Changes in the Hospital Standardized Mortality Ratio Before and During the COVID-19 Pandemic: A Disaggregated Analysis by Region and Hospital Type in Korea

  • Objectives: The coronavirus disease 2019 (COVID-19) pandemic has led to a global shortage of medical resources; therefore, we investigated whether COVID-19 impacted the quality of non-COVID-19 hospital care in Korea by comparing hospital standardized mortality rates (HSMRs) before and during the pandemic. Methods: This retrospective cohort study analyzed Korean National Health Insurance discharge claim data obtained from January to June in 2017, 2018, 2019, and 2020. Patients' in-hospital deaths were classified according to the most responsible diagnosis categories. The HSMR is calculated as the ratio of expected deaths to actual deaths. The time trend in the overall HSMR was analyzed by region and hospital type. Results: The final analysis included 2 252 824 patients. In 2020, the HSMR increased nationwide (HSMR, 99.3; 95% confidence interval [CI], 97.7 to 101.0) in comparison to 2019 (HSMR, 97.3; 95% CI, 95.8 to 98.8). In the COVID-19 pandemic zone, the HSMR increased significantly in 2020 (HSMR, 112.7; 95% CI, 107.0 to 118.7) compared to 2019 (HSMR, 101.7; 95% CI, 96.9 to 106.6). The HSMR in all general hospitals increased significantly in 2020 (HSMR, 106.4; 95% CI, 104.3 to 108.5) compared to 2019 (HSMR, 100.3; 95% CI, 98.4 to 102.2). Hospitals participating in the COVID-19 response had a lower HSMR (HSMR, 95.6; 95% CI, 93.9 to 97.4) than hospitals not participating in the COVID-19 response (HSMR, 124.3; 95% CI, 119.3 to 129.4). Conclusions: This study suggests that the COVID-19 pandemic may have negatively impacted the quality of care in hospitals, especially general hospitals with relatively few beds. In light of the COVID-19 pandemic, it is necessary to prevent excessive workloads in hospitals and to properly employ and coordinate the workforce.

  • Recently, in order to prevent the proliferation of COVID-19, which began in earnest in 2020, an increasing number of places have been measuring the temperature and required to wear a mask. However, as wearing a mask and checking the temperature are typically measured directly by a person or by a single individual positioned in front of the machine, standards may vary based on the person's manual measurement method, wasting workforce. While standing in front of a device often measures the maximum temperature of the face, the standard of fever is also unclear. Both approaches can create bottleneck situations when checking large numbers of people. Furthermore, it is unable to conduct periodic measurements and tracking because the measuring machines are generally put only at the entrance. Thus, this study suggests a method for preventing the spread of infectious diseases by automatically identifying and displaying unmasked people and those with fever in real-time using a general camera, a thermal imaging camera, and an artificial intelligence algorithm.

  • Major advanced ports around the world are preparing for environmental regulations such as increased efficiency of ports and low emission of pollutants in ports by utilizing fourth industrial technologies and ICT technologies such as AI, big data, self-driving cars and connected cars. It is also investing in developing fully unmanned terminals to solve the problem of workforce reduction caused by avoidance of 3D industries. However, the introduction of advanced technology is being delayed in domestic ports, which has led to a drop in port efficiency. In addition, port safety accidents have also occurred frequently, seriously affecting port marketing. Thus, the characteristics and types of each container terminal in Korea were analyzed and the factors for introducing autonomous cooperative driving were classified into five section factors and 15 division factors. Hierarchically classified factors will be surveyed on workers working in shipping lines, port construction, container terminals and related ministries.

