers refresher courses to guarantee social workers' continued expertise through.

BIM-based Repair&Replacement (R&R) Cost Estimating Process (BIM기반 건축물 수선교체비 산정 프로세스)

  • In a construction project, the portion for maintenance costs for a building is considerable compared to the initial construction cost. As such, Life Cycle Cost (LCC) analysis is being increasingly utilized to assess the design value of engineering work in Korea. Additionally, the Public Procurement Service in Korea announced that it will be mandatory for all domestic construction projects to adopt BIM. Furthermore, the paradigm for architectural design has shifted from 2D to 3D, and to BIM, which includes a data management system. Within this background, however, there is currently no adequate BIM-based LCC analysis software and the requirements of cost estimation for repair and replacement cost for a building is not completely adequate in BIM tools such as Revit and Archicad. Therefore, this study suggests a process of cost estimation for repair and replacement (R&R) cost based on IFC data. First, we analyzed existing R&R criteria and defined BIM-based requirements when calculating R&R costs. These requirements were extracted from relevant IFC data. Subsequently, this was saved to a database and a BIM-based database was built for R&R cost estimation. Finally, this database was connected with external databases such as R&R Criteria DB and Cost Information DB to calculate R&R costs. This process is expected to improve upon the traditional process of cost estimation of R&R cost by applying a BIM model. The proposed process can contribute to a further standardizing of BIM-based LCC analysis thru application to initial construction costs, energy costs, and other maintenance costs.

Efficacy of Pulsatile Hemodialyzer to Experimental Renal Failure in Dog (실험적으로 신부전을 유발시킨 개에서 박동형 혈액투석기의 효능)

  • The purpose of the present study was to compare pulsatile type(Twin Pulse Life Support; TPLS) with rotary type hemodialyzer(AK95) in order to reduce the dialysis time and to improve dialysis effect. Three healthy dogs(about 30 kg BW) were used. Experimental renal failure was induced by the ligation of bilateral renal artery. A pair of catheters were implanted in jugular vein for dialysis. Daily investigated parameters included clinical signs such as vomiting, fecal appearance and activity, and also laboratory data such as PCV, TP, BUN and creatinine. Hemodialysis was started above 90 mg/100 ml BUN level and, laboratory data were measured every an hour for 4 hours. Heparin was administered 300 IU/Kg before dialysis and 150 IU/Kg via IV route every 90 minutes during dialysis. Clinical signs after induction renal failure were shown severe vomiting, anorexia, diarrhea, mucous feces, ataxia, dilated pupil and episcleral hyperemia. The average of BUN value decreased hourly $99{\pm}12.1,\;84{\pm}12.2,\;72{\pm}8.0,\;58{\pm}7.1,\;48{\pm}5.2,\;and\;39{\pm}3.2mg/100ml$ by hemodialysis. The average of creatinine value decreased $7.8{\pm}0.61,\;6.4{\pm}0.40,\;5.3{\pm}0.42,\;4.5{\pm}0.23,\;4.0{\pm}0.41,\;and\;3.4{\pm}0.42mg/100ml$ according to hemodialysis an hour. There are not significantly differences BUN, creatinine, PCV and TP values between pulsatile and rotary type hemodialysis. These results suggested that effects of hemodialysis with Pulsatile type(TPLS) are not significantly difference as compared with hemodialysis of rotary type(AK95). Further research is needed in order to estimate the influence of cardiovascular and pulmonary system in hemodialysis of pulsatile type.

Chronic Physical Comorbidities and Total Medical Costs in Patients with Schizophrenia (조현병 환자의 동반 만성신체질환 현황 및 총 의료비용에 관한 연구)

  • Objectives : This study tried to explore the prevalence of chronic physical diseases in patients with schizophrenia and its effects on total medical costs. Methods : The Health Insurance Review and Assessment Service data in 2014-2015 was employed. Only the injuries and diseases, identically diagnosed 3 times or more as a major or minor injury and disease, were classified into chronic physical diseases to improve data accuracy. Total medical costs included out-of-pocket and insurer's costs from health care system perspective. Results : It was shown that 24.5%, 17.3% and 23.4% of schizophrenia patients had one, two and three or more chronic physical diseases, respectively. There was a high prevalence of not only metabolic, but also musculoskeletal, diseases in those patients. The amount of 2015 total medical costs of patients with schizophrenia in 2014 was about 1.08 trillion won. The factors affecting the costs included sex, age, number of chronic physical diseases, and health insurance status. Conclusions : It is considered that clinical practice guidelines based on personal diseases may not sufficiently solve the problems for comorbidities in schizophrenia patients. Accordingly, it should be required to develop models for new types of medical systems capable of treating and caring varied illnesses at the same time.

