  • Clinical and epidemiologic data point to a causal interrelationship between nutritional deficiency and infectious illness. Both are major contributors to childhood morbidity and mortality, particulary in underprivileged population groups. Protein-calorie malnutrition depress a variety of immune funtions. Delayed hypersensitivity and number of T-lymphocytes are consistently reduced. The interrelationship between nutritional status and immune response was studied in 80 children aged under 6 years. According to the anthropometric assessment based on weight for height for age, 36 children were classified as normal, 22 as morderate malnutrition, and 22 as severe malnutrition. The following determinations were made : hemeglobin, hematocrit, serum albumin, immunoglobulin G, complement 3, and WBC levels. Results indicate that levels of Hb, Hct, serum albumin, and C3 concentration were decreased in moderate and severe malnutrition children. However, levels of IgG and WBC were not affected by the nutritional status. It is suggested that nutritional status has more profound effects on complement system than humoral immunity.

  • Hepatitis B virus (HBV) is one of the most common intracellular parasites, of which 350 million people worldwide are chronic carriers. It also related to a high incidence of hepatocellular carcinoma. In general, it has been well known that HBV is a noncytolytic virus, so not the virus itself but any unfavorable response by host immune cells and inflammatory cytokines mainly result in chronic liver injury. From this viewpoint, we hopefully assume that Oriental therapies based on immunologic strategies may be able to provide a therapeutic alternative for caring for these illnesses. We also need to be thoroughly familiar with information about HBV epidemiology and the pathologic process of chronic HBV carriers. In this study, to clarify the important considerations of HBV infection and the high risk of HBV induced life-threatening diseases, we introduced our pilot practices given to the patients and the possibility of Oriental therapies as a novel strategy for chronic HBV carriers.

디스트레스 간호중재가 부인암 환자의 디스트레스, 면역 및 삶의 질에 미치는 효과 (Development and Evaluation of the Psychosocial Distress Nursing Intervention for Patients with Gynecological Cancer)

  • Purpose: The purpose of this research was to develop and evaluate a distress nursing intervention for relieving psychosocial distress. Methods: We developed a twelve-week non-pharmacological distress nursing intervention to decrease distress. This intervention includes cognitive behavioral therapy, mindfulness based stress reduction, psychoeducation and supportive therapy. The participants were 27 gynecologic cancer patients who agreed to participate in this study, selected by convenience sampling (experimental group-17 and control group-10). The data collection period was performed from November, 8, 2010 to February, 1, 2011. Data were analyzed by using the SPSS/WIN 18.0 program. Results: There were no difference between the two groups distress score, distress problem and quality of life. The experimental group had significant higher IL-12 and IFN-${\gamma}$ and lower TGF-${\beta}$ between before and after the distress nursing intervention. Conclusion: The findings indicate that the distress nursing intervention was an effective intervention in improving immunologic function of gynecological cancer patients.

  • Adverse reactions to foods are common and increasing problems worldwide. It is important to differentiate between the different forms of adverse reactions to foods, in particular the difference between food intolerance and food allergy. Food allergy is an adverse immunologic response to food and typically rapid in onset, whereas food intolerance is consequence of a variety of non-immune mechanisms and slow in onset. It has been difficult to diagnose food intolerance because of lack of diagnosis measure. There are now effective findings to identify food intolerance, and paradigms with food allergy are shifting. Food intolerance affects virtually every part of the body-from mildly uncomfortable symptoms to severe illness. While there is no known simple 'cure' for food allergy and food intolerance, there are a number of measures that will help avoid and lessen symptoms and correct the underlying causes. We focus the diagnosis and treatment of food intolerance through various views.

