  • Variant angina causes chest pain due to abnormal coronary vasospasms. A 60-year-old male who was diagnosed with variant angina was admitted to the Oriental Medicine Hospital and treated with herbal medicines, including Pyungjinsujeom-san and Simjeok-hwan. After treatment, the frequency of chest pain and use of nitroglycerin decreased. Subjective symptoms of chest pain measured using a numerical rating scale (NRS) also decreased. These improvements persisted throughout the hospitalization period. In conclusion, Korean medicines, including Pyungjinsujeom-san and Simjeok-hwan, can be effective for patients with chest pain due to variant angina.

  • Lung cancer, breast cancer and lymphoma are the common oncologic causes of malignant pleural effusion, comprising more than the half of the causes. However, an endocrinologic carcinoma associated malignant effusion is very rare. Recently, we encountered a case of papillary thyroid carcinoma causing malignant effusion. An 83-year-old female patient presented with dyspnea due to massive pleural effusion in her left side. The pleural biopsy, pleural fluid cytology and breast needle aspiration biopsy results were consistent with a metastatic papillary thyroid carcinoma. Thyroid ultrasonography showed two thyroid masses, but the patient refused a thyroid biopsy. This case highlights the need for considering the possibility of papillary thyroid carcinoma when the cause of malignant pleural effusion cannot be found because one of the rare clinical manifestations of a papillary thyroid carcinoma can be dyspnea due to malignant effusion.

  • Background: Falls are the most frequent adverse events reported in hospitals. The aim of this study was to investigate the incidence rate and characteristics of falls in patients who used comprehensive nursing care service in National Health Insurance Service Ilsan Hospital. Methods: Incidence rate of falls was investigated in patients using comprehensive nursing care service, from July 2013 to Jun 2017 and compared with those not using this service. The characteristics and risk factors for falls, and fall-related injuries were obtained. Results: Among the 62,445 patients who used the comprehensive nursing care service for 4 years, total of 672 falls were reported. The incidence rate of falls per 1,000 patients-day was 1.15. The percentage of fall-related injuries was 26.9% and that of major injury was 2.2%. Although the incidence rate of all falls was slightly higher in patients using comprehensive nursing care service than those not using this service, falls-related injuries were not correlated with the implementation of this service. Conclusion: The falls could be more frequently detected and reported in comprehensive nursing care service, but there was no difference in fall-related injuries.

This video will give inexperienced patients a better understanding of the processes and expectations of the radiotherapy. We have produced a radiotherapy guidance video regarding work flow and a method of radiotherapy to relieve anxiety and stress. It also improves patients satisfaction and understanding of radiotherapy to provide a high-quality health care for radiotherapy patients with indirect experience. Methods: We have evaluated the effectiveness of the video compared to our existing verbal method. See below for the evaluation criteria; 1) Patients satisfaction rate of guidance 2) a comparison of understanding of radiotherapy 3) a comparison of a time of education for patients 4) a researching of an incidence rate of radiotherapy. Results: When compared to the verbal explanation the patients had a increased level of understanding of the radiotherapy treatment. The time to educate patient was decreased and the level of incidents during the treatment was decreased due to the patient having a better understanding of the whole process. Conclusion : In conclusion, the audiovisual education increased the understanding of radiotherapy for patients compared to verbal education. The video also helped patients to cooperate in treatment room so we can provide premium radiotherapy treatment. By reducing the treatment time and education processa we improved the patients overall experience.

  • Henoch-$Sch{\ddot{o}}nlein$ purpura (HSP) is a vasculitis of the small vessels in skin, joints, gastrointestinal (GI) tract and kidney. GI symptoms occur in up to 85% of patients and may lead to severe problems such as intussusception, obstruction, and perforation. GI symptoms may not be easily controlled, showing refractoriness to the conventional corticosteroid therapy. Although GI involvements of HSP are acute, and self-limited in most instances, they may cause fatal results in some unusual cases. In such conditions all the possible therapeutic modalities should be considered. We report two cases of severe small bowel involvement of HSP. One case presented with severe abdominal pain showing refractoriness to corticosteroid, but improved with IV immunoglobulin therapy. In the second case, HSP with transmural infarction in the small bowel could be cured with surgical intervention.
