A primary goal of periodontal therapy is the creation of root surfaces that are free of plaque and calculus. It is not yet to be determined whether it is desirable to have a smooth root surface after treatment. It is also not clear what degree of roughness result from different instruments. In the present study various instruments for root surface debridement were evaluated. 20 extracted teeth were utilized, and the teeth were treated with one of the following instrument: Gracey curette, Perio Clean, and piezo ultrasonic device(Setlec, P Max) with general scaler tip, curette-like tip, and diamond tip. 3 dimensional planimetric device(Accura) was used to evaluate the average surface roughness. It was demonstrated hand and power-driven instruments did not have a significant difference in roughness of the root surface following instrumentation. And ultrasonic scaler tip tended to make a most smooth surface than other instruments. The possible reasons of the result were discussed.
Purpose: The purpose of this study is to find the effect of rare earth magnet's magnetic field of to the osteoblast around the implant by the means of observation number, and distribution around the implant which is connected to the permanent magnet but not, counted and compared by the number of cells attached to the surface of the implant. Material and method: The permanent magnets, made in the healing cap form, were connected to the implant future, and placed on the culture plate, The osteoblast-like cell: MC3T3-E1 were used for cell culture. As the control group, the implant were connected to normal healing cap, and cultured in the same conditions. 48 hours later, using inverted microscope, the number and distribution of osteoblast around the implant were observed, and 72 hours later, the number of the cells attached to the implant were counted. Results: As a result, the implant connected to the permanent magnet had proved to have a more concentrated cell distribution rate than the control group. The implant connected to the permanent magnet, neck area : which has about 10 gauss magnetic force, had more cells than apex area. The implant connected to the permanent magnet had proven to attach to the osteoblast more productively than control group's implant. Conclusions: This research showed that the magnetic field of the permanent magnet affected the distribution and growth rate of the osteoblast around the implant. In order to support this study, it also had need to monitor the progress of the permanent magnet specifically shown on the neck area, which has10 gauss magnetic force. So after additional research on the distribution and attachment of the cells, and further more, on bone formation, it will be concluded that the clinical applications ,such as immediate loading of implant treatment are possible.
This study aims to investigate the clinical outcome following treatment of peri-implantitis lesions. Five subjects with 7 implants were treated with surgical approach. Four subjects with 6 implants were initially treated with non-surgical approach or hygiene control. However, inflammation was not resolved and more bone loss was found. Therefore, surgical treatment was performed. After surgical exposure of the defect, granulation tissue was removed and implant surface was treated using tetracycline and chlorhexidine. Then, the flaps were sutured. The wound healing was performed in a non-submerged mode. The present finding demonstrates stable results without progression of bone loss. In one subject, deep V shaped bone defect was filled with bone substitute (ICB, CanCellous Bone, Rockey Mountain Tissue Bank, USA), and resorbable membrane (Lyoplant$^{(R)}$, B.Braun Aesculap AG, Germany) was placed over the grafted defect and healing abutment was connected. However, the inflammation was not resolved and more bone loss was found. At one month after regenerative surgery, the implant was removed.
A titanium based screw shaped dental implant was first introduced by Branemark and a treatment protocol where the restoration of edentulous area by connecting abutment after the osseointegration of the titanium surface of the implant and surrounding bone structure has been proposed. Although this protocol is widely accepted as a standard up to date, the healing duration of 3-6 months as well as the need for provisional prostheses during this period present as a major drawback. Immediate loading has been accomplished through the advent of various implant designs, enforced surface treatments, diverse forms of abutment, and delicate surgical techniques together with the increase in demand from the patients. The success rate of the immediate loading technique has been first reported as 85.7% by Dr. Schnitman in 1990 which recently has been reported up to 100% in the case of immediate loading in single tooth by Dr. Kan. To ameliorate the success rate of immediate loading technique, selection of patients presenting a sound bone quality and quantity, acquiring primary stability through delicate surgical techniques and fabrication of prostheses which accounts for biological stabilities should all be taken into consideration. This presentation introduces the understanding of biological stability of immediate loading, various methods for measurement of stability and clinical cases regarding immediate loading technique.
Purpose: Titanium implants are widely used in the treatment of dentition defects; however, due to problems such as osseointegration failure, peri-implant bone resorption, and periimplant inflammation, their application is subject to certain restrictions. The surface modification of titanium implants can improve the implant success rate and meet the needs of clinical applications. The goal of this study was to evaluate the effect of the use of porous titanium with a chitosan/hydroxyapatite coating on osseointegration. Methods: Titanium implants with a dense core and a porous outer structure were prepared using a computer-aided design model and selective laser sintering technology, with a fabricated chitosan/hydroxyapatite composite coating on their surfaces. In vivo and in vitro experiments were used to assess osteogenesis. Results: The quasi-elastic gradient and compressive strength of porous titanium implants were observed to decrease as the porosity increased. The in vitro experiments demonstrated that, the porous titanium implants had no biological toxicity; additionally, the porous structure was shown to be superior to dense titanium with regard to facilitating the adhesion and proliferation of osteoblast-like MC3T3-E1 cells. The in vivo experimental results also showed that the porous structure was beneficial, as bone tissue could grow into the pores, thereby exhibiting good osseointegration. Conclusions: Porous titanium with a chitosan/hydroxyapatite coating promoted MC3T3-E1 cell proliferation and differentiation, and also improved osseointegration in vitro. This study has meaningful implications for research into ways of improving the surface structures of implants and promoting implant osseointegration.
