  • Background The use of acellular dermal matrix (ADM) in implant-based immediate breast reconstruction has been increasing. The current ADMs available for breast reconstruction are offered as aseptic or sterile. No published studies have compared aseptic and sterile ADM in implant-based immediate breast reconstruction. The authors performed a retrospective study to evaluate the outcomes of aseptic versus sterile ADM in implant-based immediate breast reconstruction. Methods Implant-based immediate breast reconstructions with ADM conducted between April 2013 and January 2016 were included. The patients were divided into 2 groups: the aseptic ADM (AlloDerm) group and the sterile ADM (MegaDerm) group. Archived records were reviewed for demographic data and postoperative complication types and frequencies. The complications included were infection, flap necrosis, capsular contracture, seroma, hematoma, and explantation for any cause. Results Twenty patients were reconstructed with aseptic ADM, and 68 patients with sterile ADM. Rates of infection (15.0% vs. 10.3%), flap necrosis (5.0% vs. 7.4%), capsular contracture (20.0% vs. 14.7%), seroma (10.0% vs. 14.7%), hematoma (0% vs. 1.5%), and explantation (10.0% vs. 8.8%) were not significantly different in the 2 groups. Conclusions Sterile ADM did not provide better results regarding infectious complications than aseptic ADM in implant-based immediate breast reconstruction.

  • The purpose of this research was to develop labor-saving cultural practices of cherry tomatoes in the deep flow technique (DFT) by omitting the process of raising seedling. Three cultivars, ‘Pepe’, ‘Pico’ and ‘Koko’ were propagated by the immediate setting of cuttings(lateral shoots) to the culture bed and by setting seedlings obtained by raising cuttings for 15 days, and grown in the standard and half - strength solution. 1. The immediate setting resulted in higher plants, larger number of leaves and larger leaf area during the early growth stage. And the harvest time was hastened by six days in the direct setting than that in the seedling setting. It was also earlier in ‘Pepe’, but not significant by different between two strengths of nutrient solution. 2. The average fruit weight was affected only by cultivars, but not by the type of cutting and the strength of nutrient solution. The highest yields were obtained from the immediate setting of ‘Pepe’ in other cultivars grown in the nutrient solution of standard strength. 3. Brix degree of treated cherry tomato juice was higher in ‘Pepe’ tomato cultured by immediate setting than others.

  • The prognosis of patients with VSD and pulmonary hypertension is at least partially related to the reversibility of pulmonary hypertension after surgery. To predict postoperative pulmonary arterial pressure, immediate postbypass values were compared with preoperative hemodynamic data in 18 surgically closed VSD patients aged 6 to 80 months. The following results were obtained. 2] There was a good correlation between preoperative PP/PS and postoperative PP/PS in patients aged over 24 months [r=0.685, p<0.05], and in patients whose QP/QS were over 2.0 [r=0.686, p<0.01]. 3] There was also a good correlation between pump time and postoperative PP in total patients [r=0.697, p<0.005]. 4] Direct correlations were found between preoperative PP/PS and immediate postbypass PP/PS in patients aged over 24 months and in patients whose QP/QS>2.0, but the effect of CPB would participate in some degree. So it may be necessary to reevaluate their relationship after the effect of CPB have been disappeared.

  • Background: The purpose of this study was to investigate the immediate effect of thoracic self mobilization on balance and gait of acute stroke patients. Methods: Patients (n=10) with acute stroke applied to thoracic self mobilization. There are two thoracic self mobilization of both extension and rotation. Each thoracic self mobilization was performed ten times per direction in a total of three sets and the total time required for two thoracic self mobilization was twenty minutes. The break time between sets was one minute. Balance ability was measured using the timed up and go test and the five times sit to stand test for patients. The 10 meter walk test and the functional gait assessment were used for measurement of gait ability for patients. Results: Significant improvements were observed on balance ability (p<.05) and gait ability (p<.05). Conclusion: These results show that thoracic self mobilization is immediate effective on balance and gait ability. Thus, thoracic self mobilization will help recovery of balance and gait ability in acute stroke patients.

  • Purpose: The purpose of the study was to evaluate the effect of the immediate postoperative nutritional status and calorie adequacy on clinical outcomes in liver transplant recipients. Methods: A total number of 99 patients who received liver transplants were recruited from a tertiary university hospital. Demography, subjective global assessment, clinical outcomes and calorie adequacy were evaluated through personal interviews, electronic medical records and dietary records. Anthropometric measures, body mass index and percent of ideal body weight were also obtained. Results: At admission to the Surgical Intensive Care Unit (SICU), the triceps skinfold thickness and mid-arm muscle circumference were significantly lower in the malnourished group than in the well-nourished group (p<.05, respectively). In the clinical outcomes, transfusion of red blood cells, mechanically ventilated hours, length of stay in the Intensive Care Unit (ICU), length of stay in the hospital, and prothrombin time were significantly higher in the malnourished group than in the well-nourished group (p<.05, respectively). The mechanically ventilated hour was significantly higher in the group with less than 50% of their required energy intake (p<.05). Conclusion: Therefore, it is important to assess the nutritional status of immediate postoperative patients. Furthermore, studies on nutritional interventions are urgently needed to provide adequate nutritional care for patients in ICUs.

