Effect of Infra-Gravity Waves on Nearshore Morphodynamics in the East Coast : Case Study - Ilsan Beach (장주기 중력외파의 동해안 연안지형변화에 미치는 영향 연구 : 사례연구 - 일산해변)

  • It is widely known that infragravity waves can exert significant influence on wave run-up over beaches. Large run-ups can lead to overwash, flooding and severe coastal erosion. In spite of the importance of infragravity waves in relation to wave run-up and coastal erosion, few studies have been carried out with regard to the impact of infragravity waves on nearshore morphodynamics with respect to eastern beaches in Korea. The purpose of this study is to investigate the importance of infragravity waves in nearshore numerical modelling. For the study, XBeach model was set up to analyze morphodynamics in December 2016, in Ilsan beach which is located in Ilsan-dong, Ulsan Metropolitan City. After validation of the XBeach model, numerical experiments were conducted by using various directional spreading coefficients. As the directional spreading coefficients are increased, the effect of infragravity waves is also enhanced by narrowband frequency. With the increasing effect of infragravity waves, the amount of sediment transport is also increased and an erosion dominant pattern is found in the south part of Ilsan beach and a deposition pattern in the north part of the beach mainly due to the wave incident direction of NNE.

Identification of Woods Excavated from the Peat-bed Site of Ilsan Area (일산 신도시 개발지역 토탄층(土炭層) 출토목재의 수종)

  • Ilsan peat-bed site, newly constructed city area, is located at the northern part of Seoul. This site is situated near the mouth of Han river, which flows through central Korea. Many fossil woods were excavated from this site, and surveyed the wood anatomical features. Totally 90 specimens were identified into 10 species including two roots and five barks which were not identified in detail. Alnus japonica and A. maxmowiczii are the most common and occupied 26. 7%, 34.4% of the total specimen number respectively, and then other species which excavated from Ilsan area are follows : Fraxinus rhynchophylla(18.9%), Lepido-balanus spp.(3.3%), Maackia amurensis(2.2%), Diospyros spp.(2.2%), Juglans mandshrica(1.1%), Prunophora spp.(1.1%), Malus or Pyrus spp.(1.1%), Rhamnus spp.(1.1%). Among the identified species there are fairly some species growing in the central - or northern temperate zone, such as A. maxmowiczii, F. rhynchophylla, Lepidobalanus spp., M. amurnsis, J. mandshurica, Rhumnus spp. Therefore, it can be estimated that the former temperature of Ilsan area was same or slightly cold than today. The species bearing fruit for food as .J, mandshunca, Diospyros spp., Prunophora spp., Mulus or Pyrus spp. were regarded as artificially planting trees, because they do not grow together in the same site.

Intraosseous Calcaneal Lipoma with Subtalar Perforation through Cystic Degeneration: A Case Report

  • Intraosseous lipoma is a benign tumor that originates from proliferating mature lipocytes. It often occurs in the metaphysis of long bones of the lower extremity, and also in the calcaneus, humerus, mandible, sacrum, and rib bones. Frequently, it involutes spontaneously through a process of infarction, calcification, and cyst formation. It can either present as pain, or be asymptomatic and only discovered through an incidental radiological finding. In our case, the patient presented with heel pain. Intraoperatively, it was found that the intraosseous cavity was filled with fat along with an adjacent but separate area of cystic degeneration. There was also a cortical perforation at the cystic lesion which was communicating with the subtalar joint. This cortical breach is most likely the cause of diffuse lateral heel pain experienced by our patient, and such a pathological fracture due to intraosseous lipoma has never been reported.

Plantar Keratosis Induced by Heterotopic Ossification under the Medial Sesamoid Bone: A Case Report (족무지 내측종자골 족저부에 발생한 이소성 골화증으로 인한 족저각화증: 증례 보고)

  • Heterotopic ossification is the formation of extra-skeletal bone in the muscle and soft tissues, and an osteoma is a benign bone-forming tumor composed of compact or mature trabecular bone limited almost exclusively to the craniofacial bones. This paper reports an extremely rare case of heterotopic ossification mimicking an osteoma that occurred independently at the plantar side of the medial sesamoid bone. The patient was a 46-year-old male with a three-month history of pain and a hard mass on the plantar aspect of the left forefoot sole. After excising the lesion, the patient's symptoms were relieved, and no pain or complications occurred. This paper discusses this exceedingly rare case of heterotopic ossification around the medial sesamoid bone with a review of the relevant literature.

