  • It is a relatively common clinical experience to see an unerupted maxillary central incisor. This phenomenon is apparent at the dental age of almost eight years and over. Among the possible cause for failure of eruption, ectopic development of the tooth germ is mentioned. This is not fully understood but trauma or periapical imflammation of primary predecessors is accepted. The case with no history of trauma may be impacted by the periapical imflammation of primary predecessors. For bringing into the tooth eruption and the continued normal root developement by the Hertwig's epithelial root sheath, there are early considered of surgical invention and orthodontic traction with removable appliance. We reported successful treatment for inverted maxillary central incisor with proper eruption and normal root developement by forced eruption using removable appliance. But further observation will be required to evaluate the final root developement state and amount of keratinized attachment gingiva.

  • Impaction is generally defined as the lack of eruption of a tooth after the normal age for the eruption. An impacted tooth may appear blocked by another tooth, bone, or soft tissue, but cause of tooth impaction is often unknown. The clinician should consider the various treatment options available : (a) No treatment and observation, (b) surgical exposure and orthodontic traction (c) auto transplantation (d) extraction. These cases were about the patients with delayed eruption of maxillary central incisor. We surgically exposed impacted tooth and guided it into normal position by the orthodontic traction. Especially, in case 1, #21 was ectopic impacted state with root dilaceration. It is required to examine further root development and alignment of dentition serially.

  • Transposition has been described as an interchange of position of permanent tooth and is a relatively rare dental anomaly. Transposition of teeth may occur both in the maxillary and mandibular arches. but it appears more often in the maxilla of individual teeth, the maxillary canine is the most often involved. A canine transposes most often with a first premolar and less frequently transposes with a lateral incisor. Incomplete transposition is a condition describing an interchange in the position of the crowns of two permanent teeth, while the root apices remain in their relative position. Complete transposition is a situation in which both the crowns and entire root structure are transposed. The etiologic factors of transposition are tooth buds interchange, retained deciduous canines, migration of the erupting canine, trauma to deciduous teeth etc. This report describes a case of a transposition between a maxillary left canine and a lateral incisor and impaction of a maxillary left central incisor due to trauma to deciduous dentition.

  • A 9-year-old girl, in good health, presented a horizontal impaction of the upper left central incisor. History revealed that the patient had been involved in a trauma of the maxillary anterior portion during her childhood. This impacted tooth crown, which was exposed surgically, was bonded by plastic attachments of direct bonding system, and was carefully tried to induce on the dental arch. On the process of the orthodontic treatment the tooth was completely induced and in normal alignment on the arch. The treatment result was very satisfactory; color, vitality, and mobility were normal, periodontal support was good and the cosmetic result was excellent.

  • The clinical and radiographic features of 1,171 mesiodenses were studied with periapical radiograms and/or pantomograms in 942 persons visited the Department of Oral Radiology, School of Dentistry, Kyung Hee University during January 1980 to December 1989. The obrained results were as follows; 1. The incidence of mesiodens was higher in males (75.4%) than in females (24.6%). 2. In number of mesiodenses per individual, a single mesiodens was found to be 75.9%, 2-mesiodenses to be 23.8%, and 3-mesiodenses to be 0.2%. 3. In mesiodenses according to erupted and impacted status, the erupted mesiodens was found to be 16.5%, the impacted mesiodens in the interdental alveolar bone to be 66.6%, and in the region below root apex to be 16.9%. 4. In impacted mesiodenses according to direction of tooth crown, the inverted impaction was found to be 68.8%, the vertical impaction to be 15.1%, and the angulated impaction to be 16.1%. Among the impacted mesiodenses, a palatally located impaction was 87.5%, a middle 4.9%, and a labially 7.6%. 5. In mesiodenses according to shape of the tooth crown, a incisor-like type was found to be 1.6%, a canine-like type to be 6.8%, a conical type to be 79.2%, and a tuberculated type to e 12.4%. 6. In effect of mesiodens on adjacent tooth, non-specific influences were found to be 73.4%, crowding to be 0.3%, diastema to be 11.8%, rotation to be 5.7%, delayed eruption to be 3.8%, root resorption to be 2.6%, and dentigerous cyst to be 2.4%.

  • Calcifying odontogenic cyst(COC) is a rare developmental odontogenic cyst, which shows diverse classification and terminology. Cystic epithelial lining of COC is composed of basal cell layer of columnar cells and overlying layer of stellate reticulum. In the epithelium, ghost cells that might induce adjacent mesenchymal tissue to develop dental organ are shown characteristically. In spite of low rate of recurrence, we have to get a histopathological examination so that odontogenic lesions may recur without fully curettage of lining epithelium. 7-year-old male child came pediatric dentistry in wonkwang university dental hospital in order to check the delayed eruption of left maxillary central incisor. Radiographic examination revealed a well-defined radiopaque mass, overlapping impacted left central and lateral incisor crown. Enucleated mass was tooth-like features and also had epithelium lining. Results of histopathologic procedure, we saw the lots of ghost cell and proliferating hard dental tissues. Also we saw the cystic epithelium cells. It revealed diagnosis of the COC associated complex odontoma. For this reason one should consider of COC when patients present odontoma-like lesion with impacted tooth.

  • Dilaceration is a developmental disturbance in shape of teeth. It refers to an angulation, or a sharp bend or curve, in the root or crown of a formed tooth. Dilacerations are estimated to occur in 3% of all permanent dentitions. However, the etiology of dilaceration is not yet fully understood. Traumatic injury to the deciduous predecessors and ectopic development of the tooth germ are the two commonly cited causes of this anomaly. The involved tooth is usually the maxillary central incisor and it also shows high prevalence of impaction. At first, We tried to do surgical exposure with orthodontic traction but to no avail. After then surgical repositioning of the dilacerated tooth was performed. Finally, Satisfactory results were obtained at the end of treatment, with improvement in dental esthetics and functional aspects.
