A study on the plaque removal efficiency of new and worn toothbrushes (새 칫솔과 마모된 칫솔의 치태제거효율에 관한 비교연구)

  • 3개월 동안 사용한 마모된 칫솔의 마모 정도와 양상을 관찰하고, 새 칫솔과 마모된 칫솔의 잇솔질 전 ${\cdot}$ 후 치태제거효율을 single-use design으로 비교 ${\cdot}$ 평가하여 3개월 주기의 칫솔 교체 주기의 근거를 임상적으로 확인 해보고자 하였다. 치주적으로 건강한 치과 대학생 42명을 대상으로 설문지를 통해 잇솔질 습관을 조사하고, 3개월간 동일한 칫솔과 치약을 사용하게 하였다. 3개월 후 피시험자를 무작위로 두 군(I, II)으로 나누고, 치석제거술을 시행한 뒤 2주후에 내원하도록 하였으며 내원 전 48시간동안은 잇솔질을 하지 않도록 지시하였다. 2주후 I군은 새 칫솔을, II군은 마모된 칫솔을 사용하도록 하였으며 잇솔질 전 ${\cdot}$ 후에 각각 구강 내를 erythrosin으로 염색한 후 6개의 Ramfjord 치아의 plaque score를 Patient Hygiene Performance (PHP) index로 측정하였다. 2주간의 washout period 후에 다시 치석제거술을 시행한 뒤, I군이 마모된 칫솔을, II군은 새 칫솔을 사용하게 하여 동일한 방법으로 PHP index를 각각 측정하였다. 마모된 칫솔은 수거하여 brushing surface area의 면적으로 마모도를 평가하였다. 결과는 paired t-test와 Pearson's correlation analysis로 통계처리 하였다. 2명이 탈락하였고 잇솔질 전 ${\cdot}$ 후에 대한 전체 부위, 치간 부위, 변연치은 부위의 plaque score는 두 칫솔 모두 통계학적으로 유의성 있게 감소하였으며 (p<0.0001), 두 칫솔을 비교한 경우에는 새 칫솔이 마모된 칫솔보다 치태 감소량이 통계학적으로 유의성 있게 많았다 (p<0.0001). 칫솔의 마모도는 평균 50.6% 증가하였으며, 마모도 증가에 따른 치태 감소량에는 직선적인 상관관계가 있었으나 통계학적인 유의성은 없었다. (전체 부위 r=-0.58, p=0.72 / 변연치은 부위 r=-0.50, p=0.76). Single-use design에서 3개월 동안 마모된 칫솔은 치태제거 능력에 있어서 새 칫솔보다 덜 효율적이였다. 칫솔의 마모도는 구강 위생 관리에 영향을 미치는 중요한 요인이며, 마모된 칫솔은 정기적인 교체가 요구된다. 또한, 치간 부위를 포함한 변연치은 부위의 치태를 정확하게 평가할 수 있는 치태지수에 대한 연구가 필요하겠다.

Full-mouth rehabilitation of severely attrited dentition with missing posterior teeth: a case report using digital workflow with jaw motion tracking (심한 교모와 구치부 상실을 보이는 환자의 전악 수복: Jaw motion tracking과 digital workflow를 활용한 증례 보고)

  • Jaw motion tracking, which is introduced in recent case reports, is a method which records the patient's individualized pathway of the mandibular movements along with facebow transfer, and reproduces the information in the virtual space of computer-aided-design/computer-aided-manufacturing (CAD-CAM) software. In this present case, a collapse of the occlusal plane was observed, due the loss of posterior teeth for a long period. Full-mouth rehabilitation with an increase in the occlusal vertical dimension was planned. First, the patient's mandibular movements were recorded on the newly established jaw relation by jaw tracking, and this information was assembled with the patient's intraoral data to create a virtual patient. Implant planning and diagnostic wax-up was done on the virtual patient, leading the fabrication of the provisional prosthesis. On the newly established jaw relation with an increase in the occlusal vertical dimension, canine guidance of the provisional prosthesis was checked. Finally, the provisional prosthesis was carried out to the definitive prosthesis. Using the advantages of the technologies in the digital dentistry, the patient was satisfied with the function and the esthetics after the treatment.