Evacuation safety analysis depending on the type of subway platform and ticket barrier (지하철 승강장과 개찰구 유형별 대피안전성 분석)

  • With the rapid development of modern society, subway has become one of the most typical urban transport systems. Since fire accident occurred at Daegu subway in 2003, importance of life safety and disaster prevention have been widely recognized and many studies have been carried out. As a result of these studies, fire-retardant and non-combustible interior material and platform screen door with passenger guide indication device have been developed, but studies on a subway evacuation criteria have been in a stalemate. Therefore, this study is intended to improve the subway evacuation standard. It is very difficult to take into account whole subway system, so a typological approach to a ticket was carried out referring to previous studies focused on a subway platform. this paper selected the most common subway platforms and estimated evacuation time among 10 platforms from previous studies and 8 from this study. As a result, evacuation time exceeded 6 minutes which is the guideline of existing standard. Therefore, it is necessary to update the standard for evacuation time and add supplementary conditions which can help establishing the measures for safety facilities and prevention measures.

The Effects of Inorganic Soil Amendment on Growth of Creeping Bentgrass(Agrostis palustris) in Golf Course (무기 토양개량제가 골프장 그린의 크리핑 벤트그래스 생육에 미치는 영향)

  • Soil amendments have been used to improve the physical and chemical condition of turf soil, which might optimize turfgrass growth in golf courses. This study was to investigate the effect of inorganic soil amendment (ZC) established in USGA root zone system on growth of creeping bentgrass, cv. 'Penncross' carried out from May to Dec. in 2005 at the nursery on Sinwon Golf Course. To analyze the effects of inorganic soil amendment on pH, specific gravity, infiltration rate, water content, soil hardness, root length, tiller density and dry weight were measured. pH was 6.7-6.8, specific gravity of sand (S) 100% was 1.48 heavier than the other treatments (1.28-1.38). Infiltration rate with ZC 15% + peat moss (P) 5% + S 80% and ZC 10% + S 90% was faster than S 100%. Soil hardness of S 100% was the highest. Root length of creeping bentgrass of P 10% + S 90% (8.6-12.0 cm) was the longest. Tiller density with P 5% + S 95% was more 4-7 ea than other treatments in summer season. In growing season, however, ZC 10% + P 5% + S 85% was more 2-3 ea than others. Dry weight of creeping bentgrass treated by ZC 10% + P 5% + S 85% in summer season and P 5% + S 95% in growing season were heavier than other treatments. It is recommended to combine 5-15% inorganic soil amendment and peat moss 5% with sand in order to sustain soil balance.

Effects of Supplemental Squid Meal, Attractant, Herb or Lecithin in the Formulated Diets on Growth Performance in Juvenile Abalone (Haliotis discus hannai) (배합사료내 오징어분, 먹이유인물질, 한약제 및 lecithin의 첨가가 참전복의 성장 및 체성분에 미치는 영향)

  • An 18-week growth trial was conducted in flow-through aquarium system to investigate the effects of several additives in formulated diets on the growth and body composition of juvenile abalone (Haliotis discus hannai). Four replicate groups of abalone averaging 150 mg were fed one of four isonitrogenous ($33.8\%$) and isolipidic ($5.8\%$) diets containing $5\%$ squid meal, $0.5\%$ attractant, $1\%$ herb or $0.5\%$ lecithin. In addition, these formulated diets were compared to commercial diet. Survival rate and shell growth of abalone were not significantly affected by the different dietary additives and commercial diet (P>0.1). Body weight gain of abalone fed the diet containing herb was higher than those of abalone fed the diets containing squid meal or lecithin and commercial diet (P<0.1). Moisture, protein, lipid and ash contents of soft body were not influenced by dietary additives and commercial diet (P>0.1). The results suggest that herb as additive in this formulated diet can improve weight gain of abalone.