괴저성 농피증의 수술적 치험례 (Surgical Management of Pyoderma Gangrenosum: A Case Report)

  • Pyoderma gangrenosum(PG) is an uncommon cutaneous vascular disease that typically presents as a painful and destructive ulceration on the anterior surface of the legs. The etiology of PG is currently unknown. But, the association with many immunologic disorders and its clinical response to immunomodulating agents suggest an immune etiology. A common feature of patients with PG is the presence of pathergy(the induction of lesion following injury of the skin). The trauma of surgery can be sufficient to induce pathergy, thus paradoxically limiting the usefulness of surgical treatment of PG. For that reason, medical treatments have been commonly used, while surgical treatments have been regarded not suitable. However, the use of the classic systemic agents is limited by their side effects and contraindications. Moreover, the large, problematic ulcers take too long to heal with medical management only. We present our experience in closing large wounds with the goal of decreasing morbidity, drug side effects and hospital stay by combination of medical and surgical therapy(split thickness skin graft). And authors advocate that surgical management is not a contraindication and may be considered as a selective modality in treatment of PG.

소화기계암의 수술후 면역기능에 대한 고려홍삼의 효과 (The Effect of Red Ginseng for Postoperative Immune Response in Gastrointestinal Carcinoma)

  • (Backgrounds) This study was performed to evaluate the usefulness of red ginseng ex rant as adjuvant therapeutic agent improving immune function in immune compromizing gas-trointestinal carcinoma patient. (Material and Methods) We were treated 72 patients with two groups after we were undertaken the curative resection for gastrointestinal carcinoma; 1) only chemotherapy and immunotherapy (control group) 2) chemotherapy and immunotherapy with 4500 mg (15 tablets) red ginseng for 6 months (study group). For investigating the immunologic alternations alongside the numerical changes in peripheral blood Iymphocyte and their subsets in the gastrolntestinal carcinoma patients, Iymphocyte surface markers were determined by monoclonal antibodies on the preoperative day, postoperative 1 months, 3 months, 6 months, 12 months and 18 months in 40 controls and 32 red ginseng groups In gastrointestinal carcinoma patients which was recruited at Korea diversity Hospital from March, 1995 to January, 1997. The patient was measured and compared in both groups with the body weight, total protein and albumin, blood hematocrit and hemoglobin, total leukocyte, lymphocyte and lymphocyte subsets count in peripheral blood through planed schedules. (Couclusion) This data suggests that red ginseng may be useful as a adjuvant therapeutic agent for improving the immune function after curative operation for immune compromizing gastrointestinal carcinoma patients. Pathophysiology and protective approaches of gut injury in critical illness

  • The gut is a complex organ that has played an important role in digestion, absorption, endocrine functions, and immunity. The gut mucosal barriers consist of the immunologic barrier and nonimmunologic barrier. During critical illnesses, the gut is susceptible to injury due to the induction of intestinal hyperpermeability. Gut hyperpermeability and barrier dysfunction may lead to systemic inflammatory response syndrome. Additionally, gut microbiota are altered during critical illnesses. The etiology of such microbiome alterations in critical illnesses is multifactorial. The interaction or systemic host defense modulation between distant organs and the gut microbiome is increasingly studied in disease research. No treatment modality exists to significantly enhance the gut epithelial integrity, permeability, or mucus layer in critically ill patients. However, multiple helpful approaches including clinical and preclinical strategies exist. Enteral nutrition is associated with an increased mucosal barrier in animal and human studies. The trophic effects of enteral nutrition might help to maintain the intestinal physiology, prevent atrophy of gut villi, reduce intestinal permeability, and protect against ischemia-reperfusion injury. The microbiome approach such as the use of probiotics, fecal microbial transplantation, and selective decontamination of the digestive tract has been suggested. However, its evidence does not have a high quality. To promote rapid hypertrophy of the small bowel, various factors have been reported, including the epidermal growth factor, membrane permeant inhibitor of myosin light chain kinase, mucus surrogate, pharmacologic vagus nerve agonist, immune-enhancing diet, and glucagon-like peptide-2 as preclinical strategies. However, the evidence remains unclear.