Differences in Perception of Quality Dental Job Conditions and Job Satisfaction between Dentists and Dental Hygienists

  • Background: This study aims to investigate the perceptions of dentists and dental hygienists regarding quality dental job conditions and to identify differences in perceptions based on job type. Methods: As a result of conducting face-to-face and online surveys, data from a total of 132 people were analyzed. In order to investigate the perception of quality job conditions, the importance of a total of 13 items was investigated, and the work policies and job satisfaction of the current workplace were examined. Since the data did not follow a normal distribution, a non-parametric test, the Mann-Whitney U test, was performed. Results: Both dentists and dental hygienists perceived income and working hours to be of priority importance for quality job conditions. Dental hygienists valued holiday support and welfare, human relations, and personal development potential more than dentists (p<0.05). Looking at differences by job type, dental hygienists rated all conditions as more important than dentists except income, indicating a statistically significant difference (p<0.05). In terms of the work policies, 96.2% of the practices in the study were required to have the four types of social security contributions, but fewer had flexible working hours (19.7%), healthcare support (23.5%), and incentives (25.0%). Of the participants, 60.6% had parental leave available at their workplace, and dental hygienists had statistically significantly higher job satisfaction when parental leave was available (2.57 points) than when it was not (p<0.05). Conclusion: Quality dental jobs are an important factor in keeping workers happy and maintaining an efficient practice. Dental practice owners need to pay attention to the quality of jobs required by the dental workforce, provide flexible working hours and welfare programs such as parental leave, and create a workplace atmosphere and human resource management system that supports the use of these programs.

Density map estimation based on deep-learning for pest control drone optimization (드론 방제의 최적화를 위한 딥러닝 기반의 밀도맵 추정)

  • Global population growth has resulted in an increased demand for food production. Simultaneously, aging rural communities have led to a decrease in the workforce, thereby increasing the demand for automation in agriculture. Drones are particularly useful for unmanned pest control fields. However, the current method of uniform spraying leads to environmental damage due to overuse of pesticides and drift by wind. To address this issue, it is necessary to enhance spraying performance through precise performance evaluation. Therefore, as a foundational study aimed at optimizing drone-based pest control technologies, this research evaluated water-sensitive paper (WSP) via density map estimation using convolutional neural networks (CNN) with a encoder-decoder structure. To achieve more accurate estimation, this study implemented multi-task learning, incorporating an additional classifier for image segmentation alongside the density map estimation classifier. The proposed model in this study resulted in a R-squared (R2) of 0.976 for coverage area in the evaluation data set, demonstrating satisfactory performance in evaluating WSP at various density levels. Further research is needed to improve the accuracy of spray result estimations and develop a real-time assessment technology in the field.

Long COVID symptoms and associated factors in registered nurses with COVID-19 (코로나19 확진 간호사의 롱코비드(long COVID) 증상과 관련 요인)

  • Purpose: The coronavirus disease 2019 (COVID-19) pandemic has had significant physical and psychological impacts on registered nurses (RNs). This study aimed to identify long COVID symptoms and their associated factors specifically among RNs. Methods: This descriptive correlational study's sample comprised 189 nurses (31.57±5.98 years, 93.7% female) in Korea. Self-reported long COVID symptoms were assessed using the COVID-19 Yorkshire Rehabilitation Scale. Data were collected from December 31, 2022, to January 13, 2023, using the online survey method and were analyzed using independent t-test, Wilcoxon signed-rank test, one-way ANOVA, Pearson's correlation, and a multiple linear regression analysis with the IBM SPSS Statistics 26.0 program. Results: A total of 179 participants (94.7%) experienced one or more long COVID symptoms. The most prevalent symptoms were weakness (77.8%), fatigue (68.3%), breathlessness (67.7%), cough/throat sensitivity/voice change (50.3%), and sleep problems (50.3%). The factors related to long COVID symptoms were marital status, type of institution, working time, acute COVID-19 symptoms, and vaccination status. The quarantine period (β=.26, p<.001) and the nursing workforce after COVID-19 (β=-.17, p=.018) were significantly associated with long COVID symptoms (Adjusted R2 =.33). Conclusion: Providing comprehensive recognition is necessary for the understanding of long COVID symptoms and their associated factors among nurses and could promote a long COVID symptom management education program targeted at nurses. Moreover, it could facilitate effective nursing care and education plans for long COVID patients.