Adequate root preparation in the treatment of periodontal disease often involves mechanical instrumentation to remove plaque, calculus perhaps contaminated cementum. Although meticulous scaling and root planing may remove some cementum, the use of aggressive root planing to remove cementum does not appear warranted. So ultrasonic device and rotary instrument appear to be replacing hand instrument. But it is not clear those instruments make smooth root surface as hand instrument. The roghness of the root surface were evaluate with SEM following instrumentation with Gracey curette, Perio Clean and piezo ultrasonic device(Setlec) with various tip. 20 extracted teeth were used in vitro experiment, and 9 teeth of a patient destined for extraction for periodontal reasons were utilized in vivo experiment. It was demonstrated that hand curette created the smoothest surface, while diamond tip tended to roughen the root surface. But the hand curette, Perio Clean, and piezo ultasonic device with scaler tip tend to remove cementum completely. Piezo ultrasonic device with curette-like tip made the desirable smooth surface with partial removal of cementum.
Surface treatment play an important role in nucleating calcium phosphate deposition on surgical Ti implant. Therefore, the purpose of this study is to examine whether the precipitation of apatite on cp-Ti and Ti alloys are affected by surface modification in HCl and $H_2O_2$ solution. Specimens were then chemically treated with a solution containing 0.1 M HCl and 8.8M $H_2O_2$ at $80^{\circ}C$ for 30 mins, and subsequently heat-treated at $400^{\circ}C$ for 1 hour. All specimens were immersed in the HBSS with pH 7.4 at $36.5^{\circ}C$ for 15 days, and the surface was examined with XRD, SEM, EDX ana XPS. Also, pure Ti, Ti-6Al-4V and Ti-6Al-7Nb alloy specimens with and without surface treatment were implanted in the abdominal connective tissue of mice for 4 weeks. All specimens chemically treated with HCl and $H_2O_2$ solution have the ability to form a apatite layer in the HBSS which has inorganic ion composition similar to human blood plasma. The average thickness of the fibrous capsule surrounding the specimens implanted in the connective tissue was $38.57\;{\mu}m,\;62.27\;{\mu}m\;and\;45.64\;{\mu}m$ in the cp-Ti, Ti-6Al-4V ana Ti-6Al-7Nb alloy specimens with the chemical treatment respectively, and $52.20\;{\mu}m,\;75.62\;{\mu}m\;and\;66.56\;{\mu}m$ in the commercial specimens of cp-Ti, Ti-6Al-4V and Ti-6Al-7Nb without any treatment respectively. The results of this evaluation indicate that the chemically treated cp-Ti, Ti-6Al-4V ana Ti-6Al-7Nb alloys have better bioactivity and biocompatibility compared to the other metals tested.
Objective: The objective of the study was to evaluate the practicality and the validity of different surface treatments of self-drilling orthodontic mini-implants (OMIs) by comparing bone cutting capacity and osseointegration. Methods: Self-drilling OMIs were surface-treated in three ways: Acid etched (Etched), resorbable blasting media (RBM), partially resorbabla balsting media (Hybrid). We compared the bone cutting capacity by measuring insertion depths into artificial bone (polyurethane foam). To compare osseointegration, OMIs were placed in the tibia of 25 rabbits and the removal torque value was measured at 1, 2, 4, and 8 weeks after placement. The specimens were analyzed by optical microscopy, scanning electron microscopy (SEM), and energy dispersive X-ray spectroscopy (EDS). Results: The bone cutting capacity of the etched and hybrid group was lower than the machined (control) group, and was most inhibited in the RBM group (p < 0.05). At 4 weeks, the removal torque in the machined group was significantly decreased (p < 0.05), but was increased in the etched group (p < 0.05). In the hybrid group, the removal torque significantly increased at 2 weeks, and was the highest among all measured values at 8 weeks (p < 0.05). The infiltration of bone-like tissue surface was evaluated by SEM, and calcium and phosphorus were detected via EDS only in the hybrid group. Conclusions: Partial RBM surface treatment (hybrid type in this study) produced the most stable self-drilling OMIs, without a corresponding reduction in bone cutting capacity.
연구 목적: 본 연구는 임플란트 보철물과 인접치 사이의 식편압입을 호소하는 환자를 대상으로 각종 임상 자료를 분석하여 식편압입의 임상적 발현 양상을 알아보고자 하였다. 연구 재료 및 방법: 식편압입을 주소로 내원한 보철물 장착이 완료된 임플란트 환자 51명을 대상으로 설문조사와 구강검사를 시행하고 방사선 사진과 모형을 채득하여 식편압입과 관련이 있는 요소를 분석한 결과 다음과 같은 결과를 얻었다. 결과: 1. 식편압입은 하악(39.2%)보다 상악(60.7%)에서 많이 나타났다. 2. 식편압입은 자연치아가 임플란트 보철물의 원심 부위(13.7%)에 있는 경우보다 근심 부위(86.2%)에 있는 경우에 많이 나타났다. 3. 식편압입이 나타난 임플란트 보철물과 그 인접치 사이의 접촉점은 긴밀하지 않은 경우(94.2%)가 대부분이었다. 결론: 임플란트 보철물과 인접치 사이에 식편압입이 일어나지 않기 위해서는 임플란트 보철물과 인접치아가 적절한 긴밀도를 가지고 있어야 하겠다.
임플란트를 보조적으로 이용함으로써 후방 연장 국소의치에 부가적으로 유지와 지지를 얻는 임플란트 보조 국소의치(implant-assisted RPD, IARPD)에 대한 관심이 증가하고 있다. 본 증례는 장기간 후방연장 국소의치의 사용으로 인하여 심하게 기울어진 교합평면을 가진 부분 무치악 환자에서 상악의 고정성 보철 수복과 하악의 지르코니아 교합면을 가지는 임플란트 보조 국소의치를 이용하여 심미적이며 장기적으로 안정적인 교합평면을 가지도록 전악 수복하였으며, 기능적으로 만족스러운 결과를 얻었기에 이를 보고하고자 한다.