Comparison of digitalized fabrication method for interim removable partial denture: case reports (두 가지 프린팅 방식으로 제작한 임시 가철성 의치의 비교: 증례 보고)

  • With the recent development of digital dentistry, fully digitalized methods for fabricating dentures, using intraoral scans and computer-aided design/computer-aided manufacturing (CAD-CAM), are getting popular. Digital methods have the advantage of simplifying the fabrication process in the clinic and laboratory, supplementing digital data. This case report shows a fully digital fabrication method for interim removable dentures in a patient with anterior tooth loss in which implant placement is impossible or delayed. Interim removable dentures were fabricated using two methods. One method is printing tooth and base parts separately and combining, and the other method is printing the whole denture at one time and coloring on the base part. Afterward, dentures were delivered and adaptation was evaluated using the triple scan technique. The extracted site was scanned intraorally (first scan) and the interim removable denture was digitally scanned both intraorally (second scan) and, after removal extraorally (Third scan). In both method, denture adaptation was shown favorable. We report this case report as both the patient and the operator were satisfied with a simplified process using a fully digital method in the clinic.

Mechanical analysis of the taper shape and length of orthodontic mini-implant for initial stability (교정용 미니임플랜트의 초기 안정성에 대한 원추형태와 길이에 관한 기계역학적 분석)

  • Orthodontic mini-implants, despite its usefulness as anchorage, have some limits such as loosening. Therefore, various shapes and lengths have been studied. The aim of this study is to analyze the shape and length of mini-implants mechanically. The shapes of mini-implants (1.6 mm, Dual Top, Jeil Medical Co., Seoul, Korea) were cylindrical and taper. The lengths of mini-implants were 6 mm and 8 mm. The tested groups were 5 groups (cylindrical 6 mm, cylindrical 8 mm, taper 6 mm, taper 8 mm and taper 8 mm modified whose thread is reduced from the middle to upper part). All were inserted and removed on the polyurethane foam with the torque measured. During insertion and removal, the taper shape needed higher torque than the cylindrical shape, and the 8 mm length than the 6 mm length (p<0.001). The taper 6mm group showed superior insertion torque (p<0.001) and similar removal torque to the cylindrical 8 mm group. The taper 8 mm modified group with gradually reduced threads, showed continuous high removal torque after the peak. The initial mechanical stability can be provided by the tapered shape and also, affected by length and thread design.

Evaluation of Insertion of torque and Pull-out strength of mini-screws according to different thickness of artificial cortical bone (다양한 교정용 미니 스크류의 인공 피질골 두께에 따른 삽입 토오크와 Pull-out 강도 비교)

  • Objective: The purpose of this study was to evaluate the mechanical performance of mini-screws during insertion into artificial bone with use of the driving torque tester (Biomaterials Korea, Seoul, Korea), as well as testing of Pull-out Strength (POS). Methods: Experimental bone blocks with different cortical bone thickness were used as specimens. Three modules of commercially available drill-free type mini-screws (Type A; pure cylindrical type, Biomaterials Korea, Seoul, Korea, Type B; partially cylindrical type, Jeil Medical, Seoul, Korea, Type C; combination type of cylindrical and tapered portions, Ortholution, Seoul, Korea), were used. Results: Difference in the cortical bone thickness had little effect on the maximum insertion torque (MIT) in Type A mini-screws. But in Type B and C, MIT increased as the cortical bone thickness Increased. MIT of Type C was highest in all situations, then Type B and Type A in order. Type C showed lower POS than Type A or B in all situations. There were statistically significant correlations between cortical bone thickness and MIT, and POS for each type of the mini-screws. Conclusion: Since different screw designs showed different insertion torques with increases in cortical bone thickness, the best suitable screw design should be selected according to the different cortical thicknesses at the implant sites.