Search for Viable Alternative for the Korean Occupational Welfare System at the Crossroad (전환점에 서있는 한국기업복지제도의 한 대안으로서 근로자협조프로그램의 적용성에 관한 연구)

  • There is a general consensus that fate of the differentiated welfare state had fallen into the brink of demise during the 1970s while the integrated welfare state model has emerged instead. Indications are all that such trend spotlights occupational welfare which has been relatively neglected, especially, in the field of social welfare. Nevertheless, the occupational welfare system in Korea faced crisis due to two major incidents. One is the IMP crisis which necessitates massive scale of restructuring of the general fabric of the Korean business and industry. There is little doubt that the economic crisis will surely cut down occupational welfare expenditure substantially. The other is a report released by the Korean Labor Research Institute which argus that there is no correlation between the Korean occupational welfare system and labor productivity. Thorough investigation of the Korean occupational welfare system suggests that current system is so outdated that it doesn't meet needs of the industrial workforce which, in turn, fails to enhance workers productivity. Consequently, a need for new paradigm and system arises and employee assistance program(EAP) emerges as a viable alternative. EAP is thoroughly delianeated of it's definition, motivations, functions, ingredients, historical background as well as cost-effectiveness. But major tasks center around an investigation and an analysis of it's applicability. In-depth analysis was carried out on the issue of it's indispensable natures and stumbling blocks on the way to lay it's root in the Korean industrial environment. It seems harder than the herculean task. However, EAP is not simply an alternative but is rather a pressing imperative for the present occupational welfare system in Korea which stands at the crossroad.

  • In the performance of this research we aimed at surveying·studying the trouble factors, problems and improvement plans to extend agricultural products and agro-food and providing basic data for the globalization of Korean agricultural products and agricultural foods in future. All the respondents highly recognized, in general, the problems in exporting agricultural products and agro-food, and it was surveyed that the biggest reason lay in the insufficiency of the scout for new buyers and the lack in opening overseas market and marketing specialists. As for the problems of the supporting system of the export of agricultural products, it was pointed out that the level of recognition was fairly high, but the procedures were difficult, and they indicated the problems of non-benefits in spite of the recognition of the selection. For the purpose, it could be known that it required the publicity and education as to export supporting system, and also required the extension of export supporting system and the improvement of the system. Finally, in relation with the export promotion plan, the factors blocking the export promotion of agricultural products marked the highest in the weak management size and production foundation and the lack in export mind and special workforce. Therefore, considering that most of exporters of agricultural products are small/medium sized businesses, we foresaw that we lack in the opening of overseas market and the capacity of marketing activities, and analyzed that it was necessary to open an overseas market in the dimension of the government and cooperate with the marketing to solve the problems.

The Effect of the Educational Services and Environment of Cooking Education Institutes on Behavioral Intention of Educational Consumers - Focused on Busan Area - (조리교육학원의 교육서비스와 환경이 교육소비자의 행동의도에 미치는 영향)

  • This study has been carried out to provide marketing materials concerning recognition of parents and students for operators of private cooking education institutes and useful information for the parents who are to send their children to such training institutes, by identifying the effects of the quality of educational services and educational environment of cooking education institutes on satisfaction and positive recommendation intention of trainees. Especially, educational services and educational environment of cooking education institutes that provide education to foster skilled workforce in specialized culinary area were analyzed in detail to draw effective data. Firstly, it has been studied if educational services of cooking education institutes such as educational contents, service of instructors and educational service quality had positive effects on the level of satisfaction about the institutes. Secondly, looking into effects of satisfactory environmental service of cooking education institutes, such as educational environment and quality of administration, on behavioral intention, it has been surveyed that all factors affected satisfaction of students. Thirdly, as for the effect of use intention of action of cooking education institute on positive recommendation intention of trainees, it has been shown that satisfaction with educational services and environment had an effect on positive recommendation intention as well as on intention of reregistration. Therefore, it can be suggested that marketing strategies and management strategies need to be established in a way that quality of education services and educational environment provided by cooking education institutes can render positive behavioral intention to customers of cooking education and the education market through differentiated strategy establishment.