  • Objective: To report three cases of monozygotic twinning after IVF-ET transfer. Methods: Private practice in two different assisted reproductive technology clinics. Results: Three intrauterine monozygotic twin pregnancies occurred after IVF-ET. One of them was complicated by cord entanglement, another is progressing normal pregnancy without complication and the other was had a normal pregnancy without complication and delivered twin by cesarean section. Conclusion: The reported prevalence of multiple gestations in IVF-ET is a approximately 30%, and it is only 2.7% to be monozygotic twinning in IVF-ET. We report three cases of monozygotic twining after IVF-ET.

Sural Nerve Tuberculoma: A Case Report (비복신경에 발생한 결핵종: 증례 보고)

  • Nearly one third of the world's population have active or latent tuberculosis, resulting in 1.5 million deaths annually. Tuberculosis involving the peripheral nerve is difficult to detect. Sural nerve tuberculoma is an extremely rare case of tuberculous involvement of the peripheral nerve that has attracted the attention of physicians. This paper reports a patient with sural nerve tuberculoma. A 58-year-old female patient presented with a palpable mass on the posterolateral calf with progressive tingling sensation on the distal area. The patient had no history of trauma and it was unclear whether the patient had any contact with individuals with active tuberculosis. The histopathologic findings revealed a granuloma-like lesion with caseous necrosis that was compatible with tuberculoma.

  • Dental implants are an effective and predictable treatment for restoration of missing teeth. However, as the use of implants increases, complications are also increasing. The mechanical complications are not only highly frequent, but also increasing as life span of an implant increases, thus, solutions should be prepared. In this report, we will present a case dealing with abutment fracture and abutment screw fracture which are the most common mechanical complication of the implant, focusing on preserving and reusing existing components by reconstructing only the damaged parts.

  • Rhabdomyolysis is a syndrome caused by injury to skeletal muscles and involves leakage of large quantities of potentially toxic intracellular contents into the plasma. It is known that rhabdomyolysis results in peripheral nerve injury, however, reports of bilateral sciatic neuropathy following rhabdomyolysis are rare. We report a case involving a 42-year-old female patient with no past medical history, who presented with sudden bilateral calf pain, redness, and burning sensation with weakness of both lower extremities after sleeping on an electric heating pad following alcohol drinking. Lower extremity magnetic resonance angiography (MRA) revealed multifocal edema with enhancement of bilateral lower extremity muscles. Clinical and electrodiagnostic tests were consistent with the diagnosis of bilateral sciatic neuropathy following rhabdomyolysis. This is a rare case of bilateral sciatic neuropathy following rhabdomyolysis.

Effect of Delayed Clozapine Initiation on Acute Treatment Response in Treatment-Resistant Schizophrenia (클로자핀의 지연된 사용이 치료저항성 조현병 환자의 급성기 약물 반응에 미치는 영향)

  • Objectives: Recent studies have reported that delayed initiation of clozapine can affect clinical response in patients with treatment-resistant schizophrenia (TRS). This study aimed to explore the relationship between delayed initiation of clozapine and acute treatment response. Methods: Sixty-five inpatients with TRS who started clozapine for the first time were included through a retrospective chart review. Acute treatment response was defined as a 30% reduction in the Positive and Negative Syndrome Scale score or a Clinical Global Impression of Improvement score of 1 (very much improved) or 2 (much improved) at 4 weeks after initiating clozapine. Results: After meeting the TRS criteria, the mean delay for initiating clozapine was approximately 13.8 months. The delay was shorter in patients who showed a better response to clozapine in logistic regression analysis (p=0.037). Conclusion: Our findings suggest that reducing the delay in initiating clozapine increases the effectiveness of clozapine in patients with TRS.

Improved Symptoms of Peripheral Facial Nerve Palsy in ChAdOx1 nCoV-19 Vaccine Recipients Following Complex Korean Medicine Treatment

  • Peripheral facial nerve palsy is a neurological condition/disease characterized by unilateral paralysis of the facial muscles. We report the effects of complex Korean medicine treatment in 2 patients diagnosed with Bell's palsy who developed symptoms 3-4 days following vaccination with ChAdOx1 nCoV-19 and who received oral steroids and antiviral medications. Complex Korean medicine treatments including acupuncture, electroacupuncture, bee venom acupuncture, cupping therapy, herbal medicine, and physical therapy were performed. Symptom improvement was evaluated daily using the Numerical Rating Scale, House-Brackmann Grade, and Yanagihara's Score. Patient 1 was hospitalized for 22 days and Patient 2 visited the outpatient clinic 20 times. In both cases, postauricular pain caused by Bell's palsy disappeared, and symptoms of facial paralysis improved following completion of treatment. These findings could suggest that combined Korean medicine treatment may improve symptoms of peripheral facial nerve palsy.