  • The flowers of Lonicera japonica Thunb. (Caprifoliaceae) have been used as a traditional effective drug in treating bacillary dysentery, purulence. However, the exact role of Lonicera japonica in allergic reaction has not been clarified yet. Immediate hypersensitivity, popularly known as allergy, is a major clinical problem in humans. It has been found that the histamine release from mast cells is an essential step in the pathological process of immediate hypersensitivity. In this study, the effect of aqueous extract of Lonicera japonica (AELJ) on allergic reaction was investigated. AELJ inhibited the compound 48/80-induced anaphylactic reactions and antigen-induced passive cutaneous anaphylaxis (PCA). AELJ in vitro exhibited a dose-dependent inhibition of degranulation in RPMC stimulated by compound 48/80. AELJ also suppressed the morphological changes and the increase of intracellular free calcium level induced by compound 48/80. These results suggest that inhibitory effect of AELJ on allergic reaction may be mediated through the decrease of intracellular free calcium levels, and AELJ importantly contributes to the treatment of anaphylaxis and may be useful for other allergic disease.

  • Cloves are the dried flower buds of Syzygium aromaticum (L.) Mere et Perry (Myrtaceae). They have been successfully used for the management of various allergic disorders by oral administration in Korea. In this study, the author investigated the effect of an aqueous extract of Syzygium aromaticum on immediate-type hypersensitivity by anal administration. Anal administration of Syzygium aromaticum showed a marked inhibition rate in systemic hypersensitivity with a dose of 1 mg/kg 1 hr before intraperitoneal injection of compound 48/80. Anal administration of Syzygium aromaticum significantly reduced plasma histamine contents induced by compound 48/80. Anal administration of Syzygium aromaticum (1 mg/kg) also inhibited to $61.4\%$ (P<0.01) local a1lergic reaction activated by anti-dinitrophenyl (DNP) IgE. In addition, Syzygium aromaticum dose-dependently inhibited the histamine release from the peritoneal mast cells by compound 48/80 or anti-DNP IgE. When Syzygium aromaticum was added, the level of cAMP in peritoneal mast cells transiently and significantly increased about 47-fold at 10 second compared with that of basal cells. These results provide evidence that anal therapy of Syzygium aromaticum may be beneficial in the treatment of systemic and local immediate-type hypersensitivity by inhibition of histamine release from mast cells in vivo and in vitro.

  • Purpose: In chronic stroke patients, muscle tone and stiffness increase due to ankle spasticity. Electrotherapy may control the spasticity of patients with central nerve system damage via neurophysiological mechanisms. Therefore, this study was conducted to determine the immediate effects of interferential current therapy on gastrocnemius (GCM) muscle. Methods: This study was a one-group pretest-posttest design and 20 stroke patients participated. The experimental group underwent interferential current therapy for GCM for 30 minutes. Muscle tone (MT) and stiffness were assessed using MYOTONE(R) PRO. After 30 minutes of interferential current therapy, MT and stiffness of the affected side and unaffected side by GCM were measured. Results: After interferential current therapy, the medial GCM MT (Hz) was significantly reduced in stroke patients. There was a significant difference in MT between affected GCM muscles and unaffected side medial GCM muscles before intervention, but there was no significant difference after interferential current therapy. Conclusion: This study demonstrated that interferential current therapy had a positive effect, producing an immediate decrease in the medial GCM muscles tone of stroke patients. However, this study employed a one-group pretest-posttest design. Future studies will show differences in muscle tone compared to a control group or other electrical stimulation treatments.

  • Implant success is achieved by the synergistic combination of numerous biomechanical factors. This report examines the mechanical aspect of implants. In particular, it is focused on macrodesign such as thread shape, pitch, width and depth, and crestal module of implants. This study reviews the literature regarding the effect of implant thread geometry on primary stability and osseointegration under immediate loading. The search strategy included both in vitro and in vivo studies published in the MEDLINE database from January 2000 to June 2014. Various geometrical parameters are analyzed to evaluate their significance for optimal stress distribution, implant surface area, and bone remodeling responses during the process of osseointegration.