Clinical Characteristics and Treatment of Immune-Related Adverse Events of Immune Checkpoint Inhibitors

  • Immune checkpoint inhibitors (ICIs) have been changing the paradigm of cancer treatment. However, immune-related adverse effects (irAEs) have also increased with the exponential increase in the use of ICIs. ICIs can break up the immunologic homeostasis and reduce T-cell tolerance. Therefore, inhibition of immune checkpoint can lead to the activation of autoreactive T-cells, resulting in various irAEs similar to autoimmune diseases. Gastrointestinal toxicity, endocrine toxicity, and dermatologic toxicity are common side effects. Neurotoxicity, cardiotoxicity, and pulmonary toxicity are relatively rare but can be fatal. ICI-related gastrointestinal toxicity, dermatologic toxicity, and hypophysitis are more common with anti- CTLA-4 agents. ICI-related pulmonary toxicity, thyroid dysfunction, and myasthenia gravis are more common with PD-1/PD-L1 inhibitors. Treatment with systemic steroids is the principal strategy against irAEs. The use of immune-modulatory agents should be considered in case of no response to the steroid therapy. Treatment under the supervision of multidisciplinary specialists is also essential, because the symptoms and treatments of irAEs could involve many organs. Thus, this review focuses on the mechanism, clinical presentation, incidence, and treatment of various irAEs.

A Probiotic Preparation Alleviates Atopic Dermatitis-Like Skin Lesions in Murine Models

  • Atopic dermatitis (AD) is a chronic inflammatory skin disease with a complex etiology that encompasses immunologic responses. AD is frequently associated with elevated immunoglobulin (Ig) E levels, and common environmental factors contribute to its pathogenesis. Several recent studies have documented the role of specific lactic acid bacteria in the treatment and prevention of AD in humans and mice. In this study, the efficacy of Duolac ATP, a probiotic preparation, was determined in a mouse model with AD-like skin lesions. Alterations in the cytokine levels and histological staining suggested the alleviation of AD. The in vivo test showed that T helper (Th)2 cytokines, IgE, interleukin (IL)-4, and IL-5, were significantly downregulated, whereas Th1 cytokines, IL-12p40 and interferon (IFN)-${\gamma}$, were upregulated in all groups of mice treated with Duolac ATP compared to that observed in the group of mice treated with 1-chloro-2,4-dinitrobenzene (DNCB) alone. Moreover, the scratch score decreased in all mice treated with Duolac ATP. Staining of the dorsal area of the mice in each group with hematoxylin and eosin and toluidine blue further confirmed the alleviation of AD in mice orally treated with Duolac ATP. These results suggest that Duolac ATP inhibits the development of AD-like skin lesions in NC/Nga mice by suppressing the Th2 cell response and increasing the Th1 cell response. Thus, Duolac ATP is beneficial and effective for the treatment of AD-like skin lesions.

Association of Intracellular $T_H1-T_H2$ Balance in CD4+ T-cells and MIP-$1{\alpha}$ in CD8+ T-cells with Disease Severity in Adults with Dengue

  • Background: We tested the hypothesis that dengue haemorrhagic fever (DHF) is associated with a $T_H1$-skewed immune response as opposed to dengue fever (DF). Methods: We estimated intracellular (in T-cells) and serum levels of designate $T_H1/T_H2$ cytokines [interferon-${\gamma}$ (IFN-${\gamma}$), interleukin-4 (IL-4), and tumor necrosis factor-${\alpha}$] and macrophage inflammatory protein-$1{\alpha}$ (MIP-$1{\alpha}$) at admission, 48h, and day 5 in 20 adults with dengue (DF=10, DHF=10) and 10 dengue-naive healthy controls. Results: At admission, intracellular IFN-${\gamma}$/IL-4 ratio in CD4+ T-cells and proportion of MIP-$1{\alpha}$-positive CD8+ T-cells were significantly higher in patients with DHF [7.21 (5.36~10.81) vs. 3.04 (1.75~4.02); p=0.011 and 6.2% (3.2~8.2%) vs. 2.4% (2.0~3.6%); p=0.023]. The latter showed a significant positive correlation with IFN-${\gamma}$/IL-4 ratio in CD4+ T-cells (Spearman's rho=0.64; p=0.003), percentage-change in haematocrit (rho=0.47; p=0.048), and serum alanine amino-transferase level (rho=0.61; p=0.009). Conclusion: We conclude that DHF is associated with a $T_H1$-skewed immune response. Further, MIP-$1{\alpha}$ in CD8+ T-cells is an important immunologic correlate of disease severity in dengue.