The effect of smoking on the healing response following flap debridement surgery (흡연이 치주 판막술후 치유에 미치는 영향)

  • The ultimate goal of periodontal therapy is the regeneration of periodontal tissue which has been lost due to destructive periodontal disease. Various periodontal procedures have been used throughout the years in an attempt to reestablish attachment of periodontal tissues to root surfaces affected by periodontitis. Flap debridement surgery has been demonstrated to be a successful procedure in gaining the probing attachment level and reducing probing depth. A tendency towards impaired wound healing following periodontal procedures in smokers has been clinically documented. But, previous clinical studies on healing response in smokers are based on a retrospective design. The purpose of this study was to evaluate the treatment outcome following flap debridement surgery in smokers compared to nonsmokers. 25 patients with moderate to advanced periodontitis were included for study. Among these patients, 13 patients were smokers, and 12 patients were nonsmokers. Mucoperiosteal flap was raised with the sulcular incision. No antibiotic treatment was administered postsurgery. The patients was recalled at monthly intervals during a period of 6 months following the surgery. The patients were received supragingival scaling and oral hygiene reinforcement. All the recordings, including modified O' Leary plaque control record, bleeding on probing, probing pocket depth, probing attachment level,were recorded, presurgery and 6 months postsurgery. The changes of all the recordings at 6 months after flap debridement surgery revealed the following results: 1. PI on all the dentitions and surgical sites showed no statistical significance between smokers and nonsmokers at presurgery. But, smokers demonstrated a significantly lower % of PI than nonsmokers at 6 months postsurgery. 2. Smokers demonstrated a greater % of BOP sites than nonsmokers on the surgical sites and all the dentitions, presurgery and 6 months postsurgery. But, there was no statistical significance between two groups. 3. Smokers exhibited significantly less reduction of probing depth in the 3 mm or less probing pocket depth(PPD) group, 6mm or more PPD group and total PPD group when compared to nonsmokers at 6 months postsurgery. 4. Smokers exhibited significantly less gain of probing attachment level(PAL) in the 3mm or less PPD group, 6 mm or more PPD group and total PPD group when compared to nonsmokers at 6 months postsurgery.

Effects of Controlled-Release Local Delivery Drugs on the Treatment of Adult Periodontitis (국소약물송달제제가 성인형 치주염의 치료에 미치는 효과)

  • The present study was performed to assess and compare the clinical and microbiological effects following local application of 2% minocycline gel or 0.1% chlorhexidine subgingival irrigation to augment scaling and root planing in patients with moderate to advanced chronic adult periodontitis. 32 healthy patients with moderate to advanced chronic adult periodontitis were enrolled in the study. In each patient, the quadrants that had 2 or more teeth with $5{\sim}8mm$ probing pocket depth and radiographic evidence of alveolar bone loss were selected and divided into test side and control side according to the split-mouth design. All patients received standardized oral hygiene instructions at the beginning of the study and all remaining teeth received scaling and root planing until 0 week. The 2% minocycline gel was applied to periodontal pocket at 0, 1, 2, 3 week in the test side. The 0.1% chlorhexidine solution and the normal saline were irrigated subgingivally for about 30 seconds in the positive control side and negative control side respectively. The clinical and microbiological analysis carried out at 0, 4, 8, and 12 weeks . The results of this study were as follows; 1. In saline irrigation group, there was no adjunctive effects in probing pocket depth reduction, sulcular bleeding index and no significant changes in relative proportions of subgingival bacteria. 2. The chlorhexidine irrigation as an adjunct to scaling and root planing results in reduction in the plaque index and sulcular bleeding index, but there was not statistically significant. The relative proportion of spirochetes was significantly reduced, but the proportion of motile rods was no significant reduction. 3. The minocycline gel delivered subgingivally as an adjunct to scaling and root planing provide significant benefit in reducing probing depths and sulcular bleeding index compared to saline and chlorhexidine irrigation groups. 4. The relative proportions of spirochetes and motile rods were significantly reduced and the proportions of cocci and non-motile bacteria were correspondingly increased in the minocycline gel group. In conclusion, minocycline gel delivered subgingivally as an adjunct to scaling and root planing induces clinical and microbial responses more favorable for periodontal health than saline and chlorhexidine subgingival irrigation.

The results of this st udy were as follows; 1.2% minocycline gel delivered subgingivally as an adjunct to scaling and root planing provided benefit in reducing sulcular bleeding index and pocket depth than the use of normal saline. 2. The relative proportion of cocci and non-motile bacteria was increased in the test and control groups with time, and there was no statistically significantdifference between two groups. 3. The proportion of spirochetes was slowly reduced in the control group, but, inthe test group, they were remarkably reduced from the 4th week, and there was a statistically significant difference between two groups. 4. In both groups, the relative proportion of motile rods was notably decreasedat the beginning of the study, and remained until 12th week in the test group,but, in the control group, they were slowly increased from the 4th weekand finally similar to that of the initial examination. In conclusion, local application of 2% minocycline gel may be effective in the clinical and microbiological aspects as an adjunct to scaling and root planing in periodontal disease sites.

Paracrine effect of the bone morphogeneticprotein-2 at the experimental site on healing of the adjacent control site: a study in the rabbit calvarial defect model

  • Purpose: The aim of this study was to assess the possible paracrine effect of bone morphogeneticprotein-2 (BMP-2) at the experimental site on the adjacent control site for validating a rabbit calvarial defect model as a means of verifying the effect of BMP-2. Methods: Sixteen rabbits were divided into two groups (n=8 in each) according to whether or not BMP-2 would be used. Two circular defects (8 mm in diameter) were created side by side, 2 mm apart, in the calvarium of all of the rabbits. In each animal, one of the defects was grafted with either BMP-2-loaded carrier or carrier material alone. The control defects adjacent to these grafted defects, designated CB (the nongrafted defect adjacent BMP-2-loaded carrier-grafted defect) and CC (the nongrafted defect adjacent to carrier only-grafted defect), respectively, were the focus of this study, and were filled only with a blood clot in all of the animals. Histologic observation and histomorphometric analysis were performed at 2 and 8 weeks (n=4 animals per point in time) after surgery. Results: There was no noteworthy difference in the healing pattern, and no statistically significant differences in histomorphometric parameters such as the defect closure, new bone area, or total augmented area between the CC and CB groups. Conclusions: The results of this study suggest that rabbit calvarial defects separated by a distance of 2 mm are suitable for evaluating the effects of BMP-2 and the control defect can be regarded not to be affected by BMP-2 applied defect.

  • $T4K^{TM}$(Myofunctional Research Co, Australia) is one of the myofunctional appliance developed to be used in children of mixed dentition. Myofuncitonal appliance stimulate the facial, masticatory and tongue muscle and help to balance the muscular force. Labial bow included in the device exerts strength in excessively labial inclineded upper jaw, Lip bumper blocks strength of the mouth to prevent abnormal strength exerted in lower jaw, Tongue tag secures proper position of tongue, and additional exercise is not required for child patients. For the more, simpler design and softer texture of device prmoted cooperation of patients during use. This case report is to present the satisfactory results gained by using $T4K^{TM}$ on Class II patients. Comment 1. $T4K^{TM}$ was applied in Class II malocclusion patients of mixed dentition with expected space insufficient to gain facial improvement. 2. Excessive overjet, overbite were improved. 3. Main effects are regarded to have been achieved by development of lingual slant of upper jaw, labial slant of lower jaw, and lower part of jawbone. 4. Bad habits, such as mouth breathing, can also be adjusted.
